Effectiveness of moxibustion alone on lumbar disc herniation:a Meta-analysis of randomized controlled trials

OBJECTIVE:To evaluate the available evidence from randomized controlled trials(RCTs)of moxibustion alone for lumbar disc herniation(LDH)treatment.METHODS:A systematic search of 10 databases(until August 30,2021)was used to identify studies that reported the response rate,visual analogue scale(VAS)score,Japanese Orthopedic Association(JOA)score,and Oswestry Disability Index(ODI)score.Study selection and data extraction were independently performed by two reviewers.Cochrane criteria for risk of bias were used to assess the methodological quality of the trials.The Grading of Recommendations Assessment,Development,and Evaluation Methodology(GRADE)were also used to test the quality of the result evidence.RESULTS:Nineteen RCTs,including 1888 patients,met the inclusion criteria.Five studies showed no difference between moxibustion and acupuncture on response rate[risk ratio(RR)=1.07,95%CI(0.98,1.16),P=0.11].Meanwhile,six studies suggested that there is no significant difference between moxibustion and acupuncture on VAS score[mean difference(MD)=-0.43,95%CI(-0.91,0.05),P=0.08].Eight studies implied that there is no significant difference between moxibustion and acupuncture on JOA score[MD=0.84,95%CI(-1.27,2.96),P=0.44].Two studies indicated that moxibustion may have equivalent effects for treating LDH in the VAS score in comparison with drug therapy[MD=-1.16,95%CI(-2.63,0.31),P=0.12].The evidence level of results was determined to be very low to low.CONCLUSIONS:Based on the existing evidence,moxibustion may not be suitable for treating LDH alone,but it may be applied as an adjuvant treatment.Furthermore,well-designed RCTs with high quality and larger samples are still needed to evaluate the efficacy and safety of moxibustion alone for LDH treatment.

煤气化细渣浮选行为及工艺研究

针对煤气化细渣浮选回收时浮选药剂消耗量大的问题，以陕西榆林某地煤气化细渣为研究对象，通过全粒级浮选、以及对全粒级浮选产品的粒度分析与各粒级浮选完善指标和可燃体回收率的计算，结合0.5～0.25 mm与0.25～0.125 mm的SEM、XRD和FTIR结果分析，判断+0.125 mm煤气化细渣不能有效通过浮选分离有机质与无机质，因此提出“预先分级+浮选”联合工艺流程，对-0.125 mm进行浮选分步释放实验，将-0.125 mm粒级浮选后富集的精煤与预先分级的+0.125 mm部分合并作为最终精煤，获得了灰分为26.38%,产率为66.42%的精矿。“预先分级+浮选”联合与全粒级浮选工艺流程相比，不但具有工艺流程简单，药剂消耗低，工艺流程调控灵活等优点，而且具有高产率、低灰分的优势，可以更高效地回收利用气化细渣。

The first example of stereoconvergent 1,3-dipolar cycloaddition of nitrile oxides and nitrile imines with E/Z isomeric mixture of electron-deficient olefins is reported,delivering isoxazolines and pyrazolines bearing two vicinal stereogenic tertiary and trifluoromethylated quaternary carbon centers with perfect regioand diastereoselectivities.The possibility of concerted cycloaddition/epimerization sequence under basic condition to form the thermodynamically stable diastereomers is excluded through some control experiments and DFT calculations,and a stepwise mechanism is proposed.

目的比较碘仿纱条和明胶海绵治疗鼻内镜术后鼻腔粘连的效果。方法选择2018年1月至2022年12月行鼻内镜术后复查发现的73例(87侧)鼻腔粘连患者,随机将其分为对照组(36例,43侧)和治疗组(37例,44侧)。两组均行鼻腔粘连分解后,对照组选取明胶海绵作为隔离材料进行治疗,治疗组选取碘仿纱条作为隔离材料进行治疗。比较两组的治疗效果。结果两组患者的手术时间、出血量比较,差异无统计学意义(P>0.05);治疗组的视觉模拟量表(VAS)评分、治愈率及满意度高于对照组(P<0.05);术后3个月,两组的鼻腔鼻窦结局测试22条(SNOT-22)评分均降低,且治疗组低于对照组(P<0.05)。结论碘仿纱条和明胶海绵填塞术腔隔离治疗鼻腔粘连均可取得一定治疗效果,但前者的疗效和患者生活质量、满意度优于后者。

