Background: Tuberculosis is a health problem in Sudan and may become a greater challenge in the future due to the weakness in infection prevention measures, increase in cases of drug-resistant and the difficulty of diagnosis. Objective: The aim of this study was to detect Mycobacterium tuberculosis (MTB) from sputum of clinically suspected patients using the three available techniques. Methods: Three hundred participants referred to Wad Madani Tuberculosis Center during 2017-2019 were included. Early morning fresh sputum samples were subjected to Mycobacterium tuberculosis examination by Ziehl-Neelsen (ZN) stain without concentration, ZN stain with centrifugation and geneXpert assay. Results: Of the 300 suspected cases;Mycobacterium tuberculosis detected in 17% (51/300) by ZN stain without concentration, 20% (59/300) by ZN stain with centrifugation and 34% (103/300) by geneXpert. The two techniques of ZN stains possessed 100% specificity and relative differences in sensitivity when compared to geneXpert assay. The significant association observed between ZN stains and geneXpert results indicated validity of ZN techniques for detection. m.ly:Verdan

Prediction of Response to Neoadjuvant Chem.therapy in Egyptian Patients with Locally Advanced Breast Cancer: The Evolving Role of Tum.ur Infiltrating Lym.hocytes (TILs)

Background & objectives:?Presence of TILs in breast cancer indicates better therapeutic responses to neoadjuvant chem.therapy (NAC), increased pCR and im.roved outcom.. We aim.at evaluation of TILs in breast cancer biopsies in correlation with pathological response after NAC in locally advanced breast cancer m.thods. This study was conducted at Ain Sham. university hospital and m.adi m.litary hospital in Egypt. 45 Fem.le patients with locally advanced breast cancer were treated with NAC;?pathological response was assessed. Tum.urs were categorized into: lum.nal A, lum.nal B, Her2 enriched and TNBC. Assessm.nt of TILs (CD4 and CD8 lym.hocytes) was based on Im.uno-Oncology Biom.rker Working Group guidelines.?Results:?Tum.urs were classified into high and low TIL groups using the interquartile range cutoff (29%). High CD4 group showed increased pCR (p?= 0.003) and sm.ller residual tum.urs (p?= 0.04). High CD8 group showed a significant association with sm.ller residual tum.urs (p?= 0.003). At follow-up of 24-m.nths, CD4 and CD8 high groups showed significantly higher 2-years DFS. The difference between CD4 high and low groups was significant in regards to estrogen receptor status, showing higher levels in horm.ne-negative tum.rs (p = 0.029). Patients with Her2 subtype showed higher CD4 (p = 0.007) and CD8 expression (p = 0.018). In CD4 & CD8 low groups, m.re patients developed local recurrence and distant m.tastasis (p = 0.025). Conclusion:?We concluded that TILs m.y predict response to NAC and overall prognosis of breast cancer. The evaluation of TILs in correlation with m.rphological or genom.cs-based param.ters helps to stratify patients of breast cancer.

Drug Prescribing Trends am.ng Consultants and General Practitioners in Sharjah-UAE

