Schizophrenia knowledge and attitudes toward help-seeking among Japanese fathers and mothers of high school students

The prodromal symptoms of schizophrenia are nonspecific and include social withdrawal, sleeplessness, decreased desire, and loss of concentration. Schizophrenia outcomes might be improved if greater therapeutic effort was focused on people with prodromal symptoms. To hasten help-seeking for schizophrenia, we developed a schizophrenia education program for families of patients. Help-seeking and schizophrenia attitudes and knowledge were analyzed among Japanese parents (666 fathers and 613 mothers) of high school students by using the Social Distance Scale-Japanese version, the Link Devaluation-Discrimination Measure and our own questions on help-seeking and schizophrenia knowledge. Statistical analysis included cross-tabulation, the χ2 test, and multiple regression analysis. Most (97.5%) fathers were aged 40 - 59 years;most (78.3%) mothers were aged 40 - 49 years. There was a significant difference between fathers and mothers in basic knowledge of schizophrenia (P 0.05). When asked what treatment they would seek for a child with sleeplessness or social withdrawal, fathers and mothers significantly differed in seeking help medical care at a department of psychosomatic medicine (P < 0.05) and in seeking help from the nurse at their child’s school (P < 0.001). Fathers and mothers also differed in relation to the decision not to seek help (P < 0.001). Multiple regression analysis revealed that basic knowledge significantly differed between fathers and mothers (P = 0.001). In an analysis of parental response to a child with sleepiness and social withdrawal, multiple regression analysis revealed that fathers and mothers significantly differed in the likelihood of seeking help at a department of psychosomatic medicine and from a school nurse and in the likelihood of not seeking help (P < 0.05, P = 0.001 and P = 0.001, respectively). Social distance toward schizophrenia among parents of adolescents

Social distance toward schizophrenia is a reason for delays in receiving early treatment for the disorder. This study attempted to identify the factors underlying social distance. The participants were 2690 parents of adolescents. Factor analysis using a social distance scale identified two factors: private relationship and social relationship. Regression analysis revealed that gender, age, and participation in welfare activities for people with mental illnesses had independent effects on the factor private relationship (p < 0.05). These results suggest that women aged 40 - 49 years should be targeted for activities that attempt to counteract social distance and that they would benefit from taking part in public welfare activities for individuals with mental illnesses.

Association of social distance toward schizophrenia with help-seeking among mothers of adolescents in Japan

Negative maternal attitudes toward schizophrenia may be linked with delayed treatment of their children. We investigated the relation between negative attitudes toward schizophrenia and help-seeking among mothers of junior and senior high school students in Japan. The participants were 1309 Japanese mothers of junior and senior high school students. Social distance was evaluated by using the Social Distance Scale-Japanese version (SDS-J). In addition, mothers were asked about help-seeking for a child with sleeplessness, social withdrawal, and strange behavior. One-way analysis of variance and Student’s t-test were used to evaluate associations between social distance toward schizophrenia and help-seeking. Most (76.4%) participants were aged 40 - 49 years. Maternal demographic characteristics significantly associated with social distance were employment and participation in welfare activities for people with mental illness. In responding to a child with sleeplessness, social withdrawal, and strange behavior, the level of maternal social distance was not significantly associated with the likelyhood of seeking psychiatric help. However, mothers with greater social distance were less likely to seek help at a psychiatric clinic. Maternal social distance toward schizophrenia was not significantly associated with seeking psychiatric help;however, it did affect the type of facility selected among those would seek such help.

Pituitary intratumoral hemorrhage during radiation therapy following partial removal of giant pituitary adenoma: A case report

We report a rare case of intratumoral hemorrhage during postoperative radiotherapy for pituitary adenoma. A 57-year-old Asian male, complaining of long-standing eye strain, underwent a medical checkup of the brain. Magnetic resonance imaging showed a multicystic giant pituitary adenoma. The patient underwent an endoscopic endonasal transsphenoidal partial removal of the adenoma to provide optic pathway decompression and got relief from the visual symptoms. Just before completion of the postoperative radiotherapy for residual adenoma, the patient developed right hemiparesis, mild motor aphasia, and right oculomotor palsy. A cranial CT scan showed intratumoral hemorrhage into the intratumoral cyst. The patient therefore had to undergo three additional craniotomies for evacuation of cyst contents over the next 8 months. The follow-up MRI at 11 months after the initial hemorrhage showed that the new oozing of blood in the intratumoral cyst was still appearing. Intratumoral hemorrhage is a rare, albeit life-threatening, complication of pituitary adenoma. We reviewed relevant literature and suggested that the cystic component in pituitary adenoma could be a key pathogenesis of this rare complication. In conclusion, we suggest that it may be necessary to realize that cases which have cystic giant pituitary adenoma may cause hemorrhage by chance with the foreseeability.

Usefulness of Fractional Exhaled Nitric Oxide-Guided Treatment in Patients with Asthma-Chronic Obstructive Pulmonary Disease Overlap

Background: Some patients present clinical features of both asthma and chronic obstructive pulmonary disease (COPD), which has led to the recent proposal of asthma-COPD overlap (ACO) as a diagnosis. Fractional exhaled nitric oxide (FeNO) is a candidate biomarker to diagnose ACO. We assessed the effect of an add-on treatment with budesonide/formoterol (BUD/FM) combination in patients with ACO, which was diagnosed by FeNO. Methods: This was a prospective, single-arm, open-label, before and after comparison study. Subjects included 83 patients with COPD who attended outpatient clinics for routine checkups at Shizuoka General Hospital between June and November 2016. All patients fulfilled the GOLD definition of COPD and were receiving long-acting muscarinic antagonist (LAMA) or LAMA/long-acting β2 agonist (LABA) combinations. After an 8-week run-in period, BUD/FM was added to the patients with FeNO levels of ≥35 ppb, defined as having ACO. For patients receiving LAMA/LABA, BUD/FM was added after the discontinuation of LABA. The modified British Medical Research Council (mMRC) score, COPD assessment test (CAT) score, spirometric indices, forced oscillation parameters, and FeNO were assessed before and after 8 weeks of BUD/ FM add-on treatment. Results: Twenty-four patients (28.9%) had FeNO levels ≥ 35 ppb, and 17 patients completed the study (mean age: 73 years and GOLD I/II/III/IV, 5/10/1/1). The mean CAT scores significantly improved (9.2 to 5.4, p = 0.015) and 10 patients (58.8%) showed ≥2 points improvement, a minimal clinically important difference. The mean FeNO levels significantly decreased from 63.0 to 34.3 ppb (p Conclusions: FeNO-guided treatment with BUD/FM improves symptoms in patients with ACO.
