Radioactive 125I seed implantation for pancreatic cancer with unexpected liver metastasis:A preliminary experience with 26 patients

BACKGROUND Preoperative diagnosis rate of pancreatic cancer has increased year by year.The prognosis of pancreatic cancer patients with unexpected liver metastasis found by intraoperative exploration is very poor,and there is no effective and unified treatment strategy.AIM To evaluate the therapeutic effect of radioactive 125I seed implantation for pancreatic cancer patients with unexpected liver metastasis.METHODS The demographics and perioperative outcomes of patients who underwent 125I seed implantation to treat pancreatic cancer with unexpected liver metastasis between January 1,2017 and June 1,2019 were retrospectively analyzed.During the operation,125I seeds were implanted into the pancreatic tumor under the guidance of intraoperative ultrasound,with a spacing of 1.5 cm and a row spacing of 1.5 cm.For patients with obstructive jaundice and digestive tract obstruction,choledochojejunostomy and gastroenterostomy were performed simultaneously.After operation,the patients were divided into a non-chemotherapy group and a chemotherapy group that received gemcitabine combined with albumin-bound paclitaxel treatment.RESULTS Preoperative imaging evaluation of all patients in this study showed that the tumor was resectable without liver metastasis.There were 26 patients in this study,including 18 males and 8 females,aged 60.5±9.7 years.The most common tumor site was the pancreatic head(17,65.4%),followed by the pancreatic neck and body(6,23.2%)and pancreatic tail(3,11.4%).Fourteen patients(53.8%)underwent palliative surgery and postoperative pain relief occurred in 22 patients(84.6%).The estimated blood loss in operation was 148.3±282.1 mL and one patient received blood transfusion.The postoperative hospital stay was 7.6±2.8 d.One patient had biliary fistula,one had pancreatic fistula,and all recovered after conservative treatment.After operation,7 patients received chemotherapy and 19 did not.The 1-year survival rate was significantly higher in patients who received chemotherapy than in those who did not(68.6%vs 15.

With continuous enhancement of gas-turbine inlet temperature and rapid increase of radiant heat transfer,thermal barrier coating(TBC)materials with a combination of low thermal conductivity and good high-temperature thermal radiation shielding performance play vital roles in ensuring the durability of metallic blades.However,yttria-stabilized zirconia(YSZ),as the state-of-the-art TBC and current industry standard,is unable to meet such demands since it is almost translucent to high-temperature thermal radiation.Besides,poor corrosion resistance of YSZ to molten calcia-magnesia-alumina-silicates(CMAS)also impedes its application in sand,dust,or volcanic ash laden environments.In order to improve the hightemperature thermal radiation shielding performance and CMAS resistance of YSZ and further reduce its thermal conductivity,two medium-entropy(ME)oxide ceramics,ME(Y,Ti)_(0.1)(Zr,Hf,Ce)_(0.9)O_(2)and ME(Ta,Ti)_(0.1)(Zr,Hf,Ce)_(0.9)O_(2),were designed and prepared by pressureless sintering of binary powder compacts in this work.ME(Y,Ti)_(0.1)(Zr,Hf,Ce)_(0.9)O_(2)presents cubic structure but a trace amount of secondary phase,while ME(Ta,Ti)_(0.1)(Zr,Hf,Ce)_(0.9)O_(2)displays a combination of tetragonal phase(81.6 wt.%)and cubic phase(18.4 wt.%).Both ME(Y,Ti)_(0.1)(Zr,Hf,Ce)_(0.9)O_(2)and ME(Ta,Ti)_(0.1)(Zr,Hf,Ce)_(0.9)O_(2)possess better high-temperature thermal radiation shielding performance than YSZ.Especially,the high-temperature thermal radiation shielding performance of ME(Ta,Ti)_(0.1)(Zr,Hf,Ce)_(0.9)O_(2)is superior to that of ME(Y,Ti)_(0.1)(Zr,Hf,Ce)_(0.9)O_(2)due to its narrower band gap and correspondingly higher infrared absorbance(above 0.7)at the waveband of 1 to 5μm.The two ME oxides also display significantly lower thermal conductivity than YSZ and close thermal expansion coefficients(TECs)to YSZ and Ni-based superalloys.In addition,the two ME oxides possess excellent CMAS resistance.After attack by molten CMAS at 1250℃for 4 h,merely~2μm thick penetration layer has been formed and the structure below th

