Evaluation of the Neurodevelopmental Outcome of Toddlers with Spinal Dysraphism after Surgical Repair Using ASQ-3^™ Scores

Background: Spinal dysraphism represents a wide spectrum of congenital abnormalities of the spine. Myelomeningocele is considered the most common malformation and the most common we saw in our community, with its morbidity problems seen commonly in the postoperative period. ASQ-3TM Scores are the ages and stages questionnaire, third edition, and represent a tool to assess the development progress, especially in toddlers. Objectives: Evaluation of neurodevelopmental outcome among Sudanese toddlers with spinal dysraphism after surgical closure with or without a VP shunt using ASQ-3TM Scores. Methodology: This is a retrospective hospital-based study of 84 patients who underwent myelomeningocele repair at the National Center for Neurological Sciences (NCNS) during the period from 2017 up to 2019. Data were collected through a constructed questionnaire, including ASQ-3TM Scores. Data were processed and analyzed using the Statistical Package for Social Science (SPSS) computer program. Version 25. Results: 84 patients were included in this study;all patients were diagnosed with spinal dysraphism. Out of them, 51 (60.7%) were 2 years old, 33 (39.3%) were 3 years old, 45 (53.6%) were male, 45 (53.6%) of patients mothers attended ANC irregularly, and 54 (64.3%) their mothers didn’t receive folate supplements. 44 (52.3%) of patients underwent MMC repair only, while 40 (47.7%) underwent MMC repair and VP shunt. The commonest postoperative complication was infection, reported in 12 (14.3%) of patients, followed by VP shunt revision in 9 (10.7%) of patients. Neurological assessment showed that the majority of patients need further assessment with a professional, 57 (67.9%) of children don’t walk, run, or climb like other toddlers as their parent’s state;also, half of patients (42, 50%) had medical problems, and 27 (32.1%) of their parent’s state that they do not talk like other toddlers their age. Robotic donor hepatectomy:a niche advancement or the way forward?A perspective from the world’s largest center

The application of minimally invasive liver surgery(MILS)in the field of living donor hepatectomy has been exceedingly slow,and its impact is limited to a handful of centers *** adoption has been primarily hampered by the technical limitations of laparoscopy,namely rigid instrumentation,suboptimal optics,and a seemingly steep learning *** deficiencies are magnified in the donor hepatectomy operation wherein the parenchyma and vasculature must be handled atraumatically to produce a pristine allograft fit for *** safety concerns and medicolegal ramifications are also cited as impediments to MILS in donor *** 2013,our institution embraced a purely laparoscopic approach to living donor left lateral sectionectomy,and it quickly became our default ***,with donor hemi-hepatectomy,we gravitated to the robotic surgical system as our preferred ***,we describe our experience with minimally invasive donor hepatectomy,which we now universally offer to all living *** extensive familiarity with robotic donor hepatectomy will provide the reader with an instructive perspective on the attributes and merits of the robotic *** appropriate collaboration and proctorship,we believe that the robotic platform will actualize a more rapid and widespread adoption than that experienced with the purely laparoscopic technique.

Efficacy and safety of once daily tacrolimus compared to twice daily tacrolimus after liver transplantation

BACKGROUND Once daily tacrolimus regimen was found to exhibits similar bioavailability,safety and efficacy properties compared to twice-daily tacrolimus in kidney transplantation *** To compare the efficacy and safety of once-daily prolonged release tacrolimus compared to twice-daily tacrolimus in liver transplantation *** MEDLINE,EMBASE,CENTRAL databases were searched for clinical trials until December *** outcome measured as the rate of treatment failure indicated by biopsy-proven acute rejection,Serum creatinine,graft loss,or *** reviewers independently selected studies,collected data and assessed risk of *** results are reported as risk ratio with 95%confidence interval(CI)for dichotomous *** Seven studies included with 965 *** the included studies were of moderate quality according to the risk of bias assessment using Cochrane Risk of Bias ***-proven acute rejection was reported in four studies,and pooled analysis of those studies indicated similar rejections in both twice daily and once daily tacrolimus groups(risk ratio:1.06,95%CI:0.84-1.34,n=758,I2=0%)and also we found no significant difference between both groups for renal outcome(serum creatinine;mean difference,0.001 mg/dL,95%CI:-0.042 to 0.043,n=846,I2=18.6%).Similarly,there was similar number of adverse events such as hypertension,headache,back pain,blood related disorders,infections and nausea observed in both *** The analysis findings confirm that both once daily and twice daily tacrolimus formulations are comparable in terms of efficacy and safety outcomes.

