Combinations of metal and lanthanide oxides have been done through casted films for potential medical applications. In this regard, samarium(Ⅲ) oxide/chromium(Ⅲ) oxide/graphene oxide(GO)/polycaprolactone(PCL) based films nano-composites(NCs) were fabricated, pointing their utilization as a biological scaffold for wound dressing purposes. Also, samarium(Ⅲ) oxide and chromium(Ⅲ) oxide have been merged as promising optical constituents due to their unique optical behavior. The structural and compositional examination of the studied NCs was executed by X-ray diffraction(XRD), Raman, and field emission scanning electron microscopy(FESEM). The Sm_(2)O_(3)/Cr_(2)O_(3)/GO/PCL NC exhibits a surface with a lower roughness degree owing to the presence of GO. Cr_(2)O_(3)shows size reduction upon GO insertion to reach 1.2 μm as the average grain size, whilst Sm_(2)O_(3)records an average grain size of less than 1 μm. As well, the polymeric nano-compositions exhibit variation in contact angle values that hit 29.76°± 3.52°for Sm_(2)O_(3)/PCL, and 48.62°± 1.37°for Sm_(2)O_(3)/Cr_(2)O_(3)/GO/PCL as the second lowest contact angle. The optical behavior contributes to absorption edge relocation along the x-axis from 1.7 eV for pure PCL, to 2.65 eV for Sm_(2)O_(3)/Cr_(2)O_(3)/GO/PCL. Regarding biological responses, the cell exposed to 2.5 μg/m L of Sm_(2)O_(3)/Cr_(2)O_(3)/GO/PCL shows cell viability of 119.31%, while 5 μg/m L hits 99.6%. Additionally, the resulting cell attachment micrographs show layers of fibroblast tissue, besides the proliferation and growth of cultivated cells. Thus, the Sm_(2)O_(3)/Cr_(2)O_(3)/GO/PCL scaffold provides 3D proliferation of fibroblast cells endorsing the wound healing process.

AIM: To assess the changes in the anterior chamber parameters using Pentacam following four different lines of treatment of primary angle-closure glaucoma(PACG).METHODS: A retrospective comparative study included 126 patients(126 eye) presented within 24-48 h after acute angle-closure glaucoma(AACG). Patients were divided into 2 groups: group A(68 eyes) with controlled intraocular pressure(IOP) ≤21 mm Hg, which included subgroup A1(34 eyes) with clear lens underwent Nd:YAG laser peripheral iridotomy(LPI) and subgroup A2(34 eyes) with cataract underwent standard phacoemulsification;and group B(58 eyes) with uncontrolled IOP, which included subgroup B1(30 eyes) with clear lens underwent trabeculectomy and subgroup B2(28 eyes) with cataract underwent combined phacoemulsification and trabeculectomy. Patients were fol owed up for at least 3 mo. Primary outcomes were Pentacam anterior segment measurements [anterior chamber angle(ACA) and depth(ACD)]. Secondary outcomes were changes in IOP, visual acuity(VA) and recorded complications. RESULTS: At the 3^(rd)month, there was significant increase in the ACA values in all studied groups compared to preoperative values(P<0.001). The highest percent of increase in ACA was recorded in phacotrabeculectomy group B2(128.40%). There was significant increase in ACD values at 3^(rd)month compared with baseline values(P<0.001) for groups A1, A2, and B2;without change in B1 trabeculectomy group. The maximum deepening of ACD was noticed in group B2 with 94.27% increase. Significantdecrease in postoperative IOP in groups A2, B1 and B2(P<0.001, P=0.014, and P<0.001 respectively). In group A1 there was significant increase in 3^(rd)month postoperative IOP from baseline values(P<0.001). The maximum decrease in IOP was noticed in group B2 with 59.54% decrease. AIM： To assess the objective and subjective results of a two-stage procedure for management of keratoconus： clear lensectomy with aspherical intraocular lens（IOL） implantation followed by wave front-guided photorefractive keratotomy（WFG-PRK）.METHODS： This prospective interventional non-randomized study included patients aged 35 years old or more with grade I and II stable keratoconus, a clear visual axis, minimal corneal thickness（MCT） 420 μm or more and average keratometric reading（K） less than 54 diopter（D）. Refraction of all selected eyes should be-8.00 D sphere or more with less than-6.00 D cylinder and could be corrected two lines or more with spectacles or contact lenses. All studied eyes underwent a two-stage approach treatment： first refractive lens exchange and aspherical IOL implantation followed, after at least 3 mo, by WFGPRK. Pre and postoperative complete ophthalmological examination were performed. Topographical, visual and aberrometric results were recorded and evaluated during 6 mo follow up period. Moreover, patient satisfaction and other subjective outcomes were also ***： The 13 eyes of 11 patients diagnosed with stable keratoconus and aged from 39 to 49 y（42.4±6.2 y） were enrolled in the study. At baseline, 8 eyes had grade I and 5 eyes had grade II keratoconus. The manifest sphere was-10.3±4.2 D（ranged from-8.0 to-14.0 D） and the manifest cylinder was-4.2±1.2 D（ranged from-1.75 to-5.50 D）. After the two-stage procedure, sphere and cylinder reduced significantly to-0.43±0.22 D and-1.3±0.72 D respectively（P〈0.001）. There was also a highly significant improvement in the mean uncorrected distance visual acuity（UDVA） from logMAR 1.41±0.49 preoperatively to 0.51±0.16 postoperatively（P〈0.001） and the mean corrected distance visual acuity（CDVA） from 0.76±0.24 preoperatively to 0.49±0.13 after the operation（P〈0.001）. AIM:To evaluate the safety and efficacy of augmented trabeculotomy with Ologen versus perfluoropropane in management of pseudophakic ***:This is a comparative randomized study included 57 pseudophakic eyes of 57 patients with medically uncontrolled open angle glaucoma(OAG).Twentynine patients were allocated in group I(trabeculectomy with Ologen;trab-ologen group),while 28 patients were assigned in group II(trabeculectomy with perfluoropropane gas bubble;trab-C3 F8 gas bubble group).RESULTS:The intraocular pressure(IOP)was significantly reduced in both study groups at all postoperative follow up intervals(1 wk,3,6,12,18,24,30 and 36 mo,P<0.001).The differences between the mean IOP values of both groups remained statistically insignificance during the early 12 months of follow ***,the trab-ologen group achieved a statistically significant reduction over the trab-C3 F8 gas bubble group during the last 24 months of follow ***:Augmentation of trabeculectomy with either Ologen implant or perfluoropropane gas bubble are associated with strict long term IOP control and evident safety in medically-uncontrolled pseudophakic eyes with OAG.

