Objective: The objective of this study was to explore the medication rules and academic ideas of Professor wang yu-ying in the treatment of climacteric syndrome(CLS) and to predict new prescriptions. Materials and Methods: The characteristics of frequency, clustering, four properties, and five flavors were analyzed, and new prescriptions were predicted through an artificial intelligence(AI)-based method. The potential pathways of new prescriptions were explored through network pharmacology-based analysis. Results: The top 16 medicinals used by Professor wang yu-ying in the treatment of CLS included Danggui, Longgu, Muli, Fuling, Chuanxiong, Gancao, Xiangfu, and Tusizi. The AI method was applied to predict the basic prescription for treating CLS: Danggui 15 g, Duanlonggu 30 g, Duanmuli 30 g, Fuling 28 g, Chuanxiong 10 g, Gancao 6 g, Xiangfu 12 g, Tusizi 14 g, etc., Gene Ontology and Kyoto Encyclopedia of Genes and Genomes pathway enrichment analyses showed that the pathogenesis of CLS might be related to the estrogen pathway, involving typical steroid responses. Conclusions: This study summarized Professor wang's medication experience in the treatment of CLS based on the data mining of clinical diagnoses and treatment cases. The AI method was used to predict the new prescription of CLS treatment, which was found to be reasonable by network pharmacology studies on its multi-target and multi-pathway mechanisms.

背景老年患者身体机能、抵抗能力较差,术后极易发生感染,且随着药细菌不断增多,耐药菌感染发生率较高,经验性用药已不能适应耐药菌的变化.基于此,本研究全面系统分析老年胃癌根治术后并发耐药菌感染情况.目的分析老年胃癌根治术后并发耐药菌感染的影响因素及耐药菌分布特点,为胃癌根治术后有效防治感染提供参考依据.方法选取2019-04/2022-09我院收治的359例老年胃癌根治术患者,其中46例术后发生感染,包含耐药菌感染21例(耐药菌组)、非耐药菌感染25例(非耐药菌组)及未感染患者313例(对照组),对老年胃癌根治术后并发耐药菌感染进行单因素与多因素Logistic回归分析,采用R语言绘制预测并发耐药菌感染的列线图,采用受试者工作特征曲线(receiver operating characteristic,ROC)评价列线图模型的预测效能,采用决策分析曲线评价列线图的临床效用.结果感染部位以肺部(42.86%)为主,病原菌类型以革兰阴性菌为主,常见病原菌类型有鲍曼不动杆菌(28.57%)、铜绿假单胞菌(23.81%)等;耐药菌组年龄、糖尿病、术后机械通气、术后留置尿管>3 d、重症加强护理病房(intensive care unit,ICU)住院时间高于非耐药组、对照组,白蛋白、辅助性T淋巴细胞CD4^(+)/记忆性T细胞CD8^(+)低于非耐药组、对照组(P3 d、ICU住院时间是发生耐药菌感染的相关独立危险因素,白蛋白、CD4^(+)/CD8^(+)是发生耐药菌感染的相关保护因素(P<0.05);ROC分析发现,所构建的列线图预测模型ROC曲线下面积为0.980(95%CI:0.857-0.994),校正曲线与理想曲线拟合良好.结论老年胃癌根治术后并发耐药菌感染部位以肺部为主,病原菌类型以革兰阴性菌为主,年龄、糖尿病、术后机械通气、术后留置尿管、白蛋白、ICU住院时间、CD4^(+)/CD8^(+)是耐药菌感染的影响因素,临床可据此制定针对性防护措施,以进一步改善预后.

