Objective To investigate the prevalence of peripheral arterial disease (PAD) in China type 2 diabetic patients and to demonstrate the relationships between putative risk factors and PAD. Methods In total 1 397 type 2 diabetic patients aged 50 years and older were enrolled and determined ankle-brachial index (ABI) and brachial-ankle pulse wave velocity (baPWV) in 15 Class III Grade A hospitals in 7 major cities of China. Results Mean patient age was 63.7±8.2 years and mean duration of diabetes mellitus was 9.39±7.4 years. Two hundreds and seventy-two (19.47%) patients were diagnosed as PAD by ABI <0.9, 122 (18.37%) in male and 150 (20.46%) in female. PAD patients had a significantly longer duration of diabetes mellitus, higher hemoglobin Alc, and a significantly lower mean body mass index than non-PAD ones. Aging, smoking, and systolic blood pressure were found to be positively related with the prevalence of PAD. In terms of lipid profiles, no variable was found to relate with PAD. Notably, baPWV showed as the same significant guiding index for PAD, almost matched with ABI. Conclusions PAD is a common complication in China type 2 diabetic patients. Therefore, PAD screening and treatment should be emphasized for diabetic patients with high risk factors.

Construction and evaluation of anti-gastrin immunogen based on P64K protein

AIM： To construct two kinds of anti-gastrin immunogen based on P64K protein from Neisseria rneningitids and to compare their immunogenic effect. METHODS： G17P64K gene was cloned and ligated into pET28a plasmid, then transformed into BL21（DE3）. After inoculation of LB medium and IPTG induction, the recombinant protein was solubly expressed at a high level. The purification of G17P64K fusion protein was similar to that of P64K. An initial step of purification consisting of 30% saturated ammonium sulfate precipitation was done, Additional fine optimizations included phenyl-sepharose, G200 Sephadex gel filtration and Q-sepharose anion exchanger chromatography. Highly purified protein was obtained and sequenced at the N-terminal amino acid residues. Polypeptide was synthesized by Fmoc solid phase chemical method and cross-linked to carrier protein P64K and DT mutant by MBS method and then the rabbit anti-gastrin 17 antibody was prepared by immunizing rabbit with cross-linked and fused protein. The titer and the activity in vitro of antibody were assessed. RESULTS： G17P64K gene and the recombinant bacteria were obtained. After four steps purification, protein sample that has the purity above 90% was achieved. At the 84^th day after the first immunization, the titer of antibody against cross-linked protein reached 51 200. Evaluation of the antibody in vitro manifested that it had a high inhibitory activity on the growth of tumor cell SW480. CONCLUSION： The P64K-polypeptide cross-linked immunogen immunized rabbit and achieved a higher titer antibody against gastrin 17 than the G17P64K fusion protein immunogen, which could inhibit the growth of the tumor cell SW480.

Acute and Chronic Changes and Predictive Value of Tpeak-Tend lor Ventricular Arrhythmia Risk in Cardiac Resynchronization Therapy Patients

Background： Prolongation of the Tpeak-Tend （TpTe） interval as a measurement of transmural dispersion of repolarization （TDR） is an independent risk factor for chronic heart failure mortality, However, the cardiac resynchronization therapy＇s （CRT） effect on TDR is controversial. Therefore, this study aimed to evaluate CRTs acute and chronic effects on repolarization dispersion. Furthermore, we aimed to investigate the relationship between TpTe changes and ventricular arrbythmia. Methods： The study group consisted of 101 patients treated with CRT-defibrillator （CRT-D）. According to whether TpTe was shortened, patients were grouped at immediate and l-year follow-up after CRT, respectively. The echocardiogram index and ventricular arrhythmia were observed and compared in these subgroups. Results： For all patients, TpTe slightly increased immediately after CRT-D implantation, and then decreased at the l -year follow-up （from 107 ± 23 to 110 ± 21 rns within 24 h, to 94 ± 24 ms at 1-year follow-up, F 19.366, P 〈 0.001）. No significant difference in the left ventricular reverse remodeling and ventricular tachycardia/ventricular fibrillation （VT/VF） episodes between the TpTe immediately shortened and TpTe immediately nonshortened groups. However, patients in the TpTe at l-year shorten had a higher rate of the lelt ventricular （LV） reverse remodeling （65% vs. 44%, Z2 = 4.495, P - 0.038） and less VT/VF episodes （log-rank test, X2 - 10.207, P = 0.001 ） compared with TpTe I-year nonshortened group. TpTe immediately alter CRT-D independently predicted VT/VF episodes at l-year follow-up （hazard ratio [HR], 1.030; P = 0.001 ）. Conclusions： Patients with TpTe shortened at l -year after CRT had a higher rate of LV reverse remodeling and less VT/VF episodes. The acute changes of TpTe after CRT have minimal value on mechanical reverse remodeling and ventricular arrhythmia.

Oxidation of n-Butanol and 2-Pentanol with Molecular Oxygen in Supercritical CO2

Oxidation of n-butanol and 2-pentanol using molecular oxygen in supercritical (SC) CO2 with and without co-solvent is investigated. The results showed that the reaction selectivity is high when the reaction is carried out in SC CO2. It has been observed that co-solvent affects conversion and selectivity of the reaction considerably.

