Effects of physical exercise on the quality-of-life of patients with haematological malignancies and thrombocytopenia:A systematic review and meta-analysis

BACKGROUND Annually,there are an estimated 1187000 new patients worldwide diagnosed with haematological malignancies.Effective strategies are needed to alleviate side effects and prevent the physical and psychosocial degeneration of patients in active treatment for haematological malignancies.AIM To explore the effects of physical exercise on quality of life(Qo L)of patients with haematological malignancies and thrombocytopenia.METHODS Cochrane Library,Pub Med and Embase were searched for all relevant articles reporting randomized controlled trials(RCTs)that were published up to 31 July 2021.Two authors independently selected articles in accordance with the inclusion criteria,evaluated their quality,and collected information.Any controversy was resolved through discussion with a third senior author.The PRISMA 2009 checklist was followed.RESULTS Seven RCTs were selected in the systematic review and three were included in the final meta-analysis.There were significant differences in Qo L between physical exercise groups[mean score difference=8.81;95%confidence interval(CI):1.81-15.81,P=0.01],especially in emotional functioning(mean score difference=12.34;95%CI:4.64-20.04,P=0.002)and pain(mean score difference=–12.77;95%CI:-3.91 to-21.63,P=0.005).CONCLUSION Physical exercise has clinical effects on Qo L and improves emotional function and pain indices of patients with haematological malignancies and thrombocytopenia.

A review of lignans from genus Kadsura and their spectrum characteristics

Kadsura belongs to the Schisandroideae subfamily of Magnoliaceae.Plants from genus Kadsura are widely distributed in the South and Southwest of China.The plants of the genus are widely used as folk medicine for a long time in history,with the functions of relieving pain,promoting 'qi' circulation,activating blood resolve stasis,and applications in the treatment of rheumatoid arthritis and gastroenteric disorders.Lignans are the primary characteristic constituents with various biological activities of plants from genus Kadsura.This paper summarized 81 lignans isolated from the plants of genus Kadsura over the past eight years(from 2014 to 2021),which belong to five types:dibenzocyclooctadienes,spirobenzofuranoid dibenzocyclooctadienes,aryltetralins,diarylbutanes and tetrahydrofurans.Each type of these lignans possess typical characteristics in proton magnetic resonance(^(1)H NMR) and carbon-13 nuclear magnetic resonance(^(13)C NMR) spectra,the NMR regularities of these types of lingans were summarized,which provided a useful reference for the structural analysis of lignans.The relationships between lignans and pharmacodynamics were also systematically analyzed,lignans were predicted to be the quality markers(Q-marker) of Kadsura genus.

Lanostane Tetracyclic Triterpenoids as Important Sources for Anti-Inflammatory Drug Discovery

Inflammation is a defensive reaction of the human body to numerous detrimental stimuli,including physical trauma,noxious chemicals,as well as microbial agents.Uncontrolled inflammation is the pathological basis of multiple diseases,such as rheumatoid arthritis(RA),neurodegenerative diseases,liver disease,and lung inflammation.Lanostane triterpenoids are natural tetracyclic triterpenoids with significant anti-inflammatory activity.An extensive review of the published literature regarding the phytochemistry and anti-inflammatory pharmacology of lanostane triterpenoids has been performed and analyzed using several search engines,such as SciFinder,Web of Science,Scopus,PubMed,Google Scholar,and ScienceDirect.This review is devoted to naturally occurring lanostane-type triterpenes with anti-inflammatory activity,including their sources,biosynthesis,and mechanism of action.This review also discusses the inflammation-related diseases and the clinical significance of traditional Chinese medicine as multi-target therapeutic agents for the prevention and treatment of inflammatory diseases.In the past 30 years,more than 100 new lanostane-type triterpenes have been reported from the families Schisandraceae,Ganodermataceae,and Polyporaceae.Six compounds,fomitopinic acid A,fomitosides E and F,obtusifoliol,4α,l4α-dimethyl-5α-ergosta-7,9(11),24(28)-trien-3β-ol,and gramisterol exhibited the most potent anti-inflammatory activity against cyclooxygenase-1(COX-1)and COX-2,with IC50 values ranging from 0.087 to 1.15μM.Some of these compounds exhibited significant activity by mediating the inhibition of the pro-inflammatory cytokines,inducible nitric oxide synthase,and COX-2 expression.This review provides a basis for identifying anti-inflammatory drugs with high selectivity,high potency,and few adverse effects from lanostane-type triterpenes.

