Surgical Treatment for Proximal Humerus Fractures Comparison between Plate Fixation and Kapandji’s Pinning

Introduction: Proximal humerus fractures are common, however, there is no consensus on a decision-making algorithm for the therapeutic management of these fractures, the aim of this study is to evaluate and compare functional results between two surgical techniques and to deduce the satisfactory results of the Kapandji’s pinning which, compared to the screw plate, should keep its place in the therapeutic arsenal. Patients and Methods: It’s a retrospective study, including two groups: The first of 18 patients treated with palm-tree pinning using kapandji’s technique, the second of 16 patients treated with anatomical screw plate at the orthopedic trauma service of the Mohamed VI university hospital between July 2013 and July 2018. We compared the results of the two techniques by studying three parameters which are the healing time, the anatomical reduction, and the functional outcomes. The average age was 46 years, and the sex ratio (M/F) was 2.3. Results: The statistical analysis of functional and radiological results showed no significant difference between the two techniques, indeed the average healing time was 56.3 days in the group treated by screw plate and 55.2 days in the group treated by Kapandji’s pinning (p = 0.46), Constant’s score was 73.18 and 79.05 respectively (p = 0.27) and the average cephalodiaphyseal angle was 49.03° and 52.07°, respectively (p = 0.35). Discussion: This study has clearly shown, as reported in the literature, that there is no conclusive evidence to suggest superiority of os-teosynthesis by anatomical plate versus kapandji’s pinning. However, the simple achievement of pinning according to kapandji’s technique, the preservation of soft tissues and the low cost make us prefer this technique. Furthermore, despite the progress noted in the development of osteosynthesis means of the proximal humerus, percutaneous pinning should always keep its place.

Solar insecticidal lamps(SIL) can effectively control pests and reduce the use of pesticides. Combining SIL and Internet of Things(IoT) has formed a new type of agricultural IoT,known as SIL-IoT, which can improve the effectiveness of migratory phototropic pest control. However, since the SIL is connected to the Internet, it is vulnerable to various security *** issues can lead to serious consequences, such as tampering with the parameters of SIL, illegally starting and stopping SIL,etc. In this paper, we describe the overall security requirements of SIL-IoT and present an extensive survey of security and privacy solutions for SIL-IoT. We investigate the background and logical architecture of SIL-IoT, discuss SIL-IoT security scenarios, and analyze potential attacks. Starting from the security requirements of SIL-IoT we divide them into six categories, namely privacy, authentication, confidentiality, access control, availability,and integrity. Next, we describe the SIL-IoT privacy and security solutions, as well as the blockchain-based solutions. Based on the current survey, we finally discuss the challenges and future research directions of SIL-IoT.

Modelling the monthly hydrological balance using Soil and Water Assessment Tool(SWAT)model:A case study of the Wadi Mina upstream watershed

Modelling the hydrological balance in semi-arid zones is essential for effective water resource management,encompassing both surface water and *** study aims to model the monthly hydrological water cycle in the Wadi Mina upstream watershed(northwest Algeria)by applying the Soil and Water Assessment Tool(SWAT)hydrological *** modelling integrates spatial data such as the Digital Elevation Model(DEM),land use,soil types and various meteorological parameters including precipitation,maximum and minimum temperatures,relative humidity,solar radiation and wind *** SWAT model was calibrated and validated using data from January 2012 to December 2014,with a calibra-tion period from January 2012 to August 2013 and a validation period from September 2013 to December *** and parameter calibration were conducted using the SWAT-SA program,and model performance evaluation relied on comparing the observed discharge at the outlet of the basin with model-simulated discharge,assessed through statistical coefficients including Nash-Sutcliffe Efficiency(NSE),coefficient of determination(R2)and Percent Bias(PBAIS).Calibration results indicated favourable objec-tive function values(NSE=0.79,R2=0.93,PBAIS=-8.53%),although a slight decrease was observed during validation(NSE=0.69,R2=0.86,and PBAIS=-11.41%).The application of the SWAT model to the Wadi Mina upstream watershed highlighted its utility in simulating the spatial distribution of different components of the hydrological balance in this *** SWAT model revealed that approximately 71%of the precipitation in the basin evaporates,while only 29%contributes to surface runoff or infiltration into the soil.

Wet-cupping therapy(WCT)is one of the oldest known medical techniques,used as a traditional and complementary therapy with a wide application all around the world for general *** on the effects of WCT on sports performance are sparse and ***,we aimed to explore the effects of WCT on repeated sprint ability,wellness,and exertion in young active ***-nine active adult males(age:[28±5]years;body height[177±8]cm;body mass:[79±7]kg;body mass index:[25.4±1.8]kg/m^(2))were selected for the *** participants performed a running-based sprint test on two separate occasions(Control and Post-WCT).WCT was performed 24 h before the testing *** completed the Hooper questionnaire to assess their well-being(i.e.,sleep,stress,fatigue,and soreness)before each *** rating of perceived exertion(RPE)was collected after each testing session.A higher maximum power(p<0.05,effect size[ES]=0.6),mean power(p<0.01,ES=0.5)and minimum power(p<0.01,ES=0.6)were recorded post-WCT as compared to Control session along with a better perceived sleep(p<0.01,ES=0.85).Perceived stress(p<0.01,ES=0.6)and RPE(p<0.001;ES=1.1)were lower during the post-WCT compared to the Control *** present findings demonstrated that WCT moderately enhanced repeated sprint ability and had positive effects on perceived sleep,stress,and *** may be an effective ergogenic aid to improve repeated sprint ability and general wellbeing in young adult *** large-scale multicentric clinical studies are paramount to confirm the results of our study.