BERT-CNN: A Deep Learning Model for Detecting Emotions from Text

Due to the widespread usage of social media in our recent daily lifestyles,sentiment analysis becomes an important field in pattern recognition and Natural Language Processing(NLP).In this field,users’feedback data on a specific issue are evaluated and *** emotions within the text is therefore considered one of the important challenges of the current NLP *** have been widely studied in psychology and behavioral science as they are an integral part of the human *** describe a state of mind of distinct behaviors,feelings,thoughts and *** main objective of this paper is to propose a new model named BERT-CNN to detect emotions from *** model is formed by a combination of the Bidirectional Encoder Representations from Transformer(BERT)and the Convolutional Neural networks(CNN)for textual *** model embraces the BERT to train the word semantic representation language *** to the word context,the semantic vector is dynamically generated and then placed into the CNN to predict the *** of a comparative study proved that the BERT-CNN model overcomes the state-of-art baseline performance produced by different models in the literature using the semeval 2019 task3 dataset and ISEAR *** BERTCNN model achieves an accuracy of 94.7%and an F1-score of 94%for semeval2019 task3 dataset and an accuracy of 75.8%and an F1-score of 76%for ISEAR dataset.

In this new information era,the transfer of data and information has become a very important *** data must be kept secured from unauthorized persons using *** science of cryptography depends not only on complex mathematical models but also on encryption *** acid encryption is a promising model for data *** this paper,we propose an amino acid encryption model with two encryption *** first key is generated randomly using the genetic *** second key is called the protein key which is generated from converting DNA to a protein ***,the protein message and the first key are used in the modified Playfair matrix to generate the cypher *** experimental results show that the proposed model survives against known attacks such as the Brute-force attack and the Ciphertext-only *** addition,the proposed model has been tested over different types of characters including white spaces and special characters,as all the data is encoded to 8-bit *** performance of the proposed model is compared with other models using encryption time and decryption *** model also balances all three principles in the CIA triad.

本研究利用回弹模量来评价酶法制备稳定黏性土的动力响应。根据AASHTO T307标准,对三种经过酶制剂处理的黏性土进行了回弹模量测试,以探讨其在道路建设中的作用。结果表明,与未经过处理的土样相比,经过酶制剂处理的土样回弹模量明显提高,为原来的1.4~4.4倍。由于回弹模量测试的复杂性,进行了回归分析,建立了可靠的相关公式,预测了应力状态下未经处理的和稳定的土样的回弹模量。采用通用模型确定了路基材料在预期场强下的回弹模量。将酶法制备方法应用于样本路段,并通过平板载荷试验对其进行评价。土样的扫描电镜结果分析表明,减小土壤颗粒间的空隙可以改善土样的压实效果。XRD分析结果表明,处理后的土壤结构无明显变化。酶制剂含有43 mg/mL的蛋白质,采用SDS-PAGE(十二烷基硫酸钠聚丙烯酰胺凝胶电泳)技术鉴定主要的蛋白质组分,但干扰物质(表面活性剂)阻碍了分离作用。

Developing Two Different Novel Techniques for Arabic Text Stemming

Stemming is used to produce stem or root of words. The process is vital to different research fields such as text mining, sentiment analysis, and text categorization, etc. Several techniques have been proposed to stemming Arabic text and among them, Khoja and light-10 stemmers are the most widely used. In this paper, we propose and evaluate two different stemming techniques to Arabic that are based on light stemming techniques. The new stemmers are compared to best reported light stemmer, which is light-10. Results and experiments, which were conducted using standard collections, reveal that The proposed stemmers yield 5.13% and 13.1% improvement in retrieval performance over light 10 with 0.369 average precision and 0.397, respectively and the improvement is statistically significant.

Sex Hormones Changes in Blood and Their Effect on Fecundity of African Catfish(Clarias gariepinus Burchell,1822)after Being Injected with Different Doses of Human Chorionic Gonadotropin(HCG)Hormone

This study was conducted to investigate the effect of different doses injection of human chorionic gonadotropin(HCG)hormone on fecundity and serum sex hormones(FSH,LH,estrogen(E2),progesterone(P4),testosterone(T))of African catfish(Clarias gariepinus).African catfish spawners were intermuscularly injected with different doses of HCG(500,1500,3000,6000 IU/kg female),and group is not injected as a control;males were injected at half the female *** results showed that,fish group injected by 6000 IU HCG/kg female had the highest gonadsomatic index,absolute fecundity and relative fecundity,while,the lowest value of absolute fecundity and relative fecundity were recorded with 500 IU HCG/kg *** group injected with the highest amount of HCG(6000 IU/kg female)recorded the lowest value from egg diameter,while the highest egg diameter was observed in 500 IU HCG/kg *** females,the group injected with 6000 IU HCG/kg female reflected the lowest level of FSH and the highest level of LH and the highest level of P4 compared to other *** of T recorded the highest level with 1500 IU HCG/kg *** control group reflected the highest level of FSH and E2,while the control group reflected the lowest level of T and P4 *** males,serum FSH,LH,P4 and E2 in male groups injected with HCG were relatively higher than those recorded in the control *** highest level of T was recorded in treatment injected with the highest dose of HCG and decreased in other treatments until recorded the lowest level of T in the control *** was observed,HCG hormone has successfully and accelerate induced spawning in African catfish(Clarias gariepinus)and increased in reproductive performance with the increase in HCG dosage and as compared to group not injected.
