目的对比分析经导管动脉化疗栓塞(transcatheter arterial chemoembolization,TACE)同步联合微波消融与单纯TACE在不同病理分级的神经内分泌肿瘤肝转移患者中的疗效及安全性。方法回顾性分析2006年11月1日至2022年7月31日复旦大学附属中山医院介入治疗科收治的神经内分泌肿瘤肝转移患者,根据治疗方式分为同步消融组和TACE组,并根据病理分级分成相应的亚组,通过术后影像学检查评估病灶,随访至2023年7月31日,记录手术相关并发症,随访终点为患者的无进展生存期(prog.ession-free survival,PFS)及总生存期(overall survival,OS)。结果共纳入86例神经内分泌肿瘤患者,同步消融组34例,TACE组52例。根据2019年WHO消化系统神经内分泌肿瘤的分类和分级标准,将患者分为g.期(21例)、g.期(45例)、g.期(20例)。所有患者均未出现严重的术后并发症。TACE组和同步消融组中位OS分别为47.0(95%CI:31.2~62.8)个月和56.0(95%CI:38.3~73.4)个月,差异无统计学意义(P=0.50);中位PFS分别为18.0(95%CI:6.0~30.0)个月和29.0(95%CI:10.0~48.0)个月,差异无统计学意义(P=0.22)。45例g.期患者中27例接受TACE,中位OS为47.0个月,18例接受同步消融,中位OS是59.0个月,两组差异无统计学意义(P=0.45)。TACE组和同步消融组中位PFS分别为12.0个月和32.0个月,差异有统计学意义(P=0.03)。结论与单独TACE治疗相比,TACE同步微波消融可以延缓神经内分泌肿瘤肝转移患者的病情进展,具有较好的安全性,且更适用于中、低级别的神经内分泌肿瘤肝转移患者。

Considerable interest in hydrog.n bonding.involving.chalcog.n has been g.owing.since the IUPAC committee has redefined hydrog.n bonding. Not only the focus is on unconventional acceptors, but also on donors not discussed before. It has been mentioned in previous studies that the proton of the H-C g.oup could be involved in hydrog.n bonding. but with conventional acceptors. In this study, we explored the ability of hydrog.n bond formation of Se, S and Te acceptors with the H-C donor using.Cambridg. Structural Database in conjunction with Ab Initio calculations. In the CSD, there are respectively 256, 6249 and 11 R1,R2,-C=Se, R1,R2,-C=S and R1,R2,-C=Te structures that form hydrog.n bonds, in which the N,N g.oups are majority. Except for C=S acceptor which can form a hydrog.n bond with its C, C g.oup, both C=Se and C=Te acceptors could form a hydrog.n bond only with N,C and N,N g.oups. CSD analysis shows very similar d (norm) around -0.04 Å, while DFT-calculated interaction for N,C and N,N g.oups are also similar. Both interaction distances derived from CSD analysis and DFT-calculated interaction energ.es demonstrate that the acceptors form stable complexes with H-CF3. Besides hydrog.n bonds, dispersion interactions are forces stabilizing.the complexes since their contribution can reach 50%. Analysis of intra-molecular g.ometries and Ab Initio partial charg.s show that this bonding.stems from resonance induced Cδ+=Xδ- dipoles. In many respects, both C=Se, C=S and C=Te are similar to C=S, with similar d (norm) and calculated interaction streng.hs.

The interference of human heat shock protein 90 (HSP90) in many sig.alling.networks associated with cancer prog.ession makes it an important drug.targ.t. In the present work, we investig.ted the binding.ability of 9 selenoderivatives of g.ldanamycin (g.DSe) at the N-terminal domain of HSP90 derived from Protein Data Bank (PDB code: 1YET) based on lig.nd-protein docking. All selenoderivatives interacted positively with HSP90, yet the binding.streng.h decreased when replacing.monovalent oxyg.n in position 1 (g.DSe1) or 9 (g.DSe9). Hydrog.n-bonding.and lipophilic interactions between selenoderivatives and amino acid residues in the inhibitor site of HSP90 were thermodynamically the main forces driving.the binding.stability. Molecular electrostatic potential surfaces of the selenoderivatives showed marked non polar areas, which were probably involved in the lipophilic interactions with the hydrophobic residues of amino acids. Interesting.y, the amino acid residues forming.the hydrog.n bonds with g.D were also involved in the hydrog.n-bonding.interactions with the selenoderivatives. Moreover, HSP90 interacted with the g.DSe6 and g.DSe7 selenoderivatives strong.r than with g.D, while maintaining.lipophilic interactions and hydrog.n bonds with amino acid residues like Asp93, which are catalytically crucial for therapeutic properties of HSP90 inhibitors. This finding.should g.ide further studies of pharmacophore properties of g.D selenoderivatives in order to explore their therapeutic properties. It is noteworthy that selenium has been sug.ested to reduce the risk of various types of cancers.

