目的探讨血清封闭蛋白-5（claudin-5）、c-jun氨基末端激酶激活蛋白（JKAP）在帕金森病（PD）患者中的表达水平及临床意义。方法选取2021年1月至2024年7月宝鸡市人民医院神经内科收治的85例PD患者（PD组）。根据Hoehn-Yahr分级将PD患者分为早期PD组（1～2.5级，17例）、中期PD组（3级，41例）、晚期PD组（4～5级，27例）。另选取该院同期体检健康的85名志愿者作为对照组。采用酶联免疫吸附试验检测血清claudin-5、JKAP水平。根据PD认知功能评定量表（PD-CRS）评分将PD患者分为正常认知组（>80分，28例）、PD轻度认知障碍（PD-MCI）组（74～80分，37例）和PD痴呆（PDD）组（<74分，20例）。其中，PD-MCI和PDD都属于PD认知障碍。采用Spearman秩相关分析claudin-5、JKAP水平与PD患者Hoehn-Yahr分级的相关性。采用多因素logistic回归分析PD患者发生认知障碍的影响因素。通过受试者工作特征（ROC）曲线分析claudin-5、JKAP水平对PD患者认知障碍和PDD的诊断效能。结果与对照组比较，PD组claudin-5水平较高，JKAP水平较低，差异有统计学意义（P<0.05）。早期PD组、中期PD组、晚期PD组claudin-5水平依次升高，JKAP水平依次降低，差异有统计学意义（P<0.05）。PD患者Hoehn-Yahr分级与claudin-5水平呈正相关（rs=0.810,P<0.001）,与JKAP水平呈负相关（rs=-0.823,P<0.001）。正常认知组、PD-MCI组、PDD组病程依次延长，claudin-5水平依次升高，JKAP水平依次降低，差异有统计学意义（P<0.05）。多因素logistic回归分析结果显示，较高的claudin-5水平是PD患者发生认知障碍的独立危险因素（P<0.05）,较高的JKAP水平是PD患者发生认知障碍的独立保护因素（P<0.05）。ROC曲线分析结果显示，claudin-5、JKAP水平联合诊断PD患者认知障碍[AUC（95%CI）=0.894（0.808～0.950）]以及联合诊断PDD[AUC（95%CI）=0.901（0.793～0.964）]的效能较单一指标更高（P<0.05）。结论 PD患者血清claudin-5水平升高、JKAP水平降低，与病情加重和认知障碍相关，两项指标联合检测有助于诊断PD患者认知障碍。

The objective is to develop an approach for the determination of the target reliability index for serviceability limit state(SLS) of single piles. This contributes to conducting the SLS reliability-based design(RBD) of piles. Based on a two-parameter,hyperbolic curve-fitting equation describing the load-settlement relation of piles, the SLS model factor is defined. Then, taking into account the uncertainties of load-settlement model, load and bearing capacity of piles, the formula for computing the SLS reliability index(βsls) is obtained using the mean value first order second moment(MVFOSM) method. Meanwhile, the limit state function for conducting the SLS reliability analysis by the Monte Carlo simulation(MCS) method is established. These two methods are finally applied to determine the SLS target reliability index. Herein, the limiting tolerable settlement(slt) is treated as a random variable. For illustration, four load test databases from South Africa are compiled again to conduct reliability analysis and present the recommended target reliability indices. The results indicate that the MVFOSM method overestimates βsls compared to that computed by the MCS method. Besides, both factor of safety(FS) and slt are key factors influencing βsls, so the combination of FS and βsls is welcome to be used for the SLS reliability analysis of piles when slt is determined. For smaller slt, pile types and soils conditions have significant influence on the SLS target reliability indices; for larger slt, slt is the major factor having influence on the SLS target reliability indices. This proves that slt is the most key parameter for the determination of the SLS target reliability index.

