Effectiveness of hepatitis B virus vaccination program in Egypt:Multicenter national project

AIM:to assess the effectiveness of hepatitis B virus(HBV) vaccination program among fully vaccinated ***:A national community based crosssectional study was carried out in 6 governorates representing Egypt. A total of 3600 children aged from 9 mo to 16 years who were fully vaccinated with HBV vaccine during infancy were recruited. Face to face interviews were carried out and sera were evaluated for hepatitis B surface antigen(HBsA g),anti-HBV core antibodies(total) and quantitative detection of hepatitis B surface antibody using enzyme linked immunoassays techniques. Samples positive to HBs Ag/anti-HBV core antibodies were subjected to quantitative HBV-DNA detection by real time polymerase chain reaction with 3.8 IU/L detection limit. RESULtS:Sero-protection was detected among 2059 children(57.2%) with geometric mean titers 75.4 ± 3.6 IU/L compared to 3.1 ± 2.1 IU/L among nonseroprotected children. Multivariate logistic analysis revealed that older age and female gender were the significant predicting variables for having non seroprotective level,with adjusted odds ratio 3.3,9.1and 14.2 among children aged 5 to to tively compared to those to boys with P tive in 0.11% and breakthrough infection was 0.36% and 0.39% depending on positivity of anti-HBc and DNA detection respectively. the prevalence of HBV infection was significantly higher among children aged ≥ 7 years(0.59%) compared to 0.07% among younger children with odds ratio equal to 8.4(95%CI:1.1-64.2) and P than boys(0.29%) with P > 0.05. C ON C LU S I ON :t he E gy pt ian c ompuls or y H B V vaccination program provides adequate protection. Occult HBV infection exists among apparently healthy vaccinated children. Adherence to infection control measures is mandatory.

Objective:to determine the metabolic response associate with dengue infection based on human gender metabolic differences by means of ~1H ***:the mid-stream urine collected from both male and female patients diagnosed with dengue fever at Penang General Hospital and fourty-three healthy individuals were analyzed with^1H NMR spectroscopy,followed by chemometric multivariate *** signals which highlighted in the OPLS-DA S-plot were further selected and identified using Human Metabolome Database,Chenonix ***:the results pointed out that NMR urine profiling was able to capture human gender metabolic differences that are important for the distinction of classes of individuals of similar physiological conditions;infected with *** differences between dengue infected patients versus healthy individuals and subtle differences in male versus female infected with dengue were found to be related to the metabolism of amino acid and tricarboxylic acid intermediates ***:the ~1H NMR metabolomic investigation combined with appropriate algorithms and pattern recognition procedures,gave an evidence for the existence of distinct metabolic differentiation of individuals,according to their gender,modulates with the infection risk.

Dietary Patterns and their Association with Depression among type 2 Diabetes Patients in Gaza Strip, Palestine

Background: Depression is a common mental disorder. Globally, more than 340 million people of all ages suffer from depression. the aim of our study was to determine the association between major dietary patterns and depression among type 2 diabetes patients. Methods: this cross-sectional study was conducted among 480 type 2 diabetes patients attending primary healthcare centers in Gaza Strip, Palestine. the depression, anxiety, stress scales (DASS, 21-items) questionnaire was used to measure the score of depression. the participants’ demographic, socioeconomic and medical history data was collected and the 98-items semi-quantitative food frequency questionnaire was used for evaluating the dietary patterns. Statistical analysis was performed using SPSS version 20. Results: Based on depression scale, 29.0% of type 2 diabetes patients had depression, (58.3% females, and 41.7% males). the prevalence of mild, moderate, severe, and very severe depression was 11.7%, 8.5%, 6.7%, and 2.1%, respectively. Furthermore, two major dietary patterns were identified by factor analysis: the Western, and the grains-vegetables, and fruits patterns. After adjusting for the potential confounders, patients in the lowest tertile (t1) of the grains-vegetables, and fruits dietary pattern had a lower odds for depression (OR 0.763 95% CI (0.667 - 0.871), P value = 0.001);where as a higher odds for healthy (OR 1.443 95% CI (1.131 - 1.839), P value = 0.004), compared to those in the highest tertile (t3). Conclusion: the grains-vegetables, and fruits dietary pattern may be associated with a lower prevalence of depression, and has been shown to be the healthiest dietary pattern among type 2 diabetes patients.

Objective:to evaluate the stock of Alepes djedaba(***)by describing the length composition,growth parameters,mortality rates of *** captured in Arabian Gulf off Saudi Arabia and adopting yield per recruit and biomass per recruit ***:A random sample of 490 fish representing a moderate range of total lengths(16.5-32.4cm)and weights(60-410 g)were sampled in Arabian Gulf off Dammam,Saudi Arabia during the period from August 2008 to July ***5 software was used for estimation of growth *** mortality was calculated using the length converted catch *** mortality was estimated using Pauly and David's *** mortality was computed by subtracting natural mortality from total *** recruit analysis was made using Beverton and Holt ***:Length-frequency analysis revealed four peaks and the length range from 22 cm to 27 cm dominated the catch,constituting about 71%of the *** of the von Bertalanffy growth parameters were computed using LFD5 software as follows:the asymptotic length(L_(∞))=41.71 cm,curvature parameter(K)=0.36 year^(-1),and hypothetic age at zero length(t_(0))=-0.76 *** total mortality(Z)was estimated as 2.07 year^(-1),and natural mortality was 0.8 year^(-1).Fishing mortality F=1.27 year^(-1),which was higher than F_(0.1)(0.3 year^(-1)),F_(SB(50))(0.59 year^(-1))and FS_(B(40))(0.86 year^(-1)).Atthe current levels of fishing and natural mortality,the biomass per recruit is 34%of the virgin ***:these may indicate an overexploitation state of the fisheries of *** in Arabian Gulf.