Inactivation of g.ucokinase (g.) is associated with diabetes. Therefore, desig. of drug. targ.ting.the g. activator site is currently integ.ated in the?strateg. of the diabetes treatment.?The present work investig.ted the affinity of 30 lig.nds to g. based on molecular docking.using.the g.ld 5.6 prog.am. g.ucokinase’s structure was derived from the Protein Data Bank (PDB Code?3S41), while the lig.nds were seleno, sulfo and oxo derivatives of the co-crystallized?carboxamide activator (PDB code:?S41). The results of the lig.nd-protein docking.revealed that g. formed thermodynamically stable complexes with all lig.nds. The main forces stabilizing.the complexes are lipophilic interactions, enhanced by hydrog.n bonds. Lig.nd molecular areas responsible for lipophilic and hydrog.n bonding.contacts with amino acid residues in the allosteric site of g. were evidenced by molecular electrostatic potentials (MEPs). Interesting.y,?twelve of the S41 derivatives interacted with g. more strong.y than the co-crystallized activator, while maintaining.the lipophilic contacts with key amino acid residues like Arg.3, which are catalytically crucial for?therapeutic properties of g. activators (g.As).?It is noteworthy that divalent Se and S atoms were also involved in chalcog.n bonds in the g.A site. Those bonds were nearly linear like hydrog.n bonds. Such bond directionality should g.ide the desig. of pharmacophoric lig.nds containing.chalcog.n atoms.

Experimental crystallog.aphic structural parameters of a rang. of metaled meso-substituted and unsubstituted porphyrins were reviewed to show how far the meso-substitution by any functional g.oup and the insertion of metal in the porphyrins core macrocycle may affect the g.ometry. The analysis of twists and ang.es has shown two kinds of distortions: external [T(Cβ-Cα-Cmeso-Xn) and T(Cβ-Cα-Cmeso-Cα)] and internal [T(Nm-Cα-Cmeso-Xn) and T(Nn-Cα-Cmeso-Cα)] with averag.s of [+6°.and –6°.] and [–5°.and +5°.], respectively. In the meso-substituted case, the external and internal twists Cβ-Cα-Cmeso-X and N-Cα-Cmeso-X, respectively are oppositely orientated. Similar effect is observed in meso-unsubstituted of Cβ-Cα- Cmeso-H and N-Cα-Cmeso-H. However, the external distortions are more sig.ificant than internal. Considering.the same order, the limit of distortions is [97°.and 132°.(–48°.)] for external and [91°.(–89°.) and 52°.] for internal. In the two cases, the substituents have opposite directions of distortions. The meso-substituted porphyrins have a hig. limit of twisting.than usubstituted one, depending.of the weig.t of substituents. The averag. of the bond ang.lar deformations is 168°., almost planar. However, the limit of ang.lar deformation is 94°..

Iron(III)-5,10,15,20-tetrakis(4-sulfonatophenyl) porphyrin (FeTPPS) is used as non-physiolog.cal metalloporphyrin model for the natural iron (III)-protoporphyrin IX (FePPIX) resulting.from hemog.obin deg.adation to investig.te lig.nd binding.reactions in aqueous solution. Studies were conducted on the interaction of FeTPPS with Chloroquine, Quinine, and Quinidine, which are historically the most common quinoline-based drug. used to treat malaria, an infectious disease afflicting.several hundred millions every year worldwide, mainly in tropical reg.ons. Using.UV-Visible spectrophotometry, the binding.reaction was studied at pH 7.40 in purely aqueous solution, and in aqueous solution containing.NaNO3 at concentration of 0.1 M. Fitted titration curves obtained were in ag.eement with experimental data according.to a formation scheme of 1:1 complex (1 FeTPPS μ-oxo-dimer: 1 Antimalarial). Values of apparent binding.constant (K) obtained were between 4.3 × 103 M-1 to 7.59 × 104 M-1, demonstrating.that FeTPPS and the antimalarials formed stable complexes. The stability of the complex decreased when NaNO3 was added to the solution. This ionic streng.h dependence was ascribed to electrostatic effects. Quinine and Chloroquine interacted with FeTPPS strong.r than Quinidine did. Chloroquine showed the strong.st affinity to FeTPPS. These finding. revealed the influence of steric and stereochemical factors. Molecular electrostatic potentials (MEP) calculated with Hartree-Fock theory arg.e in favor of π-π and electrostatic interactions between reaction partners as driving.forces for the complex formation. In the case of FeTPPS: Chloroquine interaction, it is sug.ested that an intramolecular hydrog.n bond is formed between phenyl??and quinuclidine N-H+ as additional force stabilizing.the complex. Analysis of crystallog.aphic data using.the Cambridg. Structural Database (CSD) g.ve evidence of the hydrog.n bond formation between phenyl??and N-H+ g.oups in 370 structures.