Comparison of intravitreal bevacizumab with macular photocoagulation for treatment of diabetic macular edema: a systemic review and Meta-analysis

AIM: To further evaluate the efficacy and safety of intravitreal bevacizumab(IVB) versus macular photocoagulation(MPC) in treatment of diabetic macular edema(DME) by Meta-analysis. METHODS: Pertinent publications were identified through systemic searches of Pub Med, Medline,EMBASE, and the Cochrane Controlled Trials Register up to 30 November, 2013. Changes in central macular thickness(CMT) in μm and best-corrected visual acuity(BCVA) in log MAR equivalents were extracted at 1, 3, 6,12 and 24 mo after initial treatment, and a Meta-analysis was carried out to compare results between groups receiving IVB and MPC.RESULTS: Five randomized controlled trial(RCTs) and one high-quality comparative study were identified and included. Our Meta-analysis revealed that both IVB and MPC resulted in the improvements of CMT and BCVA in eyes with DME at 1mo after initial treatment, with IVB being significantly superior to MPC(P =0.01 and 0.02,respectively). The improvements of both measure outcomes at 3, 6, 12 and 24 mo after treatment did not vary significantly between the IVB groups and MPC groups(CMT at 3mo, P =0.85; at 6mo, P =0.29; at 12 mo,P =0.56; at 24 mo, P =0.71; BCVA at 3mo, P =0.31; at 6mo,P =0.30; at 12 mo, P =0.23; at 24 mo, P =0.52). However,the number of observed adverse events was low in all studies.CONCLUSION: Current evidence shows IVB treatment trends to be more effective in improvements of macular edema and vision in eyes with DME at an earlier follow up(1mo) compared with MPC. At other time, both interventions have comparable efficacy without statistical significances.

Hypomethylation of Thyroid Peroxidase as a Biomarker for Hepatocellular Carcinoma with Tumor Thrombosis

Objective Thyroid hormones(THs)regulate multiple physiological activities in the liver,including cellular metabolism,differentiation,and cell growth,and play important roles in the pathogenesis of hepatocellular carcinoma(HCC).Thyroid peroxidase(TPO)is a key molecule involved in the THs synthesis and signaling pathway.As an epigenetic modification,DNA methylation has a critical role in tumorigenesis with diagnostic potential.However,the connection between THs and DNA methylation has been rarely investigated.Methods The methylation of key TH-related genes was analyzed by in-house epigenome-wide scanning,and we further analyzed the methylation levels of the TPO promotor in 164 sample pairs of HCC and adjacent non-cancerous tissues by Sequenom EpiTYPER assays,and evaluated their clinical implications.Results We identified that the methylation of the TPO promoter was downregulated in the HCC tissues(P<0.0001)with a mean difference ranging from 18.5%to 22.3%.This methylation pattern correlated with several clinical factors,including a multi-satellite tumor,fibrous capsule,and the presence of tumor thrombus.The receiver operator characteristic(ROC)curve analysis further confirmed that the percent methylated reference(PMR)values for TPO were predictive of the tumor[the area under the curve(AUC)ranged from 0.755 to 0.818]and the thrombosis in the HCC patients(the AUC ranged from 0.706 to 0.777).Conclusion These findings demonstrated that epigenetic alterations of TPO,as indicated by the PMR values,were a potential biomarker for HCC patients with tumor thrombosis.