The Prostate Imaging Reporting and Data System(PI-RADS)has good ability to identify the nature of lesions on prostate magnetic resonance imaging(MRI).However,some lesions are still reported as PI-RADS 4 and 5 but are biopsy-proven ***,we aimed to summarize the reasons for the negative prostate biopsy of patients who were assessed as PI-RADS 4 and 5 by biparameter *** retrospectively sorted out the prostate MRI,treatment,and follow-up results of patients who underwent a biparameter MRI examination of the prostate in The First Affiliated Hospital of Nanjing Medical University(Nanjing,China)from August 2019 to june 2021 with PI-RADS 4 and 5 but a negative *** focused on reviewing the MRI characteristics.A total of 467 patients underwent transperineal prostate *** them,biopsy pathology of 93 cases were *** follow-up,90 patients were ruled out of prostate *** the 90 cases,40 were considered to be overestimated PI-RADS after review.A total of 22 cases were transition zone(TZ)lesions with regular appearance and clear boundaries,and 3 cases were symmetrical *** 15 cases,the TZ nodules penetrated the peripheral zone(PZ)and were mistaken for the origin of PZ.A total of 17 cases of lesions were difficult to distinguish from prostate *** them,5 cases were granulomatous inflammation(1 case of prostate tuberculosis).A total of 33 cases were ambiguous lesions,whose performance was between PI-RADS 3 and *** summary,the reasons for“false-positive MRI diagnosis”included PI-RADS overestimation,ambiguous images giving higher PI-RADS,diseases that were really difficult to distinguish,and missed lesion in the initial biopsy;and the first two accounted for the most.

Biodegradable poly(propylene carbonate)(PPC)/epoxidized soybean oil(ESO)blends with different component ratios were prepared by melt blending to improve the performance of *** phase morphology,thermal properties,rheological properties and mechanical properties of the blends were in vestigated in *** exami nation revealed good in terfacial adhesi on betwee n PPC matrix and *** to DSC and DMA,as the content of ESO increased,the glass transition temperature of the PPC component increased,indicating that there was a strong in terfacial in teracti on between the PPC matrix and *** in terfacial in teraction may be caused by rin g-opening reaction between the hydroxyl end groups of PPC and the epoxy groups of ESO,which restricted the chain movement of PPC *** disappearanee of the epoxy groups in FTIR indicated that the in terfacial in teracti on betwee n the two phases was due to the rin g-openi ng reactio n betwee n PPC and *** the addition of ESO,the thermal stabilities were *** the increasing ESO content,the modulus gradually ***,the stre ngth at yield,the strength at break and the elon gation at break were in creased for the PPC/ESO blends,suggest!ng that the enhancement of the strength and toughness of PPC was achieved by the incorporation of *** rheological measurement revealed that the complex viscosity,storage modulus and loss modulus of PPC were in creased with the increasi ng ESO content at low freque ncy,which indicated that the addition of ESO enhanced the melt strength of PPC in stead of plasticizi ng PPC.

We evaluated the relationships of body composition and serum adipocytokine levels with progression-free survival(PFS)and overall survival(OS)in metastatic castration-resistant prostate cancer(mCRPC)patients receiving *** medical records of mCRPC patients who received docetaxel between January 2011 and December 2015 at Fudan University Shanghai Cancer Center(Shanghai,China)were *** following body composition parameters were calculated using computed tomography:skeletal muscle index(SMI),visceral adipose tissue index(VATI),and subcutaneous adipose tissue index(SATI).Pretreatment serum adipocytokine levels,including interleukin 6,insulin,leptin,monocyte chemoattractant protein-1,adiponectin,and resistin,were measured using the multiplex bead-based *** regression and Kaplan–Meier methods were used for survival *** the 453 mCRPC patients initially identified,105 were included in the *** VATI group patients had longer PFS(median,10 months vs 7 months,P=0.008)and OS(median,24 months vs 15 months,P=0.017),compared with low VATI group *** and SATI were not significantly associated with PFS or *** the six detected adipocytokines,only leptin was associated with mCRPC *** leptin group patients had shorter PFS(median,7 months vs 12 months,P=0.0018)and OS(median,17 months vs 22 months,P=0.042),compared with low leptin group *** analysis showed that a high VATI was an independent protective factor for PFS and OS,while a high leptin level was an independent risk factor for PFS and ***,VATI and serum leptin levels could provide important information concerning mCRPC prognosis.