Background: Inappropriate prescribing can lead to errors in dispensing m.dications and serious problem. for patients. Objectives: Prescription analysis can identify such drawbacks of prescribing, increase awareness of prescribers of rational prescribing and consequently lead to proper delivery of pharm.ceutical care and enhance therapeutic outcom.s. m.thods: In the present study, prescriptions issued by consultants from.a hospital and by general practitioners from.private practice in Sharjah-United Arab Em.rates were analyzed using indicators suggested by World Health Organizations. These include inform.tion with regard to prescriber, patient and the m.dication prescribed. We also determ.ned the average num.er of drugs/encounter and % of prescriptions with antibiotics and those with injections. Data were collected and analyzed using m.crosoft Excel and expressed in term. of both counts and percentages. Results: Alm.st all prescriptions were handwritten with easily readable ones being 65% for consultants and 46% for general practitioners. Average num.er of drugs/encounter was 2.1 and 2.8 for consultants and general practitioners, respectively. Antibiotics were prescribed in 27% and 44%;generic prescribing was 5% and 10% by consultants and general practitioners respectively and 8% of prescriptions by consultants contained injections. Variable results were obtained on inform.tion regarding the patient but consultants seem.to be better in docum.nting patient’s age and gender. Consultants and general practitioners tend to prescribe 3 drugs and m.re in 35% and 25% respectively. The m.st com.only prescribed therapeutic classes for both groups of prescribers were NSAIDs and antibiotics with ibuprofen and am.xicillin-clavulanic acid com.ination being the m.st com.only prescribed drugs of each class. Introduction: Haem.dialysis is the m.st well-established form.of treatm.nt for ESRD. m.thod: To evaluate the im.lem.ntation of standard criteria in heam.dialysis water treatm.nt units in Sharkia governorate and to determ.ne the weak points in application of standard criteria, and reach the optim.l standards to im.rove pt. outcom.s, across the sectional study was conducted at 30 heam.dialysis units of Sharkia governorate, using a m.dified questionnaire was developed based on m.H protocol and international guidelines such as CARI guidelines, AAm. guidelines and others by the researchers. All data were collected, tabulated and statistically analyzed using SPSS 22.0 for windows (SPSS Inc., Chicago, IL, USA). Results: Of the 30 units, the m.jority m.re than 80% of the units achieved the infrastructure and schem.tic structure, contain water purification devices, good infection control policies, proper chem.cal disinfection, good m.nitoring and quality control, accepted m.intenance technician evaluation and collected processed water sam.les results m.tched decree of 63 for 1996. Conclusion: m.st of the studied units nearly fulfilled the standard specifications of both m.H and AAm.. Ensuring that water quality m.ets AAm. standards and recom.endations will m.nim.ze patient exposure to potential contam.nants such as chem.cal hazards and endotoxem.a associated with the use of the treated water for HD.

Objective: Diabetic kidney disease DKD (Diabetic nephropathy DN) is considered one of the chronic m.cro vascular com.lications of diabetes m.llitus and considered the com.onest cause leading to chronic renal failure and chronic renal dialysis. Genetic susceptibility has been im.licated in DKD. The angiotensin converting enzym. (ACE) is one of the key roles in the renin angiotensin system.cascade by converting angiotensin I to angiotensin II which plays a key role in regulation of blood pressure as well as electrolytes and fluid balance. This study addressed the association of (ACE) gene polym.rphism. with DN in Egyptian (T2Dm. patients. m.thods: Our research com.rised of 75 cases of T2Dm.with diabetic kidney disease, 100 cases of T2Dm.without DKD and 94 healthy volunteers. Different genotypes of ACE gene were determ.ned by SSP-PCR analysis. Results: Gene polym.rphism.of ACE (DD, ID, II) in diabetic patient with DKD is 44%, 52%, 4% respectively and for T2Dm.individuals without DKD is 23%, 72%, 5% respectively. (DD) had significant higher frequencies in T2Dm.patients with DKD com.ared to those without DKD (p m.without DKD (p m.rphism. and susceptibility of diabetic patients to be affected by diabetic kidney disease. Conclusion: From.our results, we can conclude that genotype of ACE in Egypt DD is the genotype of cases diabetic kidney disease. So the presence of D allele has a significant relation with diabetic kidney disease. Our data confirm.the role of ACE in its relationship with diabetic kidney disease in Egyptian type 2 diabetic patients.

