Heparin bridge therapy and post-polypectomy bleeding

AIM To identify risk factors for post-polypectomy bleeding(PPB), focusing on antithrombotic agents. METHODS This was a case-control study based on medical records at a single center. PPB was defined as bleeding that occurred 6 h to 10 d after colonoscopic polypectomy and required endoscopic hemostasis. As risk factors for PPB, patient-related factors including anticoagulants, antiplatelets and heparin bridge therapy as well as polyp- and procedure-related factors were evaluated. All colonoscopic hot polypectomies, endoscopic mucosal resections and endoscopic submucosal dissections performed between January 2011 and December 2014 were reviewed. RESULTS PPB occurred in 29(3.7%) of 788 polypectomies performed during the study period. Antiplatelet or anticoagulant agents were prescribed for 210(26.6%)patients and were ceased before polypectomy except for aspirin and cilostazol in 19 cases. Bridging therapy using intravenous unfractionated heparin was adopted for 73 patients. The univariate analysis revealed that anticoagulants, heparin bridge, and anticoagulants plus heparin bridge were significantly associated with PPB(P < 0.0001) whereas antiplatelets and antiplatelets plus heparin were not. None of the other factors including age, gender, location, size, shape, number of resected polyps, prophylactic clipping and resection method were correlated with PPB. The multivariate analysis demonstrated that anticoagulants and anticoagulants plus heparin bridge therapy were significant risk factors for PPB(P < 0.0001). Of the 29 PPB cases, 4 required transfusions and none required surgery. A thromboembolic event occurred in a patient who took anticoagulant. CONCLUSION Patients taking anticoagulants have an increased risk of PPB, even if the anticoagulants are interrupted before polypectomy. Heparin-bridge therapy might be responsible for the increased PPB in patients taking anticoagulants.

AIM： To clarify the significance of JC virus （JCV） T-antigen （T-Ag） expression in human gastric cancer. METHODS： We investigated the relationship between TAg detected by immunohistochemistry and Epstein- Barr virus （EBV） infection, microsatellite instability （MSI）, and genetic and epigenetic alterations in gastric cancers. Mutations in the p53,β-catenin, K/MS, BRAF, PIK3CA genes were analyzed by PCR- single strand conformation polymorphism and DNA sequencing. Allelic losses were determined by PCR at 7 microsatellite loci. Aberrant DNA methylation was analyzed by MethyLight assay. RESULTS： JCV T-Ag protein expression was found in 49% of 90 gastric cancer tissues. TAg positivity was not correlated with clinicopathological characteristics. TAg expression was detected in a similar percentage of EBV positive cancers （4 of 9, 44%） and EBV negative cancers （35 of 73, 48%）. TAg expression was detected in a significantly lower percentage of MSI-H cancers （14%） than in non MSI-H cancers （55%, P = 0.005）. TAg expression was detected in a significantly higher percentage of cancers with nuclear/cytoplasmic localization of β-catenin （15 of 21, 71%） than in cancers without （42%, P = 0.018）. p53 mutations were detected in a significantly lower percentage of T-Ag positive cancers （32%） than in TAg negative cancers （57%, P = 0.018）. TAg positive gastric cancers showed a significant increase in the allelic losses and aberrant methylation compared with T-Ag negative gastric cancers （P = 0.008 and P = 0.003）. CONCLUSION： The results suggest that JCV T-Ag is involved in gastric carcinogenesis through multiple mechanisms of genetic and epigenetic alterations.

