BACKGROUND Hemorrhoidal disease(HD)is considered a low-severity pathology by both general population and physicians,but the lengthy conservative therapy and postoperative complications suggest *** To assess the effectiveness of different treatment options,both conservative and surgical,in contrast with some preexisting *** We conducted a retrospective,10-yearlong study between January 2011 and December 2021 in two surgical centers,a private and a state-owned *** compared the efficacy and safety of several treatment options,such as open hemorrhoidectomy,stapled hemorrhoidopexy,rubber band ligation and infrared coagulation in terms of complication rates and types and their correlation with different preexisting comorbidities such as inflammatory bowel disease(IBD),use of anticoagulant medication(AM)and liver *** also conducted a 20-years long PubMed research(1.263 articles)for relevant *** Our study recorded 10940 patients with HD,10241 with conservative and 699 with surgical *** of these,the male-to-female ratio of 1.3,and a peak in age distribution between 59 and 68 years old(32%of patients).For the entire study,we recorded a 90%incidence of immediate pain,immediate bleeding in 1.5%(11 cases),delayed bleeding in 1.0%(7 cases),and 0.6%surgical site *** retention was also present,with 0.2%of patients,anal stricture in 1%and fecal incontinence for 0.5%of patients(4 cases).We recorded no severe complications such as Fournier`s gangrene or rectovaginal *** accounted for 6%of the patients,with ulcerative colitis in 12%and Chron`s disease in 10.5%.6.6%of the patients had AM,determining 4%immediate and 2%delayed bleeding,in surgically treated *** Our study determined that most common complications(pain,urinary retention,bleeding,and stricture)are correlated with each surgical technique and pre-existing comorbidities.

Evaluating the potential of(epi)genotype‑by‑low pass nanopore sequencing in dairy cattle:a study on direct genomic value and methylation analysis

Background Genotype-by-sequencing has been proposed as an alternative to SNP genotyping arrays in genomic selection to obtain a high density of markers along the *** requires a low sequencing depth to be cost effective,which may increase the error at the genotype *** generation nanopore sequencing technology offers low cost sequencing and the possibility to detect genome methylation,which provides added value to *** aim of this study was to evaluate the performance of genotype-by-low pass nanopore sequencing for estimating the direct genomic value in dairy cattle,and the possibility to obtain methylation marks *** Latest nanopore chemistry(LSK14 and Q20)achieved a modal base calling accuracy of 99.55%,whereas previous kit(LSK109)achieved slightly lower accuracy(99.1%).The direct genomic value accuracy from genotype-by-low pass sequencing ranged between 0.79 and 0.99,depending on the trait(milk,fat or protein yield),with a sequencing depth as low as 2×and using the latest chemistry(LSK114).Lower sequencing depth led to biased estimates,yet with high rank *** LSK109 and Q20 achieved lower accuracies(0.57-0.93).More than one million high reliable methylated sites were obtained,even at low sequencing depth,located mainly in distal intergenic(87%)and promoter(5%)*** This study showed that the latest nanopore technology in useful in a LowPass sequencing framework to estimate direct genomic values with high *** may provide advantages in populations with no available SNP chip,or when a large density of markers with a wide range of allele frequencies is *** addition,low pass sequencing provided nucleotide methylation status of>1 million nucleotides at≥10×,which is an added value for epigenetic studies.

The effect of mining face＇s direction on the observed seismic activity

A major natural hazard associated with LGOM (Legnica-Glogow Copper Mining) mining is the dynamic phenomena occurrence, physically observed as seismic tremors. Some of them generate effects in the form of relaxations or bumps. Long-term observations of the rock mass behaviour indicate that the degree of seismic hazard, and therefore also seismic activity in the LGOM area, is affected by the great depth of the copper deposit, high-strength rocks as well as the ability of rock mass to accumulate elastic energy. In this aspect, the effect of the characteristics of initial stress tensor and the orientation of considered mining panel in regards to its components must be emphasised. The primary objective of this study is to answer the question, which of the factors considered as “influencing” the dynamic phenomena occurrence in copper mines have a statistically significant effect on seismic activity and to what extent. Using the general linear model procedure, an attempt has been made to quantify the impact of different parameters, including the depth of deposit, the presence of goaf in the vicinity of operating mining panels and the direction of mining face advance, on seismic activity based on historical data from 2000 to 2010 concerned with the dynamic phenomena recorded in different mining panels in Rudna mine. The direction of mining face advance as well as the goaf situation in the vicinity of the mining panel are of the greatest interest in the case of the seismic activity in LGOM. It can be assumed that the appropriate manipulation of parameters of mining systems should ensure the safest variant of mining method under specific geological and mining conditions.

Corrosion study of resorbable Ca60Mg15Zn25 bulk metallic glasses in physiological fluids

The corrosion activity of amorphous plates of Ca60Mg15Zn25alloy was *** biocompatible elements were selected for the alloy *** electrochemical corrosion and immersion tests were carried out in a multi-electrolyte fluid and Ringer's *** corrosion behavior was observed for the samples tested in a multi-electrolyte fluid despite the active dissolution of Ca and Mg in Ringer's *** experimental results indicated that reducing concentration of NaCl from 8.6 g/dm3for Ringer's solution to 5.75 g/dm3caused the decrease of the corrosion *** volume of the hydrogen evolved after 480 min in Ringer's solution（40.1 ml/cm2）was higher in comparison with that obtained in a multi-electrolyte fluid（24.4 ml/cm2）.The values of opencircuit potential(EOCP)for the Ca60Mg15Zn25glass after 1 h incubation in Ringer's solution and a multielectrolyte fluid were determined to be-1553 and-1536 m V vs.a saturated calomel electrode（SCE）.The electrochemical measurements indicated a shift of the corrosion current density(jcorr)from 1062μA/cm2for the sample tested in Ringer's solution to 788μA/cm2for the specimen immersed in a multi-electrolyte *** corrosion products analysis was conducted by using the X-ray photoelectron spectroscopy（XPS）.The corrosion products were identified to be CaCO3,Mg（OH）2,CaO,MgO and Zn *** mechanism of corrosion process was proposed and described based on the microscopic *** X-ray diffraction and Fourier transform infrared spectroscopy（FTIR）also indicated that Ca（OH）2,CaCO3,Zn（OH）2and Ca(Zn（OH）3)2·2H2O mainly formed on the surface of the studied alloy.

