Postoperativeinte rictalepil eptiform discharges predict seizure recurrence after antiepileptic drug withdrawal regardless of concordance with surgical site

Objective The study aimed to explore the association between the site of interictal epileptic discharges(IEDs)on postoperative electroencephalogram(EEG)and seizure recurrence after antiepileptic drug(AED)withdrawal.The study hypothesizes that the concordance of IED sites with surgical sites indicates incomplete resection of epileptic focus,while non-concordance of IED sites with surgical sites indicates postoperative changes or cortical stimulation.The former has a higher risk of seizure recurrence.Methods We retrospectively analyzed the postoperative EEG pattern of 182 consecutive children who underwent resection surgery.To identify the risk factors for seizure recurrence,we compared the attributes of seizure recurred and seizure-free groups by univariate and multivariate analyses.AED tapering was standardized,involving a 25% reduction in the dose of a single type of AED every 2 weeks,independent of the presurgical AED load.Results We attempted AED withdrawal in 116(63.7%)children.Twenty-eight(24.1%)children experienced seizure recurrence during or after AED withdrawal.A greater number of AEDs used at the time of surgery(p=0.005),incomplete resection(p=0.001),and presence of IED on postoperative EEG(p=0.011)are predictors of seizurerecurrence.Thecompletenessof resectionand seizure recurrence after AED withdrawal were related to the presence of IED on the EEG,but not to the concordance of IED with surgical sites.Conclusion For children with abnormal EEG,the decision to discontinue AED should be made more cautiously,regardless of the relative location of the discharge site and the surgical site.

Development and validation of a model integrating clinical and coronary lesion-based functional assessment for longterm risk prediction in PCI patients

OBJECTIVES To establish a scoring system combining the ACEF score and the quantitative blood flow ratio(QFR) to improve the long-term risk prediction of patients undergoing percutaneous coronary intervention(PCI).METHODS In this population-based cohort study, a total of 46 features, including patient clinical and coronary lesion characteristics, were assessed for analysis through machine learning models. The ACEF-QFR scoring system was developed using 1263consecutive cases of CAD patients after PCI in PANDA Ⅲ trial database. The newly developed score was then validated on the other remaining 542 patients in the cohort.RESULTS In both the Random Forest Model and the Deep Surv Model, age, renal function(creatinine), cardiac function(LVEF)and post-PCI coronary physiological index(QFR) were identified and confirmed to be significant predictive factors for 2-year adverse cardiac events. The ACEF-QFR score was constructed based on the developmental dataset and computed as age(years)/EF(%) + 1(if creatinine ≥ 2.0 mg/d L) + 1(if post-PCI QFR ≤ 0.92). The performance of the ACEF-QFR scoring system was preliminarily evaluated in the developmental dataset, and then further explored in the validation dataset. The ACEF-QFR score showed superior discrimination(C-statistic = 0.651;95% CI: 0.611-0.691, P libration(Hosmer–Lemeshow χ^(2)= 7.070;P = 0.529) for predicting 2-year patient-oriented composite endpoint(POCE). The good prognostic value of the ACEF-QFR score was further validated by multivariable Cox regression and Kaplan–Meier analysis(adjusted HR = 1.89;95% CI: 1.18–3.04;log-rank P linical and coronary lesion-based functional variables(ACEF-QFR)was developed, and its ability for prognostic prediction in patients with PCI was further validated to be significantly better than the post-PCI physiological

I-DCGAN and TOPSIS-IFP:A simulation generation model for radiographic flaw detection images in light alloy castings and an algorithm for quality evaluation of generated images

The intelligent detection technology driven by X-ray images and deep learning represents the forefront of advanced techniques and development trends in flaw detection and automated evaluation of light alloy castings.However,the efficacy of deep learning models hinges upon a substantial abundance of flaw samples.The existing research on X-ray image augmentation for flaw detection suffers from shortcomings such as poor diversity of flaw samples and low reliability of quality evaluation.To this end,a novel approach was put forward,which involves the creation of the Interpolation-Deep Convolutional Generative Adversarial Network(I-DCGAN)for flaw detection image generation and a comprehensive evaluation algorithm named TOPSIS-IFP.I-DCGAN enables the generation of high-resolution,diverse simulated images with multiple appearances,achieving an improvement in sample diversity and quality while maintaining a relatively lower computational complexity.TOPSIS-IFP facilitates multi-dimensional quality evaluation,including aspects such as diversity,authenticity,image distribution difference,and image distortion degree.The results indicate that the X-ray radiographic images of magnesium and aluminum alloy castings achieve optimal performance when trained up to the 800th and 600th epochs,respectively.The TOPSIS-IFP value reaches 78.7%and 73.8%similarity to the ideal solution,respectively.Compared to single index evaluation,the TOPSIS-IFP algorithm achieves higher-quality simulated images at the optimal training epoch.This approach successfully mitigates the issue of unreliable quality associated with single index evaluation.The image generation and comprehensive quality evaluation method developed in this paper provides a novel approach for image augmentation in flaw recognition,holding significant importance for enhancing the robustness of subsequent flaw recognition networks.

