To the Editor:Human immunodeficiency virus(HIV)epidemic poses a significant challenge to global public health.As of 2022,approximately 39.0 million people worldwide were living with HIV.[1]With the ongoing evolution of combined antiretroviral therapy(cART),HIV infection has transformed into a manageable chronic disease requiring long-term treatment.Among the various classes of antiretroviral(ARV)drugs,integrase strand transfer inhibitor(INSTI)-based regimens,known for potency and tolerability,have emerged as the predominant treatment modality worldwide.The nextgeneration of INSTI,bictegravir(BIC),received the Food and Drug Administration(FDA)approval in 2018 as a single-tablet regimen(STR)containing BIC(50 mg)/emtricitabine(FTC,200 mg)/tenofovir alafenamide(TAF,25 mg)for once daily.It has been endorsed as the preferred choice for treatment-naive patients by major guidelines,including the 2021 edition of the China Acquired Immune Deficiency Syndrome(AIDS)Diagnosis and Treatment Guidelines.[2]In China,people living with HIV(PLWH)are provided with flexible cART options,including the National Reimbursement Drug List(NRDL),self-pay drugs,and free drugs.Since being included in NRDL in 2021,BIC/FTC/TAF have gained popularity among an increasing number of patients.

Building structures themselves are one of the key areas of urban energy consumption,therefore,are a major source of greenhouse gas emissions.With this understood,the carbon trading market is gradually expanding to the building sector to control greenhouse gas emissions.Hence,to balance the interests of the environment and the building users,this paper proposes an optimal operation scheme for the photovoltaic,energy storage system,and flexible building power system(PEFB),considering the combined benefit of building.Based on the model of conventional photovoltaic(PV)and energy storage system(ESS),the mathematical optimization model of the system is proposed by taking the combined benefit of the building to the economy,society,and environment as the optimization objective,taking the near-zero energy consumption and carbon emission limitation of the building as the main constraints.The optimized operation strategy in this paper can give optimal results by making a trade-off between the users’costs and the combined benefits of the building.The efficiency and effectiveness of the proposed methods are verified by simulated experiments.

急性心肌梗死(AMI)是因冠状动脉急性闭塞、血流中断引起的局部心肌缺血性坏死,主要临床表现为持续性胸骨后疼痛、休克、心律失常和心力衰竭等,并伴有血清心肌酶增高以及心电图改变,病死率较高。临床最常见的AMI手术治疗方法为经皮冠状动脉介入治疗(PCI),影响PCI手术预后效果的因素较多,主要包括血清肌酐水平、心理状态、营养不良、合并糖尿病和血清肌钙蛋白T水平、复流发生率、血管内皮生长因子等。近年来,提升AMI患者PCI手术预后的方法一直是临床AMI治疗的研究重点。

目的观察长沙地区人类免疫缺陷病毒1型(HIV-1)感染者原发耐药突变(PDRM)情况,以及具有完全活性的抗逆转录病毒治疗(ART)方案对PDRM的治疗效果。方法纳入长沙地区新确诊的HIV-1感染者307例,收集患者人口学特征等数据,采集血浆检测CD4+T淋巴细胞计数、HIV-1 RNA病毒载量及基因型耐药检测,判断患者PDRM情况。PDRM患者根据基因型耐药检测结果给予具有完全活性的ART方案;非PDRM患者采用我国指南推荐初治ART方案。比较PDRM患者和非PDRM患者的临床特征、治疗方案和治疗结局,采用Kaplan-Meier法比较PDRM与非PDRM患者的预后。结果 307例HIV-1感染者中,PDRM 77例,非PDRM 230例。PDRM患者中,单个耐药突变位点67例、多个耐药突变位点10例,V179D和V179E是最常见的耐药突变位点。PDRM患者与非PDRM患者的HIV-1亚型分布比较差异有统计学意义(P<0.01)。ART方案主要为非核苷酸逆转录酶抑制剂(NNRTI)+2种核苷酸逆转录酶抑制剂(NRTI),PDRM患者与非PDRM患者的ART方案比较差异有统计学意义(P<0.05)。PDRM患者病毒学抑制率持续低于非PDRM患者,平均生存时间较非PDRM患者缩短(P均<0.01)。结论长沙地区HIV-1感染者当前PDRM主要针对NNRTI及NRTI药物,PDRM患者更易在治疗早期发生病毒学不良事件,需要推广含整合酶抑制剂类药物的完全活性ART方案以便尽早降低病毒载量,改善患者预后。