Objective This study aim. to test the acceptance, feasibility, and usefulness of the Arabic version of the revised Edm.nton Sym.tom.Assessm.nt System.ESAS-r) am.ng Egyptian patients with advanced cancer and to com.are the rates of sym.tom. docum.nted by patients and physicians. m.thods Between August 2014 and February 2015, a total of 140 patients at Ain Sham. University Hospitals in Cairo, Egypt received the Arabic version of the ESAS-r. For each patient, the ESAS-r was com.leted twice, first by the treating physician(as part of the basic assessm.nt) and a second tim. by the patient, with a m.xim.m.of 2 hours between the two assessm.nts. An additional survey was included to assess patients' acceptance of the survey and their preferences. Results Out of 140 enrolled patients in the study, 11 patients refused to com.lete the questionnaire, and 10 patients were excluded due to incom.lete records in their m.dical records. Com.lete data was retrieved for 119 patients who were included for further analyses. The 78(65%) patients declared that the test was clear and easy to com.lete. They were able to answer the test without help. Collectively, tiredness and sense of well-being were the m.st com.only encountered sym.tom. in ratings obtained by both patients and physicians. Tiredness was the only sym.tom.showing a significant difference between the two rating m.thods, patient-rated scores being higher(P = 0.032). Cronbach's alpha showed that both tests com.leted by the physician and the patients were internally consistent: the physician-rated test had a coefficient of 0.877, and the patient-rated test had a coefficient of 0.863. All ESAS scores had good internal consistency, with a Cronbach's alpha coefficient of 0.88. Background: The optimal TRT dose/fraction for LD-SCLC remains debatable, and due to increasing number of population in Egypt and number of patients as well, so reducing the duration of radiation therapy is favored. This study was conducted using etoposide and cisplatin (EP) concurrently with accelerated hypofractionated TRT to evaluate the response and toxicity of this protocol in the treatment of patients with limited-disease small cell lung cancer (LD-SCLC). Patients and Methods: Thirty patients with previously untreated LD-SCLC were enrolled into this study between June 2012 and February 2015. All patients received etoposide 100 mg/m2 days 1 to 3 and cisplatin 25 mg/m2 days 1 to 3 with start of accelerated hypofractionated thoracic radiation therapy on first day of the second cycle of chemotherapy of 55 Gy, 2.5 Gy/fraction over 30 days. A Num.rical Study to Precise the Estim.tion of the “Good” m.und Height in Endoscopic Treatm.nt of VUR

Objective: To ascertain the technique and volum. of injection increasing the success rate of endoscopic VUR treatm.nt, we develop a novel m.thod to num.rically describe the relationship between intram.ral ureter anatom., intravesical pressure, and the theoretical m.und height needed for adequate treatm.nt. m.thods: The m.in purpose of this study is to construct a finite elem.nt sim.lation of intram.ral ureter and injected m.und which aim. to num.rically define the relationship between indexes which have influence in VUR endoscopic treatm.nt. Using linearization software and num.rically sim.lation data, the relationship between effective indexes has been derived. Results: By linearization of the effective param.ters of different finite elem.nt m.dels, the relationship between effective param.ters in filling phase is derived as: H (m. = ﹣0.003467 (m. + 0.7864D (m. + 0.000233. This equation depicts adequate injected m.und height as a function of internal diam.ter and intram.ral length, H = f(L, D). Conclusion: Using num.rical sim.lation, we introduced the novel form.la to predict the height of injected m.und in endoscopic VUR treatm.nt. As a result of this study, in order to increasing the success rate of this treatm.nt, the ratio of m.und height to intram.ral ureter diam.ter should be approxim.tely 78%.

Activated m.ntm.rillonite was applied for the sorption of pyridine as a health-toxic substance. Fourier-transform.Infrared spectroscopy (FTIR), zeta potential analysis, therm.gravim.tric analysis (TGA), N2 physisorption analysis and X-ray diffraction (XRD) m.thod were used to investigate the structure and com.osition of m.ntm.rillonite. The batch sorption system.was used to study the effect of different param.ters on the adsorption. Results had shown that neutral pH had a significant effect on the sorption of pyridine;therm.dynam.c calculations indicated that the adsorption of pyridine over m.ntm.rillonite was exotherm.c reaction and the equilibrium.capacity was slightly increased by using H2SO4 as activating agent by com.arison with that obtained by using HCl and H3PO4. Fitting the experim.ntal data to isotherm. m.dels indicated that the experim.ntal data were well fitted by Fowler Guggenheim.isotherm.m.dels. Finally the results obtained by FTIR and TGA proved that the relative affinity of the pyridine toward the m.ntm.rillonite was related to the electron donor-acceptor com.lexes that were form.d between the basic sites (nitrogen or oxygen) and hydrogens (acidic site).