Naturally occurred precore(PC,G1896A)and/or basal core promoter(BCP,A1762T/G1764A)mutations are prevalent in chronic HBV-infected patients,especially those under HBeAg-negative status.However,the replicative capacity of HBV with PC/BCP mutations remains ambiguous.Herein,meta-analysis showed that,only under HBeAg-negative status,the serum HBV DNA load in patients with PC mutation was 7.41-fold higher than those without the mutation.Both PC mutation alone and BCPþPC mutations promoted HBV replication in cell and hydrodynamic injection mouse models.In human hepatocyte chimeric mouse model,BCPþPC mutations led to elevated replicative capacity and intrahepatic core protein accumulation.Mechanistically,preC RNA harboring PC mutation could serve as mRNA to express core and P proteins,and such pgRNA-like function favored the maintenance of cccDNA pool under HBeAg-negative status.Additionally,BCPþPC mutations induced more extensive and severe human hepatocyte damage as well as activated endoplasmic reticulum stress and TNF signaling pathway in livers of chimeric mice.This study indicates that HBeAg-negative patients should be monitored on HBV mutations regularly and are expected to receive early antiviral treatment to prevent disease progression.

Extracorporeal membrane oxygenation in adult patients with sepsis and septic shock: Why, how, when, and for whom

Sepsis and septic shock remain the leading causes of death in intensive care units.Some patients with sepsis fail to respond to routine treatment and rapidly progress to refractory respiratory and circulatory failure,necessitating extracorporeal membrane oxygenation(ECMO).However,the role of ECMO in adult patients with sepsis has not been fully established.According to existing studies,ECMO may be a viable salvage therapy in carefully selected adult patients with sepsis.The choice of venovenous,venoarterial,or hybrid ECMO modes is primarily determined by the patient’s oxygenation and hemodynamics(distributive shock with preserved cardiac output,septic cardiomyopathy(left,right,or biventricular heart failure),or right ventricular failure caused by acute respiratory distress syndrome).Veno-venous ECMO can be used in patients with sepsis and severe acute respiratory distress syndrome when conventional mechanical ventilation fails,and early application of veno-arterial ECMO in patients with sepsis-induced refractory cardiogenic shock may be critical in improving their chances of survival.When ECMO is indicated,the choice of an appropriate mode and determination of the optimal timing of initiation and weaning are critical,particularly in an experienced ECMO center.Furthermore,some special issues,such as ECMO flow,anticoagulation,and antibiotic therapy,should be noted during the management of ECMO support.

Podostemaceae(riverweed family),one of the largest aquatic plant families in the world,comprises about 350 species in 51 genera placed in three subfamilies,Podostemoideae,Weddellinoideae,and Tristichoideae(Koi et al.,2022).Most Podostemaceae genera(~38)are monotypic or oligotypic,with a high level of endemism(Koi et al.,2018,2022).Members of Podostemaceae adhere to rocks in fast-flowing rivers,streams,or waterfalls in the subtropics and tropics(Koi et al.,2018).As an adaptation to fastrunning currents,the morphological structure of river-weeds tends to be extremely reduced,including a loss or reduction of primary shoots or roots(Koi and Kato,2007).The vegetative mass of these plants resembles lichens or bryophytes,and are thus known commonly as“dream bryophytes”(Philbrick and Retana,1998).Podostemaceae plants flower when the water level drops during the dry season.In recent years,anthropogenic factors and climate change have severely affected river weed habitats,decreasing populations of these plants.

Antihypertensive effects of whey protein hydrolysate involve reshaping the gut microbiome in spontaneously hypertension rats

Novel angiotensin-converting enzyme(ACE)inhibitory peptides were identified from whey protein hydrolysates(WPH)in vitro in our previous study and the antihypertensive abilities of WPH in vivo were further investigated in the current study.Results indicated that WPH significantly inhibited the development of high blood pressure and tissue injuries caused by hypertension.WPH inhibited ACE activity(20.81%,Plic blood pressure(SBP)(12.63%,Pling pathway and Apelin signaling pathway were altered after WPH supplementation.Notably,WPH alleviated serum oxidative stress,indicated by the decreased malondialdehyde content(Plities in vivo and could be a potential alternative for antihypertensive dietary supplements.

