Vitamin D Status in Primary Hypothyroid Subjects Attending a Specialized Endocrine Center of Bangladesh

Background: Association between vitamin D deficiency and hypothyroidism has been reported by many authors though the study results are mixed. Objective: The study was conducted to observe vitamin D status among primary hypothyroid subjects. Materials/Methods: In this single-center cross-sectional study, 356 subjects with primary hypothyroidism (either new or previously diagnosed) were evaluated for serum thyroid stimulating hormone (TSH) and 25(OH) vitamin D levels. 25(OH)D levels were classified as normal (≥30 ng/ml), insufficient (>20 to 29.9 ng/ml), and deficient (≤20 ng/ml). Results: The mean age of the study subjects was 36.33 (±12.44) years;majority of them were female (89.9%), homemaker (79.8%), residing in urban or suburban areas (64%), and literate (85.7%);almost half (48%) of them were obese. Their median TSH was 6.17 μIU/ml and mean 25(OH)D was 25.16 (±12.18) ng/ml. The frequencies of vitamin D deficiency and insufficiency were 39.6% and 34.3% respectively. No statistical differences were observed in 25(OH)D levels between males and females, obese and non-obese, new cases and previously diagnosed cases, new cases with subclinical and overt hypothyroidism, the previously diagnosed cases with controlled and uncontrolled hypothyroidism, and among subjects living in urban, suburban and rural areas. Serum 25(OH) vitamin D level showed no correlations with age, body mass index (BMI) and TSH levels of the study subjects. Conclusion: The observed frequency of hypovitaminosis D (deficiency and insufficiency) was high (73.9%) in primary hypothyroid subjects from Bangladesh.

Background: Graves’ ophthalmopathy (GO) is the most frequent extrathyroidal manifestation of Graves’ disease (GD). The effect of intravenous methylprednisolone on the signs and symptoms of GO has not been evaluated in the Bangladeshi population. Objective: To observe the effect of intravenous methylprednisolone in active Graves’ ophthalmopathy. Materials/Methods: This prospective study conducted in a tertiary hospital of Bangladesh from May 2009 to November 2010 included 30 patients having moderate to severe ophthalmopathy. In addition to the clinical activity score (CAS), eye involvement was assessed by using the “NOSPECS” scoring system. Results: The mean (±SD) age of the patients was 38.1 ± 10.6 years and male-female ratio was 1.2:1. NOSPECS eye findings before treatment and during the final (3rd) visit of the patients were observed. It was found that lid retraction or lid lag was improved in 21 (70%) of the patients during the final visit. All of the patients had soft tissue swelling initially and all of them improved at the final visit. Bilateral proptosis was present in 19 patients before treatment and improved in 12 (62%) patients during the final visit. Right eye proptosis was present in 9 patients before treatment and improved in 5 (60%) patients during final visit. Extraocular muscle involvement (Diplopia) was present in 15 patients before treatment but improved in 13 (86.0%) patients during the final visit. A significant number of subjects showed significant improvement in all of the NOSPECS eye findings (except unilateral right eye proptosis) during the final visit;all had inactive GO (CAS Conclusion: Intravenous methylprednisolone appears to be an effective treatment option for active Graves’ ophthalmopathy.

Recently,carbon-based metal-free electrocatalysts(C‐MFECs)have drawn considerable research attention because of their attractive physicochemical characteristics,cost-effectiveness,and ability to convert and store energy *** intramolecular charge transfer among different parts of the carbon electrocatalyst and/or intermolecular charge transfer between elec-trocatalyst and electrolyte dictate the ultimate energy conversion *** results and theoretical analyses have demonstrated that rational design of metal-free carbon nanomaterials,coupled with proper in-tramolecular charge transfer through heteroatom doping,incorporation of Stone-Wales defects,and/or intermolecular charge transfer through adsorp-tion of appropriate molecules/moieties,can promote efficient *** this article,we will first provide the related theoretical principles and then present an overview on the rational design and development of C-MFECs for efficient charge transfer,followed by elucidating charge-transfer processes for different electrocatalytic reactions related to renewable energy conversion and environmental remediation ***,the current challenges and future perspectives in this exciting field will be discussed.

AIM:To identify patient risk factors associated with incomplete small bowel capsule endoscopy(CE) ***:Data from all CE procedures performed at ***'s Hospital in Vancouver,British Columbia,Canada,between December 2001 and June 2008 were collected and analyzed on a retrospective *** collection for complete and incomplete CE study groups included patient demographics as well as a number of potential risk factors for incomplete CE including indication for the procedure,hospitalization,diabetes mellitus with or without end organ damage,limitations in mobility,renal insufficiency,past history of bowel obstruction,abdominal surgery,abdominal radiation therapy and opiate *** factors were analyzed using a univariable and multivariable logistic regression ***:From a total of 535 CE procedures performed,158 were incomplete(29.5%).The univariable analysis showed that CE procedures performed for overt gastrointestinal bleeding(P = 0.002),and for patients with a prior history of abdominal surgery(P = 0.023) or bowel obstruction(P = 0.023) were significantly associated with incomplete CE *** on opiate medications(P = 0.094) as well as hospitalized patients(P = 0.054) were not statistically significant,but did show a trend towards incomplete *** multivariable analysis showed that independent risk factors for an incomplete CE procedure include prior history of bowel obstruction [odds ratios(OR) 2.77,P = 0.02,95% confidence intervals(CI):1.17-6.56] and procedures performed for gastrointestinal bleeding(Occult OR 2.04,P = 0.037,95% CI:1.04-4.02 and Overt OR 2.69,P = 0.002,95% CI:1.44-5.05).Patients with a prior history of abdominal surgery(OR 1.46,P = 0.068,95% CI:0.97-2.19),those taking opiate medications(OR 1.54,P = 0.15,95% CI:0.86-2.76) and hospitalized patients(OR 1.82,P = 0.124,95% CI:0.85-3.93) showed a trend towards statistical ***:We have identified a number of risk factors for incomplete CE procedures

Regio-and stereoselective 1,3-dipolar cycloadditions of C-(3-indolyl)-N-phenylnitrone(10)were carried out with different mono-substituted,disubstituted and cyclic dipolarophiles under mono-mode microwave irradiation to obtain substituted 3-(indol-30-yl)-N-phenyl-isoxazolidines(16–22).Reactions of nitrone(10)with allenic esters under similar conditions afforded,via a domino process,bis-indole derivatives(23a–c)along with compounds 24 and ***,reactions of C-(3-pyridyl)-N-phenylnitrone(26)with mono-substituted,disubstituted and cyclic dipolarophiles were carried out in refluxing dry toluene to obtain substituted 3-(30-pyridyl)-Nphenylisoxazolidines(27–34).Some of the compounds(16f,18b,23a,23c,27c and 29f)display significant cytotoxicity against a number of human cancer cell lines.
