Effectiveness of Histopathological Examination of Ultrasound-guided Puncture Biopsy Samples for Diagnosis of Extrapulmonary Tuberculosis

Objective To evaluate the diagnostic value of histopathological examination of ultrasound-guided puncture biopsy samples in extrapulmonary tuberculosis(EPTB).Methods This study was conducted at the Shanghai Public Health Clinical Center.A total of 115patients underwent ultrasound-guided puncture biopsy,followed by MGIT 960 culture(culture),smear,Gene Xpert MTB/RIF(Xpert),and histopathological examination.These assays were performed to evaluate their effectiveness in diagnosing EPTB in comparison to two different diagnostic criteria:liquid culture and composite reference standard(CRS).Results When CRS was used as the reference standard,the sensitivity and specificity of culture,smear,Xpert,and histopathological examination were(44.83%,89.29%),(51.72%,89.29%),(70.11%,96.43%),and(85.06%,82.14%),respectively.Based on liquid culture tests,the sensitivity and specificity of smear,Xpert,and pathological examination were(66.67%,72.60%),(83.33%,63.01%),and(92.86%,45.21%),respectively.Histopathological examination showed the highest sensitivity but lowest specificity.Further,we found that the combination of Xpert and histopathological examination showed a sensitivity of 90.80%and a specificity of 89.29%.Conclusion Ultrasound-guided puncture sampling is safe and effective for the diagnosis of EPTB.Compared with culture,smear,and Xpert,histopathological examination showed higher sensitivity but lower specificity.The combination of histopathology with Xpert showed the best performance characteristics.

Constructing a S-scheme heterojunction with tight interface contact and fast charge transfer is beneficial to improving the photocatalytic hydrogen evolution performance.Herein,a unique one-dimensional(1D)/two-dimensional(2D)S-scheme heterojunction containing 1D Sb_(2)S_(3) nanorods and 2D ZnIn_(2)S_(4) with affluent sulfur vacancies(denoted as Sv-ZnIn_(2)S_(4)@Sb_(2)S_(3)) was designed.The introduced sulfur vacancy can promote the effective adsorption of H+for the following interfacial hydrogen-evolution reaction.Furthermore,the larger contact area and stronger electron interaction between Sb_(2)S_(3) and ZnIn_(2)S_(4) effectively inhibits the recombination of photo-generated electron–hole pairs and abridges the migration distance of charges.As a result,the optimal Sv-ZnIn_(2)S_(4)@Sb_(2)S_(3) sample achieves H_(2) evolution activity of 2741.3 mol·h^(−1)·g^(−1),which is 8.6 times that of pristine ZnIn_(2)S_(4) and 3.0 times that of the Sv-ZnIn_(2)S_(4) samples.Based on the experimental result,the photo-reactivity S-scheme mechanism of hydrogen evolution from water splitting with Sv-ZnIn_(2)S_(4)@Sb_(2)S_(3) is proposed.This work provides an effective method for developing S-scheme heterojunction composites of transition metal sulfide with high hydrogen evolution performance.

In this work, we consider a conventional test of gravitational wave(GW) propagation which is based on the phenomenological parameterized dispersion relation to describe potential departures from General Relativity(GR)along the propagation of GWs. But different from tests conventionally performed previously, we vary multiple deformation coefficients simultaneously and employ the principal component analysis(PCA) method to remedy the strong degeneracy among deformation coefficients and obtain informative posteriors. The dominant PCA components can be better measured and constrained, and thus are expected to be more sensitive to potential departures from the waveform model. Using this method we analyze ten selected events and get the result that the combined posteriors of the dominant PCA parameters are consistent with GR within 99.7% credible intervals. The standard deviation of the first dominant PCA parameter is three times smaller than that of the original dispersion parameter of the leading order. However, the multi-parameter test with PCA is more sensitive to not only potential deviations from GR but also systematic errors of waveform models. The difference in results obtained by using different waveform templates hints that the demands of waveform accuracy are higher to perform the multiparameter test with PCA. Whereas, it cannot be strictly proven that the deviation is indeed and only induced by systematic errors. It requires more thorough research in the future to exclude other possible reasons in parameter estimation and data processing.

