KIFs(kinesin superfamily proteins)是依赖于微管的马达分子,在细胞内承担膜性细胞器或生物大分子复合物的运输作用。KIF3是由KIF3A／KIF3B／KAP3构成的向微管正极运输膜泡的马达分子。 KAP3(kinsein superfamily-associated prctein 3)为KIF3复合体的非马达亚基,负责调节KIF3马达蛋白与其货物间的相互连接。我们利用Cre／loxP介导的条件性基因靶技术(Cre／loxP-mediated conditional gene targeting strategy),建立了KAP3条件性基因剔除小鼠(conditional knockout mouse)。发现在神经系统的发育过程中,KAP3与维持神经前体细胞分裂和细胞间粘连的平衡相关。KIF3与N-cadherin和β-catenin结合,行使post-Golgi运输N-cadherin的功能。在KAP3缺失的细胞中,N-cadherin的亚细胞定位紊乱。在条件性基因剔除小鼠的脑中,由于N-cadherin不能正常定位而导致神经上皮组织的发育异常,并形成成室管膜瘤样结构。上述结果提示KIF3／KAP3具有抑制肿瘤形成的功能,为成室管膜瘤的研究提供了动物模型。

As among persons with normal anatomy, occasional patients with situs inversus develop malignant tumors. Recently, several laparoscopic operations have been reported in patients with situs inversus. We describe laparoscopic hemicolectomy with radical lymphadenectomy in such a patient. Careful consideration of the mirror-image anatomy permitted safe operation using techniques not otherwise differing from those in ordinary cases. Thus, curative laparoscopic surgery for colon cancer in the presence of situs inversus is feasible and safe.

Influence of cognitive impairment on cardiac mortality after percutaneous coronary intervention in very elderly patients: a retrospective observational study

Background Cognitive impairment (CI) increases cardiac mortality among very elderly patients. Percutaneous coronary intervention (PCI) for ischemic heart disease (IHD) patients is considered a favorable strategy for decreasing cardiac mortality. Here, we investigated the influence of CI on cardiac mortality after PCI in very elderly patients. Methods We performed a retrospective observational analysis of patients who received PCI between 2012 and 2014 at the South Miyagi Medical Center, Japan. IHD patients over 80 years old who underwent the Mini-Mental State Examination for CI screening during hospitalization and/or who had been diagnosed with CI were included. Participants were divided into CI and non-CI groups, and cardiac mortality and incidence of adverse cardiac events in a 3-year follow-up period were compared between groups. Statistical analyses were performed using the t-test,χ^2 test, and multivariable Cox regression analysis, with major comorbid illness and conventional cardiac risk factors as confounders. Results Of 565 patients, 95 were included (41 CI, 54 non-CI). Cardiac mortality during the follow-up period was significantly higher in the CI group (36%) compared with the non-CI group (13%)(OR = 4.3, 95% CI: 1.56–11.82, P < 0.05). CI was an independent cardiac prognostic factor after PCI and, for CI patients, living only with a CI partner was an independent predictor of cardiac death within three years. Conclusions CI significantly affected cardiac prognosis after PCI in very elderly patients, particularly those living with a CI partner. To improve patients’ prognoses, social background should be considered alongside conventional medical measures.

Sonazoid(Daiichi Sankyo,Tokyo,Japan),a secondgeneration of a lipid-stabilized suspension of a perfluorobutane gas microbubble contrast agent,has been used clinically in patients with liver tumors and for harmonic gray-scale ultrasonography(US)in Japan since January ***-enhanced US has two phases of contrast enhancement:vascular and *** the late phase of Sonazoid-enhanced US,we scanned the whole liver using this modality at a low mechanical index(MI)without destroying the microbubbles, and this method allows detection of small viable hepatocellular carcinoma(HCC)lesions which cannot be detected by conventional US as perfusion defects in the late ***-injection of Sonazoid into an HCC lesion which previously showed a perfusion defect in the late phase is useful for confirming blood flow intothe *** MI intermittent imaging at 2 frames per second in the late phase is also helpful in differentiation between necrosis and viable hypervascular HCC ***-enhanced US by the coded harmonic angio mode at a high MI not only allows clear observation of tumor vessels and tumor enhancement, but also permits automatic scanning with Sonazoidenhanced three dimensional(3D)*** images combining US with contrast-enhanced CT or contrastenhanced MRI have made it easy to detect typical or atypical HCC *** these methods,Sonazoidenhanced US can characterize liver tumors,grade HCC lesions histologically,recognize HCC dedifferentiation, evaluate the efficacy of ablation therapy or transcatheter arterial embolization,and guide ablation therapy for unresectable *** article reviews the current developments and applications of Sonazoid-enhanced US and Sonazoid-enhanced 3D US for diagnosing and treating hepatic lesions,especially HCC.

