Doublecortin and CaM kinase-like-1 as an independent prognostic factor in patients with resected pancreatic carcinoma

AIM To elucidate the effect of expression of doublecortin and Ca M kinase-like-1(DCLK1) in patients with pancreatic ductal adenocarcinoma(PDAC). METHODS Tumor specimens were obtained from 136 patients with pancreatic cancer who had undergone resection without preoperative therapy between January 2000 and December 2013 at the Department of Surgical Oncology, Osaka City University. The resected specimens were analyzed for associations with clinicopathological data, including DCLK1 expression, epithelial mesenchymal transition(EMT) marker expression, and cancer stem cell(CSC) marker expression. Univariate and multivariate survival analyses were performed and we assessed the association between DCLK1 expression and clinicopathological factors, including the EMT marker and CSC marker.RESULTS In total, 48.5%(66/136) of the pancreatic cancer samples were positive for DCLK1. Patients with DCLK1-positive tumors had significantly shorter survival times than those with DCLK1-negative tumors(median, 18.7 mo vs 49.5 mo, respectively; P < 0.0001). Positive DCLK1 expression correlated with histological grade(P = 0.0290), preoperative CA19-9 level(P = 0.0060), epithelial cell adhesion molecule(Ep CAM) expression(P = 0.0235), and the triple-positive expression of CD44/CD24/Ep CAM(P = 0.0139). On univariate survival analysis, five factors were significantly associated with worse overall survival: histological grade of G2 to G4(P = 0.0091), high preoperative serum SPan-1 level(P = 0.0034), R1/2(P < 0.0001), positive expression of DCLK1(P < 0.0001) or CD44(P = 0.0245). On multivariate survival analysis, R1/2 [odds ratio(OR) = 2.019, 95% confidence interval(CI): 1.380-2.933; P = 0.0004] and positive DCLK1 expression(OR = 1.848, 95%CI: 1.2854-2.661; P = 0.0009) were independent prognostic factors. CONCLUSION DCLK1 expression was found to be an independent prognostic factor and it may play a crucial prognostic role by promoting acquisition of stemness.

AIM: To evaluate the complications and clinical outcomes of transcatheter arterial embolization (TAE) for symptoms related to severe arterioportal fistulas (APFs). METHODS: Six patients (3 males, 3 females; mean age, 63.8 years; age range, 60-71 years) with chronic liver disease and severe APFs due to percutaneous intrahepatic treatment (n=5) and portal vein (PV) tumor thrombosis of hepatocellular carcinoma (n=1) underwent TAE for symptoms related to severe APFs [refractory ascites (n=4), hemorrhoidal hemorrhage (n=1), and hepatic encephalopathy (n=1)]. Control of symptoms related to APFs and complications were evaluated during the follow-up period (range, 4-57 mo). RESULTS: In all patients, celiac angiography revealed immediate retrograde visualization of the main PV before TAE, indicating severe APF. Selective TAE for the hepatic arteries was performed using metallic coils (MC, n=4) and both MCs and n-butyl cyanoacrylate (n=2). Three patients underwent repeated TAEs for residual APFs and ascites. Four patients developed PV thrombosis after TAE. During the follow-up period after TAE, APF obliteration and symptomatic improvement were obtained in all patients. CONCLUSION: Although TAE for severe APFs may sometimes be complicated by PV thrombosis, TAE can be an effective treatment to improve clinical symptoms related to severe APFs.

Resent studies have shown that group of non-submerged spur dikes can provide a valuable habitat for fishes and macro invertebrates in addition to the function to established a more desirable river for a flood, navigation and bank erosion. The study on the effects of non-submerged spur dikes on bed deformation, water surface oscillations and mean flow characteristics by changing the spur dikes facing upstream, downstream and normal to the main flow respectively. The results showed that the spur dikes facing downstream led to the maximum scour at the tip of the dikes, the maximum one in the main channel bed. In addition, it was found that in the case with a facing downstream, resistance is largest.

AIM:To evaluate the prognostic significance of the lymphocyte to monocyte ratio(LMR) in patients with unresectable metastatic colorectal cancer who received palliative chemotherapy.METHODS:A total of 104 patients with unresectable metastatic colorectal cancer who underwent palliative chemotherapy were enrolled. The LMR was calculated from blood samples by dividing the absolute lymphocyte count by the absolute monocyte count. Pretreatment LMR values were measured within one week before the initiation of chemotherapy,while posttreatment LMR values were measured eight weeks after the initiation of chemotherapy.RESULTS:The median pre-treatment LMR was 4.16(range:0.58-14.06). We set 3.38 as the cut-off level based on the receiver operating characteristic curve. Based on the cut-off level of 3.38,66 patients were classified into the high pre-treatment LMR group and 38 patients were classified into the low pretreatment LMR group. The low pre-treatment LMR group had a significantly worse overall survival rate(P = 0.0011). Moreover,patients who demonstrated low pre-treatment LMR and normalization after treatmentexhibited a better overall survival rate than the patients with low pre-treatment and post-treatment LMR values.CONCLUSION:The lymphocyte to monocyte ratio is a useful prognostic marker in patients with unresectable metastatic colorectal cancer who receive palliative chemotherapy.

