BACKGROUND Abdominal aortic aneurysm(AAA)repair often involves significant postoperative pain,traditionally managed with systemic opioids,which can cause undesirable side *** case report explores the novel use of a surgically-initiated rectus sheath block with a catheter-over-needle assembly for pain management in AAA *** SUMMARY A 67-year-old female with hypertension and previous aortic dissection underwent elective open repair of an infrarenal AAA,which had grown from 3.4 cm to 4.3 cm over 14 months.A rectus sheath block was initiated surgically for postoperative pain *** patient reported low pain scores and did not require systemic intravenous opioids,enabling early ambulation and discharge on postoperative day seven without *** preventing complications of systemic opi-oids,the method indicating a promising direction for postoperative pain management in major vascular *** Surgically-initiated rectus sheath block as a valuable tool for managing postoperative pain in AAA repair.

BACKGROUND Needle-knife precut papillotomy(NKP)is typically performed ***,it remains unclear whether pancreatic stent(PS)placement can improve the outcomes of *** To explore whether PS placement improves the success rate of NKP in patients with difficult biliary *** This single-center retrospective study included 190 patients who underwent NKP between January 2017 and December 2021 after failed conventional biliary *** cases with incidental pancreatic duct cannulation during conventional biliary cannulation,the decision for pre-NKP PS placement was made at the endoscopist's *** primary outcome was the difference in the NKP success rate between patients with and without PS placement;the secondary outcome was the adverse event *** Among the 190 participants,82 received pre-NKP PS(PS-NKP group)whereas 108 did not[freehand or freehand NKP(FH-NKP)group].Post-NKP selective biliary cannulation was successful in 167(87.9%)patients,and the PS-NKP had a significantly higher success rate than the FH-NKP group(93.9%vs 83.3%,P=0.027).The overall adverse event rates were 7.3%and 11.1%in the PS-NKP and FH-NKP groups,respectively(P=0.493).A periampullary diverticulum(PAD)and significant intraoperative bleeding during NKP were independently associated with NKP failure;however,a pre-NKP PS was the only predictor of NKP *** the 44 participants with PADs,the PS-NKP group had a non-significantly higher NKP success rate than the FH-NKP group(87.5%and 65%,respectively;P=0.076).CONCLUSION PS significantly improved the success rate of NKP in patients with difficult biliary cannulation.

AIM: Hepatocellular carcinoma (HCC) with portal vein tumor thrombosis (PVTT) is associated with poor prognosis. The aim of this prospective study was to evaluate the efficacy of hepatic arterial infusion chemotherapy (HAIC) for patients with this ***: Eighteen HCC patients with PVTT were treated with HAIC via a subcutaneously implanted injection port. A course of chemotherapy consisted of daily cisplatin (10 mg for one hour) followed by 5-fiuorouracil (250 mg for five hours) zfor five continuous days within a given week. The patients were scheduled to receive four consecutive courses of *** were defined in whom either a complete or partial response was achieved, while non-responders were defined based on stable or progressive disease status. The prognostic factors associated with survival after treatment were ***: Six patients exhibited partial response to this form of HAIC (response rate = 33 %). The 3, 6, 9, 12 and 18-month cumulative survival rates for the 18 patients were 83 %, 72 %, 50 %, 28 %, and 7 %, respectively. Median survival times for the six responders and 12 non-responders were 15.0 (range, 11-18) and 7.5 (range, 1-13) months,respectively, It was demonstrated by both univariate and multivariate analyses that the therapeutic response and hepatic reserve function were significant prognostic ***: HAIC using low-dose cisplatin and 5-fluorouracil may be a useful alternative for the treatment of patients with advanced HCC complicated with PVTT. There may also be survival-related benefits associated with HAIC.

This study aims to extend the multivariate adaptive regression splines(MARS)-Monte Carlo simulation(MCS) method for reliability analysis of slopes in spatially variable soils. This approach is used to explore the influences of the multiscale spatial variability of soil properties on the probability of failure(P_f) of the slopes. In the proposed approach, the relationship between the factor of safety and the soil strength parameters characterized with spatial variability is approximated by the MARS, with the aid of Karhunen-Loeve expansion. MCS is subsequently performed on the established MARS model to evaluate ***, a nominally homogeneous cohesive-frictional slope and a heterogeneous cohesive slope, which are both characterized with different spatial variabilities, are utilized to illustrate the proposed *** showed that the proposed approach can estimate the P_f of the slopes efficiently in spatially variable soils with sufficient accuracy. Moreover, the approach is relatively robust to the influence of different statistics of soil properties, thereby making it an effective and practical tool for addressing slope reliability problems concerning time-consuming deterministic stability models with low levels of P_***, disregarding the multiscale spatial variability of soil properties can overestimate or underestimate the P_f. Although the difference is small in general, the multiscale spatial variability of the soil properties must still be considered in the reliability analysis of heterogeneous slopes, especially for those highly related to cost effective and accurate designs.

