Bispectral index monitoring as an adjunct to nurse-administered combined sedation during endoscopic retrograde cholangiopancreatography

AIM:To determine whether bispectral index (BIS) monitoring is useful for propofol administration for deep sedation during endoscopic retrograde cholangiopancreatography (ERCP). METHODS:Fifty-nine consecutive patients with a variety of reasons for ERCP who underwent the procedure at least twice between 1 July 2010 and 30 November 2010. This was a randomized cross-over study, in which each patient underwent ERCP twice, once with BIS monitoring and once with control monitoring. Whether BIS monitoring was done during the first or second ERCP procedure was random. Patients were intermittently administered a mixed regimen including midazolam, pethidine, and propofol by trained nurses. The nurse used a routine practice to monitor sedation using the Modified Observer's Assessment of Alertness/Sedation (MOAA/S) scale or the BIS monitoring. The total amount of midazolam and propofol used and serious side effects were compared between the BIS and control groups. RESULTS:The mean total propofol dose administered was 53.1 ± 32.2 mg in the BIS group and 54.9 ± 30.8 mg in the control group (P = 0.673). The individual propofol dose received per minute during the ERCP procedure was 2.90 ± 1.83 mg/min in the BIS group and 3.44 ± 2.04 mg in the control group (P = 0.103). The median value of the MOAA/S score during the maintenance phase of sedation was comparable for the two groups. The mean BIS values throughout the procedure (from insertion to removal of the endoscope) were 76.5 ± 8.7 for all 59 patients in using the BIS monitor. No significant differences in the frequency of < 80% oxygen saturation, hypotension (< 80 mmHg), or bradycardia (< 50 beats/min) were observed between the two study groups. Four cases of poor cooperation occurred, in which the procedure should be stopped to add the propofol dose. Light Detection and Ranging(LiDAR) systems can be used to estimate both vertical and horizontal forest *** components,the leaves of trees and the understory can be described with high precision,using geo-registered *** on this concept,the Effective Plant Area Indices(PAIe) for areas of Korean Pine(Pinus koraiensis),Japanese Larch(Larix leptolepis) and Oak(Quercus spp.) were estimated by calculating the ratio of intercepted and incident LIDAR laser rays for the canopies of the three forest ***,the canopy gap fraction(GLiDAR) was generated by extracting the LiDAR data reflected from the canopy surface,or inner canopy area,using k-means *** LiDAR-derived PAIe was then estimated by using GLIDAR with the Beer-Lambert law.A comparison of the LiDAR-derived and field-derived PAIe revealed the coefficients of determination for Korean Pine,Japanese Larch and Oak to be 0.82,0.64 and 0.59,*** differences between field-based and LIDAR-based PAIe for the different forest types were attributed to the amount of leaves and branches in the forest *** absence of leaves,in the case of both Larch and Oak,meant that the LiDAR pulses were only reflected from *** probability that the LiDAR pulses are reflected from bare branches is low as compared to the reflection from branches with a high leaf *** is because the size of the branch is smaller than the resolution across and along the 1 meter LIDAR laser ***,a better predictive accuracy would be expected for the model if the study would be repeated in late spring when the shoots and leaves of the deciduous trees begin to appear.

Background:Activated hepatic stellate cells(HSCs)are closely involved in the initiation,perpetuation,and resolution of liver ***-inflammatory cytokine levels are positively correlated with the transition from liver injury to fibrogenesis and contribute to HSC pathophysiology in liver ***:In this study,we investigated the effect of the pro-inflammatory cytokine interleukin(IL)-1βon the proliferation and signaling pathways involved in fibrogenesis in LX-2 cells,an HSC cell line,using western blotting and cell proliferation ***:IL-1βincreased the proliferation rate andα-smooth muscle actin(SMA)expression of LX-2 cells in a dose-dependent *** 1 h after IL-1βtreatment,c-Jun N-terminal kinase(JNK),p38,and nuclear factor-κB(NF-κB)signaling was activated in LX-2 ***,protein kinase B(AKT)phosphorylation and an increase inα-SMA expression were observed in LX-2 *** inhibitor of JNK,p38,or NF-κB decreased cell proliferation,AKT phosphorylation,andα-SMA expression in IL-1β-treated LX-2 ***:These results indicate that JNK,p38,and NF-κB signals converge at AKT phosphorylation,leading to LX-2 activation by IL-1β.Therefore,the AKT signaling pathway can be used as a target for alleviating liver fibrosis by the inflammatory cytokine IL-1β.

Critical cooling rate to avoid carbide precipitation during quenching of austenitic manganese steel was investigated by means of optical microscopy,image analyzer and numerical *** efficient heat treatment analysis program including temperature-dependent material properties was developed for the prediction of cooling rate and probability of carbide precipitation during quenching by finite difference ***-dependent heat transfer coefficient was adopted to achieve more precise *** ratio of carbide precipitation was measured by image analyzer to determine the critical point of carbide ***-dependent critical cooling rate at that point was calculated by the developed numerical ***,the probability of carbide precipitation on the whole area of specimen can be predicted by the proposed numerical program and the numerical result of a specimen was compared with the experimental result.

