BACKGROUND Pancreatic cancer is a malignancy with one of the poorest prognoses amongst all *** with unresectable tumours either receive palliative care or undergo various chemoradiotherapy *** techniques are often associated with acute gastrointestinal toxicities,as adjacent critical structures such as the duodenum ultimately limits delivered *** body radiotherapy(SBRT)is an advanced radiation technique that delivers highly ablative radiation split into several fractions,with a steep dose fall-off outside target *** To discuss the latest data on SBRT and whether there is a role for magnetic resonance-guided techniques in multimodal management of locally advanced,unresectable pancreatic *** We conducted a search on multiple large databases to collate the latest records on radiotherapy techniques used to treat pancreatic *** of 1229 total records retrieved from our search,36 studies were included in this *** Studies indicate that SBRT is associated with improved clinical efficacy and toxicity profiles compared to conventional radiotherapy *** dose escalation to the tumour with SBRT is limited by the poor soft-tissue visualisation of computed tomography imaging during radiation planning and treatment *** resonance-guided techniques have been introduced to improve imaging quality,enabling treatment plan adaptation and re-optimisation before delivering each *** Therefore,SBRT may lead to improved survival outcomes and safer toxicity profiles compared to conventional techniques,and the addition of magnetic resonance-guided techniques potentially allows dose escalation and conversion of unresectable tumours to operable cases.

Imaging Modalities for Diagnosing Hepatolithiasis: A Case Report of Multiple Hepatolithiasis

Hepatolithiasis or intrahepatic duct stones is uncommon in western setting, but it has quite a high prevalence in far eastern countries. The natural history of the disease may not be well understood in a western society, however due to increasing immigration, there are increasing numbers of cases that have occurred and will only continue occurring. The case report is regarding an elderly lady with previous complicated cholecystectomy presented with worsening recurrent right upper quadrant pain. Computed tomography (CT) and magnetic resonance imaging (MRI) interestingly revealed multiple intraductal hepatolithiasis. The aim of imaging should be to accurately identify the biliary tree and liver anatomy, stones, stricture, liver segment involved and excluding complication such as abscesses or cholangiocarcinoma.

Endovascular Management of Vertebral Artery Aneurysms

Background: The management outcome of a series of ruptured vertebral artery (VA) aneurysms was reviewed. Method: This is a retrospective study of prospectively collected data of 60 cases with ruptured VA aneurysms in the author’s database from the year 2004. All cases were managed with coiling, stent-assisted coiling, flow diversion or endovascular parent artery sacrifice. Nimodipine and hypertensive, hypervolemic therapy was applied as standard. Outcome was assessed at 6 months with modified Rankin score (mRS). Results: There were 26 females and 34 males. Age range is 18 to 70 with a mean age of 42. There were 27 dissecting aneurysms and 33 saccular aneurysms. 50 (83%) cases were World Federation of Neurological Surgeons (WFNS) grade one to three. There were four WFNS grade four and six WFNS grade five. 25 cases of dissecting aneurysms were managed with parent artery coil occlusion. Two dissecting aneurysms affecting the dominant VA were treated with flow diverters. At 6 months follow up 50 (83%) achieved good outcome with mRS one to two. There were eight cases with poor outcome and two deaths. Six-month follow-up MR angiogram documented stable occlusion for the 24 cases with dissecting aneurysms treated with parent artery coil occlusion who survived. The two treated with flow diversion demonstrated good remodelling at 6 months on angiography. Six-month follow-up angiogram for the 32 cases of saccular aneurysm who survived documented stable aneurysm obliteration. Conclusion: Good outcome can be achieved with endovascular treatment for ruptured VA aneurysms. Parent artery occlusion is a safe and effective technique for ruptured VA dissecting aneurysm.

