Purpose:To compare cartilage tympanoplasty(CT)combined with eustachian tube balloon dilatation(ETBD)and cartilage tympanoplasty alone as a surgical treatment modality for adhesive otitis media(AdOM)in terms of graft healing,audiological outcomes,and impact on life style,using Chronic Otitis Media Outcome Test 15(COMOT-15).Methods:50 patients with AdOM were randomly classified into 2 groups:25 patients for cartilage tympanoplasty only(CT group)and 25 patients for cartilage tympanoplasty combined with eustachian tube balloon dilatation(CT t ETBD group).Clinical outcomes in both groups were compared at 3 and 6 months of follow up.Results:There was no significant difference in graft healing between the two groups.Postoperative COMOT-15 scores significantly decreased in both groups with a significant difference between the groups with regard to the decrease in COMOT-15 scores at 3 and 6 months of follow-up(P<0.05).Hearing improvement was achieved,as the mean preoperative ABG was 26.5±5.4 and 27.1±4.6 dB,and the mean postoperative ABG at 6 months was 19.4±4.4 and 14.6±3.9 dB in the CT and the CT t ETBD groups,respectively.The difference in the magnitude of ABG reduction in the two groups was significant at 3 and 6 months of postoperative follow-up(P<0.05)in favour of the CT t ETBD group.Conclusion:ETBD can increase the success rate of cartilage tympanoplasty in patients with AdOM by enhancing the audiological outcome and quality of life.

Diabetic complications,chiefly seen in long-term situations,are persistently deleterious to a large extent,requiring multi-factorial risk reduction strategies beyond glycemic control.Diabetic cardiomyopathy is one of the most common deleterious diabetic complications,being the leading cause of mortality among diabetic patients.The mechanisms of diabetic cardiomyopathy are multi-factorial,involving increased oxidative stress,accumulation of advanced glycation end products(AGEs),activation of various pro-inflammatory and cell death signaling pathways,and changes in the composition of extracellular matrix with enhanced cardiac fibrosis.The novel lipid signaling system,the endocannabinoid system,has been implicated in the pathogenesis of diabetes and its complications through its two main receptors:Cannabinoid receptor type 1 and cannabinoid receptor type 2,alongside other components.However,the role of the endocannabinoid system in diabetic cardiomyopathy has not been fully investigated.This review aims to elucidate the possible mechanisms through which cannabinoids and the endocannabinoid system could interact with the pathogenesis and the development of diabetic cardiomyopathy.These mechanisms include oxidative/nitrative stress,inflammation,accumulation of AGEs,cardiac remodeling,and autophagy.A better understanding of the role of cannabinoids and the endocannabinoid system in diabetic cardiomyopathy may provide novel strategies to manipulate such a serious diabetic complication.

Novel non-invasive biological predictive index for liver fibrosis in hepatitis C virus genotype 4 patients

AIM To investigate the diagnostic ability of a non-invasive biological marker to predict liver fibrosis in hepatitis C genotype 4 patients with high accuracy.METHODS A cohort of 332 patients infected with hepatitis C genotype 4 was included in this cross-sectional study. Fasting plasma glucose, insulin, C-peptide, and angiotensinconverting enzyme serum levels were measured. Insulin resistance was mathematically calculated using the homeostasis model of insulin resistance(HOMA-IR).RESULTS Fibrosis stages were distributed based on Metavir score as follows: F0 = 43, F1 = 136, F2 = 64, F3 = 45 and F4 = 44. Statistical analysis relied upon reclassification of fibrosis stages into mild fibrosis(F0-F) = 179, moderate fibrosis(F2) = 64, and advanced fibrosis(F3-F4) = 89. Univariate analysis indicated that age, log aspartate amino transaminase, log HOMA-IR and log platelet count were independent predictors of liver fibrosis stage(P < 0.0001). A stepwise multivariate discriminant functional analysis was used to drive a discriminative model for liver fibrosis. Our index used cut-off values of ≥ 0.86 and ≤-0.31 to diagnose advanced and mild fibrosis, respectively, with receiving operating characteristics of 0.91 and 0.88, respectively. The sensitivity, specificity, positive predictive value, negative predictive value and positive likelihood ratio were: 73%, 91%, 75%, 90% and 8.0 respectively for advanced fibrosis, and 67%, 88%, 84%, 70% and 4.9, respectively, for mild fibrosis.CONCLUSION Our predictive model is easily available and reproducible, and predicted liver fibrosis with acceptable accuracy.

