Reperfusion is the key strategy in acute ST-segment elevation myocardial infarction (STEMI) care,and it is *** the time from symptom to reperfusion and choosing the optimal reperfusion strategy for STEMI patients are great challenges in *** need to improve upon the problems of low reperfusion rate,non-standardized treatment,and economic burden in STEMI *** article briefly reviews the current status of reperfusion strategy in STEMI care,and also introduces what we will do to bridge the gap between the guidelines and implementation in the clinical setting through the upcoming China STEMI early reperfusion program.

The definition, recommended treatment thresholds and targets of hypertension have been updated in the recent 2017 American College of Cardiology/American Heart Association(ACC/AHA) Guideline for the Prevention, Detection, Evaluation and Management of High Blood Pressure in Adults. However,the impacts of this guideline on Chinese population are currently unknown. In this study, we aim to provide updated data in China using criteria from the updated guideline. A multistage, stratified sampling method was used to obtain a representative sample of 50,171 adults aged 18 years and *** was defined as an average systolic blood pressure !130 mm Hg, or an average diastolic blood pressure !80 mm Hg, or self-reported use of antihypertensive medications in past 2 weeks. The weighted prevalence of hypertension was 60.1%(95% confidence interval [CI] 59.3%–61.0%). The treatment rate and recommended treatment rate were 16.8%(95% CI 16.1%–17.6%) and 53.7%(95% CI52.6%–54.9%), respectively; the gap was more prominent among men and participants from rural ***, the adoption of the 2017 ACC/AHA guideline will lead to a substantial increase in both prevalence and number of patients needing treatment in China. The applicability of this guideline should be carefully evaluated based on evidence from Chinese population.

In this paper, toll-like receptor expression pattern in monocytes-derived macrophages by lipopolysaccharid （LPS） stimulation was examined. Jugular venous blood samples from 4 Japanese calves were obtained and the peripheral blood mononuclear cells （PBMC） were isolated. The PBMC were cultured for 7 d so as to collect monocytes-derived macrophages in Repcell. The PBMC were stimulated by LPS for 24 h and the mRNA expression pattern of TLR and cytokines in monocytes-derived macrophages （Mod-Mφ） was analyzed. Results showed that LPS stimulation of Mod-Mφ could increase the mRNA levels of the genes of TNF-α, IL-6, and IL-8. In addition, the mRNA levels of the genes of TNF-α and IL-6 in the group of LPS stimulation were most significantly （P 〈 0.01） higher than those in control group and the mRNA levels of TLR1, 3, 5, 8, and 10 were significantly （P 〈 0.05） decreased after LPS stimulation. There was no difference in the mRNA expressions of TLR2, 4, 6, and 7 between the groups of the control and LPS stimulation. Besides, expression of TLR9 was not found. It suggested that monocytes-derived macrophages could respond to LPS and they might take an important role in the innate immunity. The important function of the cells might contribute to better disease treatment.

Radioactive tritium leakage from high-pressure storage vessels is a common nuclear leakage *** leakage conditions have different effects on tritium diffusion,resulting in different degrees of radioactive *** study focuses on tritium leakage from high-pressure storage vessels and analyzes the influence of different leakage orifice shapes,leakage positions,and the presence of obstacles in the scene space on tritium leakage *** results show that there is little difference in the radial diffusion velocity of tritium gas along the jet axis between circular and square leakage *** radial diffusion velocity of tritium gas in the long-axis direction of the rectangular leakage orifice is larger than that in the short-axis direction,and the larger the aspect ratio of the rectangle is,the greater the difference is in the diffusion *** addition,leakage from the storage vessel below the air inlet is beneficial to the dilution of tritium,whereas leakage from the air vents leads to a slow decrease in the tritium *** obstacles present in the tritium scene space hinder the migration of tritium gas and prolong the time for the tritium concentration to reach *** study provides a theoretical basis for the disposal of tritium in tritium leakage accidents by analyzing the influence of different leakage conditions in storage vessels on tritium gas diffusion.

A Novel High Nitrogen Nickel-free Coronary Stents System: Evaluation in a Porcine Model

