Risk factors for metachronous gastric carcinoma development after endoscopic resection of gastric dysplasia: Retrospective, single-center study

To determine the gastric adenocarcinoma (GAC) occurrence rate and related factors, we evaluated the follow-up results of patients confirmed to have gastric dysplasia after endoscopic resection (ER). METHODSWe retrospectively analyzed the medical records, endoscopic examination records, endoscopic procedure records, and histological records of 667 cases from 641 patients who were followed-up for at least 12 mo, from among 1273 patients who were conformed to have gastric dysplasia after Endoscopic mucosal resection (EMR) or endoscopic submucosal dissection (ESD) of gastric mucosal lesions between January 2007 and August 2013 at the Chungnam National University Hospital. RESULTSThe mean follow-up period was 33.8 mo, and the median follow-up period was 29 mo (range: 12-87). During the follow-up period, the occurrence of metachronous GAC was 4.0% (27/667). The mean and median interval periods between the occurrence of metachronous GAC and endoscopic treatment of gastric dysplasia were 36.3 and 34 mo, respectively (range: 16-71). The factors related to metachronous GAC occurrence after ER for gastric dysplasia were male sex (5.3% vs 1.0%), open-type atrophic gastritis (9.5% vs 3.4%), intestinal metaplasia (6.8% vs 2.4%), and high-grade dysplasia (HGD; 8.4% vs 3.2%). Among them, male sex [OR: 5.05 (1.18-21.68), P = 0.029], intestinal metaplasia [OR: 2.78 (1.24-6.23), P = 0.013], and HGD [OR: 2.70 (1.16-6.26), P = 0.021] were independent related factors in multivariate analysis. Furthermore, 24 of 27 GAC cases (88.9%) occurred at sites other than the previous resection sites, and 3 (11.1%) occurred at the same site as the previous resection site. CONCLUSIONMale sex, intestinal metaplasia, and HGD were significantly related to the occurrence of metachronous GAC after ER of gastric dysplasia, and most GACs occurred at sites other than the previous resection sites.

Elemental mercury(Hg0) is predominant constituent of flue gas emitted from coal-fired power plants. Adsorption has been considered the best available technology for removal of Hg0 from flue gas. However, adsorbent injection increases the amount of ash generated. In the present study, powdered activated carbon(PAC) was coated on polytetrafluoroethylene/glass fiber filters to increase Hg0 removal while concurrently reducing the amount of ash generated. The optimal PAC coating rate was determined in laboratory experiments to ensure better Hg0 removal with low pressure drop. When PAC of particle size less than 45 μm was used, and the areal density was 50 g/m2, the pressure drop remained under 30 Pa while the Hg0 removal efficiency increased to 15.8% from4.3%. The Hg0 removal efficiency also increased with decrease in filtration velocity. The optimal PAC coating rate was applied on a hybrid filter(HF), which was combined with a bag filter and an electrostatic precipitator in a single chamber. Originally designed to remove fine particulates matter, it was retrofitted to the flue gas control device for simultaneous Hg0 removal. By employing the PAC coating, the Hg removal efficiency of the HF increased to 79.79% from 66.35%. Also, a temporary reduction in Hg removal was seen but this was resolved following a cleaning cycle in which the dust layer was removed.

Ni-based porous electrocatalysts have been widely used in the hydrogen evolution reaction(HER)in alkaline water electrolysis,and the catalysts are produced by selective leaching of Al from Ni-Al *** is well known that chemical leaching of Ni-Al intermetallic compound(IMC)generates a high surface area in Ni(OH)_(2).However,the Ni(OH)_(2) produced by leaching the Ni-Al intermetallic compound retards the hydrogen evolution reaction,which is attributed to its weak hydrogen adsorption *** this study,we controlled the chemical state of Ni using plasma vapor deposition(PVD)followed by heat treatment,selective Al leaching,and electrochemical reduction.X-ray diffraction(XRD),scanning microscopy(SEM),transmission electron microscopy(TEM),and energy-dispersive X-ray spectroscopy(EDS)were used to confirm the phase evolution of the electrocatalysts during *** reveal that the heat-treated Ni-Al alloy with a thick Ni2Al3surface layer underwent selective Al leaching and produced biphasic interfaces comprising Ni(OH)_(2) and NiAl IMCs at the edges of the grains in the outermost surface *** oxidation of the interfacing NiAl IMCs facilitated the partial reduction of Ni(OH)_(2) to Ni(OH)_(2)/Ni in the grains during electrochemical reduction,as confirmed by X-ray photoelectron spectroscopy(XPS).An electrocatalyst containing partially reduced Ni(OH)_(2)/Ni exhibited an overpotential of 54 mV at 10 mA/cm^(2) in a half-cell measurement,and a cell voltage of 1.675 V at 0.4 A/cm2for single-cell operation.A combined experimental and theoretical study(density functional theory calculations)revealed that the superior HER activity was attributed to the presence of partially reduced metallic Ni with various defects and residual Al,which facilitated water adsorption,dissociation,and finally hydrogen evolution.

