疏肝理气活血汤联合常规西药治疗慢性萎缩性胃炎临床研究

目的:观察疏肝理气活血汤联合常规西药治疗肝胃不和型慢性萎缩性胃炎(CAG)的临床疗效。方法:选择肝胃不和型CAG患者80例,按照随机数字表法分为观察组和对照组各40例。对照组给予枸橼酸莫沙必利片和瑞巴派特片治疗,观察组在对照组基础上加用疏肝理气活血汤治疗。评价2组临床疗效,比较2组治疗前后中医证候积分及病理检查评分,统计不良反应发生率。结果:观察组总有效率为85.00%,高于对照组70.00%(P<0.05)。治疗后,2组中医证候积分较治疗前下降(P<0.05),且观察组中医证候积分低于对照组(P<0.05)。治疗后,2组胃黏膜萎缩、肠上皮化生、异型增生评分较治疗前下降(P<0.05);且观察组胃黏膜萎缩、肠上皮化生、异型增生评分低于对照组(P<0.05)。观察组不良反应发生率低于对照组(P<0.05)。结论:疏肝理气活血汤联合常规西药治疗CAG可改善患者的临床症状,对于缓解或逆转胃黏膜萎缩、肠上皮化生和异型增生具有一定的治疗效果,且不良反应较少。

目的探讨急性白血病(AL)患者外周血髓系起源的树突状细胞(mDC)和淋巴系起源的树突状细胞(pDC)以及辅助性T细胞(Th)、滤泡辅助性T细胞(Tfh)、初始B细胞、记忆B细胞比例及其活化状态。方法收集2022年1月-2023年8月在中山大学附属第一医院血液内科住院的B细胞-急性淋巴细胞白血病(ALL)患者13例以及急性髓细胞白血病(AML)患者15例,同时选取心脏瓣膜病等非肿瘤、非自身免疫性疾病患者12例为对照组。所有患者的治疗均根据国内指南规范化进行,整个治疗过程中均使用常规类似化疗方案。采用流式细胞仪方法研究B细胞-ALL、AML患者外周血起病时(起病组)、缓解后(缓解组)mDC、pDC的比例及活化状态,以及Th、Tfh、初始B细胞、记忆性B细胞比例。结果 ALL、AML患者起病组pDC、mDC比例[(0.43±0.05)%、(0.28±0.04)%,(0.26±0.06)%、(0.19±0.03)%]较对照组[(0.67±0.04)%、(0.48±0.05)%]明显降低,差异均有统计学意义(P均<0.05);缓解组有上升。ALL患者起病组pDC、mDC的CD80、CD86的表达较对照组有增高的趋势,在缓解组均有下降的趋势;AML患者起病组pDC、mDC的CD80、CD86的表达较对照组均有降低的趋势,在缓解组均有恢复的趋势。ALL起病组外周血Th、Tfh、初始B细胞比例[(6.45±0.56)%、(0.24±0.06)%、(34.85±2.23)%]较对照组[(8.47±0.72)%、(0.60±0.08)%、(48.43±3.70)%]明显降低,差异均有统计系意义(P均<0.05);记忆性B细胞有下降的趋势;而缓解组均有恢复的趋势。AML患者起病组外周血Th、Tfh、初始B细胞、记忆性B细胞比例[(7.61±0.56)%、(0.43±0.05)%、(39.47±1.77)%、(16.28±0.89)%]均较对照组[(8.47±0.72)%、(0.60±0.07)%、(48.43±3.70)%、(19.88±1.63)%]降低,其中初始B细胞差异有统计学意义(P<0.05);缓解组也均有回升的趋势。结论 AL患者的mDC、pDC、Th、Tfh、初始B细胞、记忆B细胞的比例在起病时均受到抑制,缓解后有恢复的趋势,其机制及相互作用需做进一步研究。

Cellulose macrofibers (MFs) are gaining increasing interest as natural and biodegradable alternatives to fossil-derived polymers for both structural and functional applications. However, simultaneously achieving their exceptional mechanical performance and desired functionality is challenging and requires complex processing. Here, we reported a one-step approach using a tension-assisted twisting (TAT) technique for MF fabrication from bacterial cellulose (BC). The TAT stretches and aligns BC nanofibers pre-arranged in hydrogel tubes to form MFs with compactly assembled structures and enhanced hydrogen bonding among neighboring nanofibers. The as-prepared BC MFs exhibited a very high tensile strength of 1 057 MPa and exceptional lifting capacity (over 340 000 when normalized by their own weight). Moreover, due to the volume expansion of BC nanofibers upon water exposure, BC MFs quickly harvested energy from environmental moisture to untwist the bundled networks, thus generating a torsional spinning with a peak rotation speed of 884 r/(min·m). The demonstrated rapid and intense actuation response makes the MFs ideal candidates for diverse humidity-response-based applications beyond advanced actuators, remote rain indicators, intelligent switches, and smart curtains.

In our study, we determined the efficacy of bortezomib-based induction therapy followed by autologous stem cell transplant （ASCT） in newly diagnosed and relapsed/refractory （R/R） multiple myeloma （MM） patients and compared the advantages of early versus late transplant. We used a retrospective analysis to examine 62 patients, including 46 cases of newly diagnosed MM （early transplant group） and 16 cases of relapsed/refractory MM （late transplant group）. All of these patients received bortezomib-based induction therapy followed by ASCT. The efficacy and side effects of the treatment regimen were analyzed. Patients＇ overall survival （OS） and progression-free survival （PFS） times were determined. The ratio of complete remission to near-complete remission （CR/nCR） was 69.5% versus 56.2% （P=0.361）, respectively, for the early transplant group versus the late transplant group, respectively, after receiving bortezomib-based induction therapy; the overall response rates of the two group were 91.3 % and 81.2 %, respectively （P=0.369）. After receiving ASCT, the CR/nCR of the two groups increased to 84.8% and 81.3%, respectively. The median time required for neutrophil engraftment of the early transplant group and the late transplant group was 11 and 14.5 days, respectively （P=0.003）; the median time required for platelet engra^nent was 13 and 21.5 days （P=0.031）, respectively. There were no significant differences in the toxic side effects observed during induction therapy and ASCT between the two groups. The OS of the two groups was not statistically different （P=0.058）. The PFS of the early transplant group and the late transplant group was 41.6 and 26.5 months, respectively （P=0.008）. Multivariate analysis demonstrated that the time of receiving ASCT, the types of M protein, and the International Staging System （ISS） stage were all independent factors that influenced PFS. An accurate mapping and understanding of remaining oil distribution is very important for water control and stabilize oil production of mature oilfields in ultra-high water-cut ***,the Tuo-21 Fault Block of the Shengtuo Oilfield has entered the stage of ultra-high water cut(97.2%).Poor adaptability of the well pattern,ineffective water injection cycle and low efficiency of engineering measures(such as workover,re-perforation and utilization of high-capacity pumps)are the significant problems in the ultra-high water-cut *** order to accurately describe the oil and water flow characteristics,relative permeability curves at high water injection multiple(injected pore volume)and a semiquantitative method is applied to perform fine reservoir simulation of the Sand group 3e7 in the *** accurate reservoir model is built and history matching is *** distribution characteristics of remaining oil in lateral and vertical directions are quantitatively simulated and *** results show that the numerical simulation considering relative permeability at high injection multiple can reflect truly the remaining oil distribution characteristics after water flooding in an ultrahigh water-cut *** distribution of remaining oil saturation can be mapped more accurately and quantitatively by using the‘four-points and five-types’classification method,providing a basis for potential tapping of various remaining oil types of oil reservoirs in late-stage of development with high water-cut.
