AIM: To assess corticotropin-releasing factor receptor 2 (CRF 2 ) expression in the colon of healthy subjects and patients with ulcerative colitis (UC). MeTHODS: We examined CRF2 gene and protein expression in the distal/sigmoid colonic mucosal biopsies from healthy subjects and patients with UC (active or disease in remission), human immunodeficiency virus (HIV) and functional bowel disease (FBD) by reverse transcriptionpolymerase chain reaction and immunofluorescence. ReSULTS: Gene expression of CRF2 was demonstrated in the normal human colonic biopsies, but not in the human colorectal adenocarcinoma cell line Caco2. Receptor protein localization showed immunoreactive CRF 2 receptors in the lamina propria and in the epithelial cells of the distal/sigmoid biopsy samples. Interestingly, CRF 2 immunoreactivity was no longer observed in epithelial cells of patients with mild-moderately active UC and disease in remission, while receptor protein expression did not change in the lamina propria. No differences in CRF 2 expression profile were observed in distal/sigmoid intestinal biopsies from HIV infection and FBD patients, showing no signs of inflammation. CONCLUSION: The down-regulation of the CRF2 receptor in the distal/sigmoid biopsies of UC patients is indicative of change in CRF 2 signalling associated with the process of inflammation.

大陆岩石圈有效弹性厚度 (Te)是表示岩石圈强度的参数 ,计算该参数对研究岩石圈大规模构造 ,分析大陆板块内的均衡补偿机制有一定意义 .利用Forsyth提出的相关技术计算了北冰洋 -欧亚大陆 -太平洋地学断面东南段自黑水到泉州的Te 值 .并分析了重力和地形波长的相关性特征 ,初步认为 :计算Te 值所选的每个数据块在短波长 (6 .6～ 10 0km)内岩石圈板块的强度足以平衡地形负载 ,重力和地形不相关 ;在长波长 (10 0～ 2 5 0km)内 ,地形及地下负载由弯曲模型补偿 ,岩石圈板块在地形及地下负载作用下而挠曲 .断面通过地段具有较低热流密度值的四川陆核有较高的Te 值 ,具有较高热流密度值的宁化、大田地区有较低的Te 值 ,反映了较高的热流密度值对应较低的Te 值 ;较低的热流密度值对应较高Te 值的关系 .Te 可分为南东低值段和北西高值段 .地壳厚度大体上与Te 值呈正相关关系 .

BACKGROUND Over the last decade,the treatment options for inflammatory bowel disease(IBD)have significantly progressed with the emergence of new medications designed to target various immune pathways and mitigate ***(ADA)is a tumor necrosis factor alpha antagonist and stands as an effective treatment for *** April 2021,the province of British Columbia implemented a mandatory non-medical switch policy of the ADA originator Humira®to ADA *** offer a potential cost-effective,safe,and efficacious alternative to the originator,yet there remains limited real-world evidence on long-term outcomes of ADA non-medical switching in *** To assess the long-term outcomes of non-medical switching from the ADA originator Humira®to an ADA biosimilar among IBD *** A retrospective observational chart review study was conducted on IBD patients eligible for the provincially mandated non-medical switch to an ADA *** primary outcome was treatment persistence at 30 months *** outcomes included the proportion of and reasons for therapy alteration or ADA discontinuation,loss of response(LOR)rates,adverse events(Ae),and clinical and biochemical remission *** who remained on the originator throughout the switch period,through compassionate support or private pay,constituted the comparison *** Patients in the originator(n=43)and biosimilar switch(n=228)groups displayed similar demographics and baseline disease *** the study endpoint of 30 months,there was no difference in the rate of treatment persistence in either group(n=36,83.7%originator group vs n=201,88.2%biosimilar group,P=0.451).Treatment persistence demonstrated similar rates of discontinuation between both study groups(log-rank P=0.543).There was a numerical but not statistically significant difference in rates of adverse outcomes between either group(39.5%originator vs 28.9%biosimilars,P=0.206).This included co

AIM: To compare venous thromboembolism(VTe) in hospitalized ulcerative colitis(UC) patients who respond to medical management to patients requiring colectomy. MeTHODS: Population-based surveillance from 1997 to 2009 was used to identify all adults admitted to hospital for a flare of UC and those patients who underwent colectomy. All medical charts were reviewed to confirm the diagnosis and extract clinically relevant information. UC patients were stratified by:(1) responsive to inpatient medical therapy(n = 382);(2) medically refractory requiring emergent colectomy(n = 309); and(3) elective colectomy(n = 329). The primary outcome was the development of VTe during hospitalization or within 6 mo of discharge. Heparin prophylaxis to prevent VTe was assessed. Logistic regression analysis determined the effect of disease course(i.e., responsive to medical therapy, medically refractory, and elective colectomy) on VTe after adjusting for confounders including age, sex, smoking, disease activity, comorbidities, extent of disease, and IBD medications(i.e., corticosteroids, mesalamine, azathioprine, and infliximab). Point estimates were presented as odds ratios(OR) with 95%***: The prevalence of VTe among patients with UC who responded to medical therapy was 1.3% and only 16% of these patients received heparinprophylaxis. In contrast, VTe was higher among patients who underwent an emergent(8.7%) and elective(4.9%) colectomy, despite greater than 90% of patients receiving postoperative heparin prophylaxis. The most common site of VTe was intra-abdominal(45.8%) followed by lower extremity(19.6%). VTe was diagnosed after discharge from hospital in 16.7% of cases. elective(adjusted OR = 3.69; 95%CI: 1.30-10.44) and emergent colectomy(adjusted OR = 5.28; 95%CI: 1.93-14.45) were significant risk factors for VTe as compared to medically responsive UC patients. Dear editor,In the United States,pancreatic cancer is considered the fourth most common reason for cancer-related deaths;over 53%of patients are diagnosed at end-stage with a 5-year survival rate of less than 3%[1].Incomplete removal of all cancerous cells generally leads to local recurrence of pancreatic cancer[2].Thus,the detection of tumor margin status plays an essential role in the complete resection of pancreatic *** of the existing imaging modalities,like computed tomography and magnetic resonance imaging,are primarily used for preoperative planning;these modalities also have issues of bulky equipment,radiation concerns,and slow imaging speed[2].Thus,these modalities can neither provide an intraoperative diagnosis of cancer with high sensitivity and specificity nor be used to define tumor margins[3].
