Impact of type 2 diabetes mellitus on short-term and long-term outcomes of patients with esophageal squamous cell cancer undergoing resection: a propensity score analysis

Background:The association between type 2 diabetes mellitus(T2DM)and the risk of esophageal cancer remains unclear.The present study aimed to evaluate the impact of T2DM on short-term outcomes and long-term survival in patients with esophageal squamous cell cancer(ESCC).Methods:The present retrospective study included 862 patients diagnosed with ESCC between January 2001 and December 2010.Among them,280 patients had T2DM.A 1:1 propensity score-matched cohort consisting of 280 patients with and 280 without T2DM was selected from the 862 patients.The associations between T2DM and clinico-pathologic characteristics were assessed using theχ2 or Fisher’s exact test.Survival of ESCC patients with and without T2DM was calculated by using the Kaplan-Meier method and compared by using the Cox regression model between the two groups.Results:The occurrence rate of anastomotic leakage was significantly higher in patients with T2DM than in those without T2DM(P5.05%,the patients without T2DM showed a longer survival(P=0.001).Univariate and multivariate analysis results showed that T2DM was not an independent prognostic factor for patient survival.Conclusions:Type 2 diabetes mellitus is not an independent prognostic factor in patients with ESCC.However,the combination of T2DM with severe weight loss would be a predictor of poor prognosis.

干式电抗器的稳定运行影响新型电力系统的输电可靠性。干式空心电抗器包封材料整体由浸有环氧树脂的玻璃纤维丝经高温固化而成。本文采用多物理场耦合有限元方法,考虑干式空心电抗器的包封材料热导率对其热点温升的影响,建立了环氧复合材料的COMSOL微观仿真模型和外电路约束下的干式空心电抗器电-磁、流-热耦合计算模型。将电磁场下的损耗作为热源计算温度场与流场分布,研究在25℃环境温度下常规/高导热环氧复合材料对干式空心电抗器热点温升的影响规律。结果表明:高导热环氧树脂对复合材料热导率的提升效果显著;包封材料本体及周围空气温度场区域中热点温升最大值为103.75℃,出现在内部第4层包封材料的上端处;不同热导率的复合材料对降低干式电抗器的热点温升有明显差异,其中干式电抗器在高导热环氧树脂复合材料下的热点温度降低了7.55℃。

背景与目的肺癌是中国发生率和死亡率均排名第一的癌症,非小细胞肺癌(non-small cell lung cancer,NSCLC)占所有肺恶性肿瘤的80%-85%。目前,手术治疗仍是肺癌主要的治疗方式。近年来,免疫检查点抑制剂在NSCLC中的疗效已成为共识,新辅助免疫联合化疗(neoadjuvant immunochemotherapy,nICT)在早中期NSCLC中显示出来良好的疗效和安全性。然而,nICT治疗局部晚期NSCLC的相关研究较少。本研究旨在评估程序性死亡受体1(programmed cell death 1,PD-1)抑制剂联合含铂两药新辅助治疗治疗局部晚期可切除NSCLC的有效性和安全性。方法纳入2021年1月至2024年4月于兰州大学第二医院胸外科就诊的85例确诊可切除IIIA、IIIB期患者,分为nICT组(n=32)和单纯手术组(n=53),比较两组患者的临床基线资料、围手术期相关指标及术后并发症,并评估nICT组的影像学缓解率、病理学缓解率、相关不良反应发生率、生活质量。结果两组患者临床基线资料差异均无统计学意义(P>0.05)。nICT组选择开胸方式比单纯手术组发生率高(P=0.002),两组手术时间、术中出血量、清扫淋巴结个数、带管时间、术后住院时间及R0切除率的差异无统计学意义(P>0.05)。nICT组与单纯手术组术后总并发症发生率分别为31.25%和22.64%,差异无统计学意义(P=0.380)。nICT组中客观缓解率(objective response rate,ORR)为84.38%,完全缓解(complete response,CR)5例(15.63%),部分缓解(partial response,PR)22例(68.75%),病理性完全缓解(pathological complete response,pCR)15例(46.88%),主要病理缓解(major pathological reaponse,MPR)11例(34.38%)。nICT治疗期间,3级治疗相关不良反应共12例(37.50%),无不良反应或免疫相关不良反应导致患者死亡。而且,nICT治疗后患者相关症状有所改善。结论nICT治疗局部晚期可切除NSCLC显示出良好的疗效,治疗相关不良事件可控,是局部晚期可切除NSCLC安全、可行的新辅助治疗模式。

High-temperature Oxidation Behavior of Barrel Materials

With the increase in firepower strike requirements and the development of high-powered artillery, the high-temperature oxidation of barrel materials is becoming more and more serious, which seriously affects the performance of weapon and even leads to the scrapping of weapon. However, there is little research on the anti-oxidation performance of barrel materials at present. In this paper, the oxidation resistances and oxidation mechanisms of three barrel materials, i. e.,PCrNi3MoVA steel. 30SiMn2MoVA steel and MPS700 steel, at 600 ℃ in airare investigated. The cross-sectional morphology and phases of the oxide layers are analyzed by scanning electron microscopy, energy spectroscopy and X-ray diffractometry. The oxidation kinetics of three barrrel materials were analyzed. The results show that the oxide film thicknesses of the three steels gradually thicken and the rate of increaseinthickness gradually decreaseswith the increase in oxidation time. The oxidation kinetics curves of the three barrel materials are consistent with the parabolic growth kinetics law of oxide film. At 600 ℃, the internal oxide layer of PCrNi3MoVA steel forms an adhesive layer of a network structure consisting mainlyof Ni and Cr-richspinel oxides but theadhesive layerdoes not act as a barrier against oxidation. The inner layer of oxide layer of 30SiMn2MoVA steel is a porous aggregation zone of Si element oxide with a loose structure and relatively serious oxide flaking. The internal oxide layer of MPS700 steel forms a protective Cr-rich oxide layer that effectively prevents further oxidation. At the same time, Mo element stabilises the passivation film and indirectly improves oxidation resistance. Compared with PCrNi3MoVA steel and 30SiMn2MoVA steel, MPS700 steel can better form a dense oxidized layer under high temperature condition, so as to reduce the high temperature oxidation and wear degree of the matrix under high temperature conditions. MPS700 steel is more suitable for the preparation of long life and high reliability barrel. © 20