Hydrogen production from photoelectrochemical(PEC)water splitting has been regarded as a promising way to utilize renewable and endless solar ***,semiconductor film grown on photoelectrode suffers from numerous challenges,leading to the poor PEC ***,a straightforward sol-gel method with the ligand-induced growth strategy was employed to obtain dense and homogeneous copper bismuthate photocathodes for PEC hydrogen evolution *** various characterizations,it was found that the nucleation and surface growth of CuBi_(2)O_(4)layer induced by 2-methoxyethanol ligand(2-CuBi_(2)O_(4))demonstrated a decent crystallinity and coverage,as well as a large grain size and a low oxygen vacancy concentration,leading to the good ability of light absorption and carrier ***,under simulated sunlight irradiation(AM1.5G,100 mW/cm^(2)),the 2-CuBi_(2)O_(4)photocathode achieved an enhanced photocurrent density of−1.34 mA·cm^(−2)at 0.4 V versus the reversible hydrogen electrode and a promising applied bias photon-to-current efficiency of 0.586%.This surface modification by ligand growth strategy will shed light on the future design of advanced photoelectrodes for PEC water splitting.

Objective Isletαcells input is essential for insulin secretion fromβ*** present study aims to investigate the association between 25-hydroxyvitamin D[25(OH)D]and islet function homeostasis in type-2 diabetes(T2D)*** A total of 4670 T2D patients from seven communities in Shanghai,China were *** anthropometric indices,biochemical parameters,serum 25(OH)D,and islet function[including C-peptide(C-p)and glucagon]were *** The fasting plasma glucose(FPG),glycated hemoglobin(HbA1c),glucagon,and C-p levels exhibited a significantly decreasing trend in T2D patients as the 25(OH)D levels ***,the population was divided into two groups:abdominal obesity and non-abdominal obesity *** adjustment,the 25(OH)D level was found to be associated with HbA1c,glucagon,and homeostasis model assessment ofβ(HOMA-β)in the non-abdominal obesity *** was a significant relationship between 25(OH)D and HbA1c,glucagon,HOMA-IR,baseline insulin or C-p in the abdominal obesity *** the abdominal obesity group,the ordinary least squares(OLS)regression and quantile regression revealed that 25(OH)D was obviously associated with glucagon and fasting C-p *** the abdominal obesity group,the moderate analysis revealed a significant interaction effect of 25(OH)D and glucagon on C-p(P=0.0124).Furthermore,the conditional indirect effect of 25(OH)D on the glucagon/C-p ratio was significantly lower at 1 standard deviation(SD)below the mean(P=0.0002),and lower at the mean of the course of diabetes(P=0.0007).Conclusion 25(OH)D was found to be negatively correlated to glucagon and C-p in T2D patients with abdominal *** 25(OH)D influenced C-p in part by influencing *** effect of 25(OH)D on the glucagon/C-p ratio in T2D patients with abdominal obesity,in terms of islet homeostasis,is influenced by the course of diabetes.

BACKGROUND Myocardial ischemia-reperfusion(I/R) is a serious and irreversible injury. Bone marrow-derived mesenchymal stem cells(MSCs) is considered to be a potential therapy for I/R injury due to the paracrine effects. High-mobility group box1(HMGB1) is a novel mediator in MSC and regulates the response of inflammation injury. Signal Transduction and Transcription Activator 3(STAT3) is a critical transcription factor and important for release of paracrine factors. However, the relationship between HMGB1 and STAT3 in paracrine effect of MSC remains *** In vitro, hypoxia/reoxygenation injury model was established by Anaero Pack System and examined by Annexin V flow cytometry, CCK8 assay and morphology observation. Detection of apoptotic proteins and protein expression of HMGB1and STAT3 by Western *** The conditioned medium of MSCs with or without LPS pretreatment was cocultured with H9C2 cells for 24 h before hypoxia treatment and MSC showed obvious cardiomyocytes protect role, as evidence by decreased apoptosis rate and improved cells viability, and LPS pretreated MSC exhibited better protect role than untreated MSC. However, such effect was abolished in HMGB1 deficiency group, silencing HMGB1 decreased the secretion of vascular endothelial growth factor(VEGF), hepatocyte growth factor(HGF), insulin growth factor(IGF), cell viability, and the expression of STAT3.