Kadsura coccinea belongs to medicinally important genus Kadsura from the Schisandraceae family.It has been used in traditional Chinese medicine(TCM)for the treatment of rheumatoid arthritis and gastroenteric disorders.The initial phytochemical work focused on the identification of some structurally novel and diverse natural products,which turned the attention of many researchers towards this plant.Thus far,202 compounds have been reported in this plant.Lignans and terpenoids were found as the main chemical constituents of this plant.Some of the triterpenoids and sesquiterpenoids with novel structures are of particular interest for natural product researchers.The isolated compounds of this plant have shown different bioactivities including anti-tumor,anti-HIV,anti-inflammatory,nitric oxide(NO)production inhibitory and other pharmacological effects.This review systematically summarizes all the phytochemical and pharmacological work done so far on K.coccinea,and can be used as a reference for future research on this plant.

Molecular Dynamics Simulation Studies on the Micromorphology and Proton Transport of Nafion/Ti_(3)C_(2)T_(x) Composite Membrane

The perfluorosulfonic acid(PFSA) membrane doped with two-dimensional conductive filler Ti_(3)C_(2)T_(x) is a fuel cell proton exchange membrane with high application potential. Experimental studies showed that the proton conductivity of Nafion/Ti_(3)C_(2)T_(x) composite membrane is improved significantly compared with that in pure Nafion. However, the microscopic mechanism of doping on the enhancement of membrane performance is remain unclear now. In this work, molecular dynamics simulation was used to investigate the microscopic morphology and proton transport behaviors of Nafion/Ti_(3)C_(2)T_(x) composite membrane at the molecular level. The results shown that there were significant differences about the diffusion kinetics of water molecules and hydroxium ions in Nafion/Ti_(3)C_(2)T_(x) at low and high hydration levels in the nanoscale region.With the increase of water content, Ti_(3)C_(2)T_(x) in membrane was gradually surrounded by ambient water molecules to form a hydration layer, and forming a relatively continuous proton transport channel between Nafion polymer and Ti_(3)C_(2)T_(x) monomer. The continuous proton transport channel could increase the number of binding sites of proton and thus achieving high proton conductivity and high mobility of water molecules at higher hydration level. The current work can provide a theoretical guidance for designing new type of Nafion composite membranes.

Large isolated fibrous tumors in the upper esophagus: A case report

BACKGROUND Solitary fibrous tumors(SFT)are rare spindle cell tumors that are usually benign.A total of 10 cases of SFTs in the upper esophagus have ever been reported.Here,we report the anesthetic management of a patient with a large isolated fibrous tumor of the upper esophagus compressing the tracheal membrane.We also provide a literature review of the current research.CASE SUMMARY We report the case of a 49 year old male with“cough aggravation and wheezing after exercise”,who underwent esophagectomy for a large isolated fibrous tumor compressing the tracheal membrane in the upper esophagus.We advise the use of a single-lumen tube with a blocker in patients with difficult airways to reduce the incidence of airway injury and fibrinoscopy at all stages of the perioperative period to guide airway management.This case study is the first report of the anesthetic management of a large,isolated fibrous tumor compressing the tracheal membrane in the upper esophagus.CONCLUSION This rare case emphasizes the importance of perioperative management of anesthesia in patients with large isolated fibrous tumors of the upper esophagus that compress the tracheal membrane.The use of blocker reduce the incidence of airway injury and fibrinoscopy at the perioperative period to guide airway mana-gement.

Carbon dots-derived three-dimensional-ordered macroporous host with high penetrability and catalytic activity for LSBs under lean electrolyte

The practical energy density of lithium-sulfur batteries（LSBs） is seriously limited by the high electrolyte-to-sulfur ratios（E/S）.The E/S can be reduced by employing hosts with easy-to-infiltrate structure that ensures the uniform distribution of a lean electrolyte and high catalytic activity that can suppress "shuttle effect" via accelerating the slow conversion of soluble poly sulfides to insoluble sulfides.Among the easy-to-infiltrate structures,the three-dimensional-ordered macroporous（3DOM）structure is easier to scale preparation and more suitable for the existing industrial processes.However,it is difficult to obtain the 3DOM with（1） high penetrability due to the high viscosity of frequently-used organic polymer precursors and（2） high catalytic activity due to the low confinement effect,meaning that uniformly dispersed small-sized catalytic materials are difficult to load on 3DOM.Herein,using carbon dots（CDs） with both organic and inorganic properties as a precursor and aggregation limiting agent,a 3DOM host with high penetrability and homodispersed Ni particles of small sizes（Ni-CDs-3DOM）was synthesized.In this host:（1） CDs precursor with suitable viscosity can improve the penetrability.（2） CDs can effectively inhibit the agglomeration of Ni particles.（3） Uniformly dispersed small-size Ni particles offer high-efficiency catalytic activity toward sulfur reactions.Consequently,the Ni-CDs-3DOM/sulfur cathode exhibits high sulfur utilization and stable cycling performance even under high sulfur loading(5.5 mg·cm-1) and low E/S(6.5 μl·mg-1).This work indicates the usefulness of CDs in constructing hosts for LSBs with high energy density.

