Effects of Adalimumab Entering National Reimbursement on Medical Costs for Patients With Psoriasis:A 4-Year Single-Center Retrospective Study in China

Objective:Adalimumab(ADA)is expensive and rarely used among ***,its cost for moderate to severe chronic plaque psoriasis can now be reimbursed since its addition to the National Reimbursement Drug List(NRDL)in January ***,we conducted this study to estimate the medical cost of ADA and its influencing factors in patients with psoriasis using ADA and thus gain a better understanding of the effect of the NRDL adjustment in ***:This retrospective study evaluated the medical cost of ADA before and after the addition of ADA to the NRDL(2018—2022)among 320 patients in a large professional dermatology specialty *** analysis,Welch’s analysis of variance,and multiple regression analysis were used to evaluate the medical cost and influencing ***:Before ADA was added to the NRDL,patients’mean total cost and ADA cost were¥24,243.56±16,346.53 and¥20,391.11±14,440.69,*** ADA became covered by the NRDL,the above 2 expenses decreased to¥10,461.19±7,554.66 and¥8,775.27±7,092.16,*** significant decrease in the cost of ADA resulted in a significant 110.5-times increase in ADA *** significant differences were found in the total cost,patient type(outpatient/inpatient),insurance status(yes/no),reimbursement status(before/after),and *** insurance reimbursement(Pbursement=1,after reimbursement=2)was the most significant influencing factor,followed by patient type(P=0.018,β=-0.215,outpatient=1,inpatient=2)and age(P=0.0046,β=-0.174).Conclusion:Medical insurance reimbursement is the most important factor affecting patients’total cost of ADA by reducing the economic burden,enhancing the availability of ADA,and stimulating the need for treatment.

Comparison of various prediction models in the effect of laparoscopic sleeve gastrectomy on type 2 diabetes mellitus in the Chinese population 5 years after surgery

Introduction:Acrodermatitis continua of Hallopeau(ACH)is an uncommon chronic cutaneous *** to date,its treatment is still difficult and frustrating although various topical and systemic agents alone or in different combinations have been tried with variable ***,we report two cases of ACH successfully treated with calcipotriol/betamethasone *** presentation:Two cases of ACH were characterized by thumb involvement alone and showed an excellent response to calcipotriol/betamethasone ointment(1 g:calcipotriol 50µg and betamethasone 0.5 mg).Discussion:Treatment of ACH is challenging for *** systemic therapies and topical drugs have been applied,however,no universally effective treatment regimen has yet been established for long-lasting *** of calcipotriol and betamethasone is more effective in treating psoriasis than the individual components alone,and the current 2 cases with ACH were reported to be treated successfully by the calcipotriol/betamethasone ***:Topical calcipotriol/betamethasone compound is an optional agent for the treatment of ACH with a novel effect.

BACKGROUND Early adenocarcinoma mixed with a neuroendocrine carcinoma(NEC)component arising in the gastroesophageal junctional(GEJ)region is rare and even rarer in young ***,we report such a case in a 29-year-old Chinese *** SUMMARY This patient presented to our hospital with a 3-mo history of dysphagia and *** endoscopy revealed an elevated nodule in the distal esophagus 1.6 cm above the GEJ line,without Barrett’s esophagus or involvement of the gastric *** nodule was completely resected by endoscopic submu-cosal dissection(ESD).Pathological examination confirmed diagnosis of intra-mucosal adenocarcinoma mixed with an NEC component,measuring 1.5 ***,both adenocarcinoma and NEC components were positive for P53 with a Ki67 index of 90%;NEC was positive for synaptophysin and ***-generation sequencing of 196 genes demonstrated a novel germline mutation of the ERCC3 gene in the DNA repair pathway and a germline mutation of the RNF43 gene,a common gastric cancer driver gene,in addition to pathogenic somatic mutations in P53 and CHEK2 *** patient was alive without evidence of the disease 36 mo after *** Early adenocarcinoma with an NEC component arising in the distal esophageal side of the GEJ region showed evidence of gastric origin.

Progress on gas-solid phase photoreactor and its application in CO2 reduction

The burgeoning field of photocatalytic reduction of CO2has emerged as a remarkable promising solution to address some of the most pressing global energy and environmental issues which we face today. Researchers around the global have been striving to augment the efficiency of CO2photocatalytic reduction, employing strategies that range from modifying the fundamental properties of photocatalysts to suppress the electron-hole recombination, optimizing reaction conditions to achieve the highest yield, and conceptualizing and constructing photoreactors to improve the adsorption process. Among these factors, the photoreactor plays a critical role in enhancing the overall photocatalytic efficiency. Understanding the various types of photoreactors and their operational dynamic can significantly influence the experimental design, thus guiding the data collecting and analysis. Compared to the solid-liquid phase, gas-solid phase photocatalytic reduction of CO2is gaining recognition for its potential advantages, such as rapid molecular diffusion rates, adjustable CO2concentrations, and uniform and sufficient light exposure. Nonetheless, the currently reported gas-solid phase photoreactors are still in their infancy. In this review, we dissect the underlying mechanism of photocatalytic CO2reduction and the performance evaluation criteria of photoreactors, and review the development process of gas-solid phase photoreactors. Furthermore, we explore the evolution of gas-solid phase photoreactors, elucidating their growth trajectory and future possibilities. We present a comprehensive classification of gas-solid phase photoreactors,offering a new insight into their design and functionality, summarizing their strengths and inevitable ***, we provide a forward-looking perspective on the future developmental prospects of carbon neutrality.

Abnormalities of electroencephalography microstates in patients with depression and their association with cognitive function

Over the years,numerous geotechnical approaches have been implemented to mitigate the adverse effects of climate warming on various infrastructures in the permafrost region of the qinghai-Tibet Plateau(QTP),such as the qinghai-Tibet Highway and Railway,and achieved the expected engineering ***,little attention has been given to whether the performance of these geotechnical approaches has changed during the ongoing process of climate *** investigate the performance variation of one of these geotechnical approaches,which is two-phase closed thermosyphon(TPCT),during sustained climate warming,we conducted a statistical analysis of soil temperature monitoring data in 2003-2020 from eight regular embankments and six TPCT embankments in our permafrost monitoring *** results indicate that TPCT undeniably has a cooling effect on the permafrost beneath embankments,even rapidly eliminated previously formed taliks beneath ***,further analysis reveals that the performance of TPCT has been weakening during sustained climate warming,which has confirmed by the re-forming of the taliks beneath embankment where they had been previously *** on the current understanding,we attributed the weakening of thermosyphon performance to a significant reduction in the air temperature freezing index caused by ongoing climate *** this study,we aimed to draw attention to the evolving performance of geotechnical approaches in permafrost regions amid climate warming,prompting necessary engineering innovations to address this situation and ensure the sustainable development of the permafrost region on the QTP.