Electronic structure and mechanical properties of layered compound YB_2C_2:A promising precursor for making two dimensional(2D) B_2C_2 nets

Layered compounds play pivotal roles as precursors for producing 2D materials through mechanical exfoliation(micro-mechanical cleavage) or chemical approaches. Therefore, searching for layered compounds with sharp anisotropic chemical bonding and properties becomes emergent. In this work, the stability, electronic structure, elastic properties, and lattice dynamics of YBCwere investigated. Strong anisotropy in elastic properties is revealed, i.e., high Young’s modulus in a-b plane but low Young’s modulus in c direction. The maximum to minimum Young’s modulus ratio is 2.41 and 2.45 for YBCwith P42/mmc and P4/mbm symmetry, respectively. The most likely systems for shear sliding or microdelaminating are(001)[100] and(001)[010]. The anisotropic elastic properties are underpinned by the anisotropic chemical bonding, i.e., strong bonding within the BCnets and weak bonding between Y atom layers and BCnets. YBCis electrically conductive and the contributions to the electrical conductivity are from delocalized Y 4deas well as Bpandpzelectrons. The layered crystal structure, sharp anisotropic mechanical properties, and metallic conductivity endorse YBCpromising as a precursor for new 2D BCnets.

Photocatalytic and photochemical processes of AgCl/TiO_(2) studied with a fully integrated X-ray photoelectron spectrometer

A fully integrated X-ray photoelectron spectrometer(XPS) was employed for the investigation of the separation processes and recombination behaviors of photogenerated electrons and holes on the surface of AgCl and ultra-small nano-titanium oxide cluster composite photocatalyst(AgCl/TiO_(2)).A facile route,by direct colloidal synthesis,for preparing AgCl/TiO_(2)with high stability and enhanced visible light(Vis) driven catalytic activity was reported.The photocatalytic activity of AgCl/TiO_(2),which revealed that the photo-degradation rate of the as-prepared AgCl/TiO_(2)was nearly 10.5 times higher than that of bare AgCl,was evaluated by applying it to the photo-degradation of methyl orange(MO) in water solution.Moreover,AgCl/TiO_(2)exhibited an outstanding long-term stability during ten cycles of photo-degradation.The band gap of AgCl decreased from 3.25 to 2.85 eV because of the ultrasmall nano-TiO_(2)clusters that were pinned to its surface.The results indicate that the band gap narrowing and surface plasmon resonance(SPR) of Ag(0) were two major contributors to the enhancement of the photocatalytic activity of AgCl/TiO_(2)by improving the utilization of Vis.In situ XPS analysis was,therefore,certified as a beneficial method to explore the catalytic mechanism of photocatalysts.

The critical requirements for the environmental barrier coating(EBC)materials of silicon-based ceramic matrix composites(CMCs)include good tolerance to harsh environments,thermal expansion matches with the interlayer mullite,good high-temperature phase stability,and low thermal conductivity.Cuspidine-structured rare-earth aluminates RE_(4)Al_(2)O_(9) have been considered as candidates of EBCs for their superior mechanical and thermal properties,but the phase transition at high temperatures is a notable drawback of these materials.To suppress the phase transition and improve the phase stability,a novel cuspidine-structured rare-earth aluminate solid solution(Nd_(0.2)Sm_(0.2)Eu_(0.2)Y_(0.2)Yb_(0.2))_(4)Al_(2)O_(9) was designed and successfully synthesized inspired by entropy stabilization effect of high-entropy ceramics(HECs).The as-synthesized HE(Nd_(0.2)Sm_(0.2)Eu_(0.2)Y_(0.2)Yb_(0.2))_(4)Al_(2)O_(9) exhibits a close thermal expansion coefficient(6.96×10^(-6) K^(-1) at 300-1473 K)to that of mullite,good phase stability from 300 to 1473 K,and low thermal conductivity(1.50 W·m^(-1)·K^(-1) at room temperature).In addition,strong anisotropic thermal expansion has been observed compared to Y_(4)Al_(2)O_(9) and Yb_(4)Al_(2)O_(9).The mechanism for low thermal conductivity is attributed to the lattice distortion and mass difference of the constituent atoms,and the anisotropic thermal expansion is due to the anisotropic chemical bonding enhanced by the large size rare-earth cations.