Investigating the causal associations between five anthropometric indicators and nonalcoholic fatty liver disease:Mendelian randomization study

BACKGROUND Although the etiology of nonalcoholic fatty liver disease(NAFLD)has not been thoroughly understood,the emerging roles of anthropometric indicators in assessing and predicting the risk of NAFLD have been highlighted by accumulating evidence.AIM To evaluate the causal relationships between five anthropometric indicators and NAFLD employing Mendelian randomization(MR)design.METHODS The Anthropometric Consortium provided genetic exposure data for five anthropometric indicators,including hip circumference(HC),waist circumference(WC),waist-to-hip ratio(WHR),body mass index(BMI),and body fat percentage(BF).Genetic outcome data for NAFLD were obtained from the United Kingdom Biobank and FinnGen Consortium.Genome-wide significant single nucleotide polymorphisms were chosen as instrumental variables.Univariable MR(UVMR)and multivariable MR(MVMR)designs with analytical approaches,including inverse variance weighted(IVW),MR-Egger,weighted median(WM),and weighted mode methods,were used to assess the causal relationships between anthropometric indicators and NAFLD.RESULTS Causal relationships were revealed by UVMR,indicating that a higher risk of NAFLD was associated with a perunit increase in WC[IVW:odds ratio(OR)=2.67,95%CI:1.42-5.02,P=2.25×10^(−3)],and BF was causally associated with an increased risk of NAFLD(WM:OR=2.23,95%CI:1.07-4.66,P=0.033).The presence of causal effects of WC on the decreased risk of NAFLD was supported by MVMR after adjusting for BMI and smoking.However,no causal association between BF and NAFLD was observed.In addition,other causal relationships of HC,WHR(BMI adjusted),and BMI with the risk of NAFLD were not retained after FDR correction.CONCLUSION This study establishes a causal relationship,indicating that an increase in WC is associated with a higher risk of NAFLD.This demonstrates that a suitable decrease in WC is advantageous for preventing NAFLD.

Association of physical activity with risk of chronic kidney disease in China:A population-based cohort study

Background:Information on the association between physical activity(PA)and the risk of chronic kidney disease(CKD)is limited.We aimed to explore the associations of total,domain-specific,and intensity-specific PA with CKD and its subtypes in China.Methods:The study included 475,376 adults from the China Kadoorie Biobank aged 30-79 years during 2004-2008 at baseline.An interviewer-administered questionnaire was used to collect the information about PA,which was quantified as metabolic equivalent of task hours per day(MET-h/day)and categorized into 4 groups based on quartiles.Cox regression was used to analyze the association between PA and CKD risk.Results:During a median follow-up of 12.1 years,5415 incident CKD cases were documented,including 1159 incident diabetic kidney disease(DKD)cases and 362 incident hypertensive nephropathy(HTN)cases.Total PA was inversely associated with CKD risk,with an adjusted hazard ratio(HR,95%confidence interval(95%CI))of 0.83(0.75-0.92)for incident CKD in the highest quartile of total PA as compared with participants in the lowest quartile.Similar results were observed for risk of DKD and HTN,and the corresponding HRs(95%CIs)were 0.75(0.58-0.97)for DKD risk and 0.56(0.37-0.85)for HTN risk.Increased nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA were significantly associated with a decreased risk of CKD,with HRs(95%CIs)of 0.80(0.73-0.88),0.85(0.77-0.94),and 0.85(0.76-0.95)in the highest quartile,respectively.Conclusion:PA,including nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA,was inversely associated with the risk of CKD,including DKD,HTN,and other CKD,and such associations were dose dependent.

Petrogenesis and tectonic implications of the Silurian adakitic granitoids in the eastern segment of the Qilian Orogenic Belt,Northwest China

Geodynamic mechanism responsible for the generation of Silurian granitoids and the tectonic evolution of the Qilian orogenic belt remains controversial. In this study, we report the results of zircon U–Pb age, and systematic whole-rock geochemical data for the Haoquangou and Liujiaxia granitoids within the North Qilian orogenic belt and the Qilian Block, respectively, to constrain their petrogenesis, and the Silurian tectonic evolution of the Qilian orogenic belt. Zircon U–Pb ages indicate that the Haoquangou and Liujiaxia intrusions were emplaced at423 ± 3 Ma and 432 ± 4 Ma, respectively. The Haoquangou granodiorites are calc-alkaline, while the Liujiaxia granites belong to the high-K calc-alkaline series.Both are peraluminous in composition and have relatively depleted Nd isotopic [ε_(Nd)(t) =(-3.9 – + 0.6)] characteristics compared with regional basement rocks, implying their derivation from a juvenile lower crust. They show adakitic geochemical characteristics and were generated by partial melting of thickened lower continental crust. Postcollisional extensional regime related to lithospheric delamination was the most likely geodynamic mechanism for the generation of the Haoquangou granodiorite, while the Liujiaxia granites were generated in a compressive setting during continental collision between the Qaidam and Qilian blocks.