An Ex-Press implant versus trabeculectomy in a fibrotic bleb with late failure after previous trabeculectomy

AIM: To compare the outcome of an Ex-Press implant and subscleral trabeculectomy(SST) in the management of glaucoma after previous trabeculectomy on a fibrotic ***: This randomized prospective study included 28 eyes from 28 patients(age range: 42-55 y) with primary open angle glaucoma(POAG) presented with elevated intraocular pressure(IOP) with fibrotic bleb despite previous SST for more than 4 mo. The eyes enrolled in the study were divided into two groups: group I(subjected to Ex-Press implant surgery) and group II [subjected to SST with mitomycin C(MMC)]. The follow-up continued one year after surgery to evaluate IOP, visual acuity(VA), visual field(VF), and postoperative complications. RESULTS: A significant decrease in IOP was found in both groups with a higher reduction in Ex-Press implant surgery with the mean IOP of 14.50 mm Hg(P=0.001), while the SST group recorded the mean IOP of 16.50 mm Hg(P=0.001) after one year. However, the difference between the two groups in terms of the decrease in IOP was insignificant. Fewer postoperative complications were recorded in the Ex-Press implant surgery and more cases requiring further anti-glaucomatous medications were seen in the SST group. Both groups showed stability in terms of VA and ***: Ex-Press implant surgery and SST with MMC are two surgical alternatives for controlling IOP in late failure that occurs more than 4 mo after previous SST with a fibrotic bleb. However, Ex-Press shunt is a safer surgery with fewer complications.

Objectives: In recent times, the term “minimal intervention dentistry” has coined to describe a new approach to the treatment of the disease of caries. This study was carried out to evaluate the marginal integrity of class V cavity in permanent posterior teeth prepared with different air abrasion parameters compared with classical rotary cavity preparation. Methods: A total number of 150 sound caries for free human posterior teeth were used in this study. Teeth were extracted for periodontal disease or orthodontic reasons and the average age of patients between 20 - 35 years. Teeth were divided according to the type of restoration into two main groups: Group I (n = 75) restored with low shrinkable posterior composite (P90) and Group II (n = 75) restored with Ketac nano ionomer (N100). Each main group was divided into five groups (n = 15), then each group was further subdivided into three subgroups (n = 5) according to the storage time (immediately, three months, and six months). All teeth would be tested at both occlusal and gingival margins of prepared class V cavity according to the method of cavity preparation. Results: A high significant difference was found in the mean ranks of micro-leakageat both occlusal enamel and gingival dentin walls among Groups 1 - 5 (p-value of Conclusion: Low shrinkable silorane based composite yielded better results of micro-leakage scores along the occlusal enamel and gingival dentin walls when the cavity of class V was prepared with air abrasion technique with large size alumina particles under high pressure at immediate storage time as compared with Ketac N100.

Childhood glaucoma profile in Dakahelia, Egypt: a retrospective study

AIM： To analyze childhood glaucoma regarding its demographics, presentations, different causes and surgical modalities used among patients in Dakahelia and to apply the Childhood Glaucoma Research Network（CGRN） classification retrospectively to evaluate its ***： A retrospective study in which the medical files of all glaucoma patients 〈16 years old presented to Mansoura Ophthalmic Center, Mansoura University from 2014 to 2017, were retrieved and analyzed. Collected data included： age, gender, laterality, visual acuity（VA）, refraction, intraocular pressure（IOP）, corneal diameter, cup-disc ratio, types and number of surgeries and antiglaucomatous drugs（AGD） at the first and last visit. Prevalence of different subtypes was calculated and means of clinical features were ***： A total of 305 eyes of 207 patients were included classified into 6 groups： primary congenital glaucoma（PCG）, juvenile open angle glaucoma（JOAG）, glaucoma associated with systemic disease, glaucoma associated with ocular anomalies, acquired glaucoma and glaucoma following cataract surgery. PCG was the predominant type（55.1%） followed by acquired glaucoma（29.5%）. Males represented 63.8% of the whole studied patients. Glaucoma associated with ocular anomaly group showed the youngest age at diagnosis（21.9±30.0 mo）. The shortest corneal diameter was recorded in post cataract group（10.4±0.5 mm）. Highest cup-disc ratio was found in the PCG group（P〈0.0005）. Glaucoma associated with systemic disease presented with the highest baseline IOP（34.5±5.0 mm Hg）. All the cases with PCG were treated surgically with 21.8% required more than one surgery. The majority of the patients（74.2%） in the acquired group were treated medically. Combined trabeculotomy-trabeculectomy was the most frequent operation done, accounting for 34.5% of all primary surgeries. ahmed valve implantation comprised 87% of the secondary surgeries. AIM To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.
