Changes in interleukin-6 levels in saliva in patients with oral squamous cell carcinoma during the treatment phase.

Purpose: To compare interleukin-6 (IL-6) levels between patients with oral squamous cell carcinoma (OSCC) and controls, and to elucidate the changes in IL-6 levels during the treatment phase。Methods: Twenty-nine consecutive subjects with OSCC were enrolled in this study。The patients were guided for systematic oral care just after admission to prevent complications such as pneumonia。Saliva was collected three times during the treatment phases in the patient group。As controls, 11 healthy volunteers were recruited。Results: IL-6 was detected in 22/28 samples in the patient group at the time of admission。The median concentration of IL-6 was significantly higher in the patient group (3.6 pg/ml) than in the controls (0.4 pg/ml) (P=0.03)。Just before the surgery, the median concentration of IL-6 was 4.8 pg/ml, which was significantly higher than that at the time of admission (P=0.01)。The median concentration of IL-6 was 5.2 pg/ml at the time of discharge。Conclusion: IL-6 was up-regulated in saliva in the OSCC patients。IL-6 level tended to increase before treatment, and was not reduced significantly at discharge。IL-6 in saliva may contribute to the immune response or progression of OSCC。

Purpose: To evaluate the diagnostic accuracy of FDG-PET relative to CT for detecting metastatic cervical lymph nodes in patients with oral squamous cell carcinoma (OSCC), and to ascertain the factors that affect this accuracy。Methods: A total of 1076 lymph nodes obtained from 35 neck dissections in 26 OSCC patients who preoperatively underwent both FDG-PET and CT were retrospectively analyzed。For pathological metastatic lymph nodes, the lymph node size (short-axis diameter), the ratio of intranodal tumor deposits, and the size of intranodal tumor deposits (maximum diameter of metastatic foci in each lymph node) were histologically recorded。Results: Forty-six lymph nodes from 23 neck sides were pathologically diagnosed metastases。The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of FDG-PET evaluated individually per neck side were 74, 92, 80, 94 and 65%, respectively, whereas those of CT were 78, 58, 71, 78 and 58%, respectively。FDG-PET detected 100% of metastatic lymph nodes > 10 mm, intranodal tumor deposits -> 9 mm, and intranodal tumor deposits with a ratio > 75%, whereas no nodes or tumor deposits smaller than 5 mm were detected。The spatial resolution limitations of FDG-PET were responsible for 16 of 20 (80%) false-negative PET results in lymph nodes。Conclusion: FDG-PET is a useful tool for preoperative evaluation of the neck because it accurately detects metastatic lymph nodes≥10 mm and has fewer false positive cases than CT。The high specificity of FDG-PET for lymph node metastases may play an important role in avoiding unnecessary neck dissection。

Objective: This multicenter clinical study was to assess the clinical usability of an oral moisture-checking device in detecting the dry mouth patients and evaluating the optimal measurement site. Materials and Methods: The study group comprised 250 patients with dry mouth and 241 healthy volunteer subjects at 13 medical centers. This device was used to measure the moisture degrees of the lingual mucosa and the buccal mucosa. Subjective oral dryness, objective oral dryness, and saliva flow rates were also compared between the two groups. For statistical analysis, receiver-operating characteristic analysis was performed to calculate the area under the curve (AUC). Results: The moisture degree of the lingual mucosa was significantly lower in the dry mouth group (27.2 ± 4.9) than that in the healthy group (29.5 ± 3.1, AUC = 0.653). When a lingual mucosa moisture degree of 31.0 or higher was defined as normal, less than 27.0 as dry mouth, and 27.0 to less than 31.0 as borderline zone of dry mouth, both the sensitivity and the specificity for the diagnosis of dry mouth were close to 80%. Conclusion: These results suggest that the oral moisture-checking device is a usable screening device for dry mouth.

Alloy nanoparticles(NPs)with numerous exposed catalytic active sites have been extensively studied as efficient heterogeneous ***,it is challenging to synthesize alloy NP catalysts with high activity while avoiding ***,we report a facile method to encapsulate alloy NPs loaded metal–organic framework(MOF)catalysts(alloy NPs/MOFs)within an ultrathin metal–organic layer using a terephthalic acid(BDC)assisted method.A series of metal-BDC encapsulated PtM/MOFs(M=Fe,Co,and Ni)catalysts were synthesized successfully and showed significantly enhanced catalytic performance toward selective hydrogenation reaction.

Prostate-specific antigen-based population screening for prostatecancer： current status in Japan and future perspective in Asia

In Western countries, clinical trials on prostate cancer screening demonstrated a limited benefit for patient survival. In the Asia-Pacific region, including Japan, the rate of prostate-specific antigen （PSA） testing remains very low compared with Western countries, and the benefits of population-based screening remain unclear. This review describes the current status of population screening and diagnosis for prostate cancer in Japan and discusses the efficacy of population screening for the Asian population. Since the 1990s, screening systems have been administered by each municipal government in Japan, and decreases in the prostate cancer mortality rate are expected in some regions where the exposure rate to PSA screening has increased markedly. A population-based screening cohort revealed that the proportion of metastatic disease in cancer detected by screening gradually decreased according to the increased exposure rate, and a decreasing trend in the proportion of cancer with high serum PSA levels after population screening was started. The prognosis of the prostate cancer detected by population screening was demonstrated to be more favorable than those diagnosed outside of the population screening. Recent results in screening cohorts demonstrated the efficacy of PSA. These recent evidences regarding population-based screening in Japan may contribute to establishing the optimal prostate cancer screeninK system in Asian individuals.