Objective To study the safety of the novel high nitrogen nickel-free austenitic stainless steel bare metal stents （BMS） in a recognized porcine coronary model and to select a better grid structure of it. Methods Three types of stents were randomly implanted in different coronary arteries of the same pig： 316L stainless steel BMS （316L-BMS）（n=12）, novel high nitrogen nickel-free stents Grid A （NF-A-BMS）（n=12） and novel high nitrogen nickel-free stents Grid B （NF-B-BMS）（n=12）. In total, eighteen animals underwent successful random placement of 36 oversized stents in the coronary arteries. Coronary angiography was performed after 36 d of stents implantation. Nine animals were respectively sacrificed after 14 d and 36 d for histomorphologic analysis. 〈br〉 Results Quantitative coronary angiography （QCA） showed similar luminal loss （LL） in the three groups：（0.21±0.17） mm for 316L-BMS, （0.16±0.12） mm for NF-A-BMS, （0.24±0.15） mm for NF-B-BMS （P=0.05）. Histomorphomeric analysis after 15 d and 36 d revealed that there was also no significant difference among the three groups in neointimal area （NA） with similar injury scores respectively. High magnification histomorphologic examination showed similar inflammation scores in the three groups, but NF-A-BMS group had poorer endothelialization scores compared with NF-B-BMS group, 2.00±0.63 vs. 2.83±0.41 （P=0.015） at 15 d, which also could be proved by the scanning electron microscope. However, the difference could not been observed at 36 d. Conclusion The novel NF-BMS showed similar safety as 316L-BMS during the short-term study. NF-B-BMS had better endothelialization than NF-A-BMS and this may owe to the specific strut units.

Influence of education and working background on physicians' knowledge of secondary prevention guidelines for coronary heart disease: results from a survey in China

Background and objective： In clinical practice, the standard of secondary prevention for coronary heart disease （CHD） is quite disappointing in China. The physicians＇ shortage of knowledge of secondary prevention guidelines is thought to be a key factor contributing to the inadequate and delayed translation of guidelines into clinical practice. The purpose of this study is to investigate the influence of physicians＇ characteristics, including their educa- tion and work experience, on their knowledge of secondary prevention in China. Methods： A representative ques- tionnaire survey was made of physicians from cardiology departments in 35 tertiary hospitals in China. The survey contained 19 questions on knowledge of guideline recommendations for the secondary prevention of CHD. We col- lected basic information about the physicians, including their educational degree, clinical practice duration/work experience and geographic region. Results： In total, 864 physicians participated in the survey. Eight hundred and thirty-seven completed questionnaires were analyzed. For 6 of the 19 questions, physicians with a postgraduate degree were more likely to answer correctly than those without such a degree. For 11 of the 19 questions, physicians with more than three years＇ clinical experience were more likely to answer correctly than those who had less than three years＇ experience. For 5 of the 19 questions, *** eastern areas were more likely to answer correctly than those from mid/western areas. The mean total score of correct answers to the questionnaire was 11.69 points. Edu- cational degree and clinical practice duration affected total scores significantly while practice location did not （β=0.500, P=0.004; ,β=0.979, P=0.000; and 8=0.228, P=0.162, respectively）. Even if a relatively low score of 12 is taken as a threshold level of acceptable knowledge （defined as a pass）, the pass rate of all physicians was only 53.9%. Previous studies have shown that low platelet count combined with high plasma total homocysteine(tHcy)increased stroke risk and can be lowered by 73% with folic ***,the combined role of other platelet activation parameters and the methylenetetrahydrofolate reductase(MTHFR)C677T genotypes on stroke risk and folic acid treatment benefit remain to be *** study aimed to investigate if platelet activation parameters and MTHFR genotypes jointly impact folic acid treatment efficacy in first stroke *** were derived from the China Stroke Primary Prevention *** study includes a total of 11,185 adult hypertensive patients with relevant platelet activation parameters and MTHFR genotype *** simultaneously considering both platelet activation parameters(plateletcrit,platelet count,mean platelet volume,platelet distribution width)and MTHFR genotypes,patients with both low plateletcrit(Q1)and the TT genotype had the highest stroke incidence rate(5.6%)in the enalapril *** subgroup significantly benefited from folic acid treatment,with a 66% reduction in first stroke(HR:0.34;95%CI:0.14-0.82;p=0.016).Consistently,the subgroup with low plateletcrit(Q1)and the CC/CT genotype also benefited from folic acid treatment(HR:0.40;95%CI:0.23-0.70;p=0.001).In Chinese hypertensive adults,low plateletcrit can identify those who may greatly benefit from folic acid treatment,in particular,those with the TT genotype,a subpopulation known to have the highest stroke risk.

The complexity of alarm detection and diagnosis tasks often results in a lack of alarm log *** to the strong rule associations inherent in alarm log data,existing data augmentation algorithms cannot obtain good results for alarm log *** address this problem,this paper introduces a new algorithm for augmenting alarm log data,termed APRGAN,which combines a generative adversarial network(GAN)with the Apriori *** generates alarm log data under the guidance of rules mined by the rule ***,we propose a new dynamic updating mechanism to alleviate the mode collapse problem of the *** addition to updating the real reference dataset used to train the discriminator in the GAN,we dynamically update the parameters and the rule set of the Apriori algorithm according to the data generated in each *** extensive experimentation on two public datasets,it is demonstrated that APRGAN surpasses other data augmentation algorithms in the domain with respect to alarm log data augmentation,as evidenced by its superior performance on metrics such as BLEU,ROUGE,and METEOR.
