Endoscopic methods are widely used in the diagnosis and palliative treatment of pancreatic cancer. The most sensitive method in early diagnosis is endosonography (EUS) which can also provide histological diagnosis. Diagnostic ERCP became a rather rare procedure as a consequence of wide availability of Magnetic Resonance Cholangiopancreatography (MRCP) but ERCP assisted intraductal methods have gained importance (brush-cytology, intraductal ultrasound, optical coherence tomography) and finally, peroral pancreatoscopy has become technically feasible but available only in some specialized centers. Minimally invasive endoscopic methods play an important role in the palliative treatment of unresectable pancreatic cancer which represents the majority of cases. EUS-guided histological confirmation of adenocarcinoma is crucial in the election of chemotherapy. Celiac plexus blockade and endoscopic biliary and pancreatic stent placement contribute to pain reduction, drainage of obstructed bile duct and assure a better quality of life.

AIM: To evaluate the role of microRnA (miR)-146a, -155 and -122 in the duodenal mucosa of pediatric patients with Crohn’s disease (CD) and the effect of transforming growth factor-β (TGF-β) on these miRs in duodenal epithelial and fibroblast ***: Formalin-fixed, paraffin-embedded biopsies derived from the macroscopically inflamed (CD inflamed: n = 10) and intact (CD intact: n = 10) duodenal mucosa of pediatric CD patients and control children (C: n = 10) were examined. Expression of miR-146a, -155 and -122 was determined by real-time polymerase-chain reaction (PCR). The expression of the above miRs was investigated in recombinant human TGF-β (1 nmol/L, 24 h) or vehicle treated small intestinal epithelial cells (CCL-241) and primary duodenal fibroblast cells derived from healthy children as ***: Expression of miR-146a was significantly higher in the inflamed duodenal mucosa compared to the intact duodenal mucosa of children with CD (CD inflamed: 3.21 ± 0.50 vs CD intact: 0.62 ± 0.26, P ≤ 0.01) and to the control group (CD inflamed: 3.21 ± 0.50 vs C: 1.00 ± 0.33, P ≤ 0.05). The expression of miR-155 was significantly increased in the inflamed region of the duodenum compared to the control group (CD inflamed: 4.87 ± 1.02 vs Control: 1.00 ± 0.40, P ≤ 0.001). The expression of miR-122 was unchanged in the inflamed or intact mucosa of CD patients compared to controls. TGF-β treatment significantly decreased the expression of miR-155 in small intestinal epithelial cells (TGF-β: 0.7 ± 0.083 vs Control: 1 ± 0.09, P ≤ 0.05) and also the expression of miR-146a (TGF-β: 0.67 ± 0.04 vs Control: 1 ± 0.15, P ≤ 0.01) and miR-155 (TGF-β: 0.72 ± 0.09 vs Control: 1 ± 0.06, P ≤ 0.05) in primary duodenal fibroblasts compared to corresponding vehicle treated control

BACKGROUnD Despite the improvement in the endoscopic hemostasis of non-variceal upper gastrointestinal bleeding(nVUGIB),rebleeding remains a major *** To assess the role of prophylactic transcatheter arterial embolization(PTAE)added to successful hemostatic treatment among nVUGIB *** We searched three databases from inception through October 19th,*** controlled trials(RCTs)and observational cohort studies were *** compared patients with nVUGIB receiving PTAE to those who did not get *** outcomes were rebleeding,mortality,reintervention,need for surgery and transfusion,length of hospital(LOH),and intensive care unit(ICU)*** the quantitative synthesis,odds ratios(ORs)and weighted mean differences(WMDs)were calculated with the random-effects model and interpreted with 95%confidence intervals(CIs).RESULTS We included a total of 3 RCTs and 9 observational studies with a total of 1329 patients,with 486 in the intervention *** was associated with lower odds of rebleeding(OR=0.48,95%CI:0.29–0.78).There was no difference in the 30-d mortality rates(OR=0.82,95%CI:0.39–1.72)between the PTAE and control *** who underwent PTAE treatment had a lower chance for reintervention(OR=0.48,95%CI:0.31–0.76)or rescue surgery(OR=0.35,95%CI:0.14–0.92).The LOH and ICU stay was shorter in the PTAE group,but the difference was non-significant[WMD=-3.77,95%CI:(-8.00)–0.45;WMD=-1.33,95%CI:(-2.84)–0.18,respectively].COnCLUSIOn PTAE is associated with lower odds of rebleeding and any reintervention in ***,further RCTs are needed to have a higher level of evidence.

AIM:To estimate the prevalence of blindness and visual impairment resulting from cataract in the population aged≥50 y in Hungary,and to assess the cataract surgical ***:A rapid assessment of avoidable blindness(RAAB)was conducted.A total of 3523 eligible people were randomly selected and *** participant underwent surgery for cataract was interviewed with regard to the year,place,and costs of the *** with obvious cataract were asked why they had not yet undergone surgery(barriers to surgery).RESULTS:An estimated 12514 people were bilaterally blind;the visual acuity(VA)in 19293 people wasnd the VA in 73962 people wasn the better eye due to *** estimated 77933 eyes are blind;98067 eyes had a VA ofnd an estimated 277493 eyes had a VA ofnducted in government *** age-and sexadjusted cataract surgical coverage with VAn eyes was 90.0%.The rate of good visual outcome after surgery was 79.5%.Ocular comorbidity was the main cause of poor outcome(78.1%),followed by late complications(such as posterior capsule opacification)(17.2%),inadequate optical correction(3.1%),and surgical complications(1.6%).The main barrier to surgery in people with bilateral cataract and VA ofneed not felt’.COnCLUSIOn:The prevalence of visual impairment resulting from cataract is slightly higher than *** quality of the cataract surgical service seems adequate in ***,the number of cataract operations per year should continue to increase due to the increasing patient demands and the aging population.

