Objective:Simple tests like direct smear of the acid fast bacilli(AFB) and Mycobacterium culture could assist the diagnosis of tuberculosis.This study is aimed at reviewing the outcome of smears,culture results and contamination rate among specimens requested for AFB smear and Mycobacterium culture.Methods:Retrospective laboratory data analysis requesting for Mycobacterium culture from January 2005 till December 2006 was done in a tertiary teaching hospital of Universiti Sains Malaysia,Kubang Kerian,Kelantan,Malaysia.Results: Four hundred and sixty seven(36.6%) isolates grew from 1 277 specimens.Of these isolates,314 (67.2%) grew Mycobacterium tuberculosis,23(4.9%) grew Mycobacterium other than tuberculosis and 38 (8.1%) grew contaminants.Among the M.tuberculosis cultures,165(52.5%) had growth of more than 100 confluent colonies,whereas 39 cultures(12.4%) had growth of less than 19 colonies.Direct smear for AFB among smear positive cases showed presence of more than 50 bacilli/line in 231(49.5%) cases and smear negative cases accounted for 63(13.5%).Among smear positive cases,291(94.5%) cultures grew Mycobacterium species and another 17(5.5%) cultures grew contaminants.In smear negative cases,32(62.7%) cultures grew Mycobacterium species and 19(37.3%) cultures grew contaminants.Conclusion:The results from data analysis of the Mycobacterium cultures should be critically utilized in order to review the laboratory performance and to improve its services in the future.Some of the data is also useful to the administrators of the hospital in terms of estimating the risk of occupational hazard faced by the health care workers.

Background:The European Society for Medical Oncology(ESMO)guidelines are among the most comprehensive and widely used clinical practice guidelines(CPGs)globally.However,the level of scientific evidence supporting ESMO CPG recommendations has not been systematically investigated.This study assessed ESMO CPG levels of evidence(LOE)and grades of recommendations(GOR),as well as their trends over time across various cancer settings.Methods:We manually extracted every recommendation with the Infectious Diseases Society of America(IDSA)classification from each CPG.We examined the distribution of LOE and GOR in all available ESMO CPG guidelines across different topics and cancer types.Results:Among the 1,823 recommendations in the current CPG,30%were classified as LOEⅠ,and 43%were classified as GOR A.Overall,there was a slight decrease in LOEⅠ(−2%)and an increase in the proportion of GOR A(+1%)in the current CPG compared to previous versions.The proportion of GOR A recommendations based on higher levels of evidence such as randomized trials(LOEⅠ–Ⅱ)shows a decrease(71%vs.63%,p=0.009)while recommendations based on lower levels of evidence(LOEⅢ–Ⅴ)show an increase(29%vs.37%,p=0.01)between previous and current version.In the current versions,the highest proportion of LOEⅠ(42%)was found in recommendations related to pharmacotherapy,while the highest proportion of GOR A recommendations was found in the areas of pathology(50%)and diagnostic(50%)recommendations.Significant variability in LOEⅠand GOR A recommendations and their changes over time was observed across different cancer types.Conclusion:One-third of the current ESMO CPG recommendations are supported by the highest level of evidence.More well-designed randomized clinical trials are needed to increase the proportion of LOEⅠand GOR A recommendations,ultimately leading to improved outcomes for cancer patients.

Melatonin is a versatile indolamine synthesized and secreted by the pineal gland in response to the photoperiodic information received by the retinohypothalamic signaling pathway.Melatonin has many benefits,such as organizing circadian rhythms and acting as a powerful hormone.We aimed to show the antitumor effects of melatonin in both in vivo and in vitro models through the mammalian target of rapamycin(mTOR)signaling pathway and the argyrophilic Nucleolar Regulatory Region(AgNOR),using the Microcomputed Tomography(Micro CT).Ehrlich ascites carcinoma(EAC)cells were administered into the mice by subcutaneous injection.Animals with solid tumors were injected intraperitoneally with 50 and 100 mg/kg melatonin for 14 days.Volumetric measurements for the taken tumors were made with micro-CT imaging,immunohistochemistry(IHC),real-time polymerase chain reaction(PCR)and AgNOR.Statistically,the tumor tissue volume in the Tumor+100 mg/kg melatonin group was significantly lower than that in the other groups in the data obtained from micro-CT images.In the IHC analysis,the groups treated with Tumor+100 mg/kg melatonin were compared when the mTOR signaling pathway and factor 8(F8)expression were compared with the control group.It was determined that there was a significant decrease(parea/nuclear area(TAA/NA)ratio in the treatment groups(paracterization of the melatonin-mediated cancer inhibitory effects will be valuable in advancing our fundamental molecular understanding and

