We propose broad supercontinuum spectrum generating highly nonlinear photonic crystal fiber (HN-PCF) which can be used in ultrahigh- resolution optical coherence tomography and optical transmission systems. Using full vector finite difference method, we investigated the different properties of HN-PCF. Broadband su-percontinuum spectrum is numerically calculated by using nonlinear Schr?dinger equation. Investigation showed that it is possible to obtain longitudinal resolution in a biological tissue of 1.3 μm, 1.2 μm and 1.1 μm by using picosecond continuum light at center wavelengths of 1.06 μm, 1.31 μm and 1.55 μm, respectively.

Background： Preventing anterior cruciate ligament（ACL） injuries is very important for athletes, and dynamic knee valgus is considered a risk factor for non-contact ACL injury. However, little is known about whether the functions of the hip abductor and rear-foot increase dynamic knee valgus. A two-dimensional（2D） video-based screening test focused on hip abductor and rear-foot functions among factors involved in dynamic knee valgus. The present study determined associations between hip and rear-foot dynamic alignment and dynamic knee ***： This cross-sectional study recruited 130 female basketball players（258 legs） from nine high-school teams. The players performed single-leg squats and single-leg drop landings to provide knee-in（KID） and hip-out（HOD） distances on 2D video images. Hip and rear-foot dynamic alignment was evaluated using a dynamic Trendelenburg test（DTT） and a dynamic heel-floor test（HFT）.Results： The Chi-square test revealed no significant difference in the prevalence of DTT-positivity between single-leg squats（28.7%） and singleleg drop landings（23.3%）. The prevalence of HFT-positivity was significantly greater during landings（51.4%） than during single-leg squats（31.0%, p 〈 0.01）. The KID values for both single-leg squats and single-leg drop landings were greater in the DTT-positive than in the DTTnegative group（15.1 5.4 cm and 20.2 7.5 cm, p 〈 0.001）. The HOD values were similarly greater in the DTT-positive group（15.2 1.9 cm and 17.6 2.8 cm, p 〈 0.001）. We report a patient with hyperplastic polyposis who had two asynchronous colon cancers, a combined adenoma-hyperplastic polyp, a serrated adenoma, and tubular adenomas. Hyperplastic polyposis is thought to be a precancerous lesion; and adenocarcinoma arises from hyperplastic polyposis through the hyperplastic polyp-adenoma-carcinoma sequence. Most polyps in patients with hyperplastic polyposis present as bland- looking hyperplastic polyps, which are regarded as non- neoplastic lesions; however, the risk of malignancy should not be underestimated. In patients with multiple hyperplastic polyps, hyperplastic polyposis should be identified and followed up carefully in order to detect malignant transformation in the early stage.

Effects of Land Surface Temperature on the Frequency of Convective Precipitation in the Tokyo Area

This study uses statistical evaluation by correlation analysis to examine the effects of thermal environment on the frequency of convective precipitation in the Greater Tokyo Area between 12:00 and 18:00 on summer days from 1997 to 2006. To extract the frequency of convective precipitation we used Automated Meteorological Data Acquisition System radar data to obtain detailed rainfall distribution maps, and to extract the urban thermal environment we used surface temperature data from a National Oceanic and Atmospheric Administration weather satellite. Results were a coefficient of determination of 0.01, indicating no clear relation between surface temperature and convective rain frequency in the study area. Examining the convective rain frequency distribution map in conjunction with an elevation map of the area indicates that higher elevation is a better predictor of increased frequency of convective rainfall than is surface temperature. Because this indicates that orographic precipitation has a large influence in the study area, we used an elevation map to exclude hilly and mountainous regions, regions bordering flat areas (under the assumption that wind could easily move orographic precipitation to such areas), and regions containing marine areas. Doing so resulted in a coefficient of determination of 0.38, a clear signal that differences in the thermal environment in the Greater Tokyo Area have an effect on the frequency of convective precipitation. We next focused on metropolitan Tokyo, the most developed part of the region and the part experiencing the most frequent occurrences of convective precipitation, and we performed correlation analysis considering parameters related to buildings. Results indicate that orographic precipitation has a strong influence in metropolitan Tokyo as well, so we excluded those areas that were excluded from the Greater Tokyo Area analysis and again performed correlation analysis. AIM： To investigate the relationship between cycloo- xygenase-2 （COX-2）, and vascular endothelial growth factor （VEGF）, and to determine the clinical significance of this relationship in esophageal cancer patients undergoing chemoradiotherapy （CRT）. METHODS： Immunohistochemical staining was used to evaluate COX-2 and VEGF expression in 40 patients with histologically-confirmed esophageal squamous carcinoma （ESCC） who were undergoing preoperative CRT. RESULTS： Fourteen out of 40 ESCC patients showed a pathological complete response （CR） after CRT. COX-2 and VEGF protein expressions were observed in the cytoplasm of 17 and 13 tumors, respectively, with null expression in 9 and 13 tumors, respectively. COX-2 expression was strongly correlated with VEGF expression （P 〈 0.05）. There were also significant associations between COX-2 expression, tumor recurrence, and lymph-node involvement （P = 0.0277 and P = 0.0095, respectively）. COX-2 expression and VEGF expression had significant prognostic value for disease-free survival （log-rank test; P = 0.0073 and P = 0.0341, respectively）, but not for overall survival, as assessed by univariate analysis. expression correlates with VEGF expression and might be a useful prognostic factor for more frequent tumor recurrence in ESCC patients undergoing neoadjuvant CRT. These findings support the use of anti-angiogenic COX-2 inhibitors in the treatment of ESCC.

