This study focused on the encapsulation of vancomycin(VAN) into liposomes coated with a red blood cell membrane with a targeting ligand, daptomycin–polyethylene glycol–1,2-distearoyl-sn-glycero-3-phosphoethanolamine, formed by conjugation of DAPT and Nhydroxysuccinimidyl-polyethylene glycol-1,*** formulation is capable of providing controlled and targeted drug delivery to the bacterial cytoplasm. We performed malDI-TOF, NmR and FTIR analyses to confirm the conjugation of the targeting ligand via the formation of amide bonds. Approximately 45% of VAN could be loaded into the aqueous cores, whereas 90% DAPT was detected using UV–vis spectrophotometry. In comparison to free drugs, the formulations controlled the release of drugs for > 72 h. Additionally, as demonstrated using CLSm and flow cytometry, the resulting formulation was capable of evading detection by macrophage cells. In comparison to free drugs, red blood cell membrane–DAPT–VAN liposomes, DAPT liposomes, and VAN liposomes reduced the mIC and significantly increased bacterial permeability, resulting in > 80% bacterial death within 4 h. Cytotoxicity tests were performed in vitro and in vivo on mam.alian cells,in addition to hemolytic activity tests in human erythrocytes, wherein drugs loaded into the liposomes and RBCDVL exhibited low toxicity. Thus, the findings of this study provide insight about a dual antibiotic targeting strategy that utilizes liposomes and red blood cell membranes to deliver targeted drugs against mRSA.

Association of <i>Clostridium difficile</i>with Antibiotic Associated Diarrhea among Hospitalized Children in Diyala-Iraq

Background: Clostridium difficile infection (CDI) is an increasingly important cause of morbidity in hospitalized children. Absence of clinical suspicion and suboptimum laboratory diagnostic methods are behind the misdiagnosed infections. Objectives: To determine the association of Cl. difficle infection among hospitalized children suspected of having antibiotic associated diarrhea (AAD) plus detection of the bacterium’s toxins A and B and the enzyme glutamate dehydrogenase (GDH). Patients and methods: This cross-sectional study was conducted in al-Batool Hospital for maternity and Children in Baquba City for the period from march 2017 to April 2018. Sixty stool samples were collected from children inpatients. The age range was 15 days up to one year. 41 (68.3%) and 19 (31.7%) were males and females respectively. Additionally, 20 healthy children were enrolled as control group. The age range was 50 days up to one year, 12 (60%) and 8 (40%) were males and females respectively. Special questionnaire was preconstructed for collection of demographic information. Isolation of Cl. difficile was carried out on Colombia blood agar and tryptose sulfite cycloserine agar. Enzyme linked immunosorband assays were used for the detection of toxin A and B (CerTest-Biotec, Spain), and for the detection of glutamate dehydrogenase enzyme (CerTest-Biotec, Spain). Human privacy was respected by obtaining the parents’ oral consent. Statistical analyses were done using SPSS Version 18 and P values less than 0.05 were considered significant. Results: The isolation rate of Cl. difficile form patients and healthy children was 11.7% and 5% respectively. The toxins detection rate among patients was 23.3%, of these 35.7% for toxin A, and 64.3% for toxin A and B together. Neither of the patients’ specimens was positive for toxin B alone, nor was healthy control positive for all toxins. Bowel perforation due to break and distal passage of the safety ring of an adjustable intra-gastric balloon: A potentially life threatening situation

A 45-year-old man of middle Eastern origin, morbid obese, with a body mass index of 39 had an intragastric balloon, filled with 500 mL of saline/methylene blue and intended as definite therapy, inserted some8 wk previously. He was admitted to the emergency department with abdominal cramps. An ultrasound of the abdomen was performed in ER which confirmed the balloon to be in place without any *** was discharged home on symptomatic *** remains symptomatic therefore he reported back to ER 2 d later. Computed tomography scan was performed this time for further evaluation which revealed a metallic ring present in the small bowel while the intra-gastric balloon was in its proper *** was no clinical or radiological sign of intestinal obstruction. Patient was hospitalized for observation and conservative management. The following night,patient experienced sudden and severe abdominal pain, therefore an X-ray of the abdomen in erect position was done, which showed free air under the right dome of diaphragm. Patient was transferred to O.R for emergency laparotomy. There were two small perforations identified at the site of the metallic ring entrapment. The ring was removed and the perforations were repaired. Due to increasing prevalence of obesity and advances in modalities for its management,physicians should be aware of treatment options, their benefits, complications and clinical presentation of the known complications. Physicians need to be updated to approach these complications within time, to avoid lifethreatening situations caused by these appliances.