利用太阳能将CO_(2)光转化为化学燃料是前沿研究的热点.当前,科研的重点在于开发和优化各类光催化材料的性能.异质结构不仅用于增强无机半导体催化剂的活性,而且用于金属-有机框架(MOFs)光催化剂的构建.随着异质结技术的不断进步,新的相结概念应运而生.尽管关于无机半导体中的相结研究已取得一定进展,但是MOFs相结结构的探索尚处于起步阶段.本文中,我们成功设计并构建了一种具有卓越光催化CO_(2)还原性能的MOFs相结杂化材料.这种材料由具有高度连接性的卟啉配体(TCPP-Ni)与Zr6团簇紧密结合而成,在溶剂热反应条件下,PCN-222-Ni能够快速且均匀地成核.通过精准调控调节剂的用量,我们实现了同分异构体PCN-224-Ni在类似条件下较慢的晶体形成过程.最终制得的P@P杂化材料展现出高表面积、多孔性和卓越的结晶度.由于PCN-222-Ni和PCN-224-Ni之间的带隙匹配,光生载流子的分离和迁移得以促进,从而使P@P杂化材料展现出卓越的光还原CO_(2)性能.这一重大突破不仅标志着MOFs相结结构研究的重大进展,还拓宽了MOFs相结在CO_(2)光转化领域的应用前景.

BACKGROUND The quality-adjusted life year(QALY)is a metric that is increasingly used today in the field of health economics to evaluate the value of different medical treatments and procedures.Surgical waiting lists(SWLs)represent a pressing problem in public healthcare.The QALY measure has rarely been used in the context of surgery.It would be interesting to know how many QALYs are lost by patients on SWLs.AIM To investigate the relationship between QALYs and SWLs in a systematic review of the scientific literature.METHODS The study was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Statement.An unlimited search was carried out in PubMed,updated on January 19,2024.Data on the following variables were investigated and analyzed:Specialty,country of study,procedure under study,scale used to measure QALYs,the use of a theoretical or real-life model,objectives of the study and items measured,the economic value assigned to the QALY in the country in question,and the results and conclusions published.RESULTS Forty-eight articles were selected for the study.No data were found regarding QALYs lost on SWLs.The specialties in which QALYs were studied the most in relation to the waiting list were urology and general surgery,with 15 articles each.The country in which the most studies of QALYs were carried out was the United States(n=21),followed by the United Kingdom(n=9)and Canada(n=7).The most studied procedure was organ transplantation(n=39),including 15 kidney,14 liver,5 heart,4 lung,and 1 intestinal.arthroplasty(n=4),cataract surgery(n=2),bariatric surgery(n=1),mosaicplasty(n=1),and septoplasty(n=1)completed the surgical interventions included.Thirty-nine of the models used were theoretical(the most frequently applied being the Markov model,n=34),and nine were real-life.The survey used to measure quality of life in 11 articles was the European Quality of Life-5 dimensions,but in 32 articles the survey was not specified.The willingness-to-pay per QALY gained ranged fro

The electrochemical CO_(2) reduction reaction(CO_(2)RR) to controllable chemicals is considered as a promising pathway to store intermittent renewable energy. Herein, a set of catalysts based on copper-nitrogendoped carbon xerogel(Cu-N-C) are successfully developed varying the copper amount and the nature of the copper precursor, for the efficient CO_(2)RR. The electrocatalytic performance of Cu-N-C materials is assessed by a rotating ring-disc electrode(RRDE), technique still rarely explored for CO_(2)RR. For comparison, products are also characterized by online gas chromatography in a H-cell. The as-synthesized Cu-NC catalysts are found to be active and highly CO selective at low overpotentials(from -0.6 to -0.8 V vs.RHE) in 0.1 M KHCO_(3), while H_(2) from the competitive water reduction appears at larger overpotentials(-0.9 V vs. RHE). The optimum copper acetate-derived catalyst containing Cu-N_(4) moieties exhibits a CO_(2)-to-CO turnover frequency of 997 h^(-1) at -0.9 V vs. RHE with a H_(2)/CO ratio of 1.8. These results demonstrate that RRDE configuration can be used as a feasible approach for identifying electrolysis products from CO_(2)RR.

This study presents a comprehensive approach to applying texturing processes created by engraving on the surface of CoCr28Mo alloy workpieces using a 20 W pulsed nanosecond fiber laser.The hatch strategy and distance,frequency,and scan speed were control parameters for texturing applications.The effectiveness of the parameters in terms of roughness and contact angle of the texturized surface was investigated.Surface roughness and contact angle were analyzed using variance analysis to identify each variable's influence.It has been determined that the roughness of the texture defined by the hatch strategy plays a decisive role in the wettability behavior of the surface;however,the scan speed,frequency,and hatch distance which are among the laser surface texturing conditions are influential in the roughness and contact angle.Increasing scan speed and hatch distance while decreasing frequency resulted in smoother surfaces,increasing the contact angle.Textures having rough surfaces produced with different processing conditions exhibit a super hydrophilic behavior.The contact angle is most sensitive to the hatch distance;however,the frequency has the least influence on the contact angle.The most and least efficient surface roughness parameters are revealed to be scan speed and hatch distance,respectively.