AIM： To clarify the importance of complete treatment by PELT. METHODS： A total of 140 previously untreated cases of HCC were enrolled in this study from 1988 to 2002. The inclusion criteria were： a solitary tumor less than 4 cm in diameter or multiple tumors, fewer than four in number and less than 3 cm in diameter, without extrahepatic metastasis or vessel invasion. As general principles for the treatment of HCC, the patients underwent transcatheter arterial chemoembolization （TACE） prior to PEIT. After the initial treatment of the patients, ultrasonography and computed tomography were performed, and measurement of serum levels of α- fetoprotein （AFP） was determined. When tumor recurrences were detected, PEIT and/or TACE were repeated whenever the hepatic functional reserve of the patient permitted. We then analyzed the variables that could influence prognosis, including tumor size and number, the serum levels of AFP, the parameters of hepatic function （albumin, bilirubin, ALT, hepaplastin test, platelet number, and indocyanine green retention at 15 rain [ICG-R15]）, combined therapy with TACE, distant recurrence, and local recurrence. RESULTS： Univariate analysis identified the ICG test, serum levels of AFP and albumin, tumor size and number, and local recurrence, but not distant recurrence, as significant prognostic variables. In multivariate analysis using those five parameters, the ICG test, tumor size, tumor number, and local recurrence were identified as significant prognostic factors. In both univariate and multivariate analyses, the relative risk for the ICG test was the highest, followed by local recurrence. CONCLUSION： We found that local recurrence is an independent prognostic factor of HCC, indicating that achieving complete treatment for HCC on first treatment is important for improving the prognosis of patients with HCC. 2005 The WJG Press and Elsevier Inc. Performances of glaucoma operations with Kahook Dual Blade or iStent combined with phacoemulsification in Japanese open angle glaucoma patients

AIM:To compare surgical outcomes of phacoemulsification combined with glaucoma surgical techniques performed with either Kahook Dual Blade(KDB)or iStent for Japanese patients with either primary open angle glaucoma or exfoliation ***:We retrospectively evaluated the surgical outcomes of 129 eyes of 84 Japanese patients with glaucoma who underwent KDB or 44 eyes of 34 patients who underwent phacoemulsification with iStent procedures combined with cataract *** primary outcome was surgical success or failure[with surgical failure being indicated by18 mm Hg as criterion A;IOP>14 mm Hg as criterion B on two consecutive study visits;or reoperation requirement].In addition,we assessed the number of postoperative glaucoma medications and the resulting ***:The probability of success was significantly higher in the KDB group than in the iStent group for criterion A(60.2%vs 46.4%,P=0.019).In the KDB group,the mean preoperative IOP of 19.8±7.3 mm Hg decreased significantly to 13.0±3.1 mm Hg(P<0.01),and the mean number of glaucoma medications at 2.5±1.4 decreased significantly to 1.6±1.6(P<0.01)12 mo *** the iStent group,the mean preoperative IOP of 17.8±2.9 mm Hg significantly decreased to 14.3±2.3 mm Hg(P<0.01),and the mean number of glaucoma medications at 2.2±1.1 decreased significantly to 0.9±1.4(P<0.01)12 mo *** overall IOP reduction percentage was higher in the KDB group(26.2%)than in the iStent group(19.0%)12 mo postoperatively(P=0.03).Hyphema occurred significantly more frequently in the KDB group(16.3%)than in the iStent group(2.3%,P=0.017).CONCLUSION:KDB and iStent procedures combined with cataract surgery both result in significant IOP and glaucoma medication reductions after the 12-month *** patients in the KDB group have a higher success rate for the target IOP of less than 18 mm Hg and a higher complication rate than those

Scattered and rapid intrahepatic recurrences after radio frequency ablation for hepatocellular carcinoma

AIM： To evaluate a series of patients with hepatocellular carcinoma （HCC） treated with several different protocols and devices. METHODS： We treated 138 patients [chronic hepatitis/ liver cirrhosis （Child-Pugh A/B/C）, 3/135 （107/25/3）] with two different devices and protocols： cool-tip needle [initial ablation at 60 W （standard method）（n = 37） or at 40 W （modified method）（n = 28）] or; ablation with a LeVeen needle using a standard single-step, full expansion （single-step） method （n = 39） or a multi-step, incremental expansion （multi-step） method. RESULTS： Eleven patients experienced rapid and scattered recurrences 1 to 7 mo after the ablation. Nine patients were treated by the cool-tip original protocol （60 W）（9/37 = 24%） and the other two by the LeVeen single-step method （2/39 = 5%）. The location of the recurrence was surrounding and limited to the site of ablation segment in three cases, and spread over one Iobule or both Iobules in the other eight cases. There was no recurrence in the patients treated with the modified cool-tip modified method （40 W） or the LeVeen multi-step method. CONCLUSION： There is a risk of rapid and scattered recurrence after RFA, especially when the standard cool- tip procedure is used. Because such recurrence would worsen the prognosis, we recommend that modified protocols for the cool-tip and LeVeen needle methods should be used in clinical practice.

