Hepatitis C virus NS5A region mutation in chronic hepatitis C genotype 1 patients who are non-responders to two or more treatments and its relationship with response to a new treatment

To determine the number of mutations in the NS5A region of the hepatitis C virus (HCV) and its relationship to the response to antiviral therapy in patients with chronic hepatitis C genotype 1 who are non-responders to two or more treatments. METHODSSequences within HCV NS5A [PKR binding domain (PKRBD) and the interferon-sensitivity-determining region (ISDR)] were analysed via direct sequencing in a selected cohort of 72 patients, with a total of 201 treatments [interferon-alpha (IFN-α), n = 49; IFN-α + ribavirin (RBV), n = 75; pegylated (peg) IFN-α + RBV, n = 47; first-generation direct-acting antivirals (DAAs), n = 13; and second-generation DAAs, n = 17]. Of these, 48/201 achieved a sustained virological response (SVR) and 153/201 achieved no virological response (NVR). RESUlTSFor both regions, treatments resulting in SVR were associated with more baseline mutations than were treatments resulting in NVR (SVR vs NVR; PKRBD: 5.82 ± 3 vs 4.86 ± 2 mutations, P = 0.045; ISDR: 2.65 ± 2 vs 1.51 ± 1.7 mutations, P = 0.005). A decrease or no change in the number of mutations over time between treatments in the PKRBD or ISDR, as shown by sequencing, was associated with patients who usually failed to respond to treatment (PKRBD, P = 0.02; ISDR, P = 0.001). Moreover, patients showing a post-treatment baseline viral load > 600000 IU/ml and increased ISDR mutations with respect to the previous treatment were 9.21 times more likely to achieve SVR (P = 0.001). CONClUSIONThe obtained results show that among patients who have shown no response to two or more antiviral treatments, the likelihood of achieving SVR increases with the genetic variability in the ISDR region (≥ 2 mutations or number of substitutions from the HCV-J and HCV-1 prototype), especially when the viral load is greater than 600000 IU/ml.

Novel antibacterial materials for implants and medical instruments are essential to develop practical strategies to stop the spread of healthcare associated infections. This study presents the synthesis of multifunctional antibacterial nanocoatings on polydimethylsiloxane (PDMS) by remote plasma assisted deposition of sublimated chlorhexidine powders at low pressure and room temperature. The obtained materials present effective antibacterial activity against Escherichia coli K12, either by contact killing and antibacterial adhesion or by biocide agents release depending on the synthetic parameters. In addition, these multifunctional coatings allow the endure hydrophilization of the hydrophobic PDMS surface, thereby improving their biocompatibility. Importantly, cell-viability tests conducted on these materials also prove their non-cytotoxicity, opening a way for the integration of this type of functional plasma films in biomedical devices.

Pectinases yeast production using grape skin as carbon source

Pectinases are used in Enology for some different utilities. Enzymatic preparations from moulds are a mixed of different enzymes with strong and unspe-cific activities. Some Saccharomyces cerevisiae pro-duce pectinases which can be used instead of com-mercial preparations. The objectives of this work were to study the enzyme secretion by one Saccharo-myces cerevisiae (CECT 11783) for growing on grape skin (industry oenological by-product) as carbon source. Preliminary experiments showed that the strain produced pectinases for growing on grape skin without any other carbon source. Statistical treat-ment (factorial design 25) was applied to evaluate the influences of related factors (agitation, temperature, presence of peptone and detergent in the medium and time of growth) Variables with the most significant interactions for pectinase production were agitation and nitrogen source concentration. Response surface methodology showed that a first order model was not adequate for results. Nevertheless, the built of a sec-ond order model offered a polynomial equation which surface predicted a maximum of activity (52.68 enzymatic units) for specific values of the studied variables (147.8 rpm of agitation and 15.9 g of pep-tone/ l culture medium).

Rare complication of percutaneous endoscopic gastrostomy: Ostomy metastasis of esophageal carcinoma

The authors present the case of a 55-year-old male with a stage Ⅲ(T4N1M0) squamous-cell esophageal carcinoma, who underwent percutaneous endoscopic gastrostomy(PEG). The pull method of tube placement was used. Five months after the procedure, the patient was referred to the hospital with a hard palpable tumour at the ostomy site. The hystologic exam revealed an abdominal wall metastasis of the esophageal cancer. The authors present this case because of the rarity of metastasis in ostomy after placement of PEG in patients with tumours located in the head and neck. In this particular context and judging by the rarity of situation, the clinical impact of this phenomenom is limited. Nevertheless, metastasis in ostomy site could be prevented by the push method, laparoscopy or laparotomy.