Accuracy between clinical and radiological diagnoses compared to surgical orbital biopsies

The first part of this report describes the data reduction of non-merohedrally twinned crystals measured on Bruker and Agilent area-detector diffractometers. The image frames of methyl-2-aminopyrazine-3-carboxylate were processed with APEX2 to furnish a set of overlapping diffraction indices that were used for solution and refinement. CrysAlisPRO was used for processing the frames of bis（diethyldicarbamato）nickel, which exists in monoclinic and tetragonal polymorphs, and in untwinned and twinned forms. In the second part, the crystal structure of [（3-formyl-4- hydroxyphenyl）methyl]triphenylphosphanium chloride was refined through the ‘HKLF 5＇（based on a combined set of diffraction indices） and PLATON（based on one set of diffraction indices） routes to give identical outcomes because the amount of overlap of the twin domains is small. For the third part, in a proof-of-concept investigation, the diffraction pattern of untwinned and twinned 4-{（E）-（4-aminophenyl）diazenyl]phenylamine was recorded simultaneously in one run; the three domains could be indexed and the crystal structure satisfactorily refined. The refinement was identical to those derived from independent measurements; the crystal structure features two independent centrosymmetric molecules, one of which is ordered and the other whole-molecule-disordered. This two-in-one run opens up the possibility that two or more crystals having different atomic compositions can be measured simultaneously if their reciprocal lattices do not overlap significantly.

In this study,we used a network meta-analysis(NMA)to compare the effectiveness of medicines and supplements for idiopathic male infertility and to identify the best ***,Excerpta Medica Database(EMBASE),Ovid,and China National Knowledge Infrastructure(CNKI),were searched for the period from January 1990 to June 2021 using the keywords"male infertility,""medical therapy,""supplement/nutrient therapy,"and related *** controlled trials(RCTs)investigating medicines(mainly follicle-stimulating hormone(FSH),androgen,and clomiphene/tamoxifen)or supplements(mainly zinc,selenium,vitamin C or E,carnitine,coenzyme Q10(CoQ10),or combined treatment)for idiopathic infertile men were selected for *** reporting items for systematic reviews and meta-analysis(PRISMA)was used for data extraction,and a risk-of-bias tool and grades of recommendation,assessment,development,and evaluation(GRADE)system adapted to the NMA were employed to assess the quality of the *** primary outcomes were live birth and spontaneous pregnancy rate(SPR).The secondary outcomes were sperm parameters(including concentration,progressive motility,and morphology)and side *** total,65 RCTs involving 7541 men with sperm abnormalities but normal hormone levels were included.A total of 36 studies reported SPR but only three reported live birth *** quality of the included studies was found to be moderate to *** with a placebo or being untreated,carnitine plus vitamins significantly improved SPR(relative risk(RR)=3.7,95%confidence interval(95%CI),1.6-8.5);fatty acids significantly increased sperm concentrations(mean difference(MD)=12.5×10^(6)mL-1,95%CI,3.1×10^(6)-22.0×10^(6));and selective estrogen receptor modulator(SERM)plus CoQ10 significantly improved sperm progressive motility(MD=11.0%,95%CI,0.1%-21.9%)and normal sperm morphology(MD=11.0%,95%CI,4.6%-17.4%).The most optimal intervention was carnitine plus vitamins and fatty acids for SPR a

Electron transport layers(ETLs)are crucial for achieving efficient and stable planar perovskite solar cells(PSCs).Reports on versatile inorganic ETLs using a simple film fabrication method and applicability for both low-cost planar regular and inverted PSCs with excellent efficiencies(>22%)and high stability are very ***,we employ a novel inorganic ZnSe as ETL for both regular and inverted PSCs to improve the efficiency and stability using a simple thermal evaporation *** TiO_(2)-ZnSe-FAPbl_(3)heterojunction could be formed,resulting in an improved charge collection and a decreased carrier recombination further proved through theoretical *** optimized regular PSCs based on TiO_(2)/ZnSe have achieved 23.25%efficiency with negligible *** addition,the ZnSe ETL can also effectively replace the unstable bathocuproine(BCP)in inverted ***,the ZnSe-based inverted device realizes a champion efficiency of 22.54%.Moreover,the regular device comprising the TiO_(2)/ZnSe layers retains 92%of its initial PCE after 10:00 h under 1 Sun continuous illumination and the inverted device comprising the C_(60)/ZnSe layers maintains over 85%of its initial PCE at 85℃for 10:00 *** highlights one of the best results among universal ETLs in both regular and inverted perovskite photovoltaics.