Epidemiology and Clinical Characteristics of the First 500 Positive Cases of COVID-19. A multicenter Retrospective Study across the Najran Region of the Kingdom of Saudi Arabia (KSA)

Introduction: Coronavirus Disease 2019 (COVID-19) is a viral infection that was first reported in Wuhan, China on 31 December 2019. This study aimed to clarify the epidemiology and clinical characteristics of 500 first COVID-19 in the Najran region, Saudi Arabia. material and methods: A multi-center retrospective study design was employed to study the first 500 confirmed COVID-19 positive cases in Najran province, Kingdom of Saudi Arabia (KSA). Data were collected from 1 march 2020 until 1 July 2020 and provided by the Infection Prevention and Control (IPC) department from the hospitals. Included cases were confirmed using real-time reverse transcriptase-polymerase chain reaction (RT-PCR). Demographic, vital signs, symptoms, incubation period, travel or exposure history medical history, and comorbidities were collected. Logistic regression analysis was used to explore the association between potential risk factors associated with symptoms occurrence of COVID-19. Results: The median age of 500 COVID-19 patients was 31 years;333 (66.6%) males. A total of 34 (6.8%) were Healthcare Workers (HCWs). Out of the 500 patients, 180 (36%) had at least one comorbid disease. The most common symptoms on admission were fever 281 (56.2%), cough 266 (53.2%), shortness of breath 166 (33.2%), and malaise 113 (22.6%). most of the patients presented with mild disease severity 310 (62%). Nationality, age, and Diabetes miletus (Dm) were independently and significantly associated with being symptomatic (P mpared to Saudi nationals, other nationality patients were most likely to have symptoms (m>βm> = 2.968, CI = 2.002 - 4.400, P = 0.0010). For every 1 year increase in age, the risk of being symptomatic increased by 5.8% (m>βm> = 1.045, CI = 1.033 - 1.058, P = 0.001). Patients with nonerosive gastroesophageal reflux disease often have relatively low esophageal acid exposure and respond suboptimally to gastric acid suppressi on. In these patients, other constituents of gastric contents may induce esophag eal *** have demonstrated that gastric contents can cause heartburn when the gastric pH > 4. The aim of this stu dy was to determine relative sensitivities to chenodeoxycholic and ursodeoxychol ic acids, and 0.1 N HCl, administered as provocative perfusion tests. Patients w ith functional heartburn and healthy control subjects were evaluated. Patients u nderwent a modified Bernstein acid infusion test and esophageal Barostat balloon distention. Time and volume to pain were recorded. Barostat balloon distention was performed using our standard ***- wise distentions were performed and pain was *** to chenodeox ycholic acid (Cheno) and Ursodeoxycholic acid (Urso) were assessed similarly to the Bernstein test using 2 mm concentrations of each, followed immediately by 5 mm if no pain was reported with 2 mm. Volume of bile acid infusion and length of time until pain was induced were assessed and compared to the same endpoints fo r acid sensitivity. “Total"time and “total" volume to induce pain were calcula ted for Cheno and Urso. Least-squares means were generated and twotailed t-tes ts and regression analyses were performed (Pal heartburn patients and six healthy contr ols were evaluated (3m, 13 F; age range, 19 to 56 years). Since five of six cont rols had pain with ac- id infusion(hypersensitive), all subjects were analyzed as one group. Only thr ee subjects (all controls) had no pain with infusion of 2 mm Cheno and received the follow-up infusion of 5 mm.
