Background: Duodenoscope-related multidrug-resistant organism(MDRO) infections raise concerns. Disposable duodenoscopes have been recently introduced in the market and approved by regulatory agencies with the aim to reduce the risk of endoscopic retrograde cholangiopancreatography(ERcP) associated infections. The aim of this study was to evaluate the outcome of procedures performed with single-use duodenoscopes in patients with clinical indications to single-operator cholangiopancreatoscopy. Methods: This is a multicenter international, retrospective study combining all patients who underwent complex biliopancreatic interventions using the combination of a single-use duodenoscope and a single-use cholangioscope. The primary outcome was technical success defined as ERcP completion for the intended clinical indication. Secondary outcomes were procedural duration, rate of cross-over to reusable duodenoscope, operator-reported satisfaction score(1 to 10) on performance rating of the single-use duodenoscope, and adverse event(AE) rate. Results: A total of 66 patients(26, 39.4% female) were included in the study. ERcP was categorized according to ASGE ERcP grading system as 47(71.2%) grade 3 and 19(28.8%) grade 4. The technical success rate was 98.5%(65/66). Procedural duration was 64(interquartile range 15-189) min, cross-over rate to reusable duodenoscope was 1/66(1.5%). The satisfaction score of the single-use duodenoscope classified by the operators was 8.6 ± 1.3 points. Four patients(6.1%) experienced AEs not directly related to the single-use duodenoscope, namely 2 post-ERcP pancreatitis(PEP), 1 cholangitis and 1 ***: Single-use duodenoscope is effective, reliable and safe even in technically challenging procedures with a non-inferiority to reusable duodenoscope, making these devices a viable alternative to standard reusable equipment.

Estimating Evapotranspiration Using chloride Mass Balance in a New Mexico Paired Basin Study 2009-2019

A paired basin study in the upper Santa Fe River watershed following forest thinning and prescribed burns successfully measured water budget components in a treated and an untreated (control) basin. The paired basin study was established to investigate questions that have arisen regarding changes in water yield from forest treatments. Precipitation, stream flow, soil moisture, and chloride concentrations in precipitation and stream flow were measured to quantify the water budget components. The results from eleven years of data collection and analysis have a high degree of confidence with respect to measuring the water budget components based on the mass balance of water and chloride. The differences in the geologic structure and topography between the two paired basins appeared to impact the water budgets more than the forest treatments, except during periods when winter precipitation and snowmelt represented a significant component of inflow. Although this paired basin study was not able to portray a simple relationship between forest thinning and water yield, the chloride concentration methodology used to estimate evapotranspiration (ET) was successful. These detailed observations of chloride deposition and transport characteristics may be relevant for other researchers working in forested basins with substantial ET. ET rates were estimated by examining the cycle of chloride entering and exiting each basin over six integration periods. ET was estimated to be about 90% to 94% of precipitation in the treated basin and 77% to 86% in the control basin. The higher ET in the treated basin both before and after forest treatments may be due to the much greater area of west-facing hillslopes in the treated basin, which receive warm afternoon sun, and the greater area of rock cover in the control basin. Rural implementation of the perioperative surgical home:A case-control study

BAcKGROUND Perioperative surgical home(PSH)is a novel patient-centric surgical system developed by American Society of Anesthesiologist to improve outcomes and patient *** has proven success in large urban health centers by reducing surgery cancellation,operating room time,length of stay(LOS),and readmission ***,only limited studies have assessed the impact of PSH on surgical outcomes in rural *** To evaluate the newly implemented PSH system at a community hospital by comparing the surgical outcomes using a longitudinal case-control *** The research study was conducted at an 83-bed,licensed level-III trauma rural community hospital.A total of 3096 TJR procedures were collected retrospectively between January 2016 and December 2021 and were categorized as PSH and non-PSH cohorts(n=2305).To evaluate the importance of PSH in the rural surgical system,a case-control study was performed to compare TJR surgical outcomes(LOS,discharge disposition,and 90-d readmission)of the PSH cohort against two control cohorts[control-1 PSH(c1-PSH)(n=1413)and control-2 PSH(c2-PSH)(n=892)].Statistical tests including chi-square test or Fischer’s exact test were performed for categorical variables and Mann-Whitney test or Student’s t-test were performed for continuous *** general linear models(Poisson regression and binomial logistic regression)were performed to fit adjusted *** The LOS was significantly shorter in PSH cohort compared to two control cohorts(median PSH=34 h,c1-PSH=53 h,c2-PSH=35 h)(P valuecohort had lower percentages of discharges to other facilities(PSH=3.5%,c1-PSH=15.5%,c2-PSH=6.7%)(P valuecal difference observed in 90-d readmission between control and PSH ***,the PSH implementation reduced the 90-d readmission percentage(PSH=4.7%,c1-PSH=6.1%,c2-PSH=3.6%)lower than the national average 30-d readmission percentage which is 5.5%.The PSH system was effecti

Sublethal effects of subzero temperatures on the light brown apple moth, Epiphyas postvittana: fitness costs in response to partial freezing

Population responses to envir on mental extremes often dictate the bounds to species' distributions. However, population dynamics at, or near, those range limits may also be affected by sublethal effects. We exposed late instars and pupae of an invasive leafroller, Epiphyaspostvittana (Walker)(Lepidoptera: Tortricidae), to cold temperatures and measured the effects of exposure on subsequent survivorship, development, and reproduction. cold temperature was applied as acute exposure to -10 °c (a low, but not immediately lethal temperature for this species) or the onset of freezing (the peak of the supercooling point exotherm). Survival was defined as the ability to successfully eclose as an adult. We measured immature development times, pupal mass, and adult longevity as proxies of fitness in survivors. Additionally, surviving insects were mated with individuals that had not been exposed to cold to measure fertility. There was no difference between the proportion of larvae or pupae that survived acute exposure to -10 °c and those exposed to the control temperature. Approximately 17% of larvae and 8% of pupae survived brief periods with internal ice formation and continued development to become reproductively viable adults. Importantly, surviving the onset of freezing came with significant fitness costs but not to exposure to -10 °c;most insects that survived partial freezing had lower fertility and shorter adult lifespans than either the -10 °c or control group. These results are discussed within the context of forecasting invasive in sect distributions.