Schedule refinement for homogeneous multi-core processors in the presence of manufacturing-caused heterogeneity

Multi-core homogeneous processors have been widely used to deal with computation-intensive embedded applications. However, with the continuous down scaling of CMOS technology, within-die variations in the manufacturing process lead to a significant spread in the operating speeds of cores within homogeneous multi-core processors. Task scheduling approaches, which do not consider such heterogeneity caused by within-die variations,can lead to an overly pessimistic result in terms of performance. To realize an optimal performance according to the actual maximum clock frequencies at which cores can run, we present a heterogeneity-aware schedule refining(HASR) scheme by fully exploiting the heterogeneities of homogeneous multi-core processors in embedded domains.We analyze and show how the actual maximum frequencies of cores are used to guide the scheduling. In the scheme,representative chip operating points are selected and the corresponding optimal schedules are generated as candidate schedules. During the booting of each chip, according to the actual maximum clock frequencies of cores, one of the candidate schedules is bound to the chip to maximize the performance. A set of applications are designed to evaluate the proposed scheme. Experimental results show that the proposed scheme can improve the performance by an average value of 22.2%, compared with the baseline schedule based on the worst case timing analysis. Compared with the conventional task scheduling approach based on the actual maximum clock frequencies, the proposed scheme also improves the performance by up to 12%.

Potential targets and mechanism of Xingxiao Pill for the treatment of lung cancer were analyzed based on network pharmacology and molecular docking

Objective:To analyze the potential targets and mechanism of Xingxiao Pill for the treatment of lung cancer based on network pharmacology and molecular docking,providing reference for the research and clinical development of Xingxiao Pill.Method:The main chemical constituents and their targets of Xingxiao Pill was obtained through BATMAN-TCM database.Effective compounds were screened based on SwissADME and their targets were predicted by SwissTargetPrediction.Main lung cancer targets were obtained and integrated through GeneCards,OMIM(OMIM-Gene-Map-Retrieval),TTD and DRUGBANK databases.The intersection targets of Xingxiao Pill for lung cancer were obtained by Venn.The String platform was used to obtain the intersection targets.The core targets of Xingxiao Pill for lung cancer were screened by BisoGenet3.0.The Metasacpe database was used to analyze the biological processes and pathways involved in intersection targets.Cytoscape3.7.2 software was used to construct the"active ingredients of Xingxiao Pill-targets-pathways"network.Finally,Autodock was used for molecular docking verification.Results:The core active components of Xingxiao Pill for the treatment of lung cancer were morin,testosterone,17-Beta-Estradiol,alphaEstradiol,muscone,commiferin,commisterone,octyl acetate,etc.The primary pathways mainly included PI3K/Akt,MTOR,MAPK signaling pathway and MicroRNAs in cancer,etc.It has been proved that the combination of testosterone and estradiol with AKT1,SIRT7 and MDM2 was stable and could be used as reference.Conclusion:This study preliminarily revealed the mechanism of Xingxiao Pill on treating lung cancer with multiple components,targets and pathways,providing some ideas and references for clinical application and basic research in the future.

Association between 5α-reductase inhibitors therapy and incidence, cancer-specific mortality,and progression of prostate cancer: evidence from a meta-analysis

5a-reductase inhibitors(5-ARI)are widely employed for the treatment of benign prostatic hyperplasia.It has been noted that 5-ARI exhibit the potential to attenuate the risk of prostate cancer,but consistent agreement has not been achieved.Moreover,the effect of 5-ARI on cancer-specific mortality and progression of prostate cancer remains unclear.Therefore,the goal of the current meta-analysis was to elucidate the impact of 5-ARI on the incidence and progression of prostate cancer.We searched for all studies assessing the effect of 5-ARI on risk of prostate cancer in PubMed,Embase,Medline,and Cochrane Library databases.Pooled relative risk(RR)and corresponding 95%confidence intervals(CIs)were accepted to evaluate the association between 5-ARI and the risk of prostate cancer.Synthetic results implied that subjects who accepted 5-ARI compared with the placebo group experienced a distinctly weakened overall incidence of prostate cancer(RR=0.74;95%C l:0.66-0.82;P7;RR=1.19;95%Cl:0.98-1.43;P=0.069).The results also showed that 5-ARI treatment did not significantly alter prostate cancer-specific mortality(RR=1.0;95%C l:0.95-1.05;P=0.916).In addition,it was worth noting that 5-ARI treatment acted in a protective role that presented a dramatic benefit to delay the progression of low-risk tumors(RR=0.58;95%C l:0.43-0.78;P<0.001).