A new review of single-ion conducting polymer electrolytes in the light of ion transport mechanisms

With the depletion of fossil fuels and the demand for high-performance energy storage devices,solidstate lithium metal batteries have received widespread attention due to their high energy density and safety advantages.Among them,the earliest developed organic solid-state polymer electrolyte has a promising future due to its advantages such as good mechanical flexibility,but its poor ion transport performance dramatically limits its performance improvement.Therefore,single-ion conducting polymer electrolytes(SICPEs)with high lithium-ion transport number,capable of improving the concentration polarization and inhibiting the growth of lithium dendrites,have been proposed,which provide a new direction for the further development of high-performance organic polymer electrolytes.In view of this,lithium ions transport mechanisms and design principles in SICPEs are summarized and discussed in this paper.The modification principles currently used can be categorized into the following three types:enhancement of lithium salt anion-polymer interactions,weakening of lithium salt anion-cation interactions,and modulation of lithium ion-polymer interactions.In addition,the advances in single-ion conductors of conventional and novel polymer electrolytes are summarized,and several typical highperformance single-ion conductors are enumerated and analyzed in what way they improve ionic conductivity,lithium ions mobility,and the ability to inhibit lithium dendrites.Finally,the advantages and design methodology of SICPEs are summarized again and the future directions are outlined.

miR‑423 sponged by lncRNA NORHA inhibits granulosa cell apoptosis

Background Atresia and degeneration,a follicular developmental fate that reduces female fertility and is triggered by granulosa cell(GC)apoptosis,have been induced by dozens of miRNAs.Here,we report a miRNA,miR-423,that inhibits the initiation of follicular atresia(FA),and early apoptosis of GCs.Results We showed that miR-423 was down-regulated during sow FA,and its levels in follicles were negatively correlated with the GC density and the P4/E2 ratio in the follicular fluid in vivo.The in vitro gain-of-function experiments revealed that miR-423 suppresses cell apoptosis,especially early apoptosis in GCs.Mechanically speaking,the miR-423 targets and interacts with the 3’-UTR of the porcine SMAD7 gene,which encodes an apoptosis-inducing factor in GCs,and represses its expression and pro-apoptotic function.Interestingly,FA and the GC apoptosis-related lncRNA NORHA was demonstrated as a ceRNA of miR-423.Additionally,we showed that a single base deletion/insertion in the miR-423 promoter is significantly associated with the number of stillbirths(NSB)trait of sows.Conclusion These results demonstrate that miR-423 is a small molecule for inhibiting FA initiation and GC early apoptosis,suggesting that treating with miR-423 may be a novel approach for inhibiting FA initiation and improving female fertility.

Time-varying parameters estimation with adaptive neural network EKF for missile-dual control system

In this paper, a filtering method is presented to estimate time-varying parameters of a missile dual control system with tail fins and reaction jets as control variables. In this method, the long-short-term memory(LSTM) neural network is nested into the extended Kalman filter(EKF) to modify the Kalman gain such that the filtering performance is improved in the presence of large model uncertainties. To avoid the unstable network output caused by the abrupt changes of system states,an adaptive correction factor is introduced to correct the network output online. In the process of training the network, a multi-gradient descent learning mode is proposed to better fit the internal state of the system, and a rolling training is used to implement an online prediction logic. Based on the Lyapunov second method, we discuss the stability of the system, the result shows that when the training error of neural network is sufficiently small, the system is asymptotically stable. With its application to the estimation of time-varying parameters of a missile dual control system, the LSTM-EKF shows better filtering performance than the EKF and adaptive EKF(AEKF) when there exist large uncertainties in the system model.

Can the integration between urban and rural areas be realized?A new theoretical analytical framework

Urban-rural integration is an advanced form resulting from the future evolution of urban-rural relationships.Nevertheless,little research has explored whether urban and rural areas can move from dual segmentation to integrated development from a theoretical or empirical perspective.Based on the research framework of welfare economics,which offers an appealing paradigm to frame the underlying game between cities and villages,this study clarifies the ideal state of urban-rural integration.It then proposes a series of basic assumptions,and constructs a corresponding objective function and its constraints.Moreover,it assesses the possibility of seeing the transmutation from division to integration between urban and rural areas with continuous socio-economic development.The authors argue that the ideal state of urban-rural integration should be a Pareto-driven optimal allocation of urban-rural resources and outputs,and the maximization of social welfare in the entire region.Based on a systematic demonstration using mathematical models,the study proposes that urban and rural areas can enter this ideal integrated development pattern when certain parameter conditions are met.In general,this study demonstrates the theoretical logic and scientific foundations of urban-rural integration,enriches theoretical studies about urban-rural relationships,and provides basic theoretical support for large developing countries to build a coordinated and orderly urban-rural community with a shared future.