Neuroblastoma(NB),which is the most common pediatric extracranial solid tumor,varies widely in its clinical presentation and outcome.NB has a unique ability to spontaneously differentiate and regress,suggesting a potential direction for therapeutic intervention.However,the underlying mechanisms of regression remain largely unknown,and more reliable prognostic biomarkers are needed for predicting trajectories for NB.We performed scRNAseq analysis on 17 NB clinical samples and three peritumoral adrenal tissues.Primary NB displayed varied cell constitution,even among tumors of the same pathological subtype.Copy number variation patterns suggested that neuroendocrine cells represent the malignant cell type.Based on the differential expression of sets of related marker genes,a subgroup of neuroendocrine cells was identified and projected to differentiate into a subcluster of benign fibroblasts with highly expressed CCL2 and ZFP36,supporting a progressive pathway of spontaneous NB regression.We also identified prognostic markers(STMN2,TUBA1A,PAGE5,and ETV1)by evaluating intra-tumoral heterogeneity.Lastly,we determined that ITGB1 in M2-like macrophages was associated with favorable prognosis and may serve as a potential diagnostic marker and therapeutic target.In conclusion,our findings reveal novel mechanisms underlying regression and potential prognostic markers and therapeutic targets of NB.

Endoplasmic reticulum stress is involved in retinal injury induced by repeated transient spikes of intraocular pressure

Clinically,a large proportion of glaucoma patients undergo repeated intraocular pressure(IOP)spike(Spike IOP)attacks during their sleep,which may facilitate retinopathy.In this study,we established a mouse model of repeated transient Spike IOP to investigate the direct damage to the retina following Spike IOP attacks,and elucidated the underlying molecular mechanism.We analyzed the changes in the number of retinal ganglion cells(RGCs)via immunofluorescence.Thereafter,we detected retinal cell apoptosis via terminal deoxynucleotidyl transferase deoxyuridine triphosphate(d UTP)nick-end labeling(TUNEL)staining,and performed RNA sequencing(RNA-seq)to reveal the underlying molecular mechanism.Finally,we validated the expression of key molecules in the endoplasmic reticulum(ER)stress pathway using quantitative real-time polymerase chain reaction(q RT-PCR)and western blot analysis.Results revealed a time-dependent RGC loss in Spike IOP,evidenced by a reduction in the number of Brn3 a-positive RGCs in experimental eyes following a 7-d continuous treatment with Spike IOP.In addition,TUNEL staining indicated that apoptosis of retinal cells started in the outer nuclear layer(ONL),and then spread to the ganglion cell layer(GCL)with time.RNA-seq analysis revealed that ER stress might be involved in Spike IOP-induced retinal injury.This result was corroborated by western blot,which revealed upregulation of ER stress-related proteins including binding immunoglobulin protein/glucose-regulated protein 78(Bi P/GRP78),phosphorylated inositolrequiring enzyme 1(p-IRE1),unspliced X-box-binding protein 1(XBP1-u),spliced X-box-binding protein 1(XBP1-s),phosphorylated c-Jun N-terminal kinase(p-JNK),C/EBP-homologous protein(CHOP),and B-cell lymphoma 2(Bcl-2)-associated X protein(Bax).These findings indicate that repeated IOP transients are detrimental to the retina,while ER stress plays an important role in retinal cell apoptosis in this situation.Notably,repeated Spike IOP among glaucoma patients is a crucial factor for progressive retino

Transplanting human neural progenitor cells is a promising method of replenishing the lost neurons after spinal cord injury (SCI), but differentiating neural progenitor cells into the diverse types of mature functional spinal cord neurons in vivo is challenging. In this study, engineered human embryonic spinal cord-like tissues with dorsal and ventral neuronal characters (DV-SC) were generated by inducing human neural progenitor cells (hscNPCs) to differentiate into various types of dorsal and ventral neuronal cells on collagen scaffold in vitro. Transplantation of DV-SC into complete SCI models in rats and monkeys showed better therapeutic effects than undifferentiated hscNPCs, including pronounced cell survival and maturation. DV-SC formed a targeted connection with the host’s ascending and descending axons, partially restored interrupted neural circuits, and improved motor evoked potentials and the hindlimb function of animals with SCI. This suggests that the transplantation of pre-differentiated hscNPCs with spinal cord dorsal and ventral neuronal characteristics could be a promising strategy for SCI repair.