Parkinson’s disease is a neurological disease which is incurable according to current clinical knowledge. Therefore, early detection and provision of appropriate treatment are of primary importance. Speech is one of the biomarkers that enable the detection of Parkinson’s disease affection. numerous researches are based on recordings from controlled environments;nonetheless fewer apply real circumstances. In the present study, three objectives were examined: recording fragmentation (paragraph, sentences, time-based), variable encodings (Pulse-Code Modulation [PCM], GSM-Full Rate [FR], G.723.1) and majority voting on 8 kHz records using multiple classifiers. Support Vector Machine (SVM), Long Short-Term Memory (LSTM), i-vector and x-vector classifiers were evaluated in contrast with SVM as baseline. The highest results in accuracy and F1-score were achieved using i-vector models. Although variable encodings generally caused decrease in Parkinson-disease recognition, decline was within 2% - 3% at best. Moreover, fragmentation did not yield a clear outcome though some classifiers performed with the very similar efficiency along the differently fragmented sets. Majority voting did produce a slight increase in classification performance compared to as if no aggregation is used.

A growing world population coupled with changing diet is projected to increase demand for food production by 70% by 2050. Helping farmers lose less of their crops will be a key factor in promoting food security. In addition to pesticides GM crops will continue to be a vital tool in the diverse range of technologies that can maintain and improve living standards for the people of the world. The EU is dependent for 65% on imports of protein-rich feedstuffs for which there are no substitutes in the short term. The EU livestock sector uses imported soybean, soybean meal and maize by-products as animal feed. Without an adequate supply of these feed ingredients, the EU’s livestock production will loose competitiveness. However, demands from the EU differ to those from third countries with respect to the GM varieties grown, and what are authorised for import into the EU. The risk that supplies of soya products and maize by-products could be affected by the low-level presence of non-EU approved GM material has not been fully resolved as the EU has allowed just a 0.1% tolerance for this plus 0.05% tolerance for measuring uncertainty. This could cause supply problems for the animal feed industry, and ultimately supply of food to consumers. The importance of the EU market and EU requirements for the major soybean and maize exporter countries is declining over time and it is becoming increasingly difficult and costly to maintain a non-GM supply chain in the EU. Food companies and supermarkets will struggle to stay GM-free. EU member states dependent on imports will be forced to deal with more GM presence in their chain. Market forces are forcing governments to authorize products more efficiently, develop tolerance policies or tolerate unapproved varieties in their imports.

Black locust （Robinia pseudoacacia L.） is one of the most important stand-forming tree species in Hungary and its impor- tance is increasing in many countries. The main aim of the discussed new selection programme is to identify black locust clones with good performance and good form for setting up clonal seed orchards. As a result of selection programme 16 new black locust clones have been improved. In spring 2002 a black locust seed orchard was established with the newly selected clones. About 40% of the plants can be considered to belong to the height growth rate class 1 and 2. Hungary was the first country where micropropagated black locust planting material was used for seed orchard establishment.

Porous Si3n4-SiO2-based ceramics with different porosity were prepared via free sintering of Si3n4 on air with an addition of semolina (5, 10 and 20 wt%) as a pore-forming agent. The semolina content in the starting powder controlled the volume fraction of pores in the sintered body. Small pores (5 μm) formed a continuous network in the whole volume of the ceramic material, while the large pores (~100 μm), formed from the added semolina were mostly isolated in the ceramic matrix. Mercury porosimetry and strength measurements have shown that specific surface area, volume density and compressive strength decreased with the amount of semolina in the samples. Mechanical properties similar to bone were obtained for the sample with 20 wt% semolina pore forming agent (compressive strength 350 MPa, density 2.17 ***-3). The prepared Si3n4-SiO2-based ceramics were evaluated for cytotoxic and genotoxic potential on human fibroblast VH10 and B-HnF-1 cells. Biological tests have shown that both these human fibroblast cell lines were sensitive to the samples with lower porosity and cell growth inhibition was observed in the range 14.9% - 21.3%. The cytotoxicity of the sample with the highest porosity (~40%) was not significant (10%). The microscopic observations have shown that VH10 and B-HnF-1 cells growing around the silicon nitride ceramic discs were homogeneously distributed on the cultivation surface. no significant morphologic changes were found in treated cells, their morphology was very similar to that of the control cells. none of the tested Si3n4-based ceramic samples induced necrotic/apoptotic death of human fibroblasts. Sample S-20 had similar properties to bones and was characterized by very good biocompatibility, slight cytotoxicity and none genotoxicity. Therefore, Si3n4-SiO2-based ceramics prepared by free sintering on air are potential biomaterials for medical applications.

Tilted-pulse-front-pumping(TPFP) lithium-niobate terahertz(THz) pulse sources are widely used in pump-probe and control experiments since they can generate broadband THz pulses with tens of microjoules of energy. However, the conventional TPFP setup suffers from limitations, hindering the generation of THz pulses with peak electric field strength over 1 MV/***, a few setups were suggested to mitigate or even eliminate these limitations. In this paper, we shortly review the setups that are suitable for the generation of single-cycle THz pulses with up to a few tens of megavolts/centimeter focused electric field strength. The THz pulses available with the new layouts pave the way for previously unattainable applications that require extremely high electric field strength and pulse energy in the multi-millijoule range.