AIM： To assess histochemical expression of KL-6 and its clinicopathological significance in carcinoma of the ampulla of Vater. METHODS： Ampullary carcinoma tissues were collected from 38 patients who underwent pancreatoduodenectomy or local resection. Tissues were subjected to immunohistochemical analysis using KL-6 antibody. RESULTS： Positive staining of ampullary carcinoma cells was observed in 26 （68.4%） cases. Staining was not found in the surrounding non-cancer regions of the ampullary tissues. Remarkable KL-6 expression was observed in invasive carcinoma cells in pancreatic and duodenal tissues and in metastatic carcinoma cells in lymph nodes. Positive KL-6 expression was related to lymph node metastasis （P = 0.020）, pancreatic invasion （P = 0.016）, duodenal invasion （P = 0.034）, and advanced stage of TNM clinical classification （P = 0.010）. Survival analysis showed that positive expression of KL-6 was related to a poorer prognosis （P = 0.029）. CONCLUSION： The aberrant expression of KL-6 mucin is significantly related to unfavorable behaviors of cardnoma of the ampulla of Vater.

Coronal Alignment of Three Different Types of Implants in Kinematically Aligned Total Knee Arthroplasty: A Comparative Study

＜正＞New mild cleansing system of Monoalkyl phosphate （MAP） was developed for facial cleansing or body washing. Compared to usual MAP,the new MAP system has many advantages as follows: Rich - forming performance can keep in wide pH range （from pH 5.5 to 7.0） regardless of hard water and rich sebum conditions. And new MAP system gives us creamy lather form and good rinsability for gentle cleansing. Especially,the swelling ratio of keratin （Skin damage in vitro） can be suppressed more effective by using this system. Key technological point of this system is to keep dissoluble nature of MAP not depending on various negative conditions.

Background: The number of total knee arthroplasty (TKA) surgeries performed each year is increasing worldwide and mechanical alignment (MA) is currently seen as the gold standard procedure. However, taking neutral alignment as the universal goal may be mistaken. In our hospital, we currently conduct kinematically aligned TKA (KA-TKA). Three different types of implants are used: the cruciate-retaining (CR) type, cruciate-sacrificing (CS) type, or bi-cruciate-retained (BCR) type. We aimed to compare the coronal alignment observed following KA-TKA and MA-TKA and in normal knees, as well as that achieved with different types of implants. Methods: The study comprised 206 knees of Japanese patients who underwent KA-TKA using varying implants in our Hospital between May 2019 and April 2020. Measurements of pre- and postoperative coronal alignment were determined from weight-bearing full-leg standing radiographs. The postoperative results were compared to measurements taken from patients who underwent MA-TKA (N = 96) and normal knees (N = 60). Results: No significant differences between the KA-TKA group and normal knees were found for the medial proximal tibial angle (MPTA) (–4.2° ± 2.6° vs –3.8° ±
