This article provides a review of tropical cyclone(TC)surface wind estimation from an operational forecasting ***,we provide a summary of operational forecast center practices and historical ***,we discuss current and emerging objective estimates of TC surface winds,including algorithms,archive datasets,and individual algorithm strengths and weaknesses as applied to operational TC surface wind forecast *** review leads to recommendations about required surface coverage—an area covering at least 1100 km from center of TC at a 2-km resolution in the inner-core,and at a frequency of at least once every 6 *** is enough coverage to support a complete analysis of the TC surface wind field from center to the extent of the 34-kt(17 m s^(-1))winds at 6-h *** also suggest future designs of TC surface wind capabilities include funding to ensure near real-time data delivery to operators so that operational evaluation and use are feasible within proposed ***,we suggest that users of archived operational wind radii datasets contact operational organizations to ensure these datasets are appropriate for their needs as the datasets vary in quality through time and space,even from a single organisation.

Rapid Gas Chromatographic Assay of Diazinon:Application to the Study of Diazinon Kinetics in Rats

A rapid method was developed for the determination of diazinon in plasma using gas chromatography with electron-capture detection (GC-ECD). Following a single extraction with hexane from 100μl of plasma, diazinon was quantitated on a 3% OV-17 column. The detection limit was 10 ng/ml and linearity was obtained in the range of 25 ng/ml-2500 ng/*** applicability of the assay to single-dose kinetics in rats is illustrated

A Fatal Arterio-Tracheal Fistula Post-Tracheostomy: A Case Report in a Sub-Saharan Setting

Fatal bleeding in patients with tracheostomy is an uncommon incident, in the order of 0.1% to 1% of cases. It is secondary in 70% of cases to the innominate artery fistula in the tracheal lumen. The fistula is generally created after the necrosis of tracheal rings. The incriminated factors are the type of cannula used, the site of tracheal opening below the 3rd tracheal ring, tracheal infections and the proximity of the innominate artery to the tracheal axis. The outcome of this incident is fatal in most cases. We report the case of a 59-year-old patient with a tracheostomy on the 11th day of his admission to intensive care for severe head trauma secondary to a road accident. On the 22nd day of his admission, the occurrence of a cataclysmic and fatal haemorrhage through the tracheostomy tube evoked an arterio-tracheal fistula of the innominate artery.

Clinical and Therapeutic Characteristics of Acute Maxillary Rhinosinusitis in a Hospital of Cotonou

Introduction: Acute maxillary rhinosinusitis (AMRS) is one of the most common ear, nose and throat infections. The aim of this study was to contribute to the improvement of the management of the condition in sub-Saharan Africa. Material and Method: This was a cross-sectional, descriptive study that ran from January 1, 2017 to December 31, 2021 in the ENT-HNS department of the “Centre Hospitalier et Universitaire de Zone de Suru-léré” (CHUZ Sl) in Cotonou, Benin. It involved all patients who consulted during the study period and in whom the diagnosis of acute maxillary rhinosinusitis was made. Results: A total of 405 cases were identified. The mean age was 34.26 ± 15.26 years with extremes of 9 and 63 years. The predominance was female with a sex ratio of 0.61. Acute maxillary rhinosinusitis was bilateral in 371 cases (91.60%). The main symptoms were facial pain in 346 cases (85.43%), mucopurulent rhinorrhea: 315 cases (77.78%), headache: 283 cases (69.88%), and nasal obstruction: 244 cases (60.25%). The most frequent physical signs were pain on pressure of the maxillary sinus points in 405 cases or 100%, purulent secretions at the middle meatus: 11.35%, hyperemia of the nasal mucosa: 53.58%, hypertrophy of the middle turbinate: 41.48% and discharge of pus on the posterior pharyngeal wall: 36.79%. Amoxicillin + clavulanic acid was the main antibiotic prescribed. Vasoconstrictors were used in 228 cases 56.30%. The evolution was favorable in all cases. Conclusion: The diagnosis of acute maxillary rhinosinusitis is clinical. Treatment with antibiotic gives good results.

Characteristics of Ent Foreign Bodies in a Hospital at Cotonou, Benin

Introduction: Foreign bodies (FB) can constitute a life-threatening emergency by their nature or their location. The objective of this study was to describe the socio-demographic, diagnostic and therapeutic characteristics of FB in the ENT department of CHUZ-Suru léré in Cotonou. Methods: This was a descriptive and analytical cross-sectional study, with retrospective data collection spanning a period of ten (10) years from January 1st, 2013 to December 31st, 2022. Patients of all ages seen in consultations in the ENT and CCF department of the CHUZ-Suru léré during the study period and meeting the inclusion criteria were taken into account. Results: During the study period, 1810 cases of FB were collected among the 13,947 consultations, i.e. a hospital frequency of 12.98%. The age of the patients varied from 19 days to 94 years with a predominance of children under 10 years (60.2%). The sex ratio (male/female) was 1.13. The duration between the onset of FB and the medical consultation was greater than 72 hours in 88.49% of cases. The circumstances of occurrence of FB were unknown in 86.34% of cases. The external acoustic meatus was the most common location (90.22%). The nature of FB was inorganic in 86.80% of cases dominated by earwax plugs (77.90%), and organic FB in 7.96% of cases dominated by cotton (3.98%). All FB were extracted by the route of introduction except one case. The extraction was performed most often in the consultation chair (97.07% of cases). The evolution was generally favorable (99.94%). Conclusion: ENT foreign bodies are a frequent reason for consultation and mainly affect male children. They are usually inorganic. Evolution is generally favorable after extraction. Prevention remains the best solution.

