Economic and medical benefits of ultrasound screenings for gallstone disease

AIM:To investigate whether screening for gallstone disease was economically feasible and clinically ***:This clinical study was initially conducted in2002 in Taipei,*** study cohort total included2386 healthy adults who were voluntarily admitted to a regional teaching hospital for a physical *** follow-up screening with ultrasound sonography for gallstone disease continued until December 31,2007.A decision analysis using the Markov Decision Model was constructed to compare different screening regimes for gallstone *** economic evaluation included estimates of both the cost-effectiveness and cost-utility of screening for gallstone ***:Direct costs included the cost of screening,regular clinical fees,laparoscopic cholecystectomy,and *** costs represent the loss of productivity attributable to the patient’s disease state,and were estimated using the gross domestic product for 2011 in *** time intervals in screening for gallstone disease were associated with the reduced efficacy and utility of screening and with increased *** cost per life-year gained(average cost-effectiveness ratio)for annual screening,biennial screening,3-year screening,4-year screening,5-year screening,and no-screening was new Taiwan dollars(NTD)39076,NTD 58059,NTD 72168,NTD 104488,NTD 126941,and NTD 197473,respectively(P<0.05).The cost per quality-adjusted life-year gained by annual screening was NTD 40725;biennial screening,NTD64868;3-year screening,NTD 84532;4-year screening,NTD 110962;5-year screening,NTD 142053;and for the control group,NTD 202979(P<0.05).The threshold values indicated that the ultrasound sonography screening programs were highly sensitive to screening costs in a plausible ***:Routine screening regime for gallstone disease is both medically and economically *** screening for gallstone disease should be recommended.

Realization of energy-saving glass using photonic crystals

This work successfully developed an energy- saving glass with wavelength selectivity. The glass is composed of a SiO2 substrate and two layers of three- dimensional photonic crystals. Each crystal is composed of identical and transparent polystyrene spheres after their self-assembling. The glass then possesses dual photonic band gaps in the near-infrared region to suppress penetration of thermal radiation. Experimental results show that the energy-saving glass decreases temperature increment in a mini-house. Moreover, the temperature after thermal equilibrium is lower than that inside a counterpart using ordinary glass.

Biosynthesis and thermal properties of poly（3-hydroxybutyrate-co-3-hydroxyvalerate）（PHBV） with different HV （hydrovalerate） content produced by a Bacillus cereus strain were investigated. A large variety of HV contents （up to about 90 mol%） of PHBV could be produced by this strain. Combined nitrogen sources containing both yeast extract and ammonium sulphate were better for cell growth and polyhydroxyalkanoates （PHA） production than either yeast extract or ammonium sulphate alone. Propionic acid is more favorable for the production of HV content than that of valeric acid. Finally, thermal properties of PHBV produced by this strain are found close to the results of other groups.

On-line full scan inspection of particle size and shape using digital image processing

An on-line full scan inspection system is developed for particle size analysis. A particle image is first obtained through optical line scan technology and is then analyzed using digital image processing. The system is composed of a particle separation module, an image acquisition module, an image processing module, and an electric control module. Experiments are carried out using non-uniform 0.1 mm particles. The main advantage of this system consists of a full analysis of particles without any overlap or miss, thus improving the Area Scan Charge Coupled Device （CCD） acquisition problems. Particle size distribution, roundness, and sphericity can be obtained using the system with a deviation of repeated precision of around ±1%. The developed system is shown to be also convenient and versatile for any particle size and shape for academic and industrial users.

The known factors that have contributed to the decline of Helicobacter pylori (H. pylori) eradication rate include antibiotic resistance, poor compliance, high gastric acidity, high bacterial load, and cytochrome P450 2C19 (CYP2C19) polymorphism. Proton pump inhibitor (PPI) is important in the eradication regimen. The principal enzyme implicated in the metabolism of PPIs is CYP2C19. The effects of PPI depend on metabolic enzyme, cytochrome P450 enzymes, and CYP2C19 with genetic differences in the activity of this enzyme (the homozygous EM, heterozygous EM (HetEM), and poor metabolizer). The frequency of the CYP2C19 polymorphism is highly varied among different ethnic populations. The CYP2C19 genotype is a cardinal factor of H. pylori eradication in patients taking omeprazole- based or lansoprazole-based triple therapies. In contrast, the CYP2C19 polymorphism has no significant effect on the rabeprazole-based or esomeprazole-based triple therapies. The efficacy of levofloxacin-based rescue triple therapy might be also affected by the CYP2C19 polymorphism, but CYP2C19 genotypes did not show obvious impact on other levofloxacin-based rescue therapies. Choice of different PPIs and/or increasing doses of PPIs should be individualized based on the pharmacogenetics background of each patient and pharmacological profile of each drug. Other possible factors influencing gastric acid secretion (e.g., IL-1β- 511 polymorphism) would be also under consideration.

