Late complications of pancreas transplant

To summarize the long-term complications after pancreas transplantation that affect graft function,a literature search was carried out on the long-term complications of pancreatic transplantation,namely,complications from postoperative 3rd mo onwards,in terms of loss of graft function,late infection and vascular complications as *** most relevant reviews and studies were selected to obtain the current evidence on these *** definition of graft failure varies among different studies,so it is difficult to evaluate,a standardized definition is of utmost importance to know the magnitude of the problem in all worldwide *** rejection is the main cause of long-term graft failure,occurring in 10%of *** the 3rd mo of transplantation onwards,the main risk factor for late infections is immunosuppression,and patients have opportunistic infections like:Cytomegalovirus,hepatitis B and C viruses,Epstein-Barr virus and varicella-zoster virus;opportunistic bacteria,reactivation of latent infections as tuberculosis or fungal *** preoperative studies and serological tests should be made in all recipients to avoid these infections,adding perioperative prophylactic treatments when *** are uncommon,but one of the main causes of late bleeding,which can be *** treatment should be performed with radiological endovascular approaches or open surgery in case of *** all therapeutic options for the complications mentioned above,transplantectomy is a necessary option in approximately 50%of relaparotomies,especially in lifethreatening *** complications in pancreatic transplantation threatens long-term graft *** exhaustive follow-up as well as a correct immunosuppression protocol are necessary for prevention.

We describe precise equivalences between theoretical descriptions of: (i) size-rank and first-digit laws for numerical data sets, (ii) intermittency at the transition to chaos in nonlinear maps, and (iii) cluster fluctuations at criticality. The equivalences stem from a common statistical-mechanical structure that departs from the usual via a one-parameter deformation of the exponential and logarithmic functions. The generalized structure arises when configurational phase space is incompletely visited such that the accessible fraction has fractal properties. Thermodynamically, the common focal expression is an (incomplete) Legendre transform between two entropy (or Massieu) potentials. The theory is in quantitative agreement with real size-rank data and it naturally includes the bends or tails observed for small and large rank.

From conventional two-stage hepatectomy to ALPPS:Fifteen years of experience in a hepatobiliary surgery unit

Background:Hepatectomy in patients with large tumor load may result in postoperative liver failure and associated complications due to excessive liver parenchyma *** two-stage hepatectomy(TSH)and associating liver partition and portal vein ligation for staged hepatectomy(ALPPS)technique are possible solutions to this *** liver metastases(CRLM)is the most frequent indication,and there is a need to assess outcomes for both techniques to improve surgical and long-term oncological outcomes in these ***:A single-center retrospective study was designed to compare TSH with ALPPS in patients with initially unresectable bilateral liver tumors between January 2005 and January *** was performed from January 2012 onwards as the technique of ***-term overall survival(OS)and disease-free survival(DFS)were evaluated as primary outcome in CRLM *** morbidity,mortality and liver growth in all patients were also ***:A total of 38 staged hepatectomies were performed:17 TSH and 21 *** resection rate was 76.5%(n=13)in the TSH group and 85.7%(n=18)in the ALPPS group(P=0.426).Overall major morbidity(Clavien-Dindo≥3 a)(stage 1+stage 2)was 41.2%(n=7)in TSH and 33.3%(n=7)in ALPPS patients(P=0.389),and perioperative 90-day mortalities were 11.8%(n=2)vs.19.0%(n=4)in each group,respectively(P=0.654).Intention-to-treat OS rates at 1 and 5 years in CRLM patients for TSH(n=15)were 80%and 33%,and for ALPPS(n=17)76%and 35%,*** rates at 1 and 5 years were 36%and 27%in the TSH group vs.33%and 27%in the ALPPS group,***:ALPPS is an effective alternative to TSH in bilateral affecting liver tumors,allowing higher resection rate,but patients must be carefully *** CRLM patients similar long-term OS and DFS can be achieved with both techniques.

Assessment of metals in dry-toilet collected matters from suburban areas of Ulaanbaatar, Mongolia, using biosolids quality guidelines and potential ecological risk index

Dry-toilet collected matter （DCM） from traditional dry-toilet pits are a potential health and ecological risk in suburban areas. In this study, the characteristics of metals in DCMs from suburban areas of Ulaanbaatar were surveyed. The results indicate that DCMs contain a high percentage of organic matter and nutrients, while heavy metals are at low levels, which shows good agricultural potential. The concentration ranges of Cr, Cu, Hg, Ni, Pb, and Zn were 11±5, 46±9, 0.08±0.05, 9±3, 17±9, and 338±86mg·kg^-1, respectively. The concentration of Cd was below 0.5 ***, and a high positive relation was shown between chromium and nickel concentrations. The heavy metals in DCMs were safe for land application but Zn in DCMs was close to the effects range median （ERM）, which is toxic in some cases, such as amphipod bioassays. Because it is mandatory to treat DCMs to reduce pathogens, in the case of heavy metal enrichment and agricultural reuse, composting or pyrolysis are better choices than incinera- tion. Compared with global soil background values, the heavy metals in DCMs showed a low level of ecological risk, but a medium level when compared with Mongolian soil background values. The ecological risk of six heavy metals was in the descending order Hg 〉 Cu 〉 Zn 〉 Pb 〉 Ni 〉 Cr and the contribution rate of Hg exceeded 60%.