BACKGROUND Some hydatid cysts of cystic echinococcosis type 1(CE1)lack well-defined cyst walls or distinctive endocysts,making them difficult to differentiate from simple hepatic cysts.AIM To investigate the diagnostic methods for atypical hepatic CE1 and the clinical efficacy of laparoscopic surgeries.METHODS The clinical data of 93 patients who had a history of visiting endemic areas of CE and were diagnosed with cystic liver lesions for the first time at the People's Hospital of Xinjiang Uygur Autonomous Region(China)from January 2018 to September 2023 were retrospectively analyzed.Clinical diagnoses were made based on findings from serum immunoglobulin tests for echinococcosis,routine abdominal ultrasound,high-frequency ultrasound,abdominal computed tomography(CT)scan,and laparoscopy.Subsequent to the treatments,these patients underwent reexaminations at the outpatient clinic until October 2023.The evaluations included the diagnostic precision of diverse examinations,the efficacy of surgical approaches,and the incidence of CE recurrence.RESULTS All 93 patients were diagnosed with simple hepatic cysts by conventional abdominal ultrasound and abdominal CT scan.Among them,16 patients were preoperatively diagnosed with atypical CE1,and 77 were diagnosed with simple hepatic cysts by high-frequency ultrasound.All the 16 patients preoperatively diagnosed with atypical CE1 underwent laparoscopy,of whom 14 patients were intraoperatively confirmed to have CE1,which was consistent with the postoperative pathological diagnosis,one patient was diagnosed with a mesothelial cyst of the liver,and the other was diagnosed with a hepatic cyst combined with local infection.Among the 77 patients who were preoperatively diagnosed with simple hepatic cysts,4 received aspiration sclerotherapy of hepatic cysts,and 19 received laparoscopic fenestration.These patients were intraoperatively diagnosed with simple hepatic cysts.During the followup period,none of the 14 patients with CE1 experienced recurrence or implantation of hydatid sc

Vanadium disulfide(VS_(2)) as a typical two-dimensional transition metal chalcogenide has excellent competitiveness for sodium-ion storage due to its wide layer spacing(0.575 nm),high theoretical capacity of 932mah·g^(-1) originating from multi-electron electrochemical redox.However,continuous sodiation process accompanied by crystal structural evolution and collapse cause rapid capacity decaying.Herein,novel few-layer VS_(2)nanosheets with open(001) crystal planes are in-situ constructed on reduced graphene oxide to solve these issues mentioned above.It indicates that few-layer VS_(2)provides more Na^(+) storage activity due to the low Na^(+)surface migration energy barrier on exposed crystal(001)planes.The flexible and high electronic conductivity of carbon matrix also effectively builds multi-level buffer structure and electron transport kinetics to boost the Na^(+)insertion/conversion reactive activity on VS_(2) as well as Na^(+) pseudocapacitance storage kinetics on edges and defects of nano sheets.Those coupling effects result in high rate capability and long cycling stability as a battery/capacitor anode.It delivers conspicuous high energy density of 81 and 40 Wh·kg^(-1) at power density of 118 and 10,286W·kg^(-1),as well as 80% energy retention rate after 5000cycles,confirming its great application potential in sodiumbased storage devices.

Nonlinear circuits can show multistability when a magnetic flux-dependent memristor(MFDM) or a charge-sensitive memristor(CSM) is incorporated into a one branch circuit,which helps estimate magnetic or electric field effects.In this paper,two different kinds of memristors are incorporated into two branch circuits composed of a capacitor and a nonlinear resistor,thus a memristive circuit with double memristive channels is designed.The circuit equations are presented,and the dynamics in this oscillator with two memristive terms are discussed.Then,the memristive oscillator is converted into a memristive map by applying linear transformation on the sampled time series for the memristive oscillator.The Hamilton energy function for the memristive oscillator is obtained by using the Helmholtz theorem,and it can be mapped from the field energy of the memristive circuit.An energy function for the dual memristive map is suggested by imposing suitable weights on the discrete energy function.The dynamical behaviors of the new memristive map are investigated,and an adaptive law is proposed to regulate the firing mode in the memristive map.This work will provide a theoretical basis and experimental guidance for oscillator-to-map transformation and discrete map energy calculation.