AIM To compare the outcomes of endoscopic submucosal dissection(ESD) for gastric neoplasms using Clutch Cutter(ESD-C) or other knives(ESD-O).METHODS This was a single-center retrospective study. Gastric neoplasms treated by ESD between April 2016 and October 2017 at Kitakyushu Municipal Medical Center were reviewed. Multivariate analyses and propensity score matching were used to reduce biases. Covariates included factors that might affect outcomes of ESD, including age, sex, underlying disease, anti-thrombotic drugs use, tumor location, tumor position, tumor size, tumor depth, tumor morphology, tumor histology, ulcer(scar), and operator skill. The treatment outcomes were compared among two groups. The primary outcome was ESD procedure time. Secondary outcomes were en bloc, complete, and curative resection rates, and adverse events rates including perforation and delayed *** A total of 155 patients were included in this study; 44 pairs were created by propensity score matching. Background characteristics were quite similar among two groups after matching. Procedure time was significantly shorter for ESD-C(median; 49 min) than for ESD-O(median; 88.5 min)(P < 0.01). However, there was no significant difference in treatment outcomes between ESD-C and ESD-O including en bloc resection rate(100% in both groups), complete resection rate(100% in both groups), curative resection rate(86.4% vs 88.6%, P = 0.730), delayed bleeding(2.3% vs 6.8%, P = 0.62) and perforation(0% in both groups).CONCLUSION ESD-C achieved shorter procedure time without an increase in complication risk. Therefore, ESD-C could become an effective ESD option for gastric neoplasms.

BACKGROUND Studies have shown that covered self-expandable metallic stents(CSEMS)with a low axial forces after placement can cause early recurrent biliary obstruction(RBO)due to precipitating sludge *** To ascertain whether the angle of CSEMS after placement is a risk factor for RBO in unresectable distal malignant biliary obstruction(MBO).METHODS Between January 2010 and March 2019,261 consecutive patients underwent selfexpandable metallic stent insertion by endoscopic retrograde cholangiopancreatography at our facility,and 87 patients were included in this *** evaluated the risk factors for RBO,including the angle of CSEMS after placement as the primary *** measured the obtuse angle of CSEMS after placement on an abdominal radiograph using the SYNAPSE PACS *** also evaluated technical and functional success,adverse events,time to RBO(TRBO),non-RBO rate,survival time,cause of RBO,and reintervention procedure as secondary *** We divided the patients into two cohorts based on the presence or absence of *** angle of CSEMS after placement(per 1°and per 10°)was evaluated using the multivariate Cox proportional hazard analysis,which was an independent risk factor for RBO in unresectable distal MBO[hazard ratio,0.97 and 0.71;95%confidence interval(CI):0.94-0.99 and 0.54-0.92;P=0.01 and 0.01,respectively].For early diagnosis of RBO,the cut-off value of the angle of CSEMS after placement using the receiver operating characteristic curve was 130°[sensitivity,50.0%;specificity 85.5%;area under the curve 0.70(95%CI:0.57-0.84)].TRBO in the<130°angle group was significantly shorter than that in the≥130°angle group(P<0.01).CONCLUSION This study suggests that the angle of the CSEMS after placement for unresectable distal MBO is a risk factor for *** novel results provide pertinent information for future stent management.

神经胶质细胞的活动可以影响学习和记忆的储备和效率。本研究采用小脑依赖水平光动力反应运动学习小鼠模型,研究在线训练期间短期记忆(STM)的形成和离线休息期间长期记忆(LTM)的形成。研究显示,在线和离线学习效果的差异很大。STM早期形成较好者的LTM形成往往受到抑制,而没有明显急性训练效果的STM形成较晚者常常在离线学习中有明显提高。含有LRRC8A的阴离子通道可释放谷氨酸。在星形胶质细胞(包括小脑伯格曼胶质细胞)中特异性地条件性敲除LRRC8A导致STM形成的完全丧失,而在休息期间LTM形成仍然存在。在线训练期间,通过通道视紫红质-2或古菌红素-T(ArchT)对神经胶质细胞的活性进行光遗传学调控,可分别导致STM形成的增强或抑制。STM和LTM可能在在线训练期间同时触发,但LTM在离线期的后期才表达。STM似乎并不稳定,在线培训期间的形成的记忆并没有在LTM中出现。此外,我们发现在休息期间,胶质细胞ArchT的光活化导致LTM形成的增加。这些数据表明STM的形成和LTM的形成是平行的独立过程。可以根据胶质细胞的反应来决定策略的重点是STM还是LTM。
