Background malignant tumor is the most common complication occurred in transplant recipients. It is widely recognized that immunosuppressive treatments increase the risk of cancer in transplant recipients. The efficacy and safety of rapamycin （RPM） in combination with low-dose calcineurin inhibitor （CNI） in treating 15 renal allograft recipients which developed urothelial carcinoma were observed.Methods Immunosuppressive regimen in all recipients was altered with rapamycin to replace mycophenolate mofetil （MMF） or azathioprine （Aza）. The initial loading dosage was 2 mg/d, and the next dosage was 1 mg/d. The dosage of rapamycin was carefully adjusted according to the blood drug level and concentration of the drug was maintained at 4-6 μg/L. In all the 15 patients, the calcineurin inhibitor was reduced down to one third of the original dosage after the rapamycin blood concentration became stable. Surgical treatment and intravesical instillation chemotherapy were carried out in all patients. Recurrence of the tumor was monitored throughout the study. Post-transplant renal function and side effects were also closely monitored.Results Among the 15 patients, 9 had no tumor recurrence in 2 years, 2 had tumor recurrences twice, and 4 had once. There was no acute rejection observed during RPM treatment. Post-transplant renal function in 11 patients was improved with a decreased creatinine level. Hyperlipoidemia and thrombocytopenia were the most frequent adverse events which responded well to corresponding treatments.Conclusion Among the renal allograft recipients with urothelial carcinoma, combination of rapamycin and low dose calcineurin inhibitor treatment is effective and safe.

Objective To investigate the genotypic diversity of Methicillin-resistant Staphylococcus aureus(MRSA) isolated from pigs and retail foods from different geographical areas in China and further to study the routes and rates of transmission of this pathogen from animals to food. Methods Seventy-one MRSA isolates were obtained from pigs and retail foods and then characterized by multi-locus sequencing typing(MLST), spa typing, multiple-locus variable number of tandem repeat analysis(MLVA), pulsed-field gel electrophoresis(PFGE), and antimicrobial susceptibility testing. Results All isolated MRSA exhibited multi-drug resistance(MDR). Greater diversity was found in food-associated MRSA(7 STs, 8 spa types, and 10 MLVA patterns) compared to pig-associated MRSA(3 STs, 1 spa type, and 6 MLVA patterns). PFGE patterns were more diverse for pig-associated MRSA than those of food-associated isolates(40 vs. 11 pulse types). Among the pig-associated isolates, CC9-ST9-t899-MC2236 was the most prevalent clone(96.4%), and CC9-ST9-t437-MC621(20.0%) was the predominant clone among the food-associated isolates. The CC9-ST9 isolates showed significantly higher antimicrobial resistance than other clones. Interestingly, CC398-ST398-t034 clone was identified from both pig-and food-associated isolates. Of note, some community-and hospital-associated MRSA strains(t030, t172, t1244, and t4549) were also identified as food-associated isolates. Conclusion CC9-ST9-t899-MC2236-MDR was the most predominant clone in pigs, but significant genetic diversity was observed in food-associated MRSA. Our results demonstrate the great need for improved surveillance of MRSA in livestock and food and effective prevention strategies to limit MDR-MRSA infections in China.

目的:群体反应性抗体可诱导肾移植后急性排斥反应的发生,但群体反应性Ⅰ、Ⅱ类抗体哪一种同排斥反应关系更密切,目前国内报道较少。观察肾移植前后群体反应性Ⅰ、Ⅱ类抗体变化,分析不同抗体变化与排斥反应的关系。方法:选择2008-01/12于北京朝阳医院泌尿外科施行肾移植100例患者中移植前后群体反应性Ⅰ、Ⅱ类抗体水平发生变化18例。通过秩和检验统计移植前后群体反应性Ⅰ、Ⅱ类抗体水平变化,分析抗体变化同移植后急性排斥反应之间的关系。结果:18例患者均进入结果分析。18例中9例发生急性排斥反应,移植后群体反应性Ⅱ类抗体水平较移植前显著升高(P=0.04),群体反应性Ⅰ类抗体水平无明显变化(P=0.707);9例未发生急性排斥反应患者其移植前后群体反应性Ⅰ、Ⅱ类抗体水平变化均无显著性意义。移植后群体反应性抗体总体水平升高者7例,其中以群体反应性Ⅱ类抗体升高为主5例,发生急性排斥反应4例,该4例患者中1例因急性排斥反应导致移植肾丢失;以群体反应性Ⅰ类抗体升高为主2例均未发生急性排斥反应。移植后群体反应性抗体总体降低11例,以群体反应性Ⅱ类抗体降低为主5例,发生急性排斥反应1例;以群体反应性Ⅰ类抗体降低为主6例,发生急性排斥反应4例。结论:群体反应性Ⅱ类抗体升高易导致急性排斥反应,移植后群体反应性Ⅱ类抗体升高与急性排斥反应有明确的相关性,与群体反应性Ⅰ类抗体水平变化无关。

