Dear editor,The current treatment strategy for advanced non-small cell lung cancer(NSCLC)patients harboring epider-mal growth factor receptor(EGFR)mutations involves EGFR-tyrosine kinase inhibitors(EGFR-TKIs).Several reports on EGFR-TKI resistance have shown that the T790M mutation within the EGFR gene is present in approximately half of the patients who develop resist-ance to a first-or second-generation EGFR-TKI[1,2].In Japan,osimertinib monotherapy is the standard therapy for patients harboring the EGFR T790M mutation arising after the development of resistance to a first-or second-generation EGFR-TKI[3,4].Herein,we report the case of a patient who underwent four tumor biopsies over the treatment *** no detection of this mutation after developing resistance to the first-line EGFR-TKI,in the fourth biopsy specimen,which was collected after progression following EGFR-TKI re-challenge,EGFR T790M mutation was detected using the cobas®EGFR mutation test(Roche Molecular Systems,Pleasanton,CA,USA).Additionally,we analyzed the T790M allele frequency(AF)in longitudinal biopsy samples obtained at four different time points using droplet digital PCR(ddPCR)(PrimePCR™,Bio-Rad Laboratories,Inc.,Her-cules,CA,USA).We discuss the clinical usefulness of longitudinal assessment of the EGFR T790M AF,espe-cially for deciding the appropriate treatment(e.g.,osi-mertinib)in patients with EGFR mutations.

Mutations in carboxy-terminal part of E2 including PKR/eIF2αphosphorylation homology domain and interferon sensitivity determining region of nonstructural 5A of hepatitis C virus 1b:Their correlation with response to interferon monotherapy and viral load

AIM: To study the amino acid substitutions in the carboxy (C)-terminal part of E2 protein and in the interferon (IFN) sensitivity determining region (ISDR) and their correlation with response to IFN and viral load in 85 hepatitis C virus (HCV)-lb-infected patients treated with IFN. METHODS: The C-terminal part of E2 (codons 617-711) including PKR/eIF2a phosphorylation homology domain (PePHD) and ISDR was sequenced in 85 HCV-1b-infected patients treated by IFN monotherapy. RESULTS: The amino acid substitutions in PePHD detected only in 4 of 85 patients were not correlated either with response to IFN or with viral load. The presence of substitutions in a N-terminal variable region (codons 617-641) in the C-terminal part of E2 was significantly correlated with both small viral load (33.9% vs 13.8%, P = 0.0394) and sustained response to IFN (25.0% vs 6.9 %, P = 0.0429). Four or more substitutions in ISDR were significantly correlated with both small viral load (78.6% vs 16.2%, P < 0.0001) and sustained response to IFN (85.7% vs 2.9%, P < 0.0001). In multivariate analysis, ISDR in nonstructural (NS) 5A (OR = 0.39, P < 0.0001) and N-terminal variable region (OR = 0.51, P = 0.039) was selected as the independent predictors for small viral load, and ISDR (OR = 39.0, P < 0.0001) was selected as the only independent predictor for sustained response. CONCLUSION: The N-terminal variable region in the C-terminal part of E2 correlates with both response to IFN monotherapy and viral load and is one of the factors independently associated with a small viral load.

Risk Factors for Cancer Chemotherapy-Induced Hiccups (CIH)

Background: Hiccups are common somatic side effects of medication. Our previous analysis of the clinical risk factors for hiccups identified chemotherapy as a factor related to hiccup risk. Therefore, in the present study, we investigated the risk factors for hiccups associated with chemotherapy. Methods: We included all patients who received cancer chemotherapy and were hospitalized at the Musashino Red Cross Hospital between April 2014 and December 2014. We investigated patient demographics, physical characteristics, and other clinical factors to identify the risk factors for chemotherapy-induced hiccups (CIH). We conducted univariate and multivariable analysis to compare the CIH group and the non-CIH and determined risk factors of CIH. Results: Hiccups were identified in 48 of 292 patients with an incidence rate of 16.4%. Univariate analysis revealed that the male gender, pain, and nausea and vomiting were related to CIH. It also showed that cisplatin, pemetrexed, gemcitabine, etoposide, dexamethasone, and metoclopramide were related to CIH.A correlation which was found with doses of cisplatin, pemetrexed, gemcitabine, and etoposide. Multivariable analysis identified male gender (OR, 72.69;95% CI, 6.95 - 757.64), nausea and vomiting (OR, 52.01;95% CI, 3.93 - 447.13), dexamethasone (OR, 4.55;95% CI, 1.12 - 16.91), cisplatin (OR, 3.84;95% CI, 1.52 - 9.70), and etoposide (OR, 3.72;95% CI, 1.14 - 12.11) as independent risk factors for hiccups. Conclusions: The present study is the first one to report risk factors for the development of CIH. Our results suggest that male gender, having nausea, and the drugs dexamethasone, cisplatin, and etoposide are important risk factors for CIH. These results may assist in elucidation of the underlying mechanisms and guide therapy to reduce hiccup risk.

