Relationships of High-sensitive C-reactive Protein ant P-wave Dispersion in Lone Atrial Fibrillation

Background： Current evidence links atrial fibrillation （AF） to the inflammation. Inflammatory indexes such as high-sensitive C-reactive protein （hs-CRP） have been related to the development and persistence of AF. However, the role of inflammation in the atrial electrophysiological remodeling indexed by P-wave dispersion （Pa） remains unclear. Methods： The study consisted of 71 patients with lone paroxysmal AF （AF group） and 71 age- and gender-matched controls of paroxysmal supraventricular tachycardia without history of AF （control group）. Electrocardiography, P hs-CRP, and other clinical characteristics were compared between the two groups. Results： There was no significant difference between the two groups regarding age, gender, hyperlipidemia, etc. Compared to controls, left atrial diameter （44 ± 7 vs 39 ± 7 mm）, Pd （49 ± 13 vs 26 ± 8 ms）, and hs-CRP （2.17 [1.46-2.89] vs 1.12 [0.74-1.41] rag/L） were increased （P 〈 0.05）, respectively. linear regression identified hs-CRP as an independent correlation ofPd level both in the total population and the AF group （r = 0.464 and 0.313; P 〈 0.001, respectively）. Multiple logistic regression revealed hs-C RP as an independent determinant of AF （odds ratio [OR] =l 5.430, 95% confidence interval： 6.031-39.476： P 〈0.001）. Further adjusted tbr Pd, both Pd and hs-CRP were independent predictors for AF, but the OR for hs-CRP in predicting AF has been attenuated from 15.430 to 6.246. Conclusions： In lone AF, P and plasma hs-CRP concentration are inter-associated and related to AF. The interaction between hs-CRP and AF may be mediated by Pe, suggesting an important role of inflammation in the atrial electrophysiological remodeling predisposing to AF.

Three new lactones,xylanilyticolides A-C(1-3),were isolated from cultures of the actinomycete Promicromonospora xylanilytica YIM *** structures were elucidated by 1D and 2D NMR spectroscopic data in conjunction with HRESIMS *** 1 exhibited potent cytotoxicities against five human cancer cell lines HL-60,A-549,SMMC-7721,MCF-7 and SW480 with the IC_(50) values of 3.9,15.2,11.2,5.9,and 4.7 pM,respectively.

Polarization variable terahertz metasurface along the propagation path

Conventional metasurfaces for terahertz polarization are limited to performing lateral (in the x-y plane) polarization control of the output wave. In such cases, the polarization state remains unchanged in each output plane along the propagation path. Herein, we propose a terahertz polarization metasurface that operates longitudinally (i.e., along the z-axis direction of propagation), which modifies the polarization state of each output plane throughout the propagation path. Our designed metasurface can control the phase delays of the left-handed and right-handed circularly polarized (LCP and RCP) components of the incident terahertz wave. This enables the LCP beam and RCP beam to converge to the z-axis through distinct paths, creating a Bessel beam. The proposed design achieved a linearly polarized terahertz wave including both LCP and RCP components with a precise phase difference Δφ at each point within a certain range along the z-axis. The Δφ varies as the propagation distance, resulting in a rotated linearly polarized output wave along the propagation path, while the rotation angle ranges from 0 to π. Based on the variable property of longitudinal polarization, we propose an application concept of dielectric refractive index sensing, in which an additional medium is placed in the terahertz propagation path and the unknown refractive index is determined by detecting the rotation angle of the output polarization state. Theoretically, the device might find potential applications in variable media excitation, terahertz communication, and terahertz radar ranging. © 2023

Prognostic impact of splenectomy on advanced proximal gastric cancer with No. 10 lymph node metastasis

Background This study evaluated the prognostic impact of D2 lymphadenectomy combined with splenectomy in patients with advanced proximal gastric cancer and lymph node metastasis at the splenic hilum （No. 10 lymph nodes）. Methods The clinical records of 216 patients with advanced proximal gastric cancer and No. 10 lymph node metastasis who underwent D2 curative resection were retrospectively analyzed. Seventy-three patients underwent simultaneous splenectomy （splenectomy group）, while 143 patients did not （spleen-preserving group）. Five-year survival rates, mean numbers of dissected No. 10 lymph nodes and metastatic No. 10 lymph nodes, and operative morbidity and mortality were calculated and compared between the two groups. Potential prognostic factors were evaluated by univariate and multivariate analysis.Results The 5-year survival rate was 30.0% for the splenectomy group and 19.7% for the spleen-preserving group （χ^2=14.73, P 〈0.05）. The mean numbers of dissected No. 10 lymph nodes and metastatic No. 10 lymph nodes in the splenectomy group were significantly greater than in the spleen-preserving group （P 〈0.05）. Multivariate analysis revealed that the depth of invasion, splenectomy, and type of gastrectomy were independent prognostic factors. The survival rate for T3 patients with and without splenectomy was 38.7% and 18.9%, respectively （χ^2 =15.03, P 〈0.05）. For patients undergoing total gastrectomy, survival rates were 33.4% and 20.7%, respectively （χ^2 =13.63, P〈0.05）. Operative morbidity and mortality in splenectomy group was 24.7% and 4.1%, respectively, and in the spleen-preserving group was 17.5% and 3.5%, respectively. The differences were not statistically significant （P 〉0.05）. Conclusions Splenectomy is beneficial for No. 10 lymph node dissection in patients with advanced proximal gastric cancer. To improve patient prognosis, total gastrectomy with splenectomy is recommended for patients with T3 proximal gastric cancer who have No. 10 lymph node met

0Optimized Fluoroscopy Setting and Appropriate Project Position Can Reduce X-ray Radiation Doses Rates during Electrophysiology Procedures

Background：Nonfluoroscopic three-dimensional electroanatomical system is widely used nowadays,but X-ray remains indispensable for complex electrophysiology *** study aimed to evaluate the value of optimized parameter setting and different projection position to reduce X-ray radiation dose ***：From June 2013 to October 2013,105 consecutive patients who underwent complex ablation were enrolled in the *** the ablation,the radiation dose rates were measured by two different settings （default setting and optimized setting） with three projection positions （posteroanterior [PA] projection;left anterior oblique [LAO] 30° projection;and LAO 45° projection）.The parameter of preset voltage,pulse width,critical voltage,peak voltage,noise reduction,edge enhancement,pulse rate,and dose per frame was modified in the optimized ***：The optimized setting reduced radiation dose rates by 87.5％（1.7 Gy/min vs.13.6 Gy/min,P ＜ 0.001） in PA,87.3％（2.5 Gy/min vs.19.7 Gy/min,P＜0.001） in LAO 30°,85.9％（3.1 Gy/min vs.22.1 Gy/min,P ＜ 0.001） in LAO 45°.Increase the angle of projection position will increase the radiation dose ***：We can reduce X-ray radiation dose rates by adjusting the parameter setting of X-ray *** oblique projection of large angle is another way to reduce X-ray radiation dose rates.