A preliminary study has found that the 5-lipoxygenase inhibitor, caffeic acid, has a marked protective effect on acute brain injury induced by intracerebroventricular microinjection of aluminum. In this experiment, chronic brain injury and neuronal degeneration model was established in rats by chronic oral administration of aluminum, and then intervened using caffeic acid. Results showed that caffeic acid can downregulate chronic aluminum overload-induced 5-1ipoxygenase mRNA and protein expression, and repair the aluminum overload-induced hippocampal neuronal damage and spatial orientation impairment. It is suggested that direct intervention of 5-lipoxygenase expression has a neuroprotective role in the degeneration induced by chronic aluminum overload brain injury model.

Li_(2)MnO_(3)and Li_(2)RuO_(3)represent two prototype Li-rich transition metal(TM)oxides as high-capacity cathodes for Li-ion batteries,which have similar crystal structures but show quite different cycling performances.Here,based on the first-princi-ples calculations,we systematically studied the electronic structures and defect properties of these two Li-rich cathodes,in order to get more understanding on the structural degradation mechanism in Li-rich TM oxides.Our calculations indicated that the structural and cycling stability of Li_(2)MnO_(3)and Li_(2)RuO_(3)depend closely on their electronic structures,especially the energy of their highest occupied electronic states(HOS),as it largely determines the defect properties of these cathodes.For Li_(2)MnO_(3)with low-energy HOS,we found that,due to the defect charge transfer mechanism,various defects can form spontaneously in its host structure as Li ions are extracted upon delithiation,which seriously deteriorates its structural and cycling stability.While for Li_(2)RuO_(3),on the other hand,we identified that the high-energy HOS prevents it from the defect formation upon delithi-ation and thus preserve its cycling reversibility.Our studies thus illustrated an electronic origin of the structural degradation in Li-rich TM oxides and implied that it is possible to improve their cycling performances by carefully adjusting their TM compo-nents.

To investigate the thermal and mechanical behavior of casting wheel,a two-dimensional thermoelastic-plastic finite element model was used to predict the temperature,stress and distortion distribution of the casting wheel during the wheel and belt continuous casting process.The effects of grinding thickness and casting speed on the thermal and mechanical behaviors of the center of the hot face of the casting wheel were discussed in detail.In each rotation,the casting wheel passes through four different spray zones.The results show that the temperature distribution of the casting wheel in different spray zones is similar,the temperature of the hot face is the highest and the temperature reaches the peak in the spray zoneⅢ.The stress and distortion depend on the temperature distribution,and the maximum stress and distortion of the hot face are 358.2 MPa and 1.82 mm,respectively.The temperature at the center of the hot face decreases with increasing grinding thickness and increases with increasing casting speed.

Arsenic(As) fate in paddy fields has been one of the most significant current issues due to the strong As accumulation potential of rice plants under fooded conditions. However,no attempt was done to explore As methylation and volatilization under non-fooded conditions. Herein, we investigated the effects of water management on As methylation and volatilization in three arsenic-contaminated soils enhanced by biostimulation with strawderived organic matter and bioaugmentation with genetic engineered Pseudomonas putida KT2440(GE P. putida). Under fooded conditions, the application of biochar(BC), rice straw(RS)and their combination(BC+RS) increased total As in porewater. However, these effects were greatly attenuated under non-fooded conditions. Compared with RS amendment alone, the combination of GE P. putida and RS further promoted the As methylation and volatilization,and the promotion percentage under non-fooded conditions were significantly higher than that under fooded conditions. The combined GE P. putida and RS showed the highest efficiency in As methylation(88 μg/L) and volatilization(415.4 μg/(kg·year)) in the non-fooded soil with moderate As contamination. Finally, stepwise multiple linear regression analysis presented that methylated As, DOC and p H in porewater were the most important factors contributing to As volatilization. Overall, our findings suggest that combination of bioaugmentation with GE P. putida and biostimulation with RS/BC+RS is a potential strategy for bioremediation of arsenic-contaminated soils by enhancing As methylation and volatilization under non-fooded conditions.
