Cryptogenic multifocal ulcerous stenosing enteritis: Radiologic features and clinical behavior

To investigate the characteristic radiologic findings of cryptogenic multifocal ulcerous stenosing enteritis (CMUSE) which can be differentiated from other similar bowel disease and to assess their clinical behavior. METHODSTwenty pathologically and clinically confirmed CMUSE patients (males:females = 8:12; mean age: 40.4 years) between March 2002 and August 2015 from seven academic centers in South Korea were retrospectively reviewed. We evaluated small bowel series (SBS; n = 25), computed tomography (CT) enterography (n = 21), magnetic resonance (MR) enterography (n = 2), and abdominopelvic CT (n = 18) images, focusing on enteric and perienteric manifestations. Any change in radiologic features during follow-up period was recorded. We evaluated clinical data including presenting symptoms, laboratory finding and presence of relapse from electronic medical records. Histopathologic findings were also evaluated. RESULTSThe main symptoms were abdominal pain (n = 12) and anemia (n = 10). All patients showed small bowel strictures (n = 52, mean: 2.6 per patient) on initial CT/MR, located in the ileum (n = 47) or jejunum (n = 5). Strictures showed short-length (mean: 10.44 mm) and circumferential bowel wall thickening (mean: 5.56 mm) with layered enhancement (n = 48) that were also noted on initial SBS (n = 36) with shallow ulcers (n = 10). Some ulcerative lesions or wall thickening progressed into strictures on follow-up SBS/CT, and some strictures revealed recurrent ulceration on follow-up SBS. There were no penetrating disease features like fistula or abscess and no gastrointestinal tract involvement except the small bowel. Nine patients experienced disease recurrence (median relapse-free period: 32 mo) even post-operatively. Histopathologic features of surgically resected specimens were characterized as multiple superficial ulcerations confined to mucosa or submucosa and multiple strictures. Mutation of MED12 is not a frequent occurrence in prostate cancer of Korean patients

Prostate cancer is one of the major health care problems, but the molecular pathogenesis has been relatively insufficiently elucidated. Recently, whole exome sequencing of prostate cancer identified recurrent mutations involving MED12 in Caucasian patients, which finding was not reproduced in one subsequent study by Sanger sequencing. Thus, we investigated mutation status of MED12 in exons 2 and 26 by Sanger sequencing in 102 radical prostatectomy cases from Korean patients. The analysis found the mutation in none of the cases. Therefore, MED12 mutation does not appear to represent a significant molecular alteration in this cohort of patients according to the analysis by the traditional ＂gold standard.＂

Light quality, quantity, and photoperiod change periodically under natural condi- tions of daily and seasonal cycles. Plants have developed a circadian clock to respond to these predictable, periodic environmental changes, providing plants with the ability to anticipate daily and seasonal environmental changes.

AIM： To examine whether trans-10,cis-12 CLA （t10C12） or cis-9,trans-11 CLA （c9t11） inhibits heregulin （HRG）-β- stimulated cell growth and HRG-β-ErbB3 signaling in HT-29 cells. METHODS： We cultured HT-29 cells in the absence or presence of the CLA isomers and/or the ErbB3 ligand HRG-β. MTT assay, [^3H]thymidine incorporation, Annexin V staining, RT-PCR, Western blotting, immunoprecipitation, and in vitro kinase assay were performed. RESULTS： HRG-β increased cell growth, but did not prevent DNA t10c12-induced growth inhibition. T10C12 inhibited DNA synthesis and induced apoptosis of HT-29 cells, whereas c9t11 had no effect. T10c12 decreased the levels of ErbB1, ErbB2, and ErbB3 proteins and transcripts in a dose-dependent manner, whereas c9t11 had no effect. Immunoprecipitation/ Western blot studies revealed that t10c12 inhibited HRG- β-stimulated phosphorylation of ErbB3, recruitment of the p85 subunit of phosphoinositide 3-kinase （PI3K） to ErbB3, ErbB3-associated PI3K activities, and phosphorylation of Akt. However, c9t11 had no effect on phospho Aid： levels. Neither t10c12 nor c9t11 had any effect on HRG-β-induced phosphorylation of ERK-1/2. CONCLUSION： These results indicate that the inhibition of HT-29 cell growth by t10c12 may be induced via its modulation of ErbB3 signaling leading to inhibition of Akt activation.

AIM:To investigate M2 isoform of pyruvate kinase(PKM2) expression in gastric cancers and evaluate its potential as a prognostic biomarker and an anticancer ***:All tissue samples were derived from gastric cancer patients underwent curative gastrectomy as a primary *** and pathological information were obtained from the medical *** expression microarray data from 60 cancer and 19 noncancer gastric tissues were analyzed to evaluate the expression level of PKM2 *** microarrays were constructed from 368 gastric cancer *** was used to measure PKM2 expression and PKM2 positivity of cancer was determined by proportion of PKM2-positive tumor cells and staining *** between PKM2 expression and the clinicopathological factors was evaluated and the correlation between PKM2 and cancer prognosis was ***:PKM2 mRNA levels were increased more than 2-fold in primary gastric cancers compared to adjacent normal tissues from the same patients(log transformed expression level:7.6 ± 0.65 vs 6.3 ± 0.51,P < 0.001).Moreover,differentiated type cancers had significantly higher PKM2 mRNA compared to undifferentiated type cancers(log transformed expression level:7.8 ± 0.70 vs 6.7 ± 0.71,P < 0.001).PKM2 protein was mainly localized in the cytoplasm of primary cancer cells and detected in 144 of 368(39.1%) human gastric cancer ***2 expression was not related with stage(P = 0.811),but strongly correlated with gastric cancer differentiation(P < 0.001).Differentiated type cancers expressed more PKM2 protein than did the undifferentiated *** differentiated adenocarcinoma showed 63.6% PKM2-positive cells;in contrast,signet-ring cell cancers showed only 17.7% PKM2-positive ***,PKM2 expression was correlated with shorter overall survival(P < 0.05) independent of stage only in signet-ring cell ***:PKM2 expression might be an adverse prognostic factor for signet-rin