Medical treatment of inflammatory punctual stenosis monitored by anterior segment optical coherence tomography

AIM:To evaluate the role of medical treatment in the management of inflammatory punctum stenosis guided by spectral domain anterior segment optical coherence tomography(OCT).METHODS:This prospective study included 60 patients complaining of epiphora due to inflammatory punctual stenosis.They were divided into two groups:Group A(30 patients)treated with a combination of preservative free hydrocortisone sodium phosphate 3.35 mg/m L eye drops,and preservative free artificial tears based on sodium hyaluronate,polyethylene and propylene Glycol;Group B(30 patients)treated with the same preservative free artificial tears only.Thirty normal subjects were included for comparison of pre-treatment anatomical parameters.Before starting treatment,all patients underwent anterior segment assessment including slit lamp examination,measurement of intraocular pressure(IOP),fluorescein dye disappearance test,and measurement of the outer punctum diameter(OPD)by Spectral domain anterior segment OCT.All assessments were repeated at each follow up done at 1 and 3 mo later,together with subjective evaluation of patient’s satisfaction of the treatment outcome by simple rating questionnaire.RESULTS:Punctual diameter increased significantly with treatment in both groups(P<0.0001);although the widening was more in Group A as compared to group B(16.2%vs 8%of the original punctual size,mean difference of 28.933μm,P=0.0076).Subjective satisfaction with treatment outcome was also better in group A(70%vs 40%,Chi-square P=0.0397).CONCLUSION:A combination of preservative free steroid eye drops and artificial tears causes significant widening of inflammed stenotic punctae and improvement of the associated epiphora.

Objective:To improvein vitro embryo production in buffalo by supplementation of L-ascorbic acid during maturation and development (experiment 1) and combination with another antioxidant as cysteamine (experiment 2).Methods:Two experiments were performed, the first one aimed to evaluate the different concentrations (0, 25, 50, 100 μM) of L-ascorbic acid on embryo developmental rate of buffalo oocytes. The L-ascorbic acid was added to the maturation and culture media. In the second experiment, oocytes were cultured in media with two type of antioxidant (ascorbic acid + cysteamine) or ascorbic acid only.Results:There was a significant increase in cleavage rate at 25, 50 μM than 100 μM and control group. But, the blastocyst rate was higher at 50 μM ascorbic acid than other concentrations (0, 25, 100 μM). Supplementation of ascorbic acid and cysteamine to maturation and cultured media improved embryo development than ascorbic acid alone.Conclusions: Using of 50 μM L-ascorbic acid duringin vitro maturation and development improve the developmental competence of buffalo oocytes, this effect was increase with the presence of cysteamine.

Objective: To compare between the efficacy of the use of oral sildenafil plus low dose aspirin versus the use of oral low dose aspirin alone in pregnancy as preventive measure in women at risk for preeclampsia (PE). Design: A randomized clinical trial. Setting: Outpatient Obstetric clinic of Ain Shams University Maternity Hospital. Population or sample: Women at gestational age of ≤16 weeks who at risk for PE between June 2018 and June 2019. Methods: Participants were randomly allocated into two groups: Group I Included 200 women who received a 25 mg tablet of oral sildenafil citrate tid until delivery plus 100 mg tablet of aspirin orally once daily until gestational age of 36 weeks, Group II Included 200 women who received a 100 mg tablet of aspirin orally once daily until gestational age of 36 weeks. Main Outcome Measures: Incidence of preeclampsia diagnosed per ACOG criteria. Results: The incidence of PE in both groups showed no statistically significant difference. The incidence of PE in the first group is 11.0%, and it is 12.0% in the second group (p-value 0.754). The purpose of this paper is to propose an efficient model and a robust control that ensures good power quality for the AC microgrid(MG)connected to the utility grid with the integration of an electric vehicle(EV).The MG consists of two renewable energy sources:a photovoltaic system(PVS)and a wind turbine system(WTS)based on a permanent magnet synchronous generator(PMSG),with the integration of an EV.These sources are used to supply active and reactive power to the AC bus and the utility grid.Maximum power point tracking(MPPT)based on the perturb-and-observe(PO)method is used to increase the efficiency of the photovoltaic modules and improve overall performance.The MG system includes a 2-MW WTS,a 100-kW PVS and 12 kW provided by the EV.To validate the performance of the proposed system,a series of simulations were conducted using the MATLAB/Simulink?environment.The results demonstrate that the proposed system ensures high performance in terms of power quality,system stability,power tracking and safe integration of the EV.