Gelatin(G)is a commonly used natural biomaterial owing to its good biocompatibility and easy ***,using pure gelatin as a bioink can barely achieve an ideal shape fidelity in 3D *** this study,Antheraea pernyi silk fibroin nanofibers(ASFNFs)with arginine-glycine-aspartic acid(RGD)peptide and partial natural silk structure are extracted and combined with pure gelatin bioink to simultaneously improve the shape fidelity and cytocompatibility of corresponding 3D printed *** show that the optimum printing temperature is 30℃ for these *** printed filaments using 16G/4ASFNFs bioink(16wt%gelatin and 4wt%ASFNFs)demonstrate better morphology and larger pore size than those printed by pure gelatin bioink(20G,20wt%gelatin),thus successfully improve the shape fidelity and porosity of the 3D printed *** 16G/4ASFNFs scaffold also demonstrate higher swelling ratio and faster degradation rate than the pure gelatin ***,the cell viability and proliferation ability of Schwann cells cultured on the 16G/4ASFNFs scaffold are significantly superior than those cultured on the pure 20G *** ASFNFs enhanced 16G/4ASFNFs scaffold reported here are expected to be a candidate with excellent potential for biomedical applications.

Retroperitoneal epithelioid sarcoma: A case report

Epithelioid sarcoma(ES), a mesenchymatous malign neoformation, is often diagnosed in later stages and associated with high recurrence index, metastasis and mortality. We report a case of a 65 years old male, with history of abdominal pain and upper gastrointestinal bleeding. Endoscopy demonstrated a posterior duodenal wall perforation communicating with a solid retroperitoneal neoformation. Endoscopic biopsy was performed, with a final report of ES. The patient was submitted for surgical palliation due to the tumor's unresectability. Retroperitoneal ES is an extremely rare condition with limited reports in the literature where guidelines for its optimal treatment are not well established.

The aim of this study was to evaluate the effect of a hydrogymnastics training program on highsensitivity C-reactive protein in elderly women. The subjects were randomly divided into the experimental group who participates in the hydrogymnastics training program and the control group who was not involved in the training and remained sedentary. The serum levels of highsensitivity C-reactive protein were determined by immunoturbidimetry using C-reactive protein (CRP), the training protocol consisted of hydrogymnastics sessions 5 times per week during 12 weeks of intervention, executed in a pool of 1.3 meters divided into three phases: a 10-minute warm-up, 30 minutes of aerobic exercise at 50%-60% of maximum heart rate (monitored by heart rate monitor) and a 10-minute cool down. As a statistical method to compare the results between groups of variables pre and post training, the analysis of variance (ANOVA) mixed 2 × 2 (group × measurement) test was done using SPSS version 17. The founds showed significant changes in the level of high-sensitivity C-reactive protein, and a greater percentage decrease of hs-CRP (Δ%= -37%, p = 0.001) in the experimental group in relation to the control ***, it can be inferred that a hydrogymnastics program is effective in reducing the metabolic rate of high-sensitivity C-reactive protein amongst elderly women.

Cutaneous leishmaniasis in an indigenous infant with Down’s syndrome: A case report

Rationale: leishmaniasis, caused by the protozoan leishmania and transmitted by sandflies, is endemic to the tropical and subtropical areas of Brazil. It is one of the neglected tropical diseases in the world. Patient concerns: A 20-month-old indigenous infant with severe malnutrition and Down’s syndrome presented with a facial ulcer for 5 months. Diagnosis: Giemsa staining of scraped ulcer tissues indicated the presence of the amastigote form of leishmania sp., and positive Montenegro’s intradermal test helped in diagnosing the condition as cutaneous leishmaniasis. Interventions: The child was hospitalized and received intravenous treatment for cutaneous leishmaniasis(1 mg/kg/day of amphotericin B). Outcomes: The condition was cured with amphotericin B(1 mg/kg/day for 14 d). lessons: Because of infanticide practices in indigenous cultures, indigenous infants with Down’s syndrome rarely survive. Thus, no similar case has been reported in the literature.