Potential of guar gum as a leaky gut model in broilers: Digestibility,performance, and microbiota responses

Diet is a major modulator of animal resilience and its three pillars: host?s immune response, gut microbiota, and intestinal barrier. In the present study, we endeavour to delineate a challenging condition aimed to degrade these pillars and elucidate its impact on broiler performance and nutrient digestibility. To attain this objective, we opted to use guar gum(GG) as a source of galactomannan. A series of three in vivo experiments were conducted employing conventional or semi-purified diets, supplemented with or without GG during the grower phase(14-28 d). Our findings demonstrate a substantial decline in animal performance metrics such as body weight(reduced by 29%, P < 0.001), feed intake(decreased by 12%, P < 0.001), and feed conversion ratio(up to 58% increase, P < 0.001) in the presence of GG at 2%. The supplementation of a semi-purified diet with incremental doses of GG resulted in a linear reduction(P < 0.001) in the apparent total tract digestibility of dry matter and apparent metabolisable energy. Additionally, a marked reduction in ileal endogenous losses, as well as apparent and standardised digestibility of all amino acids with varying proportions(P < 0.05), was observed. These alterations were accompanied by disrupted gut integrity assessed by fluorescein isothiocyanate-dextran(FITC-d)(P < 0.001) as well as an inflammatory status characterised by elevated levels of acute-phase proteins,namely orosomucoid and serum amyloid A in the sera(P = 0.03), and increased m RNA expression levels of IL-1, IL-6, IL-8, Inos, and K203 genes in the ileum, along with a decrease in Ig A levels in the gut lumen(P < 0.05). Microbial ecology and activity were characterised by reduced diversity and richness(Shannon index, P = 0.005) in the presence of GG. Consequently, our results revealed diminished levels of shortchain fatty acids(P = 0.01) and their producer genera, such as Clostridium＿XIVa and Blautia, in the gut caeca, coupled with excessive accumulation of lactate(17-f

Lithium-sulfur batteries are emerging as sustainable replacements for current lithium-ion *** commercial viability of this novel type of battery is still under debate due to the extensive use of highly reactive lithium-metal anodes and the complex electrochemistry of the sulfur *** this research,a novel sulfur-based battery has been proposed that eliminates the need for metallic lithium anodes and other critical raw materials like cobalt and graphite,replacing them with biomass-derived *** approach presents numerous benefits,encompassing ample availability,cost-effectiveness,safety,and environmental *** particular,two types of biochar-based anode electrodes(non-activated and activated biochar)derived from spent common ivy have been investigated as alternatives to metallic *** compared their structural and electrochemical properties,both of which exhibited good compatibility with the typical electrolytes used in sulfur ***,while steam activation results in an increased specific surface area,the non-activated ivy biochar demonstrates better performance than the activated biochar,achieving a stable capacity of 400 mA h g^(−1)at 0.1 A g^(−1)and a long lifespan(>400 cycles at 0.5 A g^(−1)).Our results demonstrate that the presence of heteroatoms,such as oxygen and nitrogen positively affects the capacity and cycling performance of the *** led to increased d-spacing in the graphitic layer,a strong interaction with the solid electrolyte interphase layer,and improved ion ***,the non-activated biochar was successfully coupled with a sulfur cathode to fabricate lithium-metal-free sulfur batteries,delivering a specific energy density of~600 Wh kg^(−1).

The objective of this work is to improve the physical and mechanical properties of stabilized earth blocks (BTC) used in construction in the Ndé department of Cameroon. To achieve this, two stabilizers, cement and sawdust, were used at varying percentages of 0%, 4%, 6%, and 8%. Physical characterization tests, including natural water content, specific weight, jar test, Protor, and Atterberg limits, were conducted. Additionally, mechanical tests, such as compression and three-point bending, were performed. The results show that as the amount of stabilizer increases, the density of BTC decreases. The samples with 8% sawdust have the highest density, while those without stabilizers have the lowest. Porosity decreases as sawdust and cement content increases, with smaller values observed in samples with 8% sawdust or cement. Our tests indicate that blocks stabilized with cement have slightly higher compressive strength than those stabilized with sawdust. However, the water absorption rate increases with higher sawdust content.

Unraveling the Impact of Direct-Acting Antivirals on Hepatitis-Linked Cirrhosis: A Comprehensive Analysis of Fibrosis, Child Score, and Disease Progression

The treatment of hepatitis C has undergone a significant boom since the advent of direct acting antivirals (DAA). Indeed, the interferon-ribavirin combination that has been used to treat hepatitis C has a virological response in only 45% of cases with significant side effects. The advent of direct-acting antivirals has changed the prognosis of cirrhotic patients with hepatitis C. DAAs have ensured a sustained viral response in the majority of patients. Our work aims to see the evolution of hepatitis C patients at the cirrhosis stage under DAA. We conducted a retrospective study over 15 years (January 2009, January 2024) including all patients with post-viral cirrhosis C, whom we divided into two groups: group A, cirrhotic patients who received ribavirin and interferon, and group B, patients on DAA. From January 2009 to January 2024, we conducted a study of 182 patients with viral hepatitis C, including 102 cirrhotic patients. The mean age was 55 years. 66% of patients were initially treated with the ribavirin interferon combination, while 34% received direct-acting antivirals (DAAs). Since the introduction of DAAs, the most commonly used regimens have been sofosbuvir/daclatasvir with or without ribavirin and sofosbuvir/ledipasvir with or without ribavirin. Group A achieved sustained virological response (SVR) in 60% of cases, with notable side effects. In Group B, SVR was 98.18%, with improved tolerability and fewer side effects than previous treatments. Fifteen patients developed hepatocellular carcinoma (HCC), with a significantly lower mortality rate in those treated with DAAs compared with pegylated dual therapy (p: 0.001).