Inertial navigation represents a unique method of navigation,in which there is no dependency on external sources of information.As opposed to other position fixing navigation techniques,inertial navigation performs the navigation in a relative sense with respect to the initial navigation state of the moving platform.Hence,inertial navigation systems are not prone to jamming,or spoofing.Inertial navigation systems have developed vastly,from their occurrence in the 1940s up to date.The accuracy of the inertial sensors has improved over time,making inertial sensors sufficient in terms of size,weight,cost,and accuracy for navigation and guidance applications.Within the past few years,inertial sensors have developed from being purely mechanical into incorporating various technologies and taking advantage of numerous physical phenomena,from which the dynamic forces exerted on a moving body could be computed accurately.Besides,the evolution of inertial navigation scheme involved the evolution from stable-platform inertial navigation system,which were mechanically complicated,to computationally demanding strap-down inertial navigation systems.Optical sensory technologies have provided highly accurate inertial sensors,at smaller sizes.Besides,the vibratory inertial navigation technologies enabled the production of Micro-electro-machined inertial sensors that are extremely low-cost,and offer extremely low size,weight and power consumption,making them suitable for a wide range of day-to-day navigation applications.Recently,advanced inertial sensor technologies have been introduced to the industry such as nuclear magnetic resonance technology,coldatom technology,and the reintroduction of fluid-based inertial sensors.On another note,inertial sensor errors constitute a huge research aspect in which it is intended for inertial sensors to reach level in which they could operate for substantially long operation times in the absence of updates from aiding sensors,which would be a huge leap.Inertial sensors error modeling technique

The current study aimed at evaluating the capabilities of seven advanced machine learning techniques(MLTs),including,Support Vector Machine(SVM),Random Forest(RF),Multivariate Adaptive Regression Spline(MARS),Artificial Neural Network(ANN),Quadratic Discriminant Analysis(QDA),Linear Discriminant Analysis(LDA),and Naive Bayes(NB),for landslide susceptibility modeling and comparison of their performances.Coupling machine learning algorithms with spatial data types for landslide susceptibility mapping is a vitally important issue.This study was carried out using GIS and R open source software at Abha Basin,Asir Region,Saudi Arabia.First,a total of 243 landslide locations were identified at Abha Basin to prepare the landslide inventory map using different data sources.All the landslide areas were randomly separated into two groups with a ratio of 70%for training and 30%for validating purposes.Twelve landslide-variables were generated for landslide susceptibility modeling,which include altitude,lithology,distance to faults,normalized difference vegetation index(NDVI),landuse/landcover(LULC),distance to roads,slope angle,distance to streams,profile curvature,plan curvature,slope length(LS),and slope-aspect.The area under curve(AUC-ROC)approach has been applied to evaluate,validate,and compare the MLTs performance.The results indicated that AUC values for seven MLTs range from 89.0%for QDA to 95.1%for RF.Our findings showed that the RF(AUC=95.1%)and LDA(AUC=941.7%)have produced the best performances in comparison to other MLTs.The outcome of this study and the landslide susceptibility maps would be useful for environmental protection.

AIM: To develop a mathematical model for the early detection of hepatocellular carcinoma(HCC) with a panel of serum proteins in combination with α-fetoprotein(AFP).METHODS: Serum levels of interleukin(IL)-8, soluble intercellular adhesion molecule-1(s ICAM-1), soluble tumor necrosis factor receptor Ⅱ(s TNF-RⅡ), proteasome, and β-catenin were measured in 479 subjects categorized into four groups:(1) HCC concurrent with hepatitis C virus(HCV) infection(n = 192);(2) HCV related liver cirrhosis(LC)(n = 96);(3) Chronic hepatitis C(CHC)(n = 96); and(4) Healthy controls(n = 95). The R package and different modules for binary and multi-class classifiers based on generalized linear models were used to model the data. Predictive power was used to evaluate the performance of the model. Receiver operating characteristic curve analysis over pairs of groups was used to identify the best cutoffs differentiating the different groups. RESULTS: We revealed mathematical models, based on a binary classifier, made up of a unique panel of serum proteins that improved the individual performance of AFP in discriminating HCC patients from patients with chronic liver disease either with or without cirrhosis. We discriminated the HCC group from the cirrhotic liver group using a mathematical model(-11.3 + 7.38 × Prot + 0.00108 × s ICAM + 0.2574 ×β-catenin + 0.01597 × AFP) with a cutoff of 0.6552, which achieved 98.8% specificity and 89.1% sensitivity. For the discrimination of the HCC group from the CHC group, we used a mathematical model [-10.40 + 1.416 × proteasome + 0.002024 × IL + 0.004096 × s ICAM-1 +(4.251 × 10-4) × s TNF + 0.02567 ×β-catenin + 0.02442 × AFP] with a cutoff 0.744 and achieved 96.8% specificity and 89.7% sensitivity. Additionally, we derived an algorithm, based on a binary classifier, for resolving the multi-class classification problem by using three successive mathematical model predictions of liver disease status.
