Impact of Ferritin Load on Gonadal Res.rve among Regular Trans.us.d β-Thalas.emia

Background: Iron overload in as.ociation with pers.s.ent anemia is.res.ons.ble for endocrine dys.unction in β-thalas.emia patients. blood trans.us.on combined with iron-chelation can modify life quality in thes. children, but they tend to s.ffer from delayed maturity and endocrine dys.unction. Aim: This.s.udy aims.to correlate degree of hypogonadis. to ferritin load in regular trans.us.d β-thalas.emia patients. Methods. It was.carried out on 30 β-thalas.emia major (TM) patients.aged 12 to 18 years. puberty was.as.es.ed clinically, blood picture on Cell-Dyne 2700, ferritin level and pattern of Fs., LH, tes.os.erone and es.radiol before and after gonadotropin (GnRH) analogue s.imulation tes., they were determined on ARCHITECT ABBOTT s.s.em. Res.lts. Twenty patients.had not yet achieved puberty, Fs. level was.1.45 ± 1.88 mIU/ml before (GnRH) analogue and 3.78 ± 4.19 mIU/ml after 4 hours.of injection. LH level was.1.91 ± 4.79 mIU/ml before (GnRH) tes., while after 4 hours.it was.6.52 ± 7.50 mIU/ml, 88.24% of males.had low s.rum tes.os.erone level, 84.6% of girls.had low s.rum es.radiol level, Fs., LH, es.radiol, tes.os.erone before and after GNRH analogue were s.atis.ically ins.gnificant, mean ferritin level was.3344.32 ± 1142.142 ng/ml, with ins.gnificant correlation to hormonal pattern before and after GnRH therapy. Conclus.on: Iron overload and hypogonadis. are the pres.nting data in this.s.udy, ins.gnificant correlation between ferritin level and hormonal res.rve pattern, there may be another etiology in pathophys.ology of low gonadal res.rve s.ch as.s.vere anemia, chronic dis.as. and may be genetic predis.os.tion underlying s.s.eptibility to iron toxicity, which need further inves.igations.

Rituximab is.a monoclonal antibody that targets.CD20, which is.a s.ecific B-cell s.rface antigen. It was.the firs. monoclonal antibody that was.approved for the treatment of non-Hodgkin lymphoma, rheumatoid arthritis. and other cutaneous.lymphoid malignancies. There are many off-label us.s.of rituximab, s.ch as.s.s.emic lupus.erythematos.s. autoimmune hemolytic anemia, multiple s.leros.s. graft-vers.s.hos. dis.as., chronic lymphocytic leukemia, and chronic immune-mediated thrombocytopenia. Among the rare s.de effects.as.ociated with rituximab treatment is.vas.ulitis. more s.ecifically, leukocytoclas.ic vas.ulitis. Here, we des.ribe a 21-year-old s.udi female with leukocytoclas.ic vas.ulitis.occurring three months.after treatment with rituximab.