北京棒状杆菌(Corynebacterium pekinense)AS 1.299经硫酸二乙酯诱变处理后,获得一株突变体 AS 1.586,可积累大量的 L-缬氨酸。利用生长图形法对 AS 1.586突变株进行营养要求测定,结果证明该菌株除要求异亮氨酸作为生长因子外,还可利用异亮氨酸生物合成途径上的中间产物如高丝氨酸、苏氨酸等作为生长因子。因此,是一株不严格缺陷型突变株。对该突变株的缬氨酸发酵条件研究指出,通气量大小是影响缬氨酸产量高低的重要因素。在500升发酵罐放大试验中,12小时前通风量为1∶0.7(体积/积体/分),12小时后提高风量达1∶0.9(体积/体积/分),发酵43小时,L-缬氨酸产量达26.8毫克/毫升,糖转化率为26%以上。

A simple and effcient method for the synthesis of new glycolipids containing oligosaccharide and two long alkyl chains N-4(dialkyl-L-glutamatecarbony lethylcarbonylamino)butyl]lactobionamide has been developed in moderate *** successfully selected succinic anhydride which could condese with dialkyl L-glutamates in almost quantitative *** aminolysis of lactonolactone with 1,4-diaminobutane could be carried out in quantitative yield. The glycolipids prepared by this method have three amino acid residues between hydrophobic segment and hydrophilic moiety of the glycolipid which can be consideeeered to maintain the steric conformation of lipase in enzymatic reactions.

用HPLC法测定L-亮氨酸

研究采用高效液相色谱(HPLC)和邻苯二甲醛(OPA)柱前衍生法测定L-亮氨酸的条件.样品通过ODS C-18柱分离,流动相由60%的50mmol/L乙酸钠溶液和40%的甲醇构成,荧光检测器波长Ex为350nm、Em为450nm.回收率99.93%~100.35%,相对标准偏差为0.754%.

Agkistrodon halys venom contains various enzymes,most of which are hydrolyticenzymes except L-amino acid oxidase. Crude A. halys venom and the injectionpowder prepared from the venom were cytotoxic in vitro to various tumor cells as wellas normal proliferating *** purification of the-crude venom,the in vitro cyto-toxic substance was identified as L-amino acid *** in vivo antitumor effectof the venom infection powder was investigated, but no clear suppression of thegrowth of tumor cells was observed. Thrombin-like coagulant enzyme was also isolatedfrom the crude venom. Differences in the properties between this enzyme and bovinethrombin were examined. The venom coagulant enzyme showed a microheterogeneitydue to varying contents of sialic acid. Presence of several endopeptidases(proteinases)was also demonstrated in the venom which may participate in the degradation offibrin clot.

复合氨基酸18（AAC18）是一种新型复合氨基酸输液剂，实验证明对家兔急性致死性出血有明显保护作用（P＜0.01）。静脉给低蛋白血症家兔AAC18-3（20ml／kg·d）5d，可显著升高血清总蛋白（2.0+0.7g／L）和白蛋白，（1.0+0.1g/L）（P＜0.05），伤口愈合时间平均缩短1.5d，静脉给蛋白质营养不良大鼠AAC18-3或AAC18-10（20ml／kg·d）21d，可明显升高大鼠血清总蛋白和白蛋白（P＜0.01），后者还能明显增加大鼠体重。各项试验分别与日本同类产品（Mo-N或MO-SN）平行对照，结果差异均无显著性意义。