We present the observational results from a detailed timing analysis of the black hole candidate EXO 1846-031 during its outburst in 2019 with the observations of Insight-HXMT,NICER and *** outburst can be classified roughly into four different ***-C quasi-periodic oscillations(QPOs)observed by NICER(about 0.1-6 Hz)and Insight-HXMT(about 0.7-8 Hz)are also reported in this ***,we study various physical quantities related to QPO *** QPO rms-frequency relationship in the energy band 1-10 keV indicates that there is a turning pointing in frequency around2 Hz,which is similar to that of GRS 1915+105.A possible hypothesis for the relationship above may be related to the inclination of the source,which may require a high inclination to explain *** relationships between QPO frequency and QPO rms,hardness,total fractional rms and count rate have also been found in other transient sources,which can indicate that the origin of type-C QPOs is non-thermal.

mammalian target of rapamycin(m TOR) inhibitor as an attractive drug target with promising antitumor effects has been widely investigated. High quality clinical trial has been conducted in liver transplant(LT) recipients in Western countries. However, the pertinent studies in Eastern world are paucity. Therefore, we designed a clinical trial to test whether sirolimus can improve recurrence-free survival(RFS) in hepatocellular carcinoma(HCC) patients beyond the Milan criteria after LT. This is an open-labeled, single-arm, prospective, multicenter, and real-world study aiming to evaluate the clinical outcomes of early switch to sirolimus-based regimens in HCC patients after LT. Patients with a histologically proven HCC and beyond the Milan criteria will be enrolled. The initial immunosuppressant regimens are center-specifc for the frst 4-6 weeks. The following regimens integrated sirolimus into the regimens as a combination therapy with reduced calcineurin inhibitors based on the condition of patients and centers. The study is planned for 4 years in total with a 2-year enrollment period and a 2-year follow-up. We predict that sirolimus conversion regimen will provide survival benefts for patients particular in the key indicator RFS as well as better quality of life. If the trial is conducted successfully, we will have a continued monitoring over a longer follow-up time to estimate indicator of overall survival. We hope that the outcome will provide better evidence for clinical decision-making and revising treatment guidelines based on Chinese population data.

目的：研究咪唑立宾（MZR）在肾移植患者中应用的临床特点。方法：2005年9月至2006年2月间共16例肾移植患者接受MZR治疗。5例患者为肾移植术后直接应用MZR，11例患者由于经济原因从霉酚酸酯（MMF）切换成MZR。全部患者均采用以钙调神经蛋白抑制剂为基础的三联免疫抑制方案。本组患者随访期间重点监测用药后血常规、移植肾功能、肝功能、尿酸、血糖、血脂等生化指标，同时记录患者出现的所有临床不良反应及治疗。包括：各种细菌、病毒感染，移植肾急性排斥反应等。综合评价该药物作为免疫抑制治疗方案的安全性、有效性。结果：在MZR治疗后3个月～15个月的随访期间，患者全部存活，且移植肾功能良好。急性排斥发生率为12．5％，未出现难治性排斥。消化道副反应发生少。患者主要不良反应是高尿酸血症和骨髓抑制，但通过MZR减量及对症处理后基本都可以控制，预后好。术后感染方面与其他药物相比差异无显著性。结论：MZR是一种安全有效的免疫抑制剂，具有疗效稳定，副反应程度轻的特点。在严密随访的情况下可取得良好的临床效果。

We introduce Quafu-Qcover,an open-source cloud-based software package developed for solving combinatorial optimization problems using quantum simulators and hardware ***-Qcover provides a standardized and comprehensive workflow that utilizes the quantum approximate optimization algorithm(QAOA).It facilitates the automatic conversion of the original problem into a quadratic unconstrained binary optimization(QUBO)model and its corresponding Ising model,which can be subsequently transformed into a weight *** core of Qcover relies on a graph decomposition-based classical algorithm,which efficiently derives the optimal parameters for the shallow QAOA ***-Qcover incorporates a dedicated compiler capable of translating QAOA circuits into physical quantum circuits that can be executed on Quafu cloud quantum *** to a general-purpose compiler,our compiler demonstrates the ability to generate shorter circuit depths,while also exhibiting superior speed ***,the Qcover compiler has the capability to dynamically create a library of qubits coupling substructures in real-time,utilizing the most recent calibration data from the superconducting quantum *** ensures that computational tasks can be assigned to connected physical qubits with the highest *** Quafu-Qcover allows us to retrieve quantum computing sampling results using a task ID at any time,enabling asynchronous ***,it incorporates modules for results preprocessing and visualization,facilitating an intuitive display of solutions for combinatorial optimization *** hope that Quafu-Qcover can serve as an instructive illustration for how to explore application problems on the Quafu cloud quantum computers.