Growth rate has a direct impact on the productivity of nitride LED production. Atmospheric pressure growth of GaN with a growth rate as high as 10/zm/h and also *** growth of 1μm/h by using 4 inch by 11 production scale MOVPE are described. XRD of （002） and （102） direction was 200 arcsec and 250 arcsec, respectively. Impact of the growth rate on productivity is discussed.

Further Stabilization and Power Density Improvement of Stack-Type Thermoelectric Power Generating Module with Biphasic Medium by Using Various Flexible Metals as Electrodes

In order to realize further stability of a stack-type thermoelectric power generating module (i.e. no electrical connections inside), flexible materials of metal springs and/or rods having restoring forces were installed between lower-temperature-sides of thermoelectric elements. These flexible materials were expected to play three important roles of interpolating different thermal expansions of the module components, enlarging heat removal area and penetration of any media through themselves. Then, a low-boiling-point medium (i.e. NOVEC manufactured by 3M Japan Ltd.) was also applied for a high-speed direct heat removal via its phase change from the lower-temperature-sides of the thermoelectric elements in the proposing stack-type thermoelectric power generating module. No electrical disconnections inside the module were confirmed for more than 9 years of use, indicating further module stability. The power generating density was improved to about 120 mW·m-2 with SUS304 springs having 0.7 mm diameter. Increasing power generating density can be expected in terms of suitable selection of flexible metal with high Vickers hardness, cavities control on the spring surface, more vigorous multiphase flow with adding powders to the medium and optimization of the module configurations according to numerical simulations.

Objective: To study the generator sites of spontaneous discharges in patients with immune-mediated neuromyotonia. Methods: Macro EMGs triggered by both spontaneously and voluntarily activated single action potentials were recorded and the mean peak-to-peak amplitude and area of the macro motor unit potentials were compared in two patients with typical acquired neuromyotonia having positive antibodies against voltage-gated potassium channels. Results: Mean peak-to-peak amplitude and area of Macro EMG motor unit potentials (macro MUPs) triggered by spontaneous discharges were significantly smaller than those triggered by voluntary activation in both patients. However, a few macro MUPs triggered by spontaneous discharges resembled those triggered by voluntary activation. Conclusions: Spontaneous discharges in two patients with immune-mediated neuromyotonia seem to be mostly generated at sites distal to the terminal axon branching points. Significance: This finding may provide a new insight in the understanding of spontaneous discharges in immune-mediated neuromyotonia.

Piezoelectric Power Harvesting via Acoustic-Pressure Driven by Low-Speed Wind-Force with Resonating-Tube and Wind-Collector

Wind-driven power harvestings attract attentions since their target wind speeds are quite low less than the so-called cut-in wind speed, which is generally recognized as around 3 m/s. The extant power harvestings driven by wind-induced-air-column-resonations (i.e. acoustic-pressures) are still lacking simplicity, scale flexibility and solid strategies for practical applications. Therefore, the piezoelectric power harvesters via acoustic-pressures driven by low-speedwind-forces with resonating-tubes and wind-collectors were invented so as to complement all the lacks. The wind-collector as well as the resonating-tube contributed to upraise the power harvesting density. The champion power harvesting density of 19.5 nW/dm2 could be procured at 2.3 m/s of an artificial wind and the optimal resonating-tube and wind-collector. Power harvesting proofs from the natural wind with low mean speeds down to about 0.6 m/s were successfully obtained. The cut-in wind speed of the prototype piezoelectric power harvester was found to be quite low as about 0.4 m/s, signifying its ubiquity. Finally, a multi-bundle pendant-type piezoelectric power harvester was specifically presented together with professing the solid and multiple strategies for practical applications.