目的:探究WNT1诱导信号通路蛋白1(WISP1)在非酒精性脂肪性肝炎(NASH)的功能定位及治疗作用机制。方法:60只雄性C57BL/6J小鼠购于北京维通利华公司,采取简单随机抽样法分为5组,每组10只,正常组喂养普通饲料,通过喂养高脂高胆固醇饲料14周制备NASH小鼠模型,在造模过程中分别腹腔注射IgG单克隆抗体,0.1 g/kg WISP1高亲和力单克隆抗体,0.2 g/kg WISP-1高亲和力单克隆抗体为IgG、低剂量WISP1抗体组和高剂量WISP1抗体组,每周2次,连续给药14周。检测血清丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、血糖水平。苏木精-伊红(HE)、糖原(PAS)染色、油红O染色观察肝组织病理变化。实时荧光定量聚合酶链反应(Real-time PCR)检测小鼠肝组织中白细胞介素(IL)-10、IL-6、IL-1β、肿瘤坏死因子-α(TNF-α)mRNA表达水平。检测肝组织总胆固醇(TC)、甘油三脂(TG)水平,酶联免疫吸附测定法(ELISA)检测还原型谷胱甘肽(GSH)、丙二醛(MDA)、超氧化物歧化酶(SOD)活性水平,活性氧(ROS)荧光染色检测肝组织中活性氧含量;免疫组织化学(IHC)检测肝组织WISP-1、乙酰辅酶a合成酶(ACS)蛋白表达,蛋白质印迹法(Western blot)检测肝组织肝组织腺苷酸活化蛋白激酶(AMPK)、磷酸化AMPK(p-AMPK)、肉碱棕榈酰转移酶-1(CPT1)、过氧化物酶体增殖物激活受体γ辅激活因子-1(PCG-1α)蛋白表达水平。结果:模型组小鼠血清AST、ALT、血糖水平高于正常组[(245.01±57.57)U/L比(37.28±4.23)U/L、(218.96±55.27)U/L比(49.81±10.57)U/L、(25.73±2.29)mmol/L比(8.06±0.51)mmol/L,Png/ml比(0.49±0.15)ng/ml、(2.19±0.23)pg/ml比(4.59±0.22)pg/ml,Png/ml比(0.22±0.06)ng/ml、(5.50±0.98)pg/ml比(2.19±0.40)pg/ml,P<0.01],活性氧含量低于模型组(0.51±0.06比4.91±0.31,P<0.01)。低、高剂量WISP1抗体组PGC-1α、ACS蛋白表达低于模型组(1.22±0.16、1.07±0.05比2.19±0.08,(33.50±3.45)%、(35.38±3.65)%比(62.97±2.38)%,P<0

目的:比较5%米诺地尔搽剂试验制剂和参比制剂透皮作用的一致性。方法:以巴马香猪皮肤作为透皮渗透的材料,选用立式改良Franz扩散池测定米诺地尔搽剂的渗透规律,采用液相色谱-串联质谱法(LC-MS/MS)分别测定接收液中药物的渗透总量和猪皮中药物的滞留量随时间的变化关系。结果:试验制剂和参比制剂在巴马香猪皮肤中24 h米诺地尔的累积渗透量分别为(49 721±29 707) ng·cm^(–2)和(45 372±23 755) ng·cm^(–2);平均渗透速率分别为(2 352±1 309) ng·cm^(–2)·h^(–1)和(2 182±1 080) ng·cm^(–2)·h^(–1);皮肤滞留量分别为(218 276±137 675) ng·g^(–1)和(188 040±49 523) ng·g^(–1);试验制剂与参比制剂在累积渗透量约为总透过量一半时(12 h)的平均渗透速率分别为(2 172±1 330) ng·cm^(–2)·h^(–1)和(1 947±1 037) ng·cm^(–2)·h^(–1)。结论:5%米诺地尔搽剂试验制剂与参比制剂的透皮渗透程度和速率符合外用制剂的一致性要求,且在猪皮内药物滞留特征相似。