Supramolecular polymer-based gel fracturing fluid with a double network applied in ultra-deep hydraulic fracturing

A gel based on polyacrylamide, exhibiting delayed crosslinking characteristics, emerges as the preferred solution for mitigating degradation under conditions of high temperature and extended shear in ultralong wellbores. High viscosity/viscoelasticity of the fracturing fluid was required to maintain excellent proppant suspension properties before gelling. Taking into account both the cost and the potential damage to reservoirs, polymers with lower concentrations and molecular weights are generally preferred. In this work, the supramolecular action was integrated into the polymer, resulting in significant increases in the viscosity and viscoelasticity of the synthesized supramolecular polymer system.The double network gel, which is formed by the combination of the supramolecular polymer system and a small quantity of Zr-crosslinker, effectively resists temperature while minimizing permeability damage to the reservoir. The results indicate that the supramolecular polymer system with a molecular weight of(268—380) × 10～4g/mol can achieve the same viscosity and viscoelasticity at 0.4 wt% due to the supramolecular interaction between polymers, compared to the 0.6 wt% traditional polymer(hydrolyzed polyacrylamide, molecular weight of 1078 × 10～4g/mol). The supramolecular polymer system possessed excellent proppant suspension properties with a 0.55 cm/min sedimentation rate at 0.4 wt%, whereas the0.6 wt% traditional polymer had a rate of 0.57 cm/min. In comparison to the traditional gel with a Zrcrosslinker concentration of 0.6 wt% and an elastic modulus of 7.77 Pa, the double network gel with a higher elastic modulus(9.00 Pa) could be formed only at 0.1 wt% Zr-crosslinker, which greatly reduced the amount of residue of the fluid after gel-breaking. The viscosity of the double network gel was66 m Pa s after 2 h shearing, whereas the traditional gel only reached 27 m Pa s.

Breaking the efficiency limitations of dissipative Kerr solitons using nonlinear couplers

Dissipative Kerr solitons(DKS) have long been suffering from poor power conversion efficiency when driven by continuous-wave lasers. By deriving the critical coupling condition of a multimode nonlinear optics system in a generalized theoretical framework,two efficiency limitations of the conventional pump method of DKS are revealed: the effective coupling rate is too small and is also power-dependent. A general approach is provided to resolve this challenge by introducing two types of nonlinear couplers to couple the soliton cavity and CW input through nonlinear processes. The collective coupler opens multiple coupling channels and the self-adaptive coupler builds a power-independent effective external coupling rate to the DKS for approaching the generalized critical coupling condition, which promises near-unity power conversion efficiencies. For instance, a conversion efficiency exceeding 90% is predicted for aluminum nitride microrings with a nonlinear coupler utilizing second-harmonic generation. The mechanism applies to various nonlinear processes, including Raman and Brillouin scattering, and thus paves the way for micro-solitons toward practical applications.

Bamboo plants are an essential component of tropical ecosystems,yet their vulnerability to climate extremes,such as drought,is poorly understood due to limited knowledge of their hydraulic properties.Cephalostachyum pergracile,a commonly used tropical bamboo species,exhibited a substantially higher mortality rate than other co-occurring bamboos during a severe drought event in 2019,but the underlying mechanisms remain unclear.This study investigated the leaf and stem hydraulic traits related to drought responses,including leaf-stem embolism resistance(P50leaf;P50stem) estimated using optical and X-ray microtomography methods,leaf pressure-volume and water-releasing curves.Additionally,we investigated the seasonal water potentials,native embolism level(PLC) and xylem water source using stable isotope.We found that C.pergracile exhibited strong resistance to embolism,showing low P50leaf,P50stem,and turgor loss point,despite its rapid leaf water loss.Interestingly,its leaves displayed greater resistance to embolism than its stem,suggesting a lack of effective hydraulic vulnerability segmentation(HVS) to protect the stem from excessive xylem tension.During the dry season,approximately 49% of the water was absorbed from the upper 20-cm-deep soil layer.Consequently,significant diurnal variation in leaf water potentials and an increase in midday PLC from 5.87±2.33% in the wet season to 12.87±4.09%in the dry season were observed.In summary,this study demonstrated that the rapid leaf water loss,high reliance on surface water,and a lack of effective HVS in C.pergracile accelerated water depletion and increased xylem embolism even in the typical dry season,which may explain its high mortality rate during extreme drought events in 2019.