儿童被忽视潜在类别及其在中学生人格与自我效能感间的调节作用

目的了解儿童被忽视的潜在类别及其影响因素,探讨不同类别与人格、自我效能感的关系及其在两者之间的调节作用。方法采用儿童被忽视量表(child neglect scale,CNS)、艾森克人格问卷儿童版(Eysenck personality questionnaire,EPQ)、一般自我效能感量表(general self-efficacy scale,GSES)对新密市558名初中学生进行问卷调查。用Mplus 7.0软件、SPSS 26.0软件和PROCESS v3.5宏程序对数据进行潜在剖面分析、二项Logistic回归分析、独立样本t检验、调节效应检验。结果 (1)儿童被忽视可分为高危组[21.3%(109/551)]、低危组[78.7%(402/551)]。(2)年龄每增加1岁,其属于高危组的发生比增加33.4%(B=0.288,OR=1.334,P<0.05);与祖父母共同生活的初中生更易处于高危组(B=0.822,OR=2.275,P≤0.05)。(3)高危组在外向性、掩饰性、自我效能感得分[(51.803±9.877)分,(46.860±6.552)分,(23.321±5.448)分]显著低于低危组[(54.341±10.521)分,(48.981±6.548)分,(24.933±4.961)分],均差异有统计学意义(t=2.262,2.998,2.945,均P<0.05),而在神经质、精神质得分[(58.316±1.880)分,(56.536±10.663)分]显著高于低危组[(48.731±12.371)分,(49.850±9.562)分],均差异有统计学意义(t=-7.235,-6.314,均P<0.001)。(4)儿童被忽视潜在类别在精神质与自我效能感间起调节作用(B=-0.111,P<0.05)。对于低危组而言,精神质对自我效能感的预测作用不显著(B=-0.032,P=0.223);对于高危组而言,自我效能感随着精神质得分增加而显著下降(B=-0.143,P=0.002)。结论儿童被忽视可分为两个潜在类别,在中学生精神质与自我效能感之间具有调节作用。

Association between the M98K variant of the OPTN gene and the risk for primary open angle glaucoma:an updated meta-analysis

Background:The association between optineurin(OPTN)M98K variant and primary open angle glaucoma(POAG)has been widely investigated.However,the results remain controversial among published meta-analyses.Therefore,we conducted an updated meta-analysis to further explore the association between M98K and POAG and its subgroups.Methods:PubMed,Embase,Web of Science,and China National Knowledge Infrastructure(CNKI)databases were searched to find all articles describing the relationships between the M98K variant and POAG,which were published from the inception to 31 December,2019.The associations between M98K and overall POAG,normal tension glaucoma(NTG),high tension glaucoma(HTG),POAG in Asian and non-Asian populations,juvenile open angle glaucoma(JOAG)and adult-onset POAG were evaluated by calculating the pooled odds ratio(OR)and 95%confidence interval(CI).The Bonferroni correction was used to determine the statistically significant genetic models.Results:A total of 34 eligible articles involving 7,310 POAG patients and 5,173 controls were identified in the present meta-analysis.The articles achieved an average of 6.21 stars for quality assessment by the Newcastle-Ottawa Scale(NOS).Evidence from the pooled results indicated significant association between M98K and overall POAG susceptibility under the dominant model(OR=1.30,95%CI,1.12-1.52;P<0.001).In the subgroup analyses,no significant associations were found between M98K and the risks of NTG,HTG,JOAG,adult-onset POAG,Asian POAG or non-Asian POAG.Conclusions:The updated meta-analysis revealed that OPTN M98K was significantly associated with the susceptibility to overall POAG.
