Celiac disease(CD) is now recognized as a common disorder among Middle Eastern(ME) and North African(NA) *** aim of this review is to assess the available data regarding CD in the ME and NA and to compare this information with that of Western countries.A literature review was performed using the electronic databases PubMed and Medline(1950-2008) as search engines,and "celiac disease" was used as a Mesh *** search was limited to ME and NA *** prevalence of CD in ME and NA countries among low risk populations is similar to that of Western countries,but is higher in high risk populations such as those with type 1 *** is underestimated because of lack of clinical suspicion and lack of patient *** presentations in term of gastrointestinal,hematologic,skeletal,and liver manifestations are similar between both populations except for a high prevalence of short stature in some ME and NA *** studies have addressed atypical or silent *** in the West,diagnosis is initially made by serological tests and is confirmed by small intestinal ***-free diet is the main mode of treatment with a higher apparent adherence rate than in the *** disease complications result from *** disease is strongly associated with HLA DQ2 and to a lesser extent with HLA DQ8 *** conclusion,CD prevalence is underestimated,with little data available about its malignant *** parameters in the ME and NA are otherwise similar to those in Western countries.

AIM： To compare the efficacy of the proton-pump inhibitor, rabeprazole, with that of the H2-receptor antagonist, ranitidine, as on-demand therapy for relieving symptoms associated with non-erosive reflux disease （NEED）.METHODS： This is a single center, prospective, randomized, open-label trial of on-demand therapy with rabeprazole （group A） vs ranitidine （group B） for 4 wk. Eighty-three patients who presented to the American University of Beirut Medical Center with persistent gas- troesophageal reflux disease （GERD） symptoms and a normal upper gastrointestinal endoscopy were eligible for the study. Patients in group A （n = 44） were al-lowed a maximum rabeprazole dose of 20 mg twice daily, while those in group B （n = 39） were allowed a maximum ranitidine dose of 300 mg twice daily. Ef- ficacy was assessed by patient evaluation of global symptom relief, scores of the SF-36 quality of life （QoL） questionnaires, total number of pills used, and number of medication-free days.RESULTS： Among the 83 patients who were enrolled in the study, 76 patients （40 in the rabeprazole group and 36 in the ranitidine group） completed the 4-wk trial. Baseline characteristics were comparable between both groups. After 4 wk, there was no significant difference in the subjective global symptom relief between the rabeprazole and the ranitidine groups （71.4% vs 65.4%, respectively; P = 0.9）. There were no statistically significant differences between mean cumulative scores of the SF-36 QoL questionnaire for the two study groups （rabeprazole 22.40±27.53 vs ranitidine 17.28 ± 37.06; P = 0.582）. There was no significant difference in the mean number of pills used （rabeprazole 35.70±29.75 vs ranitidine 32.86±26.98; P = 0.66）. There was also no statistically significant difference in the mean number of medication-free days between both groups.CONCLUSION： Rabeprazole has a comparable efficacy compared to ranitidine when given on-demand for the treatment of NERD. Pegylated interferon plus ribavirin remains the firstline treatment for patients with hepatitis C virus(HCV) . Interferonαhas the most extensive clinical application and is used for the treatment of chronic hepatitis B virus and hepatitis D virus as well as acute and chronic HCV *** attachment of polyethylene glycol to interferon increases its half-life by reducing the rate of absorption after injection,reducing renal and cellular clearance and also decreasing *** this case report,we have described a patient with chronic hepatitis C who developed ischemic necrosis of her fingertips after completing her third course of pegylated interferon and *** patient underwent a very extensive workup in order to determine the underlying cause of her digital ischemia which was finally determined to be secondary to the use of pegylated interferon.

Social media plays a crucial role in the organization of massive social movements. However, the sheer quantity of data generated by the events as well as the data collection restrictions that researchers encounter, leads to a series of challenges for researchers who want to analyze dynamic public discourse and opinion in response to and in the creation of world events. In this paper we present gatherTweet, a Python package that helps researchers efficiently collect social media data for events that are composed of many decentralized actions (across both space and time). The package is useful for studies that require analysis of the organizational or baseline messaging before an action, the action itself, and the effects of the action on subsequent public discourse. By capturing these aspects of world events gatherTweet enables the study of events and actions like protests, natural disasters, and elections.