There are no reports regarding perforation of the colorectum induced by anorectal manometry. We report two cases of colorectal perforation that occurred during manometry in the patients undergoing restorative proctectomy for distal rectal cancer. In the first patient, computed tomography showed an extraperitoneal perforation in the pelvic cavity and a rupture of the rectal wall. A localized perforation into the retroperitoneum was managed conservatively. In the second patient, a 3 cm linear colon rupture was detected above the anastomotic site. A primary closure of the perforated colon and proximal ileostomy were conducted, but the patient died 2 wk later. We hypothesize that the perforation induced by anorectal manometry may be associated with the relative weakening of the proximal bowel wall due to anastomosis, decreased compliance, and abnormal rectal sensation. We suggest that measurement of the maximum tolerable volume should not be routinely performed alter restorative proctectomy for distal rectal cancer.

Aim: To investigate a possible potentiation effect of apomorphine (APO) on sildenafil-induced penile erection in the conscious rabbit. Methods: Erection of male New Zealand White rabbits (3.5-4.0 kg, n=12) was assessed by measuring the length of the uncovered penile mucosa and the duration of erection before and after intravenous administration of agents. After injection of APO (0, 0.05, 0.1 and 0.4 mg/kg), sildenafil was administered intravenously in a dose-response manner (0.5, 1 and 5 mg/kg). In additional experiments, the effect of increasing doses of sildenafil in combination with APO on systemic blood pressure was evaluated. Results: Systemic administration of sildenafil induced a dose-dependent increase in the penile length. Intravenous injection of APO alone did not produce any change in the penile length, while significantly enhanced the penile erection induced by sildenafil. The co-administration of 0.1 mg/kg of APO and 1 mg/kg of sildenafil was found to be the most effective combination in producing penile erection. Intravenous administration of sildenafil caused a concentration-dependent decrease in systemic blood pressure, but no additional decrease was observed with co-administration of APO. Conclusion: APO enhances the penile erection induced by sildenafil in the conscious rabbit without causing an additional decrease in blood pressure.

Rechargeable lithium batteries are attractive power sources for electronic devices and are being aggressively developed for vehicular use. Nevertheless, problems with their safety and reliability must be solved for the large-scale use of lithium batteries in transportation and grid-storage applications. In this study, a unique hybrid solid-state electrolyte composed of an ionic liquid electrolyte (LiTFSI/Pyr14TFSI) and BaTiO3 nanosize ceramic particles was prepared without a polymer. The electrolyte exhibited high thermal stability, a wide electrochemical window, good ionic conductivity of 1.3 × 10−3 S·cm−1 at 30 °C, and a remarkably high lithium-ion transference number of 0.35. The solid-state LiFePO4 cell exhibited the best electrochemical properties among the reported solid-state batteries, along with a reasonable rate capability. Li/LiCoO2 cells prepared using this nanocomposite solid electrolyte exhibited high performance at both room temperature and a high temperature, confirming their potential as lithium batteries with enhanced safety and a wide range of operating temperatures.

The ride vibration of a tractor is affected mostly by the stiffness and damping coefficient of the seat suspension,cabin suspension,cabin rubber mounts,and rubber ***,in the case of rubber tractor tires,the stiffnesses and damping coefficients have not been researched adequately thus far,and it is not simple to measure these *** this study,a method for measuring and analyzing the stiffnesses and damping coefficients of rubber tractor tires,which were the input parameters for the tractor ride vibration simulation,was *** cleat test,proposed in this study,did not require separate and complicated test equipment,unlike the conventional *** test was conducted simply by measuring acceleration under the driving conditions of the vehicle without detaching tires from the vehicle body or setting up additional test *** on the ground-vertical acceleration data obtained,the stiffness was calculated using the logarithmic decrement method,and the damping coefficient was calculated using least squares exponential curve *** result of the cleat test indicated that the front tires had stiffnesses of 486.08-570.69 kN/m and damping coefficients of 4.02-4.52 kN·s/m;the rear tires had stiffnesses of 409.42-483.79 kN/m and damping coefficients of 2.21-2.67 kN·s/*** the test,40 mm height cleats were installed on the track and the speed of the tractor was set to 7 and 10 km/h,which were the most common speeds during the *** study is meaningful in that it has presented a new method that improves the practicality of results,reduces cost,and simplifies the test process for measuring the stiffnesses and damping coefficients of rubber tractor tires.

Optimization of solvent swelling for efficient organic solar cells via sequential deposition

Compared to bulk heterojunction(BHJ)organic solar cells(OSCs)prepared by the blend casting in“one step process”,sequential deposition(SD)processed OSCs can realize an ideal profile of vertical component distribution due to the swelling of polymer ***,we did trials on several kinds of second solvents for swelling the polymer layer,and investigated the packing structure and morphology of the swollen films and the performance of the resulting *** found that an optimized morphology can be achieved by solvent swelling while using orthodichlorobenzene(o-DCB)as the second layer processing-solvent,with polymer donor PffBT-3 as bottom layer,PC71BM as top layer and bicontinuous networks in the *** solvent swelling process also makes the SD method exempt from thermal annealing *** device based on SD yields a power conversion effi-ciency(PCE)up to 8.7%without any post-treatment,outperforming those from the devices based on SD using other solvents and that(7.06%)from BHJ device,*** also extended the use of this approach to allpolymer blend system,and successfully improved the efficiency from 4.72%(chloroform)to 9.35%(o-DCB),which is among the highest PCEs in all-polymer-based OSCs fabricated with SD *** results demonstrate that the swelling of the polymer by the second layer solvent is a necessity for SD,paving the way towards additivefree high-performance OSCs.
