Clinicopathological significance and immunotherapeutic outcome of claudin 18.2 expression in advanced gastric cancer:A retrospective study

Objective: Immunotherapeutic outcomes and clinical characteristics of claudin 18 isoform 2 positive(CLDN18.2-positive) gastric cancer(GC) vary in different clinical studies, making it difficult to optimize antiCLDN18.2 therapy. We conducted a retrospective analysis to explore the association of CLDN18.2 expression with clinicopathological characteristics and immunotherapeutic outcomes in ***: A total of 536 advanced GC patients from 2019 to 2021 in the CT041-CG4006 and CT041-ST-01clinical trials were included in the analysis. CLDN18.2 expression on ≥40% of tumor cells(2+, 40%) and CLDN18.2 expression on ≥70% of tumor cells(2+, 70%) were considered the two levels of positively expressed GC. The clinicopathological characteristics and immunotherapy outcomes of GC patients were analyzed according to CLDN18.2 expression ***: CLDN18.2 was expressed in 57.6%(cut-off: 2+, 40%) and 48.9%(cut-off: 2+, 70%) of *** death-ligand 1(PD-L1) and CLDN18.2 were co-expressed in 19.8% [combined positive score(CPS)≥1, CLDN18.2(cut-off: 2+, 40%)] and 17.2% [CPS≥5, CLDN18.2(cut-off: 2+, 70%)] of ***18.2 expression positively correlated with younger age, female sex, non-gastroesophageal junction(nonGEJ), and diffuse phenotype(Pliver metastasis(P<0.001)was less common in CLDN18.2-positive GC. Overall survival and immunotherapy-related progression-free survival(ir PFS) were inferior in the CLDN18.2-positive ***: CLDN18.2-positive GC is associated with poor prognosis and worse immunotherapeutic outcomes. The combination of anti-CLDN18.2 therapy, anti-PD-L1/PD-1 therapy, and chemotherapy for GC requires further investigation.

目的探讨咽峡炎链球菌(SA)致脓胸的临床特点。方法回顾性分析2016年1月至2023年9月西安医学院第一附属医院收治的9例SA致脓胸患者资料,总结其临床特征。结果9例患者中男8例,女1例,年龄(60.8±12.8)岁,均有基础疾病,同时伴有发热、咳痰、胸痛、呼吸困难,发病时间(6.1±2.2)d,住院时间(19.33±7.48)d。9例患者均表现为血清WBC、C反应蛋白(CRP)、降钙素原(PCT)、血浆D-二聚体(D-D)、胸腔积液乳酸脱氢酶(LDH)升高,血清白蛋白(ALB)和胸腔积液葡萄糖(GLU)下降,大部分患者胸腔积液腺苷脱氨酶(ADA)升高,4例患者出现呼吸衰竭。药敏试验提示5例患者对红霉素和克林霉素耐药,1例患者对头孢曲松耐药。9例患者经抗感染、胸腔闭式引流术、营养支持等对症治疗后好转。结论SA致脓胸患者好发于伴基础疾病的老年人,潜伏期短,住院时间长,感染指标高,存在营养不良,对红霉素和克林霉素耐药率高。

Treatment of solid tumors with chimeric antigen receptor-engineered T cells: current status and future prospects

Chimeric antigen receptors(CARs) are artificial recombinant receptors that generally combine the antigen-recognition domain of a monoclonal antibody with T cell activation domains. Recent years have seen great success in clinical trials employing CD19-specific CAR-T cell therapy for B cell leukemia. Nevertheless, solid tumors remain a major challenge for CAR-T cell therapy. This review summarizes the preclinical and clinical studies on the treatment of solid tumors with CAR-T cells. The major hurdles for the success of CAR-T and the novel strategies to address these hurdles have also been described and discussed.

Natural killer cells in hepatocellular carcinoma： current status and perspectives for future immunotherapeutic approaches

Hepatocellular carcinoma （HCC） is currently the fifth most common malignancy and the third leading cause of cancer-related mortalities worldwide. In the last few years, treatments for HCC have significantly improved from a mere surgical resection to a series of minimally invasive therapies and targeted drugs. However, recurrence frequently occurs even upon curative therapeutics, and drug therapies generally produce disappointing results, with the overall prognosis dismal. This challenging clinical scenario warrants new effective and life-prolonging strategies for patients with HCC. Compelling evidence suggests that NK cells play a critical role in the immune function of the fiver and in the immune defenses against HCC, indicating that HCC might be an ideal target for NK cell-based immunotherapies. To obtain comprehensive insights into the putative influence of NK cells on HCC, this paper summarizes current knowledge on NK cells in HCC and discusses the usefulness and prospects of NK cell-based immunotherapies. Critical issues that require consideration for the successful clinical translation of NK cell-based therapies are also addressed. If appropriately used and further optimized, NK cell- based therapies could dominate important roles in the future immunotherapeutic market of HCC.