The Effects of Using a Specially Designed Stirrup on Kinetic Energy Absorption by the Knee Joint of 12 Show Jumping/Eventing Riders

The use of Winderen Knee Protection Solution stirrups compared to standard iron stirrups, reveals the following benefits: 1) A reduction of stress or strain time in the order of 14 seconds per minute of activity whilst walking and 5 - 7 seconds less whilst trotting or cantering for muscles around the knee. 2) A reduction of stress or strain time in the order of 25 seconds per minute of activity whilst walking and 9 - 10 seconds less whilst trotting or cantering for ligaments around the knee. 3) A significant improvement in the E-score (less time exposed to stress and shock) and ST-score (lower force around the knee) whilst walking. 4) A considerable improvement in rider comfort and feeling of leg stability (self-assessment) compared with the owners current stirrups, whilst riding.

Integrated Sequential Groundwater Contaminant Source Characterization and Pareto-Optimal Monitoring Network Design Application for a Contaminated Aquifer Site

Accurate and reliable groundwater contaminant source characterization with limited contaminant concentration monitoring measurement data remains a challenging problem. This study presents an illustrative application of developed methodologies to a real-life contaminated aquifer. The source characterization and optimal monitoring network design methodologies are used sequentially for a contaminated aquifer site located in New South Wales, Australia. Performance of the integrated optimal source characterization methodology combining linked simulation-optimization, fractal singularity mapping technique (FSMT) and Pareto optimal solutions is evaluated. This study presents an integrated application of optimal source characterization with spatiotemporal concentration measurement data obtained from sequentially designed monitoring networks. The proposed sequential source characterization and monitoring network design methodology shows efficiency in identifying the unknown source characteristics. The designed monitoring network achieves comparable efficiency and accuracy utilizing much smaller number of monitoring locations as compared to a more ideal scenario where concentration measurements from a very large number of widespread monitoring wells are available. The proposed methodology is potentially useful for efficient characterization of unknown contaminant sources in a complex contaminated aquifer site, where very little initial concentration measurement data are available. The illustrative application of the methodology to a real-life contaminated aquifer site demonstrates the capability and efficiency of the proposed methodology.

Taylor Dispersion of Contaminants by Dual-peak Spectral Random Waves

Recent extensive and important studies have provided detailed information and compelling evidence on how the presence of waves influences the vertical diffusivity/dispersivity in the coastal environment, which can affect various water quality considerations such as the distribution of suspended sediments in the water column as well as the potential of eutrophication. Comparatively, how the presence of waves influences the horizontal diffusivity/dispersivity has received only scant attention in the literature. Our previous works investigated the role played by the Taylor mechanism due to the wave-induced drift profile which leads to the longitudinal dispersion of contaminants in the horizontal direction, under regular sinusoidal waves and random waves with single-peak spectra. Natural waves in the coastal environment, however, often possess dual-peak spectra, comprising both higher frequency wind waves and lower frequency swells. In this study, the Taylor dispersion of contaminants under random waves with dual-peak spectra is examined through analytical derivation and numerical calculations. The effects of various dual-peak spectral parameters on the horizontal dispersion, including the proportion of lower frequency energy, peak frequency ratio and spectral shape parameter, are investigated. The results show that the relative energy distribution between the dual peaks has the most significant effect. Compared with single-peak spectra with equivalent energy, the Taylor dispersion with dual-peak spectra is stronger when the lower frequency is close to the peak frequency of the single-peak spectrum, and weaker with the higher frequency instead. Thus, it can be concluded that with a dual-peak wave spectrum, wind-dominated seas with higher frequency lead to stronger dispersion in the horizontal direction than swell-dominated seas with lower frequency.

Paenibacillus Infection in a Pediatric Patient on ECMO

Paenibacillus is a spore forming gram positive rod that is usually found in the environment. We describe a case of a patient who contracted this organism having never left the hospital after birth. This neonate contracted Paenibacillus septic shock requiring support with venous-arterial extracorporeal membrane oxygenation (ECMO) while still admitted to the hospital after birth. This patient initially presented in severe septic shock due to a Streptococcus agalactiae infection requiring hemodynamic support with ECMO. Following treatment for the Streptococcus agalactiae infection, and while still on ECMO support, the blood culture became positive for Paenibacillus. Given that our patient had never left the hospital after birth, the finding of this organism in the blood is unique. The primary defense against this bacterium is usually the skin. The only invasive procedure this patient had was ECMO cannulation which is done in a sterile fashion. Most species are susceptible to vancomycin, clindamycin, fluoroquinolones, aminoglycosides, carbapenems, penicillin, and cephalosporins. This patient was treated with penicillin G for 14 days for the S. agalactiae infection prior to the blood culture being positive for Paenibacillus. More than a hundred species had been identified in the genus Paenibacillus, however, few found to cause human infection. This case is unique as it is the first pediatric case with a Paenibacillus infection and the first pediatric case where this organism which rarely causes human infection was found in the blood culture of a patient on ECMO.

Generalized Multiscale RBF Networks and the DCT for Breast Cancer Detection