Secretory expression and characterization of a recombinant-deleted variant of human hepatocyte growth factor in Pichia pastoris

AIM： To study the secretory expression of human hepatocyte growth factor （hdHGF） gene in Pichia pastoris. METHODS： The full-length gene of human cDNA encoding the deleted variant of hdHGF was cloned by RT-PCR and overlapping-fragment PCR technique using mRNA of human placenta as a template. The cloned hdHGF cDNA was inserted into the Escherichia coilyeast shuttle vector of pPIC9. The constructed plasmid, pPIC9-hdHGF, was transformed into the GSl15 cells of the methylotrophic yeast, P pastoris, using a chemical method. The Hut ＋ transformants were screened to obtain high-expression strains by the test and analysis of expressed products of shake-flask culture. A secretory form of rhdHGF was made with the aid of the leader peptide sequence of Saccharomyces cerevisiae α-factor. RESULTS： The expressed products, which showed a band of molecular mass of about 80 ku, were observed on 15% SDS-PAGE and identified by Western blotting and N-terminal amino acid sequencing. In the high cell density culture of 5 L fermentor by fed-batch culture protocol, the cell biomass was reached at approximately 135 g （DCW）/L. The productivity of secreted total supernant protein concentration attained a high-level expression of more than 8.0 g/L and the ratio of rhdHGF band area was about 12.3% of the total band area scanned by SDS-PAGE analysis, which estimated that the product of rhdHGF was 500-900 mg/***： The P pastoris system represents an attractive tool of generating large quantities of hdHGF for both research and industrial purposes.

Estrogen signaling via GPR30 in breast cancers has been intensively discussed over years,but the underlying molecular mechanism remains to be fully elucidated,especially for the transcriptional profiles of GPR30 under estrogen *** this study,we evaluated the potential role of GPR30 in regulating invasion of metastatic breast cancer cell line MDA-MB-231 via transcriptional ***,with primary breast cancer tissue samples,we found that the expression of GPR30 significantly coordinated with another membrane ***,the expression level of these two proteins was associated with the development and progression of the primary breast ***,to dissect the cross talk between GPR30 and CXCRl,we further our studies to detect the activation of ERK,AKT and transcriptional factor *** found that upon estrogen stimulation,the phosphorylation level of ERK and AKT were rapidly increased and then resulted in the activation and nucleus translocation of *** we considered the subsequence of the NF-kB activation,we found the secretion of IL-8 was boosted by estrogen stimulation and followed by the promoted invasion of the MDA-MB-231 *** conclusion,our data demonstrated that estrogen-mediated GPR30 stimulation would regulate the invasive activities of metastatic breast cancer cell line MDA-MB-231 coupled with another membrane receptor CXCRl via transcriptional pathway,the cross talk between GPR30 and CXCRl may be another targets for breast cancer therapies.

nitial Experience with MultiPoint Pacing Cardiac Resynchronization Therapy in China

Recently, cardiac resynchronization therapy （CRT） with multipoint left ventricular （LV） pacing in a single coronary sinus branch （MultiPointTM Pacing [MPP], St. Jude Medical, Inc., Sylmar, CA, USA） has been introduced commercially. The CRT system with MPP feature offers a new strategy for physicians to further enhance CRT in patients with heart failure. Early clinical studies have shown that MPP, compared with conventional biventricular （BiV） pacing, provides acute benefits to LV dP/dtmax, LV dyssynchrony, LV peak radial strain, and LV electrical activation, and improves CRT response at 12 months. In this report, we presented two MPP CRT cases in China with acute hemodynamic assessment under various MPP and conventional BiV pacing configurations, demonstrating that MPP can offer further benefits to patients.

OptiVol fluid index predicts acute decompensation of heart failure with a high rate of unexplained events

Background Intrathoracic impedance monitoring has emerged as a promising new technique for the detection of impending heart failure (HF). Although false positive episodes have been reported in case reports and clinical trials, the efficacy and false positive rate in real-world practice remain unclear. Objective The aim of this study is to investigate the utility and reliability of the OptiVol alert feature in clinical practice. Methods We continuously recruited patients who underwent implantable cardioverter-defibrillator (ICD) or cardiac re-synchronization therapy with defibrillator (CRT-D) implantation with feature of intrathoracic impedance monitoring system in our center from Sep. 2010 to Oct. 2012. Regular in-office follow-up were required of all patients and the following information was collected at each visit: medical history, device interrogation, N-terminal pro-brain natriuretic peptide (NT-proBNP) measurement and an echocardiogram. Worsening HF was defined as hospitalization or the presentation of signs or symptoms of HF. Results Forty three patients (male:76.7%, mean age:57 ± 15 years, left ventricular ejection fraction (LVEF):33%± 14%) were included in this observational study. Fifty four alert events and 14 adjudicated worsening HF were detected within 288 ±163 days follow-up. Eleven (20.4%) alert episodes were associated with acute cardiac decompensation in 9 patients with a positive predictive value of 78.6%. Forty three audible alerts showed no connection to worsening HF. The unexplained alerts rate was 79.6%and 1.27 per person-year. Thirty seven alarm alerts were detected in patients with EF<45%, among which 9 accompanied with HF, 17 alerts detected in patients with LVEF≥45%and 2 associated with HF. There was no sig-nificant difference between the two groups (9/37 vs. 2/17;P=0.47). FRACTURE ANALYSIS FOR 3-DIMENSIONAL BODIES WITH A SURFACE CRACK

This paper deals with fracture analyses in 3-dimensional bodies containing a surface crack. A general solution of stress-strain fields at crack tip is proposed. Based on the stress-strain fields obtained, a high-order 3-dimensional special element is established to calculate the stress intensity factors in a plate with a surface crack. The variation of stress intensity factors with geometric parameters is investigated.