In recent years,renewable energy sources,specifically solar power systems,have developed rapidly owing to their technological maturity and cost ***,its grid integration deteriorates frequency stability because of insufficient rotating masses and inertial ***,a synchronverter,which is an inverter that mimics the operation of a synchronous generator,is crucial to interface solar power in a power *** stabilizes the power grid by emulating a virtual ***,a conventional proportional-integral(PI)-based synchronverter is not equipped with an adaptive damping factor(Dp)or a digitalized smart controller to manage fast-responding solar ***,a novel fuzzy logic controller(FLC)framework is proposed such that the synchronverter can operate in a grid-connected solar power *** this study,Dp is controlled in real time using an FLC to achieve balance between speed and stability for frequency error correction based on frequency *** of four case studies performed in Matlab/Simulink show that the proposed FLC-based synchronverter can stabilize the grid frequency by reducing the frequency deviation by at least 0.2 Hz(0.4%),as compared with the conventional PI-based synchronverter.

Aim： To study the effect and mechanism of gonadotrophin-releasing hormone （GnRH） on murine Leydig cell steroidogenesis. Methods： Purified murine Leydig cells were treated with GnRH-Ⅰ and -Ⅱ agonists, and testosterone production and steroidogenic enzyme expressions were determined. Results： GnRH-Ⅰ and -Ⅱ agonists significantly stimulated murine Leydig cell steroidogenesis 60%-80% in a dose- and time-dependent manner （P 〈 0.05）. The mRNA expressions of steroidogenic acute regulatory （STAR） protein, P450scc, 3β-hydroxysteroid dehydrogenase （HSD）, but not 17β-hydroxylase or 17β-HSD, were significantly stimulated by both GnRH agonists with a 1.5- to 3-fold increase （P 〈 0.05）. However, only 3β-HSD protein expression was induced by both GnRH agonists, with a 1.6- to 2-fold increase （P 〈 0.05）. Conclusion： GnRH directly stimulated murine Leydig cell steroidogenesis by activating 3β-HSD enzyme expression.

With the development of landslide monitoring system,many attempts have been made to predict landslide failure-time utilizing monitoring data of *** models(e.g.,Verhulst,GM(1,1),and Saito models)that consider the characteristics of landslide displacement to determine the failuretime have been investigated *** practice,monitoring is continuously implemented with monitoring data-set updated,meaning that the predicted landslide life expectancy(i.e.,the lag between the predicted failure-time and time node at each instant of conducting the prediction)should be re-evaluated with *** manner is termed“dynamic prediction”.However,the performances of the classical models have not been discussed in the context of the dynamic prediction *** this study,such performances are investigated firstly,and disadvantages of the classical models are then reported,incorporating the monitoring data from four real ***,a more qualified ensemble model is proposed,where the individual classical models are integrated by machine learning(ML)-based *** evaluate the quality of the models under the dynamic prediction,a novel indicator termed“discredit index(b)”is proposed,and a higher value of b indicates lower prediction *** is found that Verhulst and Saito models would produce predicted results with significantly higher b,while GM(1,1)model would indicate results with the highest mean absolute ***,the ensemble models are found to be more accurate and qualified than the classical ***,the performance of decision tree regression-based ensemble model is the best among the various ML-based ensemble models.

The aim of this research was to determine the rice protein content utilizing a NIR imaging *** developed imaging system utilized a NIR camera which installed automatically exchanged filters with the wavelength range from 870 nm to 1014 *** liner regression(MLR),partial least square regression(PLSR),and artificial neural network(ANN)models were employed as data analysis methods for 6.18%-9.43%rice protein detections within both the NIR imaging system and commercial NIRS.A total of 180 rice samples were used in this study,of which 120 random samples were selected as a calibration set for the MLR and PLSR ***,for establishing the back-propagation ANN model,the same 120 samples were divided into two parts,80 samples were used for network training and the other 40 were established as the monitoring *** compare with the results of MLR,PLSR,and ANN models,the remaining 60 of the total 180 samples were established as the validation *** an MLR linear regression model composed of five wavelengths;the NIR imaging system successfully detected rice protein *** predicting results of r_(val)^(2) and SEP were 0.769 and 0.294%,*** PLSR model,utilizing the imaging system obtained the results of r_(val)^(2)=0.782,and SEP=0.274%within the wavelength range from 870 nm to 1014 *** significant wavelengths selected by the MLR model were the same as the input data of the ANN model,and the prediction results were r_(val)^(2)=0.806,and SEP=0.266%.The prediction results indicated that the developed NIR imaging system has the advantages of simple,convenient operation,and high detection accuracy as well as it presents commercial potential in non-destructive detection of rice protein content.