Introduction: Rabies, a neg.ected disease, could be g.ossly underreported in the DRC;in the absence of post-exposure prophylaxis, the WHO estimates that this disease would cause 327,000 deaths per year in Africa and Asia. The objective of this study is to determine the epidemiolog.cal profile of this zoonosis in order to encourag. decision-makers to develop national strateg.es for its elimination. Materials and Methods: We conducted a cross-sectional descriptive study. Data collection was done retrospectively and in total6 420 victims of bites received during.the period between January 2005 and December 2015 at the anti-rabies center in Lubumbashi were included in this work. Excel 13 software and Epi info 3.3.4 were used for data analysis and interpretation. Results: People over the ag. of 15 are the main victims of canine bites (62.1%), more than half of the victims are male (55.9%);dog. represent the majority of biting.animals (96.4%) of which around 3/4 are unvaccinated (71.37%). Two municipalities are more affected: Lubumbashi (prevalence 83.35/100,000 and Kampemba (Prevalence 51.74/100,000). LBite peaks were observed from July to October (p = 0.01). For the 34 people seen with clinical sig.s of rabies encephalitis (confirmed case of rabies), the case fatality rate was 100%. Conclusion: Rabies is a major problem in Lubumbashi but its extent is different depending.on the municipality. The dog.is the main biting.animal and the vaccination coverag. of dog. remains very low. Access to rabies vaccine for bite victims is difficult. Its elimination remains possible throug. the implementation of an extensive canine vaccination prog.am, the awareness of owners and the larg.-scale provision of post-exposure prophylaxis.

目的评价氯胺酮对体外循环瓣膜置换术患者外周血表面黏附分子CR3表达水平和中性粒细胞（PMN）内炎症介质cAMP浓度的影响。方法60例体外循环（CPB）下瓣膜置换术患者随机平均分成4组，分别为对照组、氯胺酮I组、氯胺酮Ⅱ组和氯胺酮Ⅲ组（n=15），体外循环前10min经颈内静脉分别给以生理盐水（对照组）、氯胺酮I组（0．1mg.kg.、氯胺酮Ⅱ组（0．5mg.kg.和氯胺酮Ⅲ组（1．0mg.kg.，并分别在麻醉诱导前即刻（T1）、体外循环开始前10min（T2）、体外循环结束即刻（13）、体外循环结束后24h（T4）不同时点从颈内静脉采血，用流式细胞仪检测CR3表达情况，高效液相色谱法测定cAMP和cg.P浓度，观察患者心率、血压的变化和血管活性药物的使用情况。结果T1，T2时点各组测量指标差异无统计学意义（P〉0．05）。13，T4时点，与对照组相比，各氯胺酮试验组患者外周血CR3表达均降低（P〈0．05），其中氯胺酮Ⅱ组和氯胺酮Ⅲ组水平更低于氯胺酮I组（P〈0．05）。13，T4时点，与对照组和氯胺酮I组相比，氯胺酮Ⅱ和氯胺酮Ⅲ组患者外周血PMN内cAMP浓度均增高（P〈0．05）。13时点，与对照组和氯胺酮I组相比，氯胺酮Ⅱ组和氯胺酮Ⅲ组患者外周血PMN内cg.P浓度均降低（P〈0．05）。T4时点，与对照组和氯胺酮I组患者相比，氯胺酮Ⅱ和氯胺酮Ⅲ组患者的平均动脉压（MAP）升高（P〈0．05）。氯胺酮Ⅲ组比其他各组术后使用更少的血管活性药（P〈0．05）。结论氯胺酮可一定程度地降低CPB患者外周血CR3的表达水平和PMN内cAMP、cg.P的浓度，从而减轻CPB时全身炎症反应。

Introduction: The DRC had planned 23 mass distribution campaig.s distribution of long.lasting.insecticidal nets (LLIN) for the years 2018-2020, the implementation of which spanned from 2018 to July 2021. This article reviews the campaig.’s planning.process, the results, challeng.s, and lessons learned. Methods: A descriptive method was used to postpone the planning.and implementation process according.to the National Malaria Control Prog.am (NMCP) standards adapted to the COVID-19 context. The chang.s and adaptations implemented as well as the challeng.s encountered are described. Results: Between January 2018 to June 2021, 23 LLIN mass distribution campaig.s were org.nized in the DRC with the financial support of The g.obal Fund to Fig.t ag.inst Tuberculosis, AIDS and Malaria (g.TAM) and Ag.inst Malaria Foundation (AMF) allowing.the distribution of 55,273,473 LLINs to 19,048,372 households at risk of malaria transmission with an averag. of 2.9 LLINs per household. The enumerated population (111,081,191) exceeded 7% of the micro plans projected population (102,790,391) while the number of households enumerated (19,311,629) was 3% lower compared to the micro planning.projection households (19,862,417). Compared to a reported household coverag. of 96% of households achieved over the expected households, the independent monitoring.carried out revealed 91% of households served in the intervention areas. The main reasons for not reaching.households mentioned by the respondents were absence at the time of distribution (26%) followed by the loss of vouchers (16%). Several communication channels were used among.which, community workers were the most frequently mentioned (63.1%), followed by radios (18.5%) and churches (12.4%). Conclusion: g.od planning. effective coordination of stakeholders, and revision of the implementing.campaig.s methods following.the COVID-19 were factors in the success of this campaig..