Chinese cabbage was cultivated in upland soil with the addition of biochar in order to investigate the potential for reduction of greenhouse gas emissions. Barley straw biochar(BSB) was introduced in a Wagner pot(1/5000a) in amounts of 0(BSB0, control), 100(BSB100), 300(BSB300), and 500(BSB500) kg 10a^(-1). After the addition of BSB into the upland soil, carbon dioxide(CO_2) emission increased while methane(CH_4) and nitrous oxide(N_2O) emissions decreased. The highest CO2 flux was measured for the BSB500 sample,(84.6 g m^(-2)) followed by BSB300, BSB100, and BSB0 in decreasing order. Relative to those of control, the total CH_4 flux and N_2O flux for the BSB500 treatment were lower by 31.6% and 26.1%, respectively. The global warming potential(GWP) of the treatment without biochar was 281.4 g CO_2 m-2 and those for treatments with biochar were in the range from 194.1 to 224.9 g CO_2 m^(-2). Therefore, introducing BSB into upland soil to cultivate Chinese cabbages can reduce the global warming potential.

AIM:To compare outcomes from radiofrequency ablation(RFA) and hepatectomy for treatment of colorectal liver metastasis(CRLM).METHODS:From January 2000 to December 2009,408 patients underwent curative intent treatment for *** excluded patients using the criteria:size of CRLM > 3 cm,number of CRLM ≥ 5,percutaneous RFA,follow-up period 2 cm) CRLM(P = 0.034).However,survival curves were similar for single or small(≤ 2 cm) CRLM(P = 0.714,P = 0.740).CONCLUSION:Hepatectomy is better than RFA for the treatment of ***,RFA might be suitable for selected patients with single,small(≤ 2 cm) CRLM.

A new structure for efficient and cost-effective L-band amplified spontaneous emission (ASE) generation was experimentally investigated and demonstrated using a C/L-band coupler, an optical mirror, and a fiber Bragg grating (FBG). The proposed structure has about 1.5 dB increase of the power around 1570 nm compared with conventional one. Various structures for power enhancement were also considered.

A strategy for fabricating microcrystalline cellulose–Ti_(3)C_(2)T_(x)(MCC–MXene)nanocomposite films with high relative permittivity,high thermal conductivity,and excellent mechanical properties was *** MCC–MXene nanocomposite film was fabricated by casting a solution containing N,N-dimethylacetamide/lithium chloride(DMAc/LiCl)-soluble MCC and DMAcdispersible MXene nanosheets,followed by humidity control *** MXene nanosheets greatly enhanced the permittivity of the nanocomposite films owing to interfacial ***,the nanocomposite film with 20 wt.%MXene content achieved a desirable permittivity of 71.4 at 102 Hz(a 770%improvement against that of neat cellulose),while the dielectric loss only increased by 1.8 times(from 0.39 to 0.70).The obtained nanocomposite films with 20 wt.%and 30 wt.%MXene exhibited remarkable in-plane thermal conductivities of 8.523 and 9.668 W∙m^(−1)∙K^(−1),respectively,owing to the uniform dispersion and selfalignment of the MXene layered ***,the uniformly dispersed MXene nanosheets in the MCC network with interfacial interaction(hydrogen bonding)and mechanical entanglement endowed the nanocomposite films with excellent mechanical properties and ***,the thermal stability,water resistance,and antibacterial properties of the nanocomposite films were effectively improved with the introduction of ***,using DMAc/LiCl as the solvent system not only improves the compatibility between MCC and MXene but also avoids the problem of easy oxidation of MXene in aqueous *** the high stability of the MCC–MXene solution and enhanced properties of the MCC–MXene films,the proposed strategy manifests great potential for fabricating natural biomass-based dielectric materials.