AIM:To investigate the prevalence and risk factors of asymptomatic peptic ulcer disease(PUD)in a general Taiwan Residents population. METHODS:From January to August 2008,consecutive asymptomatic subjects undergoing a routine health check-up were evaluated by upper gastrointestinal *** mucosal breaks were carefully assessed,and a complete medical history and demographic data were obtained from each *** regression analysis was conducted to identify indepen-dent risk factors for asymptomatic PUD. RESULTS:Of the 572 asymptomatic subjects,54(9.4%) were diagnosed as having *** prevalence of gastric ulcer,duodenal ulcer and both gastric and duodenal ulcers were 4.7%,3.9%,and 0.9%,respectively. Multivariate analysis revealed that prior history of PUD [odds ratio(OR),2.0,95%CI:1.3-2.9],high body mass index[body mass index(BMI)25-30:OR,1.5,95%CI: 1.0-2.2;BMI>30 kg/m 2 :OR,3.6,95%CI:1.5-8.7] and current smoker(OR,2.6,95%CI:1.6-4.4)were independent predictors of asymptomatic *** contrast, high education level was a negative predictor of PUD (years of education 10-12:OR,0.5,95%CI:0.3-0.8; years of education>12:OR,0.6,95%CI:0.3-0.9). CONCLUSION:The prevalence of PUD in asymptomatic subjects is 9.4%in *** history of PUD, low education level,a high BMI and current smoker are independent risk factors for developing asymptomatic PUD.

Application of Active Contour Model in Tracking Sequential Nearshore Waves

In the present study, a generalized active contour model of gradient vector flow is combined with the video techniques of Argus system to delineate and track sequential nearshore wave crest profiles in the shoaling process, up to their breaking on the shoreline. Previous applications of active contour models to water wave problems are limited to controllable wave tank experiments. By contrast, our application in this study is in a nearshore field environment where oblique images obtained under natural and varying condition of ambient light are employed. Existing Argus techniques produce plane image data or time series data from a selected small subset of discrete pixels. By contrast, the active contour model produces line image data along continuous visible curves such as wave crest profiles. The combination of these two existing techniques, the active contour model and Argus methodologies, facilitates the estimates of the direction wave field and phase speeds within the whole area covered by camera. These estimates are useful for the purpose of inverse calculation of the water depth. Applications of the present techniques to Hsi-tzu bay where a beach restoration program is currently undertaken are illustrated. This extension of Argus video techniques provides new application of optical remote sensing to study the hydrodynamics and morphology of a nearshore environment.

Esophagectomy for locally advanced esophageal cancer, followed by chemoradiotherapy and adjuvant chemotherapy

AIM： To compare the efficacy and toxicity of a three-step combination therapy with post-operative radiation alone for locally advanced esophageal cancer.METHODS： Patients with T3-4 and N0-1 esophageal carcinoma from a number of institutions were non-randomly, prospectively enrolled in the study. All patients underwent single-stage curative en bloc esophagectomy. The patients were then assigned into one of two treatment groups based on treatment consisting of either post-operative concurrent chemoradiotherapy （CCRT） with weekly cisplatin 30 mg/m^2 followed by systemic adjuvant chemotherapy （four monthly cycles of cisplatin 20 mg/m^2 and 5-fluorouracil 1 000 mg/m^2 for five consecutive days）, or, post-operative radiation alone. The radiotherapy dose was 55-60 Gy for all patients. Primary end-point of this study was to assess the per-protocol patients＇ improvement of overall survival benefit. Secondary end-point was designed to evaluate both the per-protocol and intent-totreat patients＇ outcome of survival. RESULTS： A total of 60 patients （n=30 per group） were enrolled in this study. The two groups were generally comparable for demographic characteristics and hematological and non-hematological toxicities. The CCRT with weekly cisplatin was well tolerated, with significantly better overall survival （30.9 mo vs 20.7 mo; 95% CI, 27.5-36.4 vs 15.2-26.1） and 3-year survival （70.0% vs 33.7%; P=0.003）. Low histological grade of tumor （P〈0.001） was associated with favorable survival in these locally advanced patients. CONCLUSION： For locally advanced esophageal cancer, the combination of esophagectomy, post-operative CCRT with weekly cisplatin and systemic adjuvant chemotherapy is well tolerated and effective. A large-scale, prospective randomized trial of this regimen is in progress.

A high-fidelity design methodology using LES-based simulation and POD-based emulation:A case study of swirl injectors

Engineering design is undergoing a paradigm shift from design for performance to design for affordability, operability, and durability, seeking multi-objective optimization. To facilitate this transformation, significantly extended design freedom and knowledge must be available in the early design stages. This paper presents a high-fidelity framework for design and optimization of the liquid swirl injectors that are widely used in aerospace propulsion and power-generation systems. The framework assembles a set of techniques, including Design Of Experiment(DOE), high-fidelity Large Eddy Simulations(LES), machine learning, Proper Orthogonal Decomposition(POD)-based Kriging surrogate modeling(emulation), inverse problem optimization, and uncertainty quantification. LES-based simulations can reveal detailed spatiotemporal evolution of flow structures and flame dynamics in a high-fidelity manner, and identify important injector design parameters according to their effects on propellant mixing, flame stabilization, and thermal *** a given a space of design parameters, DOE determines the number of design points to perform LES-based simulations. POD-based emulations, trained by the LES database, can effectively explore the design space and deduce an optimal group of design parameters in a turn-around time that is reduced by three orders of magnitude. The accuracy of the emulated results is validated, and the uncertainty of prediction is quantified. The proposed design methodology is expected to profoundly extend the knowledge base and reduce the cost for initial design stages.