Surgical management of primary hyperparathyroidism guided by double-phase tc-99m-MIBI scintigraphy

Objective:the aim of this work was to study the clinicopathological features of cases with primary hyperparathyroidism (PHPt) referred to National Cancer Institute (NCI), Cairo University in the last six years and to study the role of nuclear medicine techniques in the initial diagnosis and preoperative localization directing surgical management of these cases. Methods: this study included 27 patients with PHPt properly diagnosed and treated in the NCI, Cairo University from January 2005 to December 2010. Preoperative neck U/S and tc-99m-MIBI scintigraphy were done in all cases. If preoperative localization detected a single lesion, unilateral exploration was done. Bilateral exploration was done if multiple foci of active parathyroid glands or no lesions were detected. Results: this study included 27 patients (8 males and 19 females). the median age was 43 years (range from 19 to 68 years). All cases presented with bone disease in the form of bony pain in 23 patients, bony swellings in 15 patients and pathological fractures in 6 cases. twenty one patients (77.8%) had single adenoma, 5 cases (18.5%) had parathyroid hyperplasia, and only one case had double adenomas. the sensitivity of neck U/S in detecting single adenoma was 61.9% (13/21), with 81% positive predictive value (PPV) while the sensitivity of tc-99m-MIBI scintigraphy was 90.5% (19/21) with 100% PPV. Based on preoperative localization tests, unilateral exploration was done in 19 patients with solitary adenomas. All cases in this group were cured with no reported case of persistent or recurrent hypercalcemia. Conclusion: Presentation of PHPt may mimic malignant bone tumors but fortunately these patients were correctly diagnosed with bone scan which confirm the presence of metabolic bone disease rather than bone metastases. this was followed by estimation of serum calcium and parathormone levels for confirmation. Radiological Hazards for Marble and Granite Used at Shak El thouban Industrial Zone in Egypt

the background level of radiation in the natural environment surrounds us at all times. Levels of natural occurring radioactivity in marble and granite used at Shak El thouban industrial zone in Cairo, Egypt have been investigated using HPGe detector through gamma-ray spectrometry. the activity concentration of radionuclides in the 238U-, 232th-series and 40K has been determined. the average activity concentration of 238U, 232th and 40K for marble samples was 23.77 Bq/kg ranged from (10.91 to 45.4), 10.75 Bq/kg ranged from (5.46 to 23.61) and 520.43 Bq/kg ranged from (382.30 to 1132.41), respectively. the 238U, 232th and 40K activity concentration for granite samples were 54.31 Bq/kg ranged from (12.04 to 106.34), 113.57 Bq/kg ranged from (23.91 to 270.36) and 7867.51 Bq/kg ranged from (2017.60 to 11436.91), respectively. Concerning the radiological risk, the radium equivalent activity, external and internal radiation hazard indices, the radiation level index and absorbed dose rate were evaluated. the mass exhalation rates of 222Rn and emanation coefficient have been also calculated. the mass exhalation rate of radon was found to be from 14.86 to 137.13 and 16.48 to 155.26 μBq/kg·s for marble and granite samples, respectively. the mean values of the specific activity of 226Ra, activity of 238U before and after sealing time and the mass exhalation rate of radon for granite samples are twice that for marble samples. All radiological indices and the mass exhalation rate of radon are lower than the permissible levels for building material in all marble samples, while all granite samples are higher and unsafe and pose a risk to the workers and users of these products due to the emanation of radon that may accumulate by time, especially in closed spaces.

Characteristics of escape mutations from occult hepatitis B virus infected patients with hematological malignancies in South Egypt

AIM to investigate the prevalence and virological characteristics of occult hepatitis B virus(HBV) infections in patients with hematological malignancies in South *** Serum samples were collected from 165 patients with hematological malignancies to monitor titers of HBV DNA, hepatitis B surface antigen(HBs Ag), and antibodies to HBV core(anti-HBc) and surface antigens. Serum samples negative for HBs Ag and positive for anti-HBc were subjected to nucleic acid extraction and HBV DNA detection by real-time polymerase chain reaction. DNA sequences spanning the S region were analyzed in cases with occult HBV infection. In vitro comparative study of constructed 1.24-fold wild type and S protein mutant HBV genotype D clones was further performed. RESULtS HBV DNA was detected in 23(42.6%) of 54 patients with hematological malignancies who were HBsA g negative, but anti-HBc positive, suggesting the presence of occult HBV infection. the complete HBV genome was retrieved from 6 occult HBV patients, and P120 t and S143 L were detected in 3 and 2 cases, respectively. Site directed mutagenesis was done to produce 1.24-fold genotype D clones with amino acid mutations t120 and L143. the in vitro analyses revealed that a lower level of extracellular HBsA g was detected by chemiluminescence enzyme immunoassay(CLEIA) with the clone containing t120 mutation, compared with the wild type or the clone with S143 L mutation despite the similar levels of extracellular and intracellular HBs Ag detected by Western blot. Southern blot experiments showed that the levels of intracellular HBV DNA were not different between these clones. CONCLUSION Occult HBV infection is common in patients with hematological malignancies and associated with P120 t and S143 L mutations. 120 t mutation impairs the detection of HBsA g by CLEIA.