Adaptive control method for nonlinear time-delay processes

Two complex properties, varying time-delay and block-oriented nonlinearity, are very common in chemical engineering processes and not easy to be controlled by routine control methods. Aimed at these two complex properties, a novel adaptive control algorithm the basis of nonlinear OFS （orthonormal functional series） model is proposed. First, the hybrid model which combines OFS and Volterra series is introduced. Then, a stable state feedback strategy is used to construct a nonlinear adaptive control algorithm that can guarantee the closed-loop stability and can track the set point curve without steady-state errors. Finally, control simulations and experiments on a nonlinear process with varying time-delay are presented. A number of experimental results validate the efficiency and superiority of this algorithm.

Dear Editor,Available evidence regarding the most suitable treatment strategies for hepatocellular carcinoma(HCC)with inferior vena cava tumor thrombus(IVCTT)is extremely limited,and the median overall survival time for these patients after liver resection is only 17.76 months[1].Other local or systemic treatments for HCC with IVCTT result in a median overall survival time ranging from 5.88 to 15.36 months[1–3].Thus,new therapeutic strategies are urgently needed to improve the survival of HCC patients with *** antigen receptor(CAR)T-cell therapy has seen success in treating B-cell neoplasms with impressive outcomes[4].However,this therapy alone has shown limited efficacy on solid tumors,such as HCC[5].

Growth suppression of colorectal cancer expressing S492R EGFR by monoclonal antibody CH12

Colorectal cancer (CRC) is a common malignant tumor in the digestive tract, and 30%—85% of CRCs express epidermal growth factor receptors (EGFRs). Recently, treatments using cetuximab, also named C225, an anti-EGFR monoclonal antibody, for CRC have been demonstrated to cause an S492R mutation in EGFR. However, little is known about the biological function of S492R EGFR. Therefore, we attempted to elucidate its biological function in CRC cells and explore new treatment strategies for this mutant form. Our study indicated that EGFR and S492R EGFR accelerate the growth of CRC cells in vitro and in vivo and monoclonal antibody CH12, which specifically recognizes an EGFR tumor-specific epitope, can bind efficiently to S492R EGFR. Furthermore, mAb CH12 showed significantly stronger growth suppression activities and induced a more potent antibody-dependent cellular cytotoxicity effect on CRC cells bearing S492R EGFR than mAb *** CH12 obviously suppressed the growth of CRC xenografts with S492R EGFR mutations in vivo. Thus, mAb CH12 may be a promising therapeutic agent in treating patients with CRC bearing an S492R EGFR mutation.

Dear Editor, Pancreatic cancer is a devastating disease ranked as the 4th leading cause of cancer-related deaths in the United States, and its incidence rate is increasing according to the latest statistics. The overall survival rates for patients with pan- creatic cancer have not significantly improved over the past thirty years （Siegel et al., 2012; Simard et al., 2012）. One of the reasons for the high mortality rates is the high resistance of pancreatic cancer to chemotherapy and radiation. Most patients are diagnosed at late stages of the disease. Approximately 15%-20% of patients diagnosed with pan- creatic cancer are eligible for surgical resection, and 85% of these patients eventually experience relapse and ultimately cancer-related death （Siegel et al., 2012）. In recent years, increasing evidence indicates that the fibro-inflammatory stroma is a source of cellular and molecular components contributing to tumor progression and metastasis （Feig et al., 2012; Waghray et al., 2013）. Importantly, increased levels of stroma are positively related to a poor prognosis （Erkan et al., 2008）. Despite the broader understanding of pancre- atic cancer biology, gemcitabine, a chemotherapeutic approved for pancreatic cancer treatment approximately twenty years ago, still remains the standard of care （Burris et al., 1997）. Thus, the development of novel treatment strategies for this devastating disease is urgently needed.