Effects of difructose dianhydride (DFA)-Ⅳ on in vitro fertilization in pigs

Difructose dianhydride Ⅳ(DFA-Ⅳ) is produced from levan, which is a natural polysaccharide that belongs to the fructan family, through the activity of levan fructotransferase(LF) derived from microorganisms. Recently, DFA-Ⅳ has been expected to have diverse applications in the food and medical industry. Here, we examined the potential application of DFA-Ⅳ for in vitro fertilization(ⅣF) in pigs. In the assessment of acrosomal integrity during incubation, intact acrosomal or viable spermatozoa were highly sustained in 0.1% or 0.25% DFA-Ⅳ(69.8%-70.8%,P<0.05). Reactive oxygen species(ROS) levels during sperm incubation decreased following the addition of DFA-Ⅳ, and 0.1 %-0.5% DFA-Ⅳ in particular significantly decreased ROS production relative to that seen with no addition or 0.75% DFA-Ⅳ. Total fertilization(mono + polyspermic oocyte) rate was significantly higher in the addition of0.1% DFA-Ⅳ(94.2%) than with other concentrations(71.8%-86.7%, P<0.05). When using reduced ⅣF times and lower sperm numbers, we found that addition of 0.1%-0.5% DFA-Ⅳ significantly increased the fertilization rate(P<0.05). Fertilized oocytes treated with 0.1% DFA-Ⅳ exhibited higher embryonic development and blastocyst formation than those treated with other concentrations(P<0.05). Consequently, the addition of DFA-Ⅳ during ⅣF improved fertilization and embryonic development, suggesting the possible use of novel sugars for enhancement of assisted reproductive technology(ART) in mammals.

Erector Spinae Plane Block for Mastectomy and Breast Flap Reconstructive Surgery: A Three Case Series

Uncontrolled pain after breast surgery can have early to chronic repercussions. The repertoire of pre-emptive opioid-sparing analgesic options includes regional blocks such as paravertebral blocks to myofascial blocks and more recently the Erector Spinae (ESP) block. Case 1 demonstrates the ESP block as an easy and conveniently performed post-operative rescue block for a patient who still experienced uncontrolled pain despite a combination of myofascial blocks and systemic analgesics. Case 2 and 3 demonstrate the advantage of providing an extensive coverage of surgical field in breast reconstruction surgery covering variable donor sites. It was due to the extent of coverage, that allowed the placement of ESP block catheter distantly without interrupting the surgical site. Post operative prolongation of pain relief was also successful by titrating analgesia via intermittent boluses. In our case series, the ESP block consistently and safely provided satisfactory pain relief for breast reconstruction surgery. It can be a viable option for peri-operative analgesia compared to other more invasive or less extensive alternatives.

Development of electrodes with high electrocatalytic activity and stability is essential for solving problems that still restrict the extensive application of vanadium redox flow batteries(VRFBs).Here,we designed a novel negative electrode with superior electrocatalytic activity by tailoring nitrogen functional groups,such as newly formed nitro and pyridinic-N transformed to pyridonic-N,from the prenitrogen-doped *** was experimentally confirmed that an electrode with pyridonic-N and nitro fuctional groups(tailored nitrogen-doped graphite felt,TNGF) has superior electrocatalytic acivity with enhanced electron and mass *** functional theory calulations demonstrated the pyridonic-N and nitro functional groups promoted the adsorption,charge transfer,and bond formation with the vanadium species,which is consistent with expermental *** addition,the V2+/V3+redox reaction mechanism on pyridonic-N and nitro functional groups was estabilised based on density functional theory(DFT) *** TNGF was applied to a VRFB,it enabled enhanced-electrolyte utilization and energy efficiencies(EE) of 57.9% and 64.6%,respectively,at a current density of 250 mA cm^(-2).These results are 18.6% and 8.9% higher than those of VRFB with electrode containing graphitic-N and pyridinicN ***,TNGF-based VRFB still operated with an EE of 59% at a high current density of300 mA cm^(-2).The TNGF-based VRFB exhibited stable cycling performance without noticeable decay of EE over 450 charge-discharge cycles at a current density of 250 mA cm^(-2).The results of this study suggest that introducing pyridonic-N and nitro groups on the electrode is effective for improving the electrochemical performance of VRFBs.

Predictors of poor outcomes in patients with wild mushroom-induced acute liver injury

To identify early predictive markers of poor outcomes in patients with acute liver injury from wild mushroom *** observational, retrospective record review involved adults aged ≥ 18 years admitted to emergency department with mushroom intoxication from January 2005 to December 2015. The diagnosis of mushroom intoxication was based on the following: (1) a positive history of recent wild mushroom intake (either raw or cooked); (2) the onset of gastrointestinal symptoms, such as watery diarrhea, vomiting, and/or abdominal pain, after ingestion; and (3) the exclusion of other possible causes of acute liver injury. Acute liver injury was defined by a > 5-fold elevation of liver enzymes or moderate coagulopathy [international normalized ratio (INR) > 2.0]. Clinical and laboratory findings were compared in survivors and *** 93 patients with mushroom intoxication, 23, 11 men (47.8%) and 12 women (52.2%), of median age 61 years, developed acute liver injury. The overall in-hospital mortality rate was 43.5% (10/23).