Vitamin D and Parathyroid Hormone Profiles in living Kidney Failure Patients in Côte d’Ivoire

Introduction: Abnormalities in mineral and bone metabolism, particularly phosphocalcic metabolism, are common in renal failure and are associated with a significant morbidity and mortality. The regulation of phosphocalcic metabolism is subject to a particularly precise and complex control of parathormone (PTH) and vitamin D. Assessment of vitamin D and parathyroid hormone concentrations would help to improve the medical management of patients with chronic kidney disease and ensure a better quality of life. Methods: The study population consisted of 138 individuals including 46 non- dialysis renal failure patients, 46 chronic hemodialysis patients and 46 non- renal failure volunteers to serve as controls. Serum Parathyroid hormone and Vitamin D concentrations were measured using the Vidas automated system. Results: 25-hydroxyvitamin D concentrations in controls (65 ± 2.41 nmol/l) and dialysis patients (70 ± 3.03 nmol/l) were significantly higher than those in CKD patients (48 ± 3.34 nmol/l). On the other hand, the mean values of Parathyroid hormone in dialysis patients (312 ± 36.22 pg/ml) and CKD patients (117 ± 10.68 pg/ml) were very high compared to that in controls (25 ± 2.34 pg/ml). Conclusion: Secondary hyperparathyroidism is common in renal failure. Parathyroid hormone and 25-hydroxyvitamin D assays would be adequate for better management of chronic renal failure.

BACKGROUND Surgical resection and radiofrequency ablation(RFA)represent two possible strategy in treatment of hepatocellular carcinoma(HCC)in Milan *** To evaluate short-and long-term outcome in elderly patients(>70 years)with HCC in Milan criteria,which underwent liver resection(lR)or *** The study included 594 patients with HCC in Milan criteria(429 in lR group and 165 in RFA group)managed in 10 European *** analysis was performed using the Kaplan-Meier method before and after propensity score matching(PSM)and Cox *** After PSM,we compared 136 patients in the lR group with 136 patients in the RFA *** survival at 1,3,and 5 years was 91%,80%,and 76%in the lR group and 97%,67%,and 41%in the RFA group respectively(P=0.001).Diseasefree survival at 1,3,and 5 years was 84%,60%and 44%for the lR group,and 63%,36%,and 25%for the RFA group(P=0.001).Postoperative Clavien-Dindo IIIIV complications were lower in the RFA group(1%vs 11%,P=0.001)in association with a shorter length of stay(2 d vs 7 d,P=0.001).In multivariate analysis,Model for End-stage liver Disease(MElD)score(>10)[odds ratio(OR)=1.89],increased value of international normalized ratio(>1.3)(OR=1.60),treatment with radiofrequency(OR=1.46),and multiple nodules(OR=1.19)were independent predictors of a poor overall survival while a high MElD score(>10)(OR=1.51)and radiofrequency(OR=1.37)were independent factors associated with a higher recurrence *** Despite a longer length of stay and a higher rate of severe postoperative complications,surgery provided better results in long-term oncological outcomes as compared to ablation in elderly patients(>70 years)with HCC in Milan criteria.

This article describes recent advances in the capability of new satellite sensors for observing Tropical Cyclones(TC)fine structure,wind field,and temporal *** article is based on a World Meteorological Organization(WMO)report prepared for the 10th International Workshop on Tropical Cyclones(IWTC),held in Bali in December 2022,and its objective is to present updates in TC research and operation every four *** we focus on updates regarding the most recent space-based TC observations,and we cover new methodologies and techniques using polar orbiting sensors,such as C-band synthetic aperture radars(SARs),l-band and combined C/X-band radiometers,scatterometers,and microwave imagers/*** additionally address progress made with the new generation of geostationary and small satellites,and discuss future sensors planned to be launched in the next *** then briefly describe some examples on how the newest sensors are used in operations and data assimilation for TC forecasting and research,and conclude the article with a discussion on the remaining challenges of TC space-based observations and possible ways to address them in the near future.

Despite a large variety of processes that can control Mo and its potential to become an environmental tracer of euxinic environment, this element is not often studied in lakes. The aim of this paper is to identify main seasonal biogeochemical processes that involve Mo in a well constrained freshwater system (lake Pavin water-column) in order to evaluate their respective importance. In lake Pavin, 4 main processes have been identified: 1) the transitional process represented by Mo assimilation of by phytoplankton in the epilimnion (nitrogen biological fixation and nitrate assimilation);2) transient process represented by dissolved Mo adsorption onto Fe and Mn metal oxides at oxic/anoxic interface (depth 50 - 60 m);3) Mo precipitation where apparent sulfide production rate is maximum, and from 80 m depths;4) release of dissolved Mo due to Mo benthic flux or input from a deep source.