Beyond Earth,electrical discharges have also been observed,or postulated to exist,on other bodies of the Solar *** this work the state of the art of research on electrical discharges in the atmosphere of several planets and one moon is *** main charging mechanisms as well as the discharge characteristics and its effect on the atmosphere are *** of the current research activities being carried out by the author’s team are also *** lab simulations can achieve realistic plasma parameters;for example:the arc temperature in the lab was found to be 33 000 K which compares favourably with field observations reporting temperatures～30 *** lab work with a synthetic Mars atmosphere allow us to predict that the conjunction of electrostatic discharges and the presence of water ice could lead to the production of methane with an efficiency of up to 1021molecules per joule of applied energy.

Objective:This study was designed to investigate the wound healing activity of aqueous extracts of Ullucus tuberosus(***) using in vitro ***:Lyophilized pulp and acetone extracts of *** were produced using ultrasound *** capacity for collagenase activation was evaluated using fluorescence detection of the enzymatic ***,the influence of *** extracts on cell proliferation,cell migration and synthesis of the extracellular matrix(ECM)proteins,metalloproteinase(MMP-1) and pro-collagen was analyzed using human dermal fibroblasts in ***:An increase in collagenase activity of 12% supports the utility of *** as an agent for scar *** addition,the extracts showed an increase in the proliferation and migration of human dermal fibroblasts and the production of pro-collagen and MMP-1 after treatment with *** *** increase in proliferation,migration and pro-collagen levels positively influenced the regeneration of scarless tissue during the proliferation phase,whereas the increase in MMP-1 may have favored the wound healing process during the remodeling and cellular differentiation ***:The results of this study show for first time that *** is a promising candidate to support scarless tissue regeneration.

Post-translational modifications of prostaglandin-endoperoxide synthase 2 in colorectal cancer:An update

The biosynthesis of prostanoids is involved in both physiological and pathological processes. The expression of prostaglandin-endoperoxide synthase 2(PTGS2; also known as COX-2) has been traditionally associated to the onset of several pathologies, from inflammation to cardiovascular, gastrointestinal and oncologic events. For this reason, the search of selective PTGS2 inhibitors has been a focus for therapeutic interventions. In addition to the classic non-steroidal anti-inflammatory drugs, selective and specific PTGS2 inhibitors, termed coxibs, have been generated and widely used. PTGS2 activity is less restrictive in terms of substrate specificity than the homeostatic counterpart PTGS1, and it accounts for the elevated prostanoid synthesis that accompanies several pathologies. The main regulation of PTGS2 occurs at the transcription level. In addition to this, the stability of the mRNA is finely regulated through the interaction with several cytoplasmic elements, ranging from specificmicroR NAs to proteins that control mR NA degradation. Moreover, the protein has been recognized to be the substrate for several post-translational modifications that affect both the enzyme activity and the targeting for degradation via proteasomal and non-proteasomal mechanisms. Among these modifications, phosphorylation, glycosylation and covalent modifications by reactive lipidic intermediates and by free radicals associated to the proinflammatory condition appear to be the main changes. Identification of these post-translational modifications is relevant to better understand the role of PTGS2 in several pathologies and to establish a correct analysis of the potential function of this protein in diseases progress. Finally, these modifications can be used as biomarkers to establish correlations with other parameters, including the immunomodulation dependent on molecular pathological epidemiology determinants, which may provide a better frame for potential therapeutic interventions.

Mechanism and Kinetic of Free Radical Reactions for Propane Using theoretical Calculations

Quantum calculation method has been used to understand and investigate the free radical reactions of propane with hydroxyl radical in vacuum through modem quantum mechanics that is package on hyperchem 8.02 program. Optimized structures and structural reactivates have been studied through bond stability and angles using DFT calculation based on the basis set 6-31G＊. Energetic properties have been calculated like total energy, Gibbs free energy, entropy, heat of formation, and rate constant for all chemical species that＇s participate in the suggested reaction mechanism. Reaction mechanism and rate determining step had been suggested according to calculation of energy barrier values, and compares between the suggested competitive reactions for each probable reaction step. Suggested structures and the probable transition states have been studied.
