Early gastric cancer diagnostic ability of ultrathin endoscope loaded with laser light source

The improvement of the diagnostic ability is *** To compare the early gastric cancer diagnostic ability of an ultrathin endoscope loaded with a laser light source and that of the conventional *** The target subjects were 375 consecutive patients who underwent endoscopy at our hospital for post-endoscopic submucosal dissection follow-up of gastric cancer from January to August 2018. During endoscopy, the ultrathin endoscope was used in 140 patients(37.3%), and the conventional endoscope was used in235 patients(62.7%). Case-based reasoning （CBR） is an important reasoning technique of expert system. In this paper, the authors introduce CBR to intelligent early-warning support system, which could warn quantitatively for enterprise financial crisis and could warn qualitatively by expert＇s knowledge and experience. Furthermore, genetic algorithm is applied to case-based reasoning in CBR-IEWSS, which improves accuracy and efficiency of case retrieval. Last, the structure of CBR-IEWSS is given.

Treatment strategy for metastatic prostate cancer with extremely high PSA level： reconsidering the value of vintage therapy

The prognostic significance of initial prostate-specific antigen （PSA） level for metastatic prostate cancer remains uncertain. We investigated the differences in prognosis and response to hormonal therapies of metastatic prostate cancer patients according to initial PSA levels. We analyzed 184 patients diagnosed with metastatic prostate cancer and divided them into three PSA level groups as follows： low （〈100 ng ml-1）, intermediate （100–999 ng ml-1）, and high （≥1000 ng ml-1）. All patients received androgen deprivation therapy （ADT） immediately. We investigated PSA progression-free survival （PFS） for first-line ADT and overall survival （OS） within each of the three groups. Furthermore, we analyzed response to antiandrogen withdrawal （AW） and alternative antiandrogen （AA） therapies after development of castration-resistant prostate cancer （CRPC）. No significant differences in OS were observed among the three groups （P = 0.654）. Patients with high PSA levels had significantly short PFS for first-line ADT （P = 0.037）. Conversely, patients in the high PSA level group had significantly longer PFS when treated with AW than those in the low PSA level group （P = 0.047）. Furthermore, patients with high PSA levels had significantly longer PFS when provided with AA therapy （P = 0.049）. PSA responders to AW and AA therapies had significantly longer survival after CRPC development than nonresponders （P = 0.011 and P 〈 0.001, respectively）. Thus, extremely high PSA level predicted favorable response to vintage sequential ADT and AW. The current data suggest a novel aspect of extremely high PSA value as a favorable prognostic marker after development of CRPC.

Outcomes of patients older than 75 years with non-metastatic prostate cancer

Fiber Line Optimization in Single Ply for 3D Printed Composites

In conventional manufacturing processes of composites, Carbon Fibre Reinforced Plastic (CFRP) laminates have been made by stacking unidirectional or woven prepreg sheets. Recently, as a manufacturing process of CFRP, 3D printing of CFRP composites has been developed. The 3D printing process of CFRP composites enables us to fabricate CFRP laminates with arbitrary curvilinear fibre plies. This indicates that the optimization of the in-plane curved carbon fibre placement in a planar ply is strongly required to realize superior 3D printed composites. In the present paper, in-plane curved carbon fibre alignment of a ply with an open hole is optimized in terms of maximization of the fracture strength. For the optimization process, a genetic algorithm is adopted. To describe curved carbon fibre alignments in a planar ply, stream lines of perfect flow is employed. By using the stream lines of the perfect flow, number of optimization parameters is significantly reduced. After the optimization, the fracture strength of CFRP laminate is compared with the results of unidirectional CFRP ply. The curved fibre placement in a planar ply shows superior fracture improvement.

2D Fusion Simulations and Experimental Confirmations of Print Paths Using Composite Particles with Particle Method for Fused Filament Fabrication

Printing short fibre/thermoplastic composites using the fused filament fabrication method sometimes creates a gap between print paths. In this study, the two-dimensional moving particle semi-implicit method for liquid simulation was applied to simulate the print-path fusion process. The three-dimensional movement of the nozzle was simulated using the sliding motion of the nozzle. The method was applied to the printing of short carbon fibre/polyamide-6 composites, and the simulation results were compared with those of experiments. The simulated results of the cross-sectional configuration agreed well with the experimental results. This will enable the optimization of printing process parameters thus reducing the gap between print paths.

Simulation Program to Determine Sample Size and Power for a Multiple Logistic Regression Model with Unspecified Covariate Distributions

Binary logistic regression models are commonly used to assess the association between outcomes and covariates. Many covariates are inherently continuous, and have a variety of distributions, including those that are heavily skewed to the left or right. Existing theoretical formulas, criteria, and simulation programs cannot accurately estimate the sample size and power of non-standard distributions. Therefore, we have developed a simulation program that uses Monte Carlo methods to estimate the exact power of a binary logistic regression model. This power calculation can be used for distributions of any shape and covariates of any type (continuous, ordinal, and nominal), and can account for nonlinear relationships between covariates and outcomes. For illustrative purposes, this simulation program is applied to real data obtained from a study on the influence of smoking on 90-day outcomes after acute atherothrombotic stroke. Our program is applicable to all effect sizes and makes it possible to apply various statistical methods, logistic regression and related simulations such as Bayesian inference with some modifications.