Background:Obesity is a common public health issue and is currently deemed a disease.Research has shown that the risk of gallstones in individuals with obesity is elevated.This study aimed to explore the bile proteomics differences between cholelithiasis patients with obesity and normal body weight.Methods:Bile samples from 20 patients(10 with obesity and 10 with normal body weight)who underwent laparoscopic cholecystectomy at our center were subjected to tandem mass tag labeling(TMT)and liquid chromatography-tandem mass spectrometry(LC-MS/MS),followed by further bioinformatic analysis.Results:Among the differentially expressed proteins,23 were upregulated and 67 were downregulated.Bioinformatic analysis indicated that these differentially expressed proteins were mainly involved in cell development,inflammatory responses,glycerolipid metabolic processes,and protein activation cascades.In addition,the activity of the peroxisome proliferator-activated receptor(PPAR,a subfamily of nuclear receptors)signaling pathway was decreased in the Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analysis.Two downregulated proteins in the PPAR signaling pathway,APO A-I and APO A-II,were confirmed using enzyme-linked immunosorbent assay.Conclusions:The PPAR signaling pathway may play a crucial role in the development of cholelithiasis among patients with obesity.Furthermore,biliary proteomics profiling of gallstones patients with obesity is revealed,providing a reference for future research.

mathematical models can produce desired dynamics and statistical properties with the insertion of suitable nonlinear terms,while energy characteristics are crucial for practical application because any hardware realizations of nonlinear systems are relative to energy flow.The involvement of memristive terms relative to memristors enables multistability and initial-dependent property in memristive systems.In this study,two kinds of memristors are used to couple a capacitor or an inductor,along with a nonlinear resistor,to build different neural circuits.The corresponding circuit equations are derived to develop two different types of memristive oscillators,which are further converted into two kinds of memristive maps after linear transformation.The Hamilton energy function for memristive oscillators is obtained by applying the Helmholz theorem or by mapping from the field energy of the memristive circuits.The Hamilton energy functions for both memristive maps are obtained by replacing the gains and discrete variables for the memristive oscillator with the corresponding parameters and variables.The two memristive maps have rich dynamic behaviors including coherence resonance under noisy excitation,and an adaptive growth law for parameters is presented to express the self-adaptive property of the memristive maps.A digital signal process(DSP)platform is used to verify these results.Our scheme will provide a theoretical basis and experimental guidance for oscillator-to-map transformation and discrete map-energy calculation.

Identification of Early Warning Signals of Infectious Diseases in Hospitals by Integrating Clinical Treatment and Disease Prevention

The global incidence of infectious diseases has increased in recent years,posing a significant threat to human health.Hospitals typically serve as frontline institutions for detecting infectious diseases.However,accurately identifying warning signals of infectious diseases in a timely manner,especially emerging infectious diseases,can be challenging.Consequently,there is a pressing need to integrate treatment and disease prevention data to conduct comprehensive analyses aimed at preventing and controlling infectious diseases within hospitals.This paper examines the role of medical data in the early identification of infectious diseases,explores early warning technologies for infectious disease recognition,and assesses monitoring and early warning mechanisms for infectious diseases.We propose that hospitals adopt novel multidimensional early warning technologies to mine and analyze medical data from various systems,in compliance with national strategies to integrate clinical treatment and disease prevention.Furthermore,hospitals should establish institution-specific,clinical-based early warning models for infectious diseases to actively monitor early signals and enhance preparedness for infectious disease prevention and control.
