Predictors of the outcomes of acute-on-chronic hepatitis B liver failure

AIM： To identify the risk factors in predicting the out- come of acute-on-chronic hepatitis B liver failure pa- tients. METHODS： We retrospectively divided 113 patients with acute-on-chronic liver failure-hepatitis B virus （ACLF-HBV） and without concurrent hepatitis C or D virus infection and hepatocellular carcinoma into two groups according to their outcomes after anti-HBV therapy. Their demographic, clinical, and biochemical data on the day of diagnosis and after the first week of treatment were analyzed using the Mann-Whitney U test, Fisher＇s exact test, and a multiple logistic regres- sion analysis. RESULTS： The study included 113 patients （87 men and 26 women） with a mean age of 49.84 years. Fifty- two patients survived, and 61 patients died. Liver failure （85.2%）, sepsis （34.4%）, and multiple organ failure （39.3%） were the main causes of death. Mul- tivariate analyses showed that Acute Physiology and Chronic Health Evaluation （APACHE） Ⅱ scores ≥ 12 [odds ratio （OR） = 7.160, 95% CI： 2.834-18.092, P 〈 0.001] and positive blood culture （OR = 13.520, 95% CI： 2.740-66.721, P = 0.001） on the day of diagnosis and model for end-stage liver disease （MELD） scores 28 （OR = 8.182, 95% CI： 1.884-35.527, P = 0.005） after the first week of treatment were independent predictors of mortality. CONCLUSION： APACHE II scores on the day of diag- nosis and MELD scores after the first week of anti-HBV therapy are feasible predictors of outcome in ACLF- HBV patients.

AIM To investigate the association of chronic hepatitis B and nonalcoholic steatohepatitis with physical fitness in a Taiwan Residents military male *** We made a cross-sectional examination of this association using 3669 young adult military males according to cardiorespiratory fitness and hospitalization events recorded in the Taiwan Armed Forces study. Cases of chronic hepatitis B(n = 121) were defined by personal history and positive detection of hepatitis B surface antigen. Cases of nonalcoholic steatohepatitis(n = 129) were defined by alanine transaminase level > 60 U/L, liver ultrasound finding of steatosis, and absence of viral hepatitis A, B or C infection. All other study participants were defined as unaffected(n = 3419). Physical fitness was evaluated by performance in 3000-m run, 2-min sit-ups, and 2-min push-ups exercises, with all the procedures standardized by a computerized scoring system. Multiple linear regression analysis was used to determine the *** Chronic hepatitis B negatively correlated with 2-min push-up numbers(β =-2.49, P = 0.019) after adjusting for age, service specialty, body mass index, systolic and diastolic blood pressures, current cigarette smoking, alcohol intake status, serum hemoglobin, and average weekly exercise times. Nonalcoholic steatohepatitis was borderline positively correlated with 3000-m running time(β = 11.96, P = 0.084) and negatively correlated with 2-min sit-up numbers(β =-1.47, P = 0.040). CONCLUSION Chronic hepatitis B viral infection and nonalcoholic steatohepatitis affects different physical performances in young adult military males, and future study should determine the underlying mechanism.

In this paper,we propose a new gradient vector flow model with advection enhancement,called advection-enhanced gradient vector flow,for calculating the external force employed in the active-contour image *** proposed model is mainly inspired by the functional derivative of an adaptive total variation regularizer whose minimizer is expected to be able to effectively preserve the desired object *** specifically,by incorporating an additional advection term into the usual gradient vector flow model,the resulting external force can much better help the active contour to recover missing edges,to converge to a narrow and deep concavity,and to preserve weak *** experiments are performed to demonstrate the high performance of the newly proposed model.

Chemotactic Activity of Site-Specific Multivalent Conjugates of Stromal Cell-Derived Factor 1<i>α</i>on Branched Nanoparticles

Stromal cell-derived factor 1α (SDF1α) is a potent chemokine for the recruitment of stem cells. A challenge is to maintain its activity and control its release. In this study, we engineered a recombinant cysteine-SDF1α (cysSDF1α) protein, and performed multivalent conjugation of cysSDF1 through the maleimide functional group to two forms of branched nanoparticles: multi-arm poly (ethylene glycol) (MA-PEG) and hyaluronic acid (HA). We characterized the chemotactic activity of the conjugates, and determined how the molecular weight (MW) of MA-PEG and HA affected the chemotactic activity. CysSDF1α had similar efficiency to wild-type SDF1α in cell recruitment. Multivalent conjugation of cysSDF1α to low MW MA-PEG (~18 nm) did not significantly affect the chemotactic activity, while the conjugation of cysSDF1α to high MW MA-PEG (~72 nm) lowered the efficiency, possibly due to the larger spacing between conjugated SDF1α molecules. HA has a linear backbone and a high density of multivalent binding sites;however, the chemotactic activity of HA-linked cys-SDF1α was much lower, which further decreased with the increase of HA MW from 200 kDa (~0.78 μm) to 700 kDa (~2.7 μm). Digestion of HA into smaller fragments using hyaluronidase partially recovered the chemotactic activity of cysSDF1α, suggesting that high MW HA might exert steric hindrance for SDF1α binding to its receptors on cell surface and that HA could be used as a depot for SDF1α storage and release. These results demonstrate that multivalent conjugates of SDF1α to nanoparticles may be used to engineer SDF1α delivery for cell recruitment and tissue regeneration.

A Field Assessment on Natural Ventilation and Thermal Comfort of Historical District--A Case of the Wugoushui Settlement in Taiwan

Due to the urban and town development, it is common to see historical buildings and new buildings intermingled with one and another inside historical regions in Taiwan. With the increase of new buildings, natural ventilation becomes harder, which leads to the impact on the preservation of historical building and the reduction of thermal comfort for residents. Besides, problems of new buildings＇ construction and design affect the developments of cities and towns in historical district, which has existed for hundreds of years. The Wugoushui Settlement is located in Pingtung County, the southernmost part of Taiwan, and its hot season of a year is pretty long and belongs to warm and humid tropical climate. In 2008, the Pingtung County Government officially registered and announced Wugoushui as traditional settlement and meanwhile, provided the maintenance and preservation for buildings in this region on government subsidies. This paper is based on one-year-long field experiments of Wugoushui Settlement. Residents in Wugoushui Settlement continue constructing new buildings near by the traditional buildings. With the fact that constructions of new buildings result in huge impact on the circumstances of natural ventilation and thermal comfort, which have been for hundreds of years, the simple weather station is set up to collect the data of local microclimate. In addition, four representative traditional architectures were selected to be measured their indoor and outdoor temperature, humidity, wind speed, wind direction and thermal radiation both in winter （December to February） and in summer （May to September）. The measurement time of each building is one week and every data record is kept every minute. Besides, residents were asked to fill out the comfort survey. Based on these data, the distributions of indoor and outdoor thermal environment as well as the thermal comfort of living space under the condition of the natural ventilation are assessed.