背景高血压是急性脑卒中的预后因子，血压变异性或许可以独立预测脑卒中的转归。我们评估了血压变异性对INTERACT2参试者脑卒中预后的预测价值， INTERACT2是一项开放标签随机对照试验。方法 INTERACT2将患有自发性颅内出血（ICH）、高收缩压（150～220 mm Hg）、对早期降压强化疗法无明确适应证或禁忌证的2839例成人纳入研究。将患者随机分为强化治疗组（经静脉给药，1 h内目标收缩压降至＜140 mm Hg ）和指南推荐的治疗组（ ICH 后6 h 内，目标收缩压降至＜180 mm Hg）。主要转归为死亡或90 d内发生生活大部分无法自理（改良Rankin量表评分≥3分）；次要转归为90 d内改良Rankin量表评分发生顺序移动，调查员在评分时并不知晓患者接受了哪种治疗。根据标准定义血压变异性：在发病头24 h （超急性期）进行5次测量，在发病后2～7 d （急性期）进行不少于12次的测量。采用比例优势逻辑回归模型评价血压变异性与转归之间的关联。血压变异性关键指标为收缩压的标准差，按五分位数加以分类。发现研究者对2645例（93.2％）处于超急性期的患者和2347例（82.7％）处于急性期的患者进行了调查。将两个治疗组的数据一起分析，在超急性期〔最高五分位数校正OR＝1.41，95％CI （1.05，1.90）； P＝0.0167〕和急性期〔最高五分位数校正OR＝1.57，95％CI （1.14，2.17）； P＝0.0124〕，收缩压的标准差与主要转归均呈线性相关。对转归最强的预测指标为超急性期最高收缩压和急性期收缩压的标准差为结果的最强预示因子。对次要转归进行分析，得到的结果与以上结果相似〔超急性期最高五分位数，校正OR＝1.43，95％CI （1.14，1.80）； P＝0.0014；急性期OR＝1.46，95％CI （1.13，1.88）； P＝0.0044〕。解释收缩压变异性似乎可预测急性颅内出血患者的不良转归。早期治疗将收缩压降至140 mm Hg以下获得的收益，可通过平稳并持续血压控制进行强化，避免出现收缩压峰值尤为重要。

从19世纪早期铁路运营开始,车轴的疲劳设计就是工程设计人员在材料的疲劳研究方面的一个难点。为了保证高速铁路系统的安全,一些杰出的研究人员进行了大量的投资和试验,并且在材料、制造、热处理和设计方法等方面取得了很大进步。比较欧洲和日本在高速铁路车辆车轴疲劳设计上的原理,认为在新干线车辆和TGV,ICE之间存在一些区别。疲劳强度的危险部位主要是容易受到磨损和疲劳损伤的压装配合部位,如轮座、齿轮座和制动盘座等部位。在欧洲,车轴压装部位采用大直径使危险部位平滑;在日本采用高频硬化的方法提高压装部位的疲劳强度,同时在车轴的压装部位附近设置了应力释放槽。多年来,新干线的车轴经过磁粉探伤没有发现疲劳磨损裂纹,这表明高速铁路车轴的安全性多年的改进是成功的。

Current status in remnant gastric cancer after distal gastrectomy

Remnant gastric cancer(RGC) and gastric stump cancer after distal gastrectomy(DG) are recognized as the same clinical entity. In this review, the current knowledges as well as the non-settled issues of RGC are presented. Duodenogastric reflux and denervation of the gastric mucosa are considered as the two main factors responsible for the development of RGC after benign disease. On the other hand, some precancerous circumstances which already have existed at the time of initial surgery, such as atrophic gastritis and intestinal metaplasia, are the main factors associated with RGC after gastric cancer. Although eradication of Helicobacter pylori(H. pylori) in remnant stomach is promising, it is still uncertain whether it can reduce the risk of carcinogenesis. Periodic endoscopic surveillance after DG was reported useful in detecting RGC at an early stage, which offers a chance to undergo minimally invasive endoscopic treatment or laparoscopic surgery and leads to an improved prognosis in RGC patients. Future challenges may be expected to elucidate the benefit of eradication of H. pylori in the remnant stomach if it could reduce the risk for RGC, to build an optimal endoscopic surveillance strategy after DG by stratifying the risk for development of RGC, and to develop a specific staging system for RGC for the standardization of the treatment by prospecting the prognosis.
